Rastúci vplyv expozície environmentálnemu hluku a jeho vplyv na zdravie, patrí k významným problémom súčasnej doby. Týka sa to predovšetkým obyvateľstva, žijúceho v mestských aglomeráciách s vysokou hladinou environmentálneho hluku. Príčinou tohto nežiaduceho javu je najmä narastajúca doprava a priemysel. Hladiny hluku narastajú nielen v niektorých typických pracovných prevádzkach, ale obťažujú ľudí už aj pri rekreácii, oddychu, či spoločenských aktivitách. Tento negatívny faktor životného prostredia sa odlišuje od iných škodlivín tým, že sa jeho hladiny stále zvyšujú a pôsobí na človeka neustále, teda aj v čase určenom na relaxáciu ako napríklad pri spánku. Z verejno-zdravotníckeho hľadiska je významné, že rušivé pôsobenie environmentálneho hluku sa prejavuje neskôr rastúcim rizikom chronických ochorení, ktoré majú priamu súvislosť s poruchami spánku a iných funkcií vo vegetatívnej, endokrinnej a regulačnej sfére. Cieľom tejto prehľadovej práce je zosumarizovať aktuálne poznatky o škodlivých sluchových a nesluchových účinkoch environmentálneho hluku, ktoré bude možné využiť pri kvantifikácií rizika ako aj pri plánovaní preventívnych opatrení., The growing impact of environmental noise exposure on health is a major problems of our times. It particularly concerns the urban population living in areas with high levels of environmental noise. Increased levels of traffic and industrial activity are the major causative factors of this phenomenon. Increased noise levels cause interpersonal problems not only in occcupational settings, but during recreation, leisure, and social activities. This negative environmental factor differs from others in specifically affecting sleep and relentlessly increasing. In terms of public health, the disturbing effects of environmental noise manifest in increased risk of chronic diseases dircetly associated with sleep disorders and other vegetative, endocrine and regulatory functions. Ovarian cancer is the fifth most common malignancy in the world's female population and with the highest lethality index among gynecological tumors. The prognosis of metastatic disease is usually poor, especially in platinum-resistant cases. There are several options for the treatment of metastatic disease resistant to platinum derivates (e.g. paclitaxel, topotecan and pegylated liposomal doxorubicin), all of which are considered equipotent. Pegylated liposomal doxorubicin (PLD) is a liposomal form of the anthracycline antibiotic doxorubicin. It is characterized by more convenient pharmacokinetics and a different toxicity profile. Cardiotoxicity, the major adverse effect of conventional doxorubicin, is reduced in PLD as well as hematotoxicity, alopecia, nausea and vomiting. Skin toxicity and mucositis, however, emerge as serious issues since they represent dose and schedule-limiting toxicities. The pharmacokinetics of PLD (prolonged biological half-life and preferential distribution into tumor tissue) provide new possibilities to address these toxicity issues. The extracorporeal elimination of circulating liposomes after PLD saturation in the tumor tissue represents a novel and potent strategy to diminish drug toxicity. Extrakardiální patologie u CT srdce prováděného z jakékoliv indikace nalézáme u 10 až 60 % pacientů v závislosti na populaci, průměru zobrazované oblasti (FOV), podání kontrastní látky, dále na tom, zda vyšetření interpretuje radiolog nebo kardiolog a zda je v dané geografické oblasti praktikována defenzivní medicína. U vyšetření provedených se širokým FOV nalézáme oproti úzkému FOV extrakardiální patologii u dalších zhruba 10% pacientů. Přibližně 19-26% z těchto nálezů vyžaduje další vyšetření. Výhodou identifikace extrakardiálních nálezů je možnost stanovení alternativní diagnózy, která vysvětluje obtíže pacienta (např. pleuritida, hiátová hernie). Hlavní nevýhodou je nízká výtěžnost, finanční náklady a radiační zátěž při následných vyšetřeních. Na druhou stranu u značné části pacientů není i potenciálně významný nález podroben dalšímu vyšetření. Otázkou zůstává, zda riziko přehlédnutí extrakardiální patologie (riziko sporu) je větší než potenciální benefit pro pacienta - to je však společné téma i pro další vyšetření (např. CT páteře)., Non-cardiac pathology in CT imaging of the heart can be found in 10-60% of patients depending on the population, field of view (FOV), administration of contrast material, practicing of defensive medicine, and whether the examination is interpreted by a radiologist or a cardiologist. When large FOV is used, non-cardiac findings can be found in additional 10% of patients compared to small FOV. Around 19-26% of secondary findings require further workup. Identification of non-cardiac findings has its merits: alternative diagnosis that may explain patients complaints (eg. pleurisy, hiatal hernia). Its major drawbacks include low yield of clinically significant pathology, cost and radiation burden of subsequent examinations. On the other hand, in many patients even a potentially significant finding is not subject to further workup. The adhesive material used to bond orthodontic brackets to teeth should neither fail during the treatment period, resulting in treatment delays, untoward expenses or patient inconvenience nor should it damage the enamel on debonding at the end of the treatment. Although the effectiveness of a bonding system and any unfavorable effects on the enamel may be studied by conducting in-vivo studies, it is nearly impossible to independently analyze different variables that influence a specific bonding system in the oral environment. In-vitro studies, on the other hand, may utilize more standardized protocols for testing different bonding systems and materials available. Thus, the present review focused attention on in-vitro studies and made an attempt to discuss material-related, teeth-related (fluorotic vs non-fluorotic teeth) and other miscellaneous factors that influences the shear bond strength of orthodontic brackets. Within the limitations of this review, using conventional acid-etch technique, ceramic brackets and bonding to non-fluorotic teeth was reported to have a positive influence on the shear bond strength of orthodontic brackets, but higher shear bond strength found on using ceramic brackets can be dangerous for the enamel. and W. U pacientů, u kterých se manifestoval diabetes mellitus, dochází po měsících až desetiletích trvání choroby k vyhasnutí sekrece inzulinu, což je téměř jistým znamením úbytku B-buněk Langerhansových ostrůvků. U souboru 30 pacientů, u kterých se choroba manifestovala mezi 30–45 roky a byli diagnostikováni jako diabetici 2. typu, má po 30 letech trvání choroby polovina zachovanou nebo vyšší sekreci inzulinu, druhá polovina pak sekreci výrazně sníženou nebo vyhaslou. Faktory, které postihují B-buňky a vedou k jejich destrukci, můžeme shrnout do následujících skupin: 1. Faktory chemické: faktory metabolické: hyperglykemie a glukotoxicita, lipotoxicita, hypoxie, volné kyslíkové radikály, faktory farmakologické: anitimikrobiální prostředek pentamidin, antidepresiva typu SSRI, faktory spojené s poruchou sekrece inzulinu: MODY typy diabetu, toxické látky ze zevního prostředí: jed na krysy Vacor, streptozotocin, polychlorované či polybromované uhlovodíky 2. Onemocnění zevně sekretorické části pankreatu: nádorová infiltrace, vazivová infiltrace, chronická pankreatitida 3. Infekce, zánět a autoimunita: faktory virové: Coxsackie viry, virus chřipky H1N1, enteroviry, záněty: autoimunní faktory, představující patogenetický faktor diabetu 1. typu. V současné době pracujeme jak na další specifikaci dalších faktorů vedoucích k poškození B-buněk, tak na studiu poznání jejich účinku na buněčnou apoptózu respektive nekrózu, a konečně na definici ochranných faktorů, které by účinky působení těchto faktorů snížily. S nárůstem vědomostí o mechanizmech poškození a destrukce B-buněk se rýsují návrhy některých opatření, která by je mohla chránit. V našem přehledu podáváme zestručnělý a s ohledem na rozsah článku také notně zjednodušený přehled některých znalostí, které se poškození a destrukce B-buněk týkají. In our sample of 30 type 2 diabetic patients, whose disease manifested between 30 and 45 years of age, about a half have retained or even increased insulin secretion 30 years later, while the other half exhibit a much diminished or lost insulin secretion. Factors that can damage or destroy beta-cells can be divided into the following groups: Metabolic factors: hyperglycemia and glucotoxicity, lipotoxicity, hypoxia, reactive oxygen species; Pharmacological factors: antimicrobial medication pentamidine, SSRI antidepressants; Factors related to impaired insulin secretion: MODY type diabetes; Environmental toxic factors: rat poison Vacor, streptozotocin, polychlorinated and polybrominated hydrocarbons; Disorders of the exocrine pancreas: tumor infiltration, fibrous infiltration, chronic pancreatitis, cystic fibrosis; Infections, inflammation, autoimmunity, viral factors: Coxsackie viruses, H1N1 influenza, enteroviruses. We are currently working on finding other factors leading to beta-cell damage, studying their effect on apoptosis and necrosis and looking for possible protective factors to prevent this damage. Familial Mediterranean Fever is an autosomal recessive inherited disease with a course of autoinflammation, which is characterized by the episodes of fever and serositis. It affects the populations from Mediterranean basin. Genetic mutation of the disease is on MEFV gene located on short arm of Chromosome 16. The disease is diagnosed based on clinical evaluation. Amyloidosis is the most important complication. The only agent that decreases the development of amyloidosis and the frequency and severity of the episodes is colchicine, which has been used for about 40 years. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. Farmakogenetika je vedný odbor, ktorý skúma efekt jednotlivých liekov v závislosti od genotypu. V súčasnosti sú liečebné odporúčania pre liečbu niektorých monogenénových diabetov založené na genetickej diagnostike. Aj v oblasti farmakogenetiky perorálnych antidiabetík boli už publikované prvé štúdie, ktoré zistili asociácie jednotlivých génových variantov s liečebnou odozvou. Odozva na deriváty sulfonylurey bola signifikantne asociovaná s variantmi KCNJ11/ABCC8, TCF7L2 a CYP2C9. Odozva na liečbu metformínom bola asociovaná s variantmi génov ATM a SLC47A1. Odozva na liečbu glitazónmi bola asociovaná s variantom génu PPARG. Terapeutická odozva na liečbu gliptínmi bola asociovaná s variantmi génov TCF7L2 a CTRB1/2. Je možné očakávať, že v blízkej budúcnosti budú farmakogenetické poznatky využívané aj pri personalizácii liečby diabetu 2. typu., Pharmacogenetics is the study of how genes (individual genotypes) affect a person‘s response to drugs. At present, recommendations made about the treatment of some monogenic forms of diabetes are based on genetic diagnostics. The first studies in the field of pharmacogenetics of oral antidiabetics have now been published which have identified associations of individual genetic variants with response to treatment. The response to sulfonylurea derivatives was significantly associated with the variants KCNJ11/ABCC8, TCF7L2 and CYP2C9. The response to metformin treatment was associated with the genetic variants ATM and SLC47A1. The response to treatment with glitazones was associated with the genetic variant PPARG. The therapeutic response to the treatment with gliptins was associated with the genetic variants TCF7L2 and CTRB1/2. Období gravidity a laktace může být pro pacientky s revmatickým onemocněním rizikové. V graviditě dochází k zvýšené zátěži pohybového systému i vnitřních orgánů. U některých onemocnění ke zhoršení stavu vlivem těhotenství obvykle nedochází (např. revmatoidní artritida), u jiných onemocnění však dochází častěji k relapsům (např. systémový lupus erythematodes). Farmakoterapie léky užívanými v revmatologii je rovněž spojena s rizikem pro matku a plod. Stále častěji se uplatňuje biologická léčba, a to i u těhotných žen. V současnosti se uznává, že podávání těchto léků je u žen se závažným onemocněním, které nereagují na jiné imunomodulační léky, možné, protože nepříznivý vliv nedostatečně léčeného základního onemocnění je na zdraví matky a plodu rovněž významný. Pregnancy has a huge impact on the musculoskeletal system and internal organs. Certain rheumatic diseases (e.g. rheumatoid arthritis) are normally not affected negatively by pregnancy. Other diseases including systemic lupus erythematosus may aggravate the course of the disease, with a tendency for more frequent flares of the disease and increased complications in the pregnancy. Drugs used in rheumatology appear to be at higher risk for the mother and foetus. The use of biologic agents becomes more common in these patients including pregnant women. At present use of biological agents throughout pregnancy in patients with severe disease is reserved to those that do not respond to immunosuppressive antirheumatic treatment. Specifically, the use of biological agents is advocated in diseases not properly treated according to the maternal or foetal compromise expected. Fekálna mikrobiálna terapia (FMT) je liečebná metóda, pri ktorej sa prenesie fekálna mikroflóra chorému jedincovi od zdravého darcu, a tým sa obnoví normálne mikrobiálne zloženie čreva. V súčasnosti vzrastá záujem o využitie FMT pri liečbe rôznych chorôb, avšak neexistujú štandardné terapeutické protokoly. Postupy pri realizácii FMT sa líšia vo viacerých aspektoch, ako je výber darcu, príprava fekálnych materiálov, príprava príjemcu a spôsob aplikácie. FMT sa zdá byť najúspešnejšia v liečbe rekurentnej infekcie Clostridium difficile, randomizované kontrolované štúdie dokázali úspešnosť približne 90 %. Existujú aj obmedzené výsledky pre liečbu ulceróznej kolitídy, publikované boli malé série prípadov, ktorých výsledky však nie sú jednotné. Využitie FMT sa skúma aj v liečbe iných chorôb, pri ktorých je narušená črevná mikroflóra, ako sú napr. kardiovaskulárne, autoimunitné a metabolické ochorenia. Nateraz je pri FMT veľa nezodpovedaných otázok, a preto je nevyhnutný ďalší výskum v tejto oblasti., Fecal microbiota transplantation (FMT) is a therapeutic method, in which the fecal microflora from healthy donors is transmitted to the patient to restore the healthy microbial composition of the gut. In the recent years, there is a growing interest in the therapeutic potential of FMT in various diseases. The standard FMT protocols do not exist. Procedures of FMT vary in several aspects such as donor selection, preparation of fecal material, preparation of the recipient and administration way. FMT appears to be the most successful in the treatment of recurrent Clostridium difficile infection (CDI), randomized controlled studies reported 90 % success rate. There is a limited evidence for FMT as a treatment of ulcerative colitis. FMT has been also studied as treatment of diseases with impaired gut microbiota, such as cardiovascular, autoimmune and metabolic diseases.
