Cíle studie: Cílem této studie je sledování vzájemného vztahu plasmatických hladin imatinibu s jeho denní dávkou a časovým intervalem od poslední užité dávky u pacientů s chronickou myeloidní leukemií na terapii imatinibem. Chronická myeloidní leukemie (CML) je myeloproliferativní onemocnění charakteristické přítomností chromozomu Philadelphia a leukemického fúzního genu BCR-ABL vedoucího k expresi konstitutivně aktivní tyrosinkinázy zodpovědné za maligní transformaci. Největší úspěšnost v léčbě byla zaznamenána prostřednictvím cílených inhibitorů vzniklého onkoproteinu, proto je dnes imatinib používán jako lék první linie. Terapeutické monitorování plasmatických hladin imatinibu u pacientů s CML je důležitým nástrojem k optimalizaci a individualizaci léčby. Typ studie: Klinická Materiál a metody: V naší studii jsme analyzovali soubor 1790 vzorků od 168 pacientů s CML na terapii imatinibem. Veškerá měření byla provedena metodou kapalinové chromatografie ve spojení s tandemovou hmotnostní spektrometrií v rámci rutinního vyšetřování plasmatických hladin tyrosinkinázových inhibitorů. Denní dávka imatinibu s pohybovala v rozmezí 100 až 800 mg; vzorky byly odebrány 1–120 hodin od poslední užité dávky. Výsledky a závěr: Analýza výsledků odhalila významnou korelaci plasmatické hladiny imatinibu s jeho denní dávkou (R2 = 0,075) a časovým intervalem od poslední užité dávky (R2 = 0,177). Z celého souboru bylo vybráno 616 vzorků odebraných 24±4 hodiny od poslední dávky od 111 pacientů s denní dávkou 400 mg imatinibu a byla stanovena inter- (48 %) a intraindividuální (36 %) variabilita., Objective: The aim of the study is monitoring of correlation of imatinib plasma levels with daily dose and sampling time in patients with chronic myeloid leukemia on imatinib therapy. Chronic myeloid leukemia (CML) is myeloproliferative disorder characterized by the presence of chromozome Philadelphia and leukemic fusion gene BCR-ABL leading expression of constitutively active tyrosin kinase responsible for malignant transformation. The most successful treatment was recorded via targeted inhibitors of generated oncoprotein and therefore imatinib is used as a first-line treatment, today. Therapeutic monitoring of imatinib plasma levels in patients with chronic myeloid leukemia is important tool for treatment individualization. Design: Clinical Materials and methods: In our study we have analyzed a group of 1790 samples from 168 patients with CML on imatinib therapy. All measurements were performed by the liquid chromatography coupled with tandem mass spectrometry method within routine monitoring of plasmatic levels of tyrosin kinase inhibitors. Imatinib daily dose vary from 100 to 800 mg and samples were obtained 1 - 120 hours after last drug administration. Results and conclusion: Analysis of the results revealed correlation between imatinib plasma level and daily dose (R2 = 0.075) or with sampling time (R2 = 0.177). Cílem této pilotní studie bylo srovnat výsledky ze dvou používaných nepřímých metod pro stanovení maximální spotřeby kyslíku (odhadu VO2max) a zhodnotit jejich využitelnost pro posuzování fyzické zdatnosti v rámci primárně preventivního vyšetření. K tomuto srovnání jsme použili Cooperův 12minutový běžecký test (CT 12) na běhátku a stupňovaný zátěžový test na bicyklovém ergometru (BE) do maxima. Naše výsledky ukázaly signifikantně nižší hodnoty odhadu VO2max z CT 12 na běhátku oproti stupňovanému zátěžovému testu na BE do maxima napříč různými skupinami populace. V celkovém souboru byly tyto hodnoty v průměru o 8,6 % (p<0,001) nižší. Rozdíly v odhadech VO2max se lišily svojí mírou v jednotlivých skupinách podle trénovanosti a převažujícího typu pohybové aktivity; největší rozdíly mezi oběma testy byly u cyklistů (-12,0 %), studentů medicíny (-10,9 %) a "nevyhraněných" (-10,4 %) a u těchto skupin dosáhly rovněž statistické významnosti (p<0,01). Podstatně nižší rozdíly byly naopak u běžců (-4,6 %, statisticky nevýznamné) a ve skupině triatlonistů prakticky žádné rozdíly nebyly. Výsledky a pozorování z tohoto pilotního testování naznačují, že CT 12 na běhátku je oproti stupňovanému testu na BE do maxima podstatně náchylnější k vlivům jako trénovanost, převládající typ pohybové aktivity, motivace apod. Po celkovém zhodnocení výsledků se nám pro vyšetření fyzické zdatnosti v rámci primárně preventivního vyšetření u běžné populace s průměrnou, ale i mírně nadprůměrnou fyzickou zdatností jako vhodnější jeví stupňovaný zátěžový test na BE do maxima., The aim of this pilot study was to compare the results of two commonly used indirect methods of assessing maximum oxygen uptake (VO2max estimation) and to assess the applicability of these two methods for physical fitness assessment of an individual as part of primary preventive examination. For this comparison we selected the Cooper run/walk test (CT 12) on a treadmill and an incremental maximum test on a bicycle ergometer (BE). Our results showed significantly lower VO2max estimations gained from CT 12 on a treadmill in comparison to incremental maximum testing on a BE in the population groups examined. In total the results were lower by about 8.6% on average (p<0.001). The differences varied in the groups according to how trained the subjects were and according to the prevailing type of their physical activity. The highest differences between the two tests appeared in the group of cyclists (-12.0%), medical students (-10.9%) and in the group of subjects without a preferred type of physical activity. In these groups the differences were also statistically significant (p<0.01). They were less substantial in the group of runners (-4.6%, statistically insignificant), and in the group of triathletes there were practically no differences. This suggests that a CT 12 on a treadmill is much more susceptible to influences such as training experience, prevailing type of physical activity and motivation compared with an incremental maximum test on a BE. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Introduction: We studied influence of mud-bath on bone status in male Wistar rats with subchronic arthritis. Methods: Arthritis was induced by 2 subplantar injections of Freund’s adjuvans with heat-killed Streptoccocus pyogenes into paw. Groups: intact (int) on chippings; (con) arthritis on chippings; (san38) arthritis on hot sand; (mu38) arthritis on hot mud; (mu21) arthritis on mild mud. Bone mineral density (BMD, g/cm2) was measured by dual energy X-ray absorptiometry and femurs were tested biomechanically. Bone markers osteocalcin (OC), PINP and CTX were analysed in bone. Results: BMD of right femur decreased vs. left in san38 (p = 0.030) and mu38 (p = 0.047). Fracture load of right/left femur (N) decreased in experimental groups, significantly in san38 (p = 0.05). Fracture threshold of neck decreased in right vs. left in experimental groups, but significantly in san38 (p = 0.05). OC decreased in mu38 vs. con (1.84 ± 0.14/2.62 ± 0.23). PINP decreased in int vs. san38 (p = 0.005) and mu21 (p < 0.001). CTX decreased in int vs. mu38 (p = 0.006) and mu21 (p = 0.005). Conclusion: The hot bath appears indifferent in relation to osteoporosis, while cold mud-bath shows good effect on bone metabolism. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation. BACKGROUND: The presence of several risk factors (genetic and non-genetic) has greater impact on the risk of premature coronary artery disease (CAD) than single risk factor. OBJECTIVE: The aim of the study was to establish possible relations between genotypes and alleles of 677C>T polymorphism of MTHFR gene and some traditional risk factors e.g. elevated levels of lipid parameters and smoking in development of premature CAD. METHODS: The groups comprised 152 patients with angiographically documented premature CAD (aged 42.9 +/- 5.5) and 121 age-matched blood donors (aged 42.3 +/- 6.5) were studied. The MTHFR 677C>T polymorphism was genotyped with Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) method. RESULTS: Patients with TT genotype who simultaneously smoked had increased risk of premature CAD compared to non-smoking cases with CC genotype (OR = 24.62). We also found that individuals with TT genotype and elevated LDL-cholesterol (LDL-chol.) level had significantly higher risk of CAD (OR = 9.92) than individuals with normal LDL-chol. level and CC genotype. CONCLUSIONS: The present study shows that simultaneous presence of MTHFR TT genotype and smoking or elevated levels of LDL-chol. influences the risk of premature CAD. Cíl. Korelace ultrazvukového vyšetření polohy disku temporomandibulárního kloubu s vyšetřením na magnetické rezonanci. Metoda. Celkem bylo vyšetřeno 95 pacientů, 112 čelistních kloubů, soubor zahrnoval 10 mužů, 85 žen, věkové rozhraní 14-74 let, průměrný věk 37 let. Vyšetření na ultrazvuku probíhalo na přístroji Philips iu22 lineární 17 MHz sondou ve dvou fázích - klidové a dynamické - při otevírání úst. Na magnetické rezonanci byli pacienti vyšetření na přístroji Philips Achieva 1,5 T s povrchovou cívkou a mikroskopickou cívkou, každé vyšetření zahrnovalo sekvenci s postupným otevíráním úst. Výsledky. Souhlas mezi ultrazvukovým vyšetřením a magnetickou rezonancí při ventrální poloze byl 96%, z toho byla diagnostikována repozice disku v 94%, u pacientů bez repozice byla senzitivita nižší - 65%., Aim. The aim of this study was the correlation of an ultrasound examination of the disk of the temporomandibular joint with the magnetic rezonance examination. Method. In total has been examined 95 patients, 112 mandibular joints, file include 10 men, 85 women, age interface 14-74 years old, the average age of 37 years. The patients were examined using Philips iu22 with linear 17 MHz probe with dynamic phase - with opening the mouth. On the MRI, the patients were examined using Philips Achieva 1.5 T with the surface coil and microscopic coil, each examination included a sequence with the gradual opening of the mouth. Results. Článek pojednává o historii politiky podpory zdraví v ČR v období let 1990 až 2012. Cílem článku je 1) zachytit odlišné přístupy k politice podpory zdraví (a potažmo ke zdraví a determinantám zdraví) formulované vládami ve významných koncepčních materiálech a 2) na základě studia sekundárních dokumentů identifikovat významné události samotné implementace sledované politiky. Vládou vnímaná (resp. v koncepčních dokumentech deklarovaná) hodnota zdraví a konkrétní pojetí determinant zdraví, které, jak článek ukazuje, do značné míry souvisí s politickou orientací vlád, jsou nikoliv jedinými, ale významnými faktory následně realizované politiky podpory zdraví., The paper deals with the history of health promotion policy in the Czech Republic during 1990–2012. The aims of the paper are as follows: 1) to reveal different approaches to health promotion policy (and to health and health determinants), defined by Czech governments in significant conceptual documents and 2) based on the study of secondary documents to identify significant events of the implementation of the policy under consideration.
