Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Ucelený přehled duodenálních nádorů se v současné literatuře prakticky nevyskytuje. S jednotlivými typy zhoubných nádorů se lze seznámit v rámci přehledu malignit tenkého střeva, popsanými soubory kazuistik či jednotlivých případů. Výjimkou jsou nádory ampulární oblasti, které jsou podrobně uvedeny v poslední WHO histologické klasifikaci nádorů trávicího ústrojí. Z domácích ani zahraničních literárních pramenů nelze vyhledat ani ucelený náhled na léčbu nádorů duodena. Obdobně je tomu i při vyhledávání chirurgických postupů. Resekční výkony na duodenu by proto měly být prováděny na pracovištích, které mají dostatečnou zkušenost s výkony v oblasti hepato-pankreato-biliární., No comprehensive knowledge of duodenal tumours exists in the current literature; individual types of malignant tumours may be described within malignancies of the small bowel, sets of case reports, or individual cases. Ampullary carcinomas are the exception and they are detailed in the current WHO histological classification of tumours of digestive system. Neither national nor international literature sources provide a comprehensive review of their therapy. The situation is similar when searching for surgical procedures. Závažný defekt koagulačného faktora XII (FXII) je veľmi zriedkavou, záhadnou a málo známou vrodenou poruchou. Na rozdiel od defektu ostatných koagulačných faktorov je úplne asymptomatický. Prekvapivo nevedie k abnormálnemu krvácaniu, dokonca ani pri rozsiahlejších chirurgických výkonoch. Vysvetlenie pre chýbanie krvácavých prejavov nie je známe. Predpokladá sa, ale nie je to dokázané, že pacienti nie sú dostatočne chránení pred trombózou. Defekt FXII sa obyčajne odhalí náhodou rutinným hemokoagulačným vyšetrením uskutočneným pred operáciou. Keďže FXII zohráva dôležitú úlohu pri tvorbe koagula in vitro, jeho defekt spôsobuje pri laboratórnom vyšetrení výrazné predĺženie aktivovaného parciálneho tromboplastínového času. Hlavným problémom spojeným s defektom FXII sú potom ďalšie zbytočné vyšetrenia, strata času pri poskytovaní zdravotnej starostlivosti a obavy z chirurgických výkonov, ktoré môžu vyplývať z abnormálneho laboratórneho výsledku. Unlike other coagulation factor deficiencies, it is totally asymptomatic. Surprisingly, it does not lead to abnormal bleeding, even with major surgical procedures. The explanation for the lack of bleeding manifestations is unknown. It is suggested, but unproven, that patients are not sufficiently protected from thrombosis. FXII deficiency is usually discovered by accident through a routine coagulation testing done prior to surgery. Since FXII plays an important role in clot formation during in vitro measurements, its deficiency causes a marked prolongation of the activated partial thromboplastin time in the laboratory examination. The main concern related to FXII deficiency is the unnecessary testing, delay in health care and worry of surgical interventions that may be prompted by the abnormal laboratory result. Typ studie: retrospektivní. Cíl: Cílem studie bylo zjistit úspěšnost porodů u žen s císařským řezem v anamnéze na našem pracovišti. Rovněž bylo cílem určit rizikové faktory pro úspěšnost porodu po předchozím císařském řezu a míru rizika, kterou tyto pacientky podstupují. Metodika: Výzkumný soubor tvořilo 1 060 porodů, které na našem pracovišti proběhly ve sledovaném období. Z toho 103 žen mělo císařský řez v anamnéze a 70 z nich se pokusilo porodit vaginálně. Výsledky: Z 1 060 porodů bylo 19,2 % sekcí, ze 70 TOLAC (trial of labour after caesarean – pokus o spontánní porod po císařském řezu) bylo 54,3 % úspěšných (porodily vaginálně), ze 13 indukcí po sekci bylo úspěšných 23,1 %. Jako prediktivní faktor pro úspěšný TOLAC byl prokázán vaginální porod v anamnéze (p = 0,00), BMI < 35 (p = 0,04) a trend naznačil věk rodičky < 25 let (p = 0,05). Byl prokázán signifikantní rozdíl v krevní ztrátě (měřené odhadem) mezi pokusem o vaginální porod po sekci a bez sekce v anamnéze (p = 0,000), pokusem o vaginální porod po sekci a primární sekcí po sekci (p = 0,000) a mezi úspěšným a neúspěšným pokusem o vaginální porod po sekci (p = 0,000). Byl rovněž prokázán signifikantní rozdíl Apgar(ové) skóre v 1. a 5. minutě u dětí z úspěšných a neúspěšných pokusů o vaginální porod po sekci. Tyto výsledky byly dány do kontextu s doporučenými postupy z USA, Kanady, Spojeného království, Německa, Rakouska a Austrálie a Nového Zélandu. Závěr: Studie poskytla informace, se kterými je možné seznámit pacientku plánující pokus o vaginální porod po sekci na našem pracovišti. We also wanted to determine the predictive factors for successful vaginal birth and the amount of risk which the vaginal birth after caesarean brings to the patient. Metodics: There were 1,060 births in our study, that took place in our clinic at the specified time. Out of this set there were 103 women, who had caesarean delivery in medical history and 70 of those tried to deliver vaginally. Results: Out of 1,060 births there were 19.2 % caesarean sections, from 70 patients who tried to give birth vaginally after caesaren (TOLAC – trial of labour after caesarean) 54.3 % succeeded (the rest gave birth per sectionem caesaream), out of 13 induced labours after caesarean there were only 23.1 % successful. We determined following predictive factors for successful TOLAC: at least one vaginal birth in medical history (p = 0.00), BMI under 35 (p = 0.04) at the time of birth and age under 25 (p = 0.05). We found significant difference in the blood loss between following groups: TOLAC vs deliveries without caesarean in medical history (p = 0.000), TOLAC vs. elective repeat sections (p = 0.000) and successful vs. unsuccessful TOLAC (p = 0.000). We also found significant difference in Apgar score in first and fifth minute between the group of successful TOLAC and unsucessful TOLAC. These results were compared with the current guidelines in UK, USA, Canada, Australia and New Zealand, Germany and Austria. Conclusion: This study has brought us data, that should be provided to the patients who think about giving birth in our clinic and who have already caesarean section in medical history. Tranzitorní ischemická ataka (TIA) a minor stroke patří mezi akutní cévní onemocnění mozku, jež sama o sobě nemají ve většině případů invalidizující dopad na pacienta. Jejich podcenění však může vést k recidivě cévní mozkové příhody (CMP) s fatálními následky. Cílem tohoto přehledového článku je upozornit na hlavní problémy současné klinické praxe v České republice a navrhnout racionální algoritmus péče o pacienty s TIA/minor stroke. Článek podává informace o nové definici TIA, která se opírá o fakt, že až u 50 % pacientů po TIA je na magnetické rezonanci (MR) přítomen infarkt, o stratifikaci rizika a jejím využití při managementu pacientů, o významu zobrazovacích vyšetření a o možnostech terapie. Poukazuje na skutečnost, že stratifikace rizika není v České republice dostatečně používána, což vede k neadekvátnímu poměru mezi rizikem pacienta a zvoleným diagnosticko-terapeutickým postupem. Urgentní péči potřebují obzvláště pacienti s vysokým rizikem recidivy CMP – tj. pacienti s poruchou řeči nebo motoriky trvající ≥ 5 min, se skóre ABCD2 ≥ 4, s fibrilací síní a pacienti s infarktem na difuzně vážených sekvencích (DWI) MR či CT (počítačové tomografii) a s okluzí nebo významnou ≥ 50% stenózou extra-/intrakraniální velké mozkové tepny. Přehledový článek zdůrazňuje nutnost nejen okamžitého zahájení medikamentózní terapie, ale i provedení časné endarterektomie a shrnuje nové možnosti léčby, např. duální antiagregace, jejíž přínos byl v časné fázi po TIA a minor stroke nedávno dokumentován., Transient ischemic attack (TIA) and minor stroke are acute cerebrovascular diseases that are usually non-disabling. However, if not followed adequately, they may lead to potentially fatal stroke recurrence. The aim of this review is to summarize the main challenges of the current clinical practice (in the Czech Republic) and suggest a rational algorithm for TIA/minor stroke management. This review informs about the new TIA definition based on the fact that up to 50% of TIA patients have a brain infarction on MRI (magnetic resonance imaging), about risk stratification and its use in patients management, about the importance of imaging studies and about the available treatment options. Risk stratification for TIA is underused in the Czech Republic and this affects risk-benefit ratios for the available management plans. In particular, follow-up of high-risk patients need urgent attention - patients with speech and/or motor symptoms lasting ≥ 5 min, with ABCD2 score ≥ 4, with atrial fibrillation, and patients with an infarction on DWI MRI or CT (computed tomography), and with an occlusion or significant ≥ 50% stenosis of a precerebral/cerebral large vessel. The review highlights the importance of an immediate medical intervention with early endarterectomy and provides an overview of new therapeutic options, e.g. dual antiplatelet therapy, the benefit of which has recently been documented for the first time. Magnetoencephalography (MEG) is the recording of the magnetic field produced by the flowing of ions in the brain. This article reports our experience in the application of MEG in patients and healthy volunteers in the Greek population. We provide a brief description of our research work. The MEG data were recorded in a magnetically shielded room with a whole-head 122 channel or an one-channel biomagnetometer. Our results lead us to believe that the MEG is an important research field which is evolving quickly with a number of interesting findings with respect to normal and abnormal functions of the human brain. It could provide clinical practice with an easy to perform non invasive method, which could be adjunct to conventional methods for the evaluation of brain disorders. and P. Anninos, A. Diabetická dyslipidemie je komplexní změnou metabolizmu lipidů, které musíme věnovat při péči o diabetika velkou pozornost. Samotný diabetes mellitus 2. typu je onemocnění spojené s mnoha makrovaskulárními a mikrovaskulárními komplikacemi a od počátku onemocnění je nutné s ohledem na možnost rozvoje těchto komplikací o pacienta pečovat komplexně. Prevence kardiovaskulárních komplikací je jedním z nejdůležitějších faktorů při péči o diabetika, protože kardiovaskulární příhody jsou mezi hlavními příčinami morbidity a mortality pacientů s diabetem. Riziko vzniku aterosklerotických komplikací je u diabetiků výrazně vyšší než u nediabetické populace, což více než z poloviny podmiňuje rozdílný metabolizmus plazmatických lipoproteinů a rozvoj typické diabetické dyslipidemie s vysoce aterogenním potenciálem. Léčba diabetické dyslipidemie je založená na dodržování režimových opatření, která mají pozitivní efekt i na diabetes obecně, prakticky vždy doplněných farmakoterapií vhodně zvolenou k dosažení cílových hodnot plazmatických lipoproteinů a ovlivnění jejich kvality., Diabetic dyslipidemia is a complex disorder associated with changes in lipid metabolism, which must be considered during care of the diabetic patient. Type 2 diabetes mellitus is a disease associated with a number of macrovascular and microvascular complications and it is necessary to treat the patient with regard to the possibility of developing these disorders. Prevention of cardiovascular complications is one of the most important factors of diabetes care, because cardiovascular diseases are among the leading causes of morbidity and mortality in patients with diabetes mellitus. The risk of developing atherosclerotic changes is significantly higher in diabetics in comparison with people without diabetes, which is mostly due to the differences in the plasma lipid metabolism and development of typical diabetic dyslipidemia with high atherogenic potential. Pacienti s chirurgicky neřešitelnými metastázami kolorektálního karcinomu v játrech mají při současné systémové chemoterapii odhad přežití 1 a 2 roky 55 % a 33 %. Radiologická intervenční léčba prováděná pod kontrolou zobrazovacích metod jako je chemoembolizace (TACE), intraarteriální chemoterapie (HAI) a radiofrekvenční termoablace (RFA) má proto u těchto nemocných rostoucí význam. Využití endovaskulární a perkutánní cílené léčby jaterních metastáz má dvojí efekt. Dojde ke kontrole růstu metastáz a k redukci vedlejších účinků systémové léčby. Vývoj nových embolizačních materiálů jako DC beads (DEB), na které lze navázat různá cytostatika jako doxorubicin či irinotecan, pak umožňuje stejně jako u radiofrekvenční termoablace standardizovat protokoly a omezit rozdílné výsledky této léčby mezi pracovišti., Unresectable colorectal liver metastases have a 1- and 2-year survival of 55 % and 33 % with current systemic therapies. Currently image- -guided liver-directed therapy like trans-arterial chemoembolization (TACE), hepatic arterial infusion (HAI) and termoablation (RFA) has a growing role in the therapy of patients with liver metastases. Endovascular and percutaneous hepatic-directed therapy offers the dual benefit of true local neoplastic control and reduction of sideeffects. Biguanidy jsou léky určené pro terapii hyperglykemie u diabetu 2. typu. V poslední době se ukazuje, že nejčastěji předepisovaný biguanid metformin vykazuje také protinádorový účinek. Jeho možné využití v protinádorové léčbě podporují epidemiologické, preklinické a klinické studie. Příznivé ovlivnění karcinomu prsu a tlustého střeva by mohlo souviset s primární schopností metforminu snižovat hladinu cirkulujícího inzulinu, ale metformin má nepochybně i přímý cytostatický efekt, který se uskutečňuje prostřednictvím inhibice mTOR (mammalian target of rapamycin) signální dráhy a blokádou syntézy proteinů. Využití metforminu v protinádorové léčbě nebo chemoprevenci nádorů ale ukáže teprve budoucnost., Metformin, an oral biguanide antidiabetic drug, is the most widely prescribed agent for treatment of type 2 diabetes. The drug mainly works by lowering glucose production by the liver, and thus lowering fasting blood glucose. Metformin acts as an insulin sensitizer, which leads to inhibition of gluconeogenesis in the liver, and has been found to be beneficial especially in overweight patients with diabetes. Metformin use has been postulated to contribute to a reduced risk of pancreatic, prostate, ovarian, and breast cancers. Metformin‘ s mechanism of action was examined recently and determined to be initiated by metformins role as an adenosine 5‘-mono-phosphate– activated protein kinase (AMPK) activator through a tumor suppressor protein kinase known as LKB-1, which regulates AMPK levels. Inzulin byl při léčbě diabetu v těhotenství vždy považován za lék první volby. Metformin a další perorální antidiabetika naopak patřily mezi léčiva, která podávána nebyla, a to z obavy možného poškození plodu. Poslední studie však ukazují, že bezpečnost metforminu je ve srovnání s inzulinem naprosto srovnatelná a že užívání metforminu dokonce může přinášet jistá pozitiva. On the contrary, metformin and other oral hypoglycemic agents have not been used because of concerns about possible harm caused to the fetus. However, the latest studies prove that the safety of using metformin during pregnancy is equal to insulin therapy and it might even offer certain positives.
