Autoři uvádějí kazuistiku pacienta s epiteloidním hemangioendoteliomem jater (HEHE), který byl léčen chemoembolizací (TACE). Při dlouhodobém sledování 9 let od stanovení diagnózy je klinicky bez obtíží a bez progrese onemocnění při vyšetření zobrazovacími metodami. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma. Diastematomyelie je vzácná vrozená vada patřící mezi spinální dysrafizmy a obvykle je asociována se syndromem míšního ukotvení. Diastematomyelie je tvořena dvěma polovinami rozštěpené míchy s vlastními durálními vaky, které jsou rozděleny osteokartilaginózním septem. Tato vada bývá diagnostikována především v dětském věku. U dospělých je vzácná a dominujícím příznakem je bolest. Autoři prezentují kazuistiku ženy ve věku 44 let s diastematomyelií, která si stěžovala na chronické bolesti zad a progredující slabost dolních končetin. U pacientky jsme provedli laminektomii, deliberaci míchy a plastiku durálního vaku. Po operaci pacientka zůstala bez neurologického deficitu a udávala výraznou úlevu od bolestí., Diastematomyelia is a rare congenital defect, a type of spinal dysraphisms, often associated with the tethered cord syndrome. Diastematomyelia is formed by the two halves of the split spinal cord that each has own dural sac separated by a rigid or fibrotic septum. This defect is mainly diagnosed in children. Pain is the dominant symptom in adult patients. The authors present a case of a woman aged 44 years with diastematomyelia who complained of chronic back pain and progressive weakness in her legs. In this patient, laminectomy was performed, the spinal cord released and the dural sac restored. After the surgery, the patient had no neurological deficits and reported significant pain relief. Úvod: Klivus je centrální struktura baze lební v těsné blízkosti mozkového kmene a životně důležitých cév. K frakturám klivu dochází většinou při vysokoenergetických úrazech, jednou z nejčastějších komplikací bývá nazální likvorea. Kazuistika: Představujeme případ muže ve středním věku, u kterého se po běžném úrazu hlavy rozvinula nazální likvorea. CT vyšetření ukázalo masivní pneumocefalus a frakturu klivu v oblasti sinus sphenoidalis. Provedli jsme endoskopický endonazální výkon a defekt úspěšně uzavřeli. Diskuze: Pneumatizace sfenoidální dutiny vykazuje velkou interindividuální variabilitu. V našem případě se jedná o extrémně vzdušný sinus v kombinaci s gracilním klivem, což bylo predisponujícím faktorem pro zlomeninu. Traumatická likvorea představuje riziko likvorové hypotenze a vzniku meningitidy. Klasický chirurgický přístup je mikroskopický transnazální. Endoskopie je moderní miniinvazivní variantou. Závěr: Endoskopické endonazální ošetření traumatické likvorey u zlomenin baze lební v oblasti klivu je metodou volby. Klíčová slova: zlomenina baze lební – neurochirurgie – endoskopie, Introduction: Clivus is a central structure of the skull base located in the vicinity of the brainstem and vital brain vessels. Clival fractures are usually caused by a high-energy trauma. Cerebrospinal fluid leak is one of the most common complications. Case report: A middle-aged male sustained a mild head trauma, followed by a nasal cerebrospinal fluid leak. CT scan revealed the massive pneumocephalus and the fracture of the clivus in the posterior wall of the sphenoidal sinus. We performed an endoscopic endonasal surgery to seal the defect. Discussion: Pneumatization of a sphenoidal sinus shows high variability. In the presented case, extreme pneumatization of the sinuses was combined with the gracile clivus, which was the predisposing factor for fracture. Traumatic cerebrospinal fluid leak carries the risk of intracranial hypotension and meningitis. Microscopic transseptal management is the classical surgical approach, while endoscopy provides the modern miniinvasive option. Conclusion: Endoscopic endonasal treatment of traumatic cerebrospinal fluid leak is the treatment of choice for the clival fractures. Key words: skullbase fracture – neurosurgery – endoscopy, and M. Gastrointestinální stromální tumory (GISTy) představují většinu mezenchymálních nádorů zažívacího traktu. Extragastrointestinální tumory (EGISTy) jsou nádory, které se v histologickém a imunohistochemickém obraze GISTům podobají, ale nacházejí se mimo trávicí trubici, nemají žádnou spojitost se stěnou žaludku či střeva. V naší kazuistice představujeme případ 68letého pacienta s EGISTem vycházejím z mezenteria. Intramurální hematom jícnu je vzácně se vyskytující diagnóza. Prezentuje se retrosternální bolestí, dysfagií či hematemezou. Je způsoben barotraumatem či přímým poraněním jícnu, nebo se může objevit spontánně bez zjevné příčiny, což je častější u pacientů užívajících antiagregační či antikoagulační preparáty. Ve většině případů je léčba konzervativní, v ojedinělých případech endoskopická, endovaskulární či chirurgická. Tento článek prezentuje kazuistiku pacientky se spontánním hematomem jícnu řešeným konzervativně a diskuzi o dalších možnostech průběhu a léčby onemocnění., Intramural hematoma of the oesophagus is a rare diagnosis presenting with retrosternal pain, dysphagia and haematemesis. It is caused by barotrauma or direct injury to the esophagus, or it may occur spontaneously without apparent cause, which is more common in patients taking antiplatelet or anticoagulant medications. In most cases, treatment is conservative. Rarely, endoscopic, endovascular or surgical treatment is effective. Kolorektální karcinom z prstenčitých buněk je vzácné agresivní nádorové onemocnění. Incidence u mladých pacientů je velmi nízká a zaujímá do 1 % všech chirurgicky léčených kolorektálních karcinomů. Raritní výskyt u mladých pacientů koresponduje s diagnostikováním tohoto onemocnění v pokročilých stadiích a s nepříznivou prognózou. Pro verifikaci karcinomu z prstenčitých buněk je zásadní bioptické vyšetření a léčba je obdobná jako u ostatních kolorektálních karcinomů. Autoři popisují případ mladé ženy s diagnostikovaným kolorektálním karcinomem z prstenčitých buněk. Cílem sdělení je upozornit na raritní výskyt vzácného kolorektálního karcinomu u mladé nemocné., Signet ring cell colorectal carcinoma is a rare aggressive tumor. The incidence of this desease is very low in young patients and accounts up to 1% of all surgical patients with colorectal cancer. The infrequency of the disease among young patients makes the diagnosis more difficult and the prognosis less favourable. Biopsy results are crucial for verification of signet ring cell carcinoma. Treatment is similar as in other types of colorectal cancer. We report a case of signet ring cell colon cancer in a 22 years old female patient. Je prezentován případ 29leté pacientky, u které bylo zjištěno vzácné nádorové onemocnění – primární leiomyosarkom ledviny. Diagnóza byla stanovena u pacientky ve 32. týdnu gravidity pomocí ultrazvuku, nádor se projevoval výraznou bolestivostí levého boku, kvůli které byla pacientka odeslána na naši ambulanci. Při vyšetření byl zjištěn objemný tumor levé ledviny, který byl po porodu, indukovaném v dřívějším termínu, potvrzen i při vyšetření počítačovou tomografií (CT). Nádor byl úspěšně odoperován, histologicky byl verifikován nález leiomyosarkomu. Nyní je pacientka 19 měsíců od operačního výkonu bez nálezu progrese onemocnění či metastáz., We present a case of 29-year-old patient with diagnosis of a rare cancer: primary leiomyosarcoma of the kidney. The diagnosis was determined in the patient in 32th week of pregnancy using ultrasound. Tumour manifested with severe pain on left side, for which the patient was referred to our clinic. During examination an extensive tumour of the left kidney was detected, which was confirmed by CT scan after an early delivery. The tumour was successfully surgically removed. Histologic examination proved diagnosis of leiomyosarcoma. Nineteen months after the surgery the patient has no evidence of disease progression or metastasis., and Wasserbauer R., Ivičič J.
Maligní mezoteliom je vzácný nádor, jehož incidence je 0,5−3 případy na milion obyvatel. Pouze přibližně ve 20 % případů se vyskytuje na peritoneu. Autoři na příkladu kazuistiky nemocného s výskytem maligního mezoteliomu v peritoneální dutině dokladují obtížnost předoperační diagnostiky, kdy dle předoperačních vyšetření bylo nejprve pomýšleno na primární tumor jater a kdy teprve definitivní histologické vyšetření stanovilo diagnózu. V diskuzi se pak věnují epidemiologii, diagnostice a léčbě tohoto raritního onemocnění a zdůrazňují nezastupitelnou roli chirurgického řešení při jeho léčbě. Only about 20% of cases occur in the peritoneum. The authors use the case report of a male patient with malignant mesothelioma occurring in the peritoneal cavity to demonstrate the difficulty of preoperative diagnosis: preoperative examinations made the authors consider a primary liver tumour and only definitive histological examination revealed the diagnosis. The discussion deals with the epidemiology, diagnosis and treatment of this rare disease and underlines the unique role of surgical therapy in its management.
