Cíl. Zhodnotit první zkušenosti s využitím 18F-FCH-PET/CT v hodnocení účinků embolizační léčby hepatocelulárního karcinomu (HCC). Metoda. Byl hodnocen soubor 5 zobrazení 18F-FCH-PET/CT u mužů nemocných s transarteriální embolizaci (průměrný věk 69,6 let v rozpětí 59-83 let). Vyšetření byla provedena po aplikaci 18F-fluorocholinu ( 18F-FCH) v dávce 2 MBq/kg, součástí vyšetření bylo plně diagnostické dvoufázové MDCT. Byla zhodnocena vaskularizace ložiska a akumulace radiofarmaka. Výsledky. U všech nemocných šlo o dobře diferencovaný HCC. Ve čtyřech případech po embolizační terapii (1krát prostá embolizace pro krvácení, ve třech případech TACE) bylo nalezeno reziduum, ve třech případech byla rezidua tkáně HCC spíše hypervaskularizovaná, ale akumulace FCH byla zvýšená v menším rozsahu, naopak u hypovaskularizované tkáně ukázala vysoká metabolická aktivita viabilitu nádoru. U dvou nemocných byla nalezena nová ložiska. Závěr. 18F-FCH-PET/CT poskytuje komplexní informace o vaskularizaci a metabolické aktivitě HCC a zpřesňuje informace o viabilitě nádorového postižení., Aim. To evaluate an early experience with 18F-FCH-PET/CT in the therapy response assessments after trans-arterial embolisations of hepatocellular carcinoma (HCC). Method. The sample 18F-FCH-PET/ CT examinations was assessed in five men (mean age 69.6 years range 59-83) suffered from HCC after trans-arterial embolisation. All examinations were performed after intravenous administration of 18F-gluorocholine in the dose of 2 MBq per kilogram of body weight, the full diagnostic dual-phase MDCT forms the integral part of the examination. The vascularisation and the radio-pharmaceutical accumulation were assessed. Results. The well differentiated HCC was present in all patients. Signs of the recurrence were found in four men, hyper-metabolic all of them, in three, joined with hyper-vascularisation. But the increased 18F-FCH uptake was depicted in area less than one third of hypervascularized tissue. Hypo-vascularized hyper-metabolic viable tissue of HCC was found in one patient. Newly developed HCC foci were found in two patients. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. Jedním z významných faktorů ovlivňujících stravu je obliba jednotlivých potravin, potravinové preference. Základní chuťové preference jsou vrozené, typicky preference pro sladkou a slanou chuť a naopak odmítání hořké a kyselé chuti. Vrozené preference spolu s konkrétními zkušenostmi a dalšími vlivy utváří míru obliby jednotlivých potravin. Obecně se má za to, že preference patří mezi silné determinanty stravy, že lidé, a obzvláště děti, jedí především to, co jim chutná. Cílem předložené studie bylo zjistit, do jaké míry a jakým způsobem se potravinové preference promítají do skutečné konzumace u rozsáhlého souboru adolescentů. Výzkum byl proveden u 1655 dospívajících ve věku 15 let v rámci studie ELSPAC v oblasti města Brna. U celkem 56 položek v osmi potravinových a jedné nápojové skupině byly dotazníkovými nástroji zjišťovány jednak preference, jednak frekvence konzumace a byl analyzován jejich vzájemný vztah. Výsledky ukázaly, že preference a skutečná konzumace se výrazně liší. Preference se tedy do konzumace promítají jen částečně. Z deseti nejpreferovanějších položek byly pouze dvě mezi deseti nejkonzumovanějšími. Z deseti nejméně preferovaných se pět položek objevilo mezi deseti nejméně konzumovanými, "negativní preference" se tedy s konzumací shodují poněkud více. Konzumace byla celkově výrazně zdravější, než odpovídalo preferencím. Bližší analýza ukázala, že lze identifikovat několik typů souvislosti mezi preferencemi a konzumací, podle různých tvarů regresních křivek. Celkem nejvíce položek (27) vykazovalo závislost tvaru "J", ale například položky ve skupině sladkosti vykazovaly výhradně závislost "U", zatímco skupina masa vykazovala převážně lineární závislost. Praktické důsledky nelineárních závislostí spočívají zejména v tom, že některé potraviny jsou konzumovány výrazně více (nebo naopak méně), než odpovídá jejich preferencím. Jsou diskutována možná vysvětlení, spočívající v různých externích vlivech. Hlavním závěrem je, že preference nejsou rozhodujícím faktorem pro konzumaci, což dává jasný signál, že strava adolescentů je výrazně ovlivnitelná vnějšími vlivy, v optimálním případě příznivým směrem, cílenými preventivními aktivitami., Food preferences belong to significant factors influencing diet. Basic taste preferences are inherited, typically the preferences for sweet and salty tastes or dislike of bitter and sour tastes. Inherited taste preferences together with experience and other influences form the extent of preference of particular foods. Generally it is believed that food preferences belong to strong determinants of food choice, and that people, especially children, eat what they like. The objective of the study was to investigate to what extent and how food preferences are reflected in consumption in a large sample of adolescents. The research involved 1,655 adolescents aged 15 years, and was part of the ELSPAC study in the Brno district. Both food preferences (liking) and consumption frequency were investigated by questionnaire survey for 56 items in 8 food groups and 1 beverage group, and the relationship between preferences and consumption was analysed. The results show that preferences and consumption differ substantially and thus preferences are only partially reflected by consumption. From 10 most preferred items only 2 were among the 10 most consumed. From 10 least preferred items, 5 were classed among the 10 least consumed, thus "negative preferences" coincide with consumption to a greater extent. Consumption was generally healthier than could be assumed according to preferences. Detailed analyse showed several types of association between preferences and consumption, as demonstrated by different shapes of regression curves. Most items, 27 altogether, showed the "J" type of association, but items in the sweets group showed solely "U" type, while items in the meat group largely had a linear association. The main practical consequences of nonlinear associations are that some foods are consumed considerably more (or less) than preferences would allow. The possible explanations, based on various external influences, are discussed. The main conclusion is that preferences are not the most important determinates for consumption, which gives a clear signal that the diet of adolescents is highly influenceable by external factors, optimally in a favourable manner, by targeted preventive activities., Jindřich Fiala, Lubomír Kukla, and Literatura
Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. AIMS: Authors studied potential side effects of fetal calf serum (FCS) in cultivation media on human dental pulp stem cells (DPSC) during long-term cultivation. METHODS: Two lines of DPSC obtained healthy donors (male 22 years, female 23 years) were used. Both lines were cultivated under standard cultivation conditions in four different media containing 10% or 2% FCS and substituted with growth factors. During long-term cultivation proliferation ability, karyotype and phenotype of DPSC were measured. RESULTS: Both lines of DPSC cultivated in a media containing 2% FCS and ITS supplement showed the highest number of population doublings. On the other hand the proliferation rate of DPSC cultivated in a media with 2% FCS without ITS supplement was slowest. Proliferation rate of DPSC cultivated in 10% FCS media with or without FGF-2 was comparable. DPSC cultivated in a media with 10% FCS showed a significantly higher amount of chromosomal aberrations. These chromosomal aberrations do not seem to be clonal but surprisingly we found large amounts of tetraploid cells in the 9th passage in both media containing 10% FCS. CONCLUSIONS: Our study proved that cultivation of DPSC in media containing higher concentration of FCS has critical side effects on cell chromosomal stability. and J. Suchánek, TS. Kleplová, M. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. Cardiac surgery is inseparably linked to the activation of innate immunity cells recognizing danger signals of both endogenous and exogenous origin via pattern recognition receptors such as TLR receptors. Therefore, we followed by flow cytometry TLR2 and TLR4 expression on blood monocytes and granulocytes of patients who underwent coronary artery bypass grafting using beating heart surgery (off-pump, n = 34), with use of standard cardiopulmonary bypass (CPB), (on-pump, n = 30), and miniinvasive CPB (mini on-pump, n = 25), respectively, before, during surgery, and up to 7th postoperative day. TLR2 and TLR4 expression both on monocytes and granulocytes was significantly diminished already at the end of CPB being highly significantly decreased at the end of surgery in all patients' groups. TLR2 and TLR4 expression reached preoperative value at the 1st postoperative day being significantly higher at the 3rd postoperative day. Using intracellular staining we found the peak of TLR2 and TLR4 expression inside of monocytes and granulocytes at the first postoperative day in a subgroup of on-pump patients. In conclusion, TLR2 and TLR4 expression is significantly modulated in patients undergoing coronary artery bypass grafting as a part of adaptive homeostatic mechanisms induced by major surgery. The very surgical trauma is responsible for TLR2 and TLR4 modulation. Surprisingly, cardiopulmonary bypass itself was little contributing to the modulation of TLR2 and TLR4 expression. and J. The aim of this study is to evaluate the results of total hip arthroplasty in patients with Parkinson's disease during a period of five years, focusing on the assessment of the risks and benefits of surgery. During this period we performed total hip arthroplasty in 14 patients (15 hips) with Parkinson's disease. Patients were evaluated by subjective symptoms and objective findings, with a focus on the use of support while walking and walking distance, severity of Parkinson's disease before surgery and at the time of the last follow-up. During the postoperative period, the following parameters were assessed: length of ICU stay, mobilization, complications, the total duration of hospitalization and follow-up care after discharge. Of the 11 patients (12 hips) followed-up 1-5 years with an average of 3 years after operation 8 cases showed progression of neurological disability. 5 patients (6 hips) showed an increased dependence on the use of support when walking and reduced distance that the patient was able to walk. Subjectively, 10 hip joints were completely painless and 2 patients complained of only occasional mild pain in the operated hip. Complications that were encountered were urinary tract infection (5 patients), cognitive impairment (3 patients) and pressure ulcer (2 patients). We did not observe any infection or dislocation of the prosthesis. Three patients fell and fractured the femur and 3 patients in our cohort died during follow up. Implantation of total replacement is possible with judicious indication after careful evaluation of neurological finding in patients with minimal or mild functional impairment of the locomotor system. Z 30 166 případů leukemií, evidovaných v českém registru nádorů v letech 1976–2005 bylo 3 937 vícečetných případů spojeno s výskytem jiných novotvarů. Z nich se 2 450 (62,2 %) vyskytlo u mužů a 1 487 (37,8 %) u žen, tj. 14,7% případů u mužů a 11 % u žen ze všech evidovaných leukemií. Po 1 715 primárních leukemiích bylo diagnostikováno 2 064 jiných novotvarů a před 2 222 následnými leukemiemi 2 642 jiných primárních nádorů. Ze 14 krajů se ve třech vyskytlo 35,5 % vícečetných leukemií (Jihomoravský a Severomoravský kraj, Praha). V průměru za 4,9 roku u mužů a 5,5 roku u žen následovalo 35,1 % nádorů kůže a melanom, 16,9 % nádorů trávicích, 13 % dýchacích a 8,4 % močových cest. Očekávaný nízký počet pokročilých klinických stadií u následných nádorů se nepotvrdil: u mužů zahrnoval 43,8 % nádorů dutiny ústní, 41,5 % dýchacích, 34,2% trávicích, 10,6 % močových cest a 15,7 % pohlavních orgánů; u žen 39,4 % dýchacích, 30,6 % močových, 30 % trávicích cest a 28,2 % pohlavních orgánů, 25 % prsů, 25 % dutiny ústní. Bohužel mnoho klinických stadií nebylo v registru nahlášeno. Před následnými leukemiemi bylo nejvíce primárních karcinomů kůže a melanomu u 43,6 % případů jako ukazatel maligního stavu. Do října 2007 přežívalo u mužů jen 12,4 % primárních a 10,3 % následných leukemií, respektive 19,7 % a 14 % u žen. Kolik dalších novotvarů se vyskytne u osmi tisíc přežívajících s leukemií, očekávaných v roce 2015? Rozbory vícečetných novotvarů by mohly osvětlit mechanizmy jejich vzniku a poskytnout náhled na dispenzární péči a preventivní opatření, ze kterých budou mít prospěch rostoucí počty onkologicky nemocných v remisi., A total of 30,166 cases of leukemia, based on the Czech Cancer Registry in 1976–2005, there were notificated 3,937 multiple cases associated with the ocurrence of other neoplasms. Of them there were 2,450 (62.2 %) in males and 1,487 (37.8 %) in females, i.e. 14.7 % cases in males and 11 % in females of total registered leukemia. There were diagnosed 2,064 other neoplasms after 1,715 primary leukemias and 2,642 other primary neoplasms before 2,222 subsequent leukemias. A total of 14 Czech regions were distributed 35.5 % multiple leukemias in the three regions (Southern and Northern Moravia, Prague). In the average interval 4.9 years in males and 5.5 years in females followed 35.1 % cancer of skin and melanoma, 16.9 % digestive, 13 % respiratory and 8.4 % urinary system. The expected low representation of advanced clinical stages in subsequent cancers was not confirmed: in males 43.8 % cancer of oral cavity, 41.5 % respiratory, 34.2 % digestive, 10.6 % urinary and 15.7 % genital organs; in females 39.4 % respiratory, 30.6 % urinary, 30 % digestive and 28.2 % genital organs, 25 % breast, 25 % oral cavity. Unfortunatelly a lot of clinical stages were not registered. Before the subsequent leukemias the most frequent were 43.6 % primary cancers of skin and melanoma as a sign of malignant status. Up to October 2007 in males survived only 12.4 % of primary and 10.3 % subsequent leukemias, respective 19.7 % and 14 % in females. How many other neoplasms will be diagnosed among 8,000 surviving with leukemia, estimated in the Czech Republic in 2015?
