Je jen mále oblastí ve vnitřním lékařství, které zaznamenaly extrémně rychlý rozmach a implementaci do klinické praxe v posledních 5 letech, jako je katetrizační renální denervace (RDN) u pacientů nejen s rezistentní hypertenzí. Základním problémem, který nebyl dostatečně řešen ve všech studiích s RDN, je skutečnost, že více než 50 % pacientů s tzv. rezistentní hypertenzí jsou ve skutečnosti nemocní neukáznění a neadherující k zavedené mediaci. Výsledky studie SYMPLICITY HTN-3 neprokázaly žádné rozdíly mezi změnou krevního tlaku u pacientů s provedenou RDN proti skupině s tzv. falešnou procedurou, tedy pokračující medikamentózní terapií. Budoucnost RDN je více než otazná a je potřeba vrátit se zpět k animálním studiím, které ověří reálnou efektivitu RDN redefinovat populaci pacientů indikovaných k RDN a ověřit použitelnost nově vyvinutých technologií., Transcatheter renal denervation (RDN) has been markedly applied and developed in clinical practice in past five years and not only for patients with resistant hypertension. The issue that more than 50% patients with so-called resistant hypertension are in fact patients without adherence to this type of mediation has not been resolved in RDN trials till now. The study Symplicity HTN-3 showed no differences in blood pressure change between the patients with RDN and group of patients after false procedure who continued in their drug therapy. Pectus excavatum je najčastejšou vrodenou deformitou hrudníka. Deformitu ťažkého stupňa musíme považovať za absolútnu indikáciu na operačnú korekciu. Závažnosť deformity určená s pomocou rentgenových zobrazovacích vyšetrení je najdôležitejším indikačným kritériom. Štandardné RTG vyšetrenie hrudníka v dvoch projekciách, natívne špirálové MDCT vyšetrenie ponúkajú možnosť zhodnotiť rôzne základné ukazovatele závažnosti deformity, ako sú napríklad Halierov index, korekčný vertebrálny a fronto-sagitálny index. Rozvoj minimálne invazívnych operačných techník a narastajúci počet pacientov s asymetrickou deformitou liečených touto technikou priniesol potrebu nových indexov, ktoré operatérovi umožnia individuálne predtvarovať dlahu aj v tejto skupine pacientov. Autori práce sa v práci zamerali aj na komplikácie operačnej korekcie a typické nálezy, ktoré umožnia rentgenológovi tieto komplikácie zachytiť., Pectus excavatum is the most common inborn deformity of the chest. Severe deformity is considered to be an absolute indication for surgical correction. Indication criterias are dependent on multiple factors, but the severity of the malformation based on rádiológie assessment is the most important. Standard chest X-ray and native MDCT examination offer the evaluation of the severity of the deformity with the use of basic indices like Halier, correctional, vertebral or fronto-sagittal index. The evolution of minimally invasive pectus excavatum repair (MIRPE) and high number of asymmetric deformities treated with this procedure has brought the necessity for the creation of new indices. These indices help surgeons to individually preshape implanted bar in complex cases. Účinná léčba nádorových onemocnění často způsobuje trvalou ztrátu reprodukčních schopností pacienta. Jsou představeny moderní ukazatele ovariální rezervy, které mohou v běžné klinické praxi významně pomoci při zhodnocení míry rizika poškození gonadálních buněk v důsledku onkologické léčby. Práce dále popisuje zavedené metody reprodukční ochrany – kryokonzervace embryí a oocytů, kryokonzervace ovariální tkáně včetně možnosti její autotrasplantace a metody ochrany ovariální funkce aplikací analog gonadoliberinu. V diskuzi je pozornost věnována zejména výhodám a limitům popsaných metod ovariální protekce. V závěru jsou formulována stručná doporučení pro konzultace s pacientkami v adolescentním a reprodukčním věku před onkologickou léčbou, která významně ohrožuje jeho fertilitu., Effective treatment of cancer often causes patients irreversible damage of reproductive abilities. Úvod: Cílem práce je identifikovat pacienty operované pro vývodový karcinom pankreatu, kteří přežili více než 5 let a analyzovat jejich charakteristiky a faktory s vlivem na přežití. Zkušenosti a výsledky terciárního centra. Metoda: Prospektivně vedená databáze 155 pankreatických resekcí v období leden 2006 až červen 2010 byla prozkoumána se záměrem identifikace pacientů operovaných kurativně pro vývodový karcinom pankreatu. Klinická a patologická data byla analyzována retrospektivně. Kaplan-Meierova analýza byla použita pro hodnocení přežívání, Log-rank test a Coxova regresní analýza pro analýzu faktorů s vlivem na přežití. Testy byly dělány na hladině signifikance 0,05. Jako nezávisle proměnné v Coxově regresní analýze byly vybrány faktory, u nichž byl pomocí Kaplan-Meierovy analýzy prokázán signifikantní vliv na přežívání. Výsledky: V sestavě 155 resekcí pankreatu s kurativním záměrem bylo identifikováno 73 nemocných operovaných pro vývodový karcinom pankreatu, u nichž bylo možné hodnotit 5leté přežití. Více než 5 let přežilo 15 nemocných (20,5 %), z nichž 13 dosud žije. Průměrná doba přežití ve skupině přeživších 5 let je 77,1 měsíce (60–110) a medián 74 měsíce, průměrná doba přežití bez nemoci (relaps free survival) je 63,3 měsíce (14−110), medián 62 měsíce. Ze sledovaných faktorů statisticky významný vliv na přežití měly: nepřítomnost infiltrace lymfatických uzlin (p=0,031), nekomplikovaný pooperační průběh (p=0,025), nepřítomnost vaskulární invaze v tumoru (p=0,017), léčba bez nutnosti podání transfuze krve (p=0,015) a pooperační onkologické léčba – převážně chemoterapie (p=0,009). Významnými nezávislými prediktory přežívání jsou: přítomnost angioinvaze HR=2,239 (95%CI: 1,093−4,590; p=0,028), pooperační chemoterapie HR=2,587 (95%CI: 1,301−5,145; p=0,007) a nutnost podání transfuze HR=2,080 (95%CI: 1,027−4,212; p=0,042). Přítomnost angioinvaze zvyšuje riziko úmrtí pacienta 2,2x, podání transfuze zvyšuje riziko úmrtí 2,1x a nepodání chemoterapie zvyšuje riziko úmrtí 2,6x. Závěr: Vliv na dlouhodobé přežití u nemocných operovaných pro karcinom hlavy pankreatu byl prokázán u těchto faktorů: nepřítomnost infiltrace lymfatických uzlin, nekomplikovaný pooperační průběh, nepřítomnost vaskulární invaze v tumoru, léčba bez nutnosti podání transfuze krve a pooperační onkologická léčba – chemoterapie. Významnými nezávislými prediktory přežívání jsou: vaskulární invaze, chemoterapie a nutnost podání transfuze., Introduction: The purpose was to identify 5-year survivors among a group of radically resected patients with pancreatic cancer and analyse the characteristics and factors associated with their 5-year survival. Single tertiary centre experience. Method: A prospectively maintained database of 155 pancreatic resections from January 2006 to June 2010 was scanned to identify patients after curative radical resections for pancreatic ductal adenocarcinoma. The clinical and pathological data was analysed retrospectively. The outcomes of the PDAC group were evaluated using Kaplan-Meier analysis (survival) with the Log-rank test and Cox regression analysis (evaluation of prognostic factors). Characteristics of the survivors were discussed. Significance level of 0.05 was used. Those factors were used as independent variables for Cox regression analysis whose significant effect on survival was shown based on Kaplan-Meier analysis. Results: Among 155 patients undergoing a curative pancreatic resection, 73 had a pancreatic ductal adenocarcinoma. Fifteen patients (20.5%) after radical surgery survived over 5 years, 13 of whom are still alive. In the group of the survivors, the mean overall survival was 77.1 months (60−110) and the median survival was 74 months. The mean relapse-free interval in the group of the survivors was 63.3 months (14−110) with the median of 65 months. Factors associated with a longer survival included the absence of lymph node infiltration (p=0.031), uncomplicated postoperative course (p=0.025), absence of vascular invasion (p=0.017), no blood transfusions (p=0.015) and the use of postoperative therapy – predominantly chemotherapy (p=0.009). Significant independent predictors of survival included vascular invasion HR=2.239 (95%CI: 1.093−4.590; p=0.028), postoperative chemotherapy HR=2.587 (95%CI: 1.301−5.145; p=0.007) and blood transfusion HR=2.080 (95%CI: 1.027−4.212; p=0.042). The risk of death was increased 2.2 times in patients with vascular invasion, 2.1 times in patients with transfusions, and finally 2.6 times in those with no chemotherapy. Conclusion: Factors associated with an improved overall survival included: the absence of lymph node infiltration, an uncomplicated postoperative course, absence of vascular invasion, no need of blood transfusions, and finally the use of postoperative chemotherapy. Infekce močových cest vznikající v důsledku katetrizace (CAUTI) představují jednu z hlavních příčin infekčních onemocnění, k nimž dochází během hospitalizace pacienta. Vedle významného dopadu na morbiditu i mortalitu pacienta má tento typ infekce rovněž nezanedbatelné ekonomické důsledky. Vhodná péče o pacienty se zavedeným katetrem má tedy zcela zásadní význam. V tomto přehledovém článku předkládáme nejnovější informace (založené na důkazech) o léčbě CAUTI. Speciální pozornost je věnována různým možnostem prevence a léčby, jako jsou např. programy pro kontrolu infekce a upomínací systémy, povrchové úpravy různých materiálů, profylaxe antibiotiky a vhodná léčba symptomatických infekcí souvisejících s katetrizací. Optimální technologie pro prevenci CAUTI nebyla dosud vyvinuta a ačkoli nejrůznější přístupy vykazují slibné výsledky, zatím nejúčinnější formou zůstává prevence zbytečného zavádění katetru a jeho co možná nejrychlejší odstranění. Při manipulaci s katetry je nezbytné dodržovat doporučení stanovená na základě důkazů a postupovat v souladu s patřičnými guidelines., Catheter‑associated urinary tract infections (CAUTI) are a major source of hospital acquired infections with significant consequences to morbidity and mortality and having substantial financial implications. Therefore appropriate catheter care is of utmost importance. In this review we present an evidence‑based update on the management of CAUTI, with special emphasis on the different measures of prevention and treatment, such as infection control programs and reminder systems, surface material modifications of the foreign bodies, antibiotic prophylaxis, and the appropriate treatment of catheter associated symptomatic infections. An optimal technology to prevent CAUTI is still not available and although there are promising results with different approaches the most efficient means are still to avoid unnecessary catheterizations and to remove catheters as soon as possible. Cyclic vomiting syndrome (CVS) is a disorder characterized by recurrent, stereotypic episodes of nausea, vomiting, and other symptoms, separated by intervals of comparative wellness. These episodes carry on for hours or days. The patient is healthy between the episodes and has no clinical finding. For the treatment of the CVS, antiemetic, antimigraine and sedative medications were used. However, in some cases CVS treatment is very difficult. Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. V současnosti neexistuje standardní léčba cílená na inhibici HER2 pro pacientky s metastatickým HER2-pozitivním karcinomem prsu po progresi na léčbě trastuzumabem a lapatinibem. V retrospektivních studiích se prokázalo, že trastuzumab si může v této situaci zachovávat terapeutickou aktivitu a být validní léčebnou možností pro pacientky v dobrém celkovém stavu., Currently there are no standard therapeutical options for patients with metastatic HER2-positive breast cancer after progression on trastuzumab and lapatinib. Retrospective studies have indicated promising activity of trastuzumab retreatment in this clinical situation. Pojem „personalizovaná medicína“ sa v posledných rokoch spomína čoraz častejšie v súvislosti so snahami čo najlepšie prispôsobiť medikamentóznu, ale aj režimovú liečbu potrebám a požiadavkám jednotlivých pacientov. Personalizácia antidiabetickej liečby prekonala svoj vývoj v kontexte nárastu poznatkov týkajúcich sa cukrovky. Od úrovne empirickej sa posunula na úroveň fenotypickú, ktorá umožnila rozlišovanie medzi jednotlivými typmi diabetu. V rámci diabetu 2. typu sa od 60. rokov minulého storočia aplikovala patogenetická personalizácia, ktorá vychádzala z predpokladov, že u niektorých pacientov prevažuje inzulínová rezistencia a u iných deficit sekrécie inzulínu. Bioštatistická personalizácia (medicína dôkazov) viedla k dôkazom, na základe ktorých bol metformín zaradený do odporúčaní pre liečbu diabetu 2. typu ako liek prvej voľby. Randomizované štúdie v prvom desaťročí 21. storočia síce nedokázali superioritu žiadnej z iných liečebných modalít ako prídavnej liečby k metformínu, ale viedli k individualizácii cieľov glykemickej kompenzácie. V súčasnej dobe sa personalizácia posúva na úroveň farmakogenetickú, od ktorej môžeme v najbližších rokoch očakávať individualizáciu liečby v zmysle výberu liekov prvej, druhej a tretej voľby v závislosti od panelu kľúčových génových polymorfizmov charakterizujúcich citlivosť jedinca na konkrétne antidiabetiká. V konečnom dôsledku by mala byť „liečba ušitá na mieru“ vybratá na podklade syntézy patogenetických, bioštatistických a farmakogenetických poznatkov, čo bude reflektovať transláciu výsledkov základného biomedicinískeho výskumu do klinickej praxe. Kľúčové slová: diabetes 2. typu – farmakogenetika – medicína dôkazov – patogenéza – personalizovaná liečba, In recent years, the term “personalized medicine“ has been increasingly mentioned in relation to the endeavours to tailor the pharmaceutical as well as regimen therapy to the needs and requirements of individual patients. The personalization of antidiabetic treatment has undergone a dramatic advancement in relation to the expansion of knowledge about diabetes. From the empirical it moved forward to the phenotypic level which made it possible to differentiate between individual types of diabetes. The pathogenetic personalization which began to be used within Type 2 diabetes in the 1960s, was based on the assumption that while insulin resistance predominates in some patients, others are mainly affected by insulin secretion deficit. Biostatistics-personalized medicine (evidence based medicine) gathered evidence based on which metformin was included in recommendations on the therapy for Type 2 diabetes as a first-line drug. Although randomized studies during the first decade of the 21st century did not prove superiority of any other treatment modality as an adjunctive therapy used with metformin, they brought with them individualization of the goals of glycemic control. At present, personalization is heading towards the pharmacogenetic level that will enable in the near future individualized therapy in terms of choice of first-, second- and third-line drugs depending on the panel of key gene polymorphisms which characterize sensitivity of an individual to specific antidiabetics. Finally, the “tailor-maded therapy“ should be chosen based on a synthesis of pathogenetic, biostatistic and pharmacogenetic knowledge that will reflect the translation of results of the basic biomedical research into the clinical practice.
