Adolescent hallux valgus (HV) is a progressive deformity of adolescent age consisting of metatarsus primus varus and hallux valgus. It has a high recurrence rate after conventional surgical correction. Ten feet in nine patients (two males, seven females) were treated surgically with the Peterson Newman bunion procedure, with a minimum follow-up of one year. During the final follow-up all these patients had no complaints of pain, joint stiffness or limping. Even though the patients had some mild loss of range of movements at the MTP joints 4–6° compared to preoperative value, it did not cause any functional impairment and all were satisfied with the final outcome. The double ostetomy for treatment of hallux valgus is technically precise procedure, provides excellent correction and stability and has low rate of recurrence of deformity. Karcinom jícnu je onemocnění s 90% mortalitou, ale část pacientů s lokálně pokročilým onemocněním může dlouhodobě přežívat díky multidisciplinární léčbě. Dokladem je tato kazuistika 40letého muže s dlaždicovým karcinomem středního jícnu prorůstajícím do dýchacích cest ve stadiu T4N1M0. Prvním léčebným zákrokem bylo zavedení Dumon Y-stentu do dýchacích cest jako urgentní zábrana sufokace. V dalším postupu byla použita konkomitantní chemoradioterapie, s dávkou záření 66Gy/33 frakcí/47 dní normofrakcionací. Chemoterapie v kombinaci cisplatina a fluorouracil byla podávána v celkové délce 6 cyklů, s ozářením během 2. a 3. cyklu. Na CT po léčbě byla zjištěna částečná regrese nádoru, endoskopické biopsie z jícnu i průdušek byly negativní. V prvním roce po chemoradioterapii bylo nutno řešit těsnou stenózu jícnu s příznaky až úplné afagie, což se podařilo zvládnout dilatacemi. V dalším období stav nemocného komplikovaly endobronchiální granulace řešené lokálním ošetřením laserem a jejich mechanickým snesením. Opakované biopsie ukazovaly na lokální progresi tumoru endobronchiálně, takže byla indikována paliativní chemoterapie docetaxelem, která byla podávána po dobu 1 roku. Během tohoto období opakované endobronchiální biopsie neprokázaly karcinom, CT hrudníku se normalizovalo, stav pacienta se zlepšil. Posléze byl odstraněn Y-stent 25 měsíců po zavedení a ukončena chemoterapie. Při kontrole 3 roky po diagnóze byl pacient bez známek nádoru., Esophageal cancer is characterized by 90 % mortality. Nevertheless, the part of the patients suffering from locally advanced disease can survive for a long time thanks to the multidisciplinar treatment. This fact can be documenetd by this case report of 40 year old man with squamous cell carcinoma of medial oesophagus intergrowing to respiratory tract, stage T4N1M0. At the start of the treatment Dumon Y stent was introduced into airways to avoid suffocation. Then, the concurrent chemoradiotherapy was used, with the radiation dose of 66Gy/33 fractions/47 days. Chemotherapy using combinationof cisplatin and fluorouracil was administered in the total length of 6 cycles, with the radiation applied during the second and third cycle. After the treatment, CT revealed the partial regression of the tumor, while endoscopic biopsies from oesophagus and bronchial tubes being negative for malignancy. During the first year after the chemoradiotherapy the tight stenosis of the oesophagus with the total aphagia was to be solved using the repeated dilatations. During the following period, the illness course was complicated by endobronchial granulations, treated by the local laser treatment and mechanical ablation, as well. Repeated biopsies shown the local endobronchial progression of the tumor, so that the paliative chemotherapy with the docetaxel was applicated, lasting one-year. During this period, repeated endobronchial biopsies did not proved a malignancy, CT finding was normalized and the patient´s status was improved. Finally, the Y stent was removed 25 months after insertion, and the chemotherapy was ended. Autoři popisují případ polékového halucinatorního syndromu u 42leté pacientky s karcinomem prsu po infuzi paklitaxelu v rámci adjuvantní chemoterapie dle protokolu AC-T. Kazuistika poukazuje na potencionální riziko paklitaxelu ve smyslu psychotropního nežádoucího účinku a nutnost mezioborové spolupráce, konkrétně pak s psychiatrem. Časná symptomatická psychofarmakologická léčba umožnila bezproblémové dokončení adjuvantní chemoterapie., The authors report on a case of drug-induced hallucinatory syndrome after paclitaxel infusion in a 42-year-old woman with breast carcinoma treated with adjuvant chemotherapy AC-T. This case report highlights the potential risk of paclitaxel causing hallucinatory syndrome. At the same time it indicates the need to cooperate with the psychiatrist. Úvod: Měření podílu tělesného tuku je důležitým ukazatelem výživového a zdravotního stavu člověka. Na zvýšeném obsahu tělesného tuku se podílejí vedle faktorů genetických a hormonálních především výživa a pohybová aktivita. Existují různé metody na stanovení množství tělesného tuku, jejichž výsledky se mohou navzájem výrazně lišit. Cíl práce: Cílem této studie bylo srovnání výsledků různých metod zaměřených na zjištění množství a distribuce tělesného tuku. Metodika: Zkoumaný soubor tvořilo 86 žen (21–67 let). Byly měřeny parametry: tělesná hmotnost, výška, obvod pasu v polovině vzdálenosti posledního žebra a crista illiaca, vypočítán body mass index (BMI). Tělesné složení bylo zjišťováno pomocí kaliperace kožních řas metodami podle Pařízkové a podle Matiegky, bioelektrické impedance (BIA), duální rentgenové absorpciometrie (DXA). U 27 žen bylo provedeno stanovení abdominálního tuku pomocí CT ve výši L4–5. Výsledky: Jednotlivé námi použité metodiky navzájem velmi dobře korelují. Nejlepší korelace byla zjištěna mezi metodou DXA a BIA (r = 0,929) a metodou DXA a podle Pařízkové (r = 0,925), horší korelace byla mezi metodou DXA a podle Matiegky (r = 0,870), a to jak pro váhové, tak pro procentuální vyjádření množství tuku. V absolutním množství tuku se jednotlivé metody v rámci celého souboru navzájem výrazně liší (p < 0,001). Metody DXA a BIA dávaly srovnatelné výsledky ve skupině obézních žen (p = 0,684). BMI koreloval velmi dobře s metodami DXA, BIA, metodami podle Pařízkové, Matiegky a s CT břicha (r = 0,920–0,812). Obvod pasu dobře koreluje s množstvím intraabdominálního tuku stanoveným pomocí CT (r = 0,737)., Introduction: Body composition is an important indicator of male nutrition and health state and depends on a number of factors. Nutrition and physical activity are primary determinants of body composition aside from heredity and hormonal aspects. We can use different methods for assessment of body composition. If the various methods are used the results can differ widely. Objective: The aim of this study was to compare selected methods for measurement of body fat and its distribution. Methods: Body composition was assessed in a group of 57 overweight adults (21–67 years old). The following parameters were measured: body height, body weight, circumference of waist (between the 12th rib and crista illiaca), weight/height ratio (BMI). These methods were used for determination of body composition: skin fold thickness measurement by Parizkova, skin fold thickness measurement by Matiegka, bioelectrical impedance (BIA), dual-energy X-ray absorptiometry (DXA). Abdominal computer tomography (CT) at L4–L5 was used for total abdominal fat assessment in a group of 27 women. Results: The results of these methods correlate very well. The best correlation was found with DXA and BIA methods (r = 0.929) and DXA and Parizkova method (r = 0.925). Worse correlation was found with DXA and Matiegka method (r = 0.870) for the total amount of body fat and body fat percentage. There were significant differences in body fat amount assessed by DXA and other methods (p < 0.001). The results for DXA and BIA methods were similar in the group of obese women (p = 0.684). BMI correlated with DXA, BIA, Parizkova, Matiegka and abdominal CT very well (r = 0.920–0.812). Cíl: Cílem studie bylo vyhledat poznatky týkající se NANDA International ošetřovatelské diagnózy Porucha polykání (00103) se zaměřením na výskyt této ošetřovatelské diagnózy u různých skupin pacientů a její stanovení prostřednictvím znaků uvedených v klasifikačním systému dle NANDA International. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system. Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Background: To evaluate the clinical usefulness of serum levels of soluble form of endoglin in stage III colorectal adenocarcinomas (CRC) patients for detection of recurrence. Methods: The case-control study consisted of 80 stage III CRC patients who underwent surgery with curative intent and 70 age-and sex-matched healthy volunteers. Serum levels of soluble form of endoglin (sol-end) were measured in both groups. Also, predictive factors of recurrence were evaluated using multivariate analyses. Results: Serum levels of sol-end in stage III CRC patients were significantly higher than those in controls. There was not a significant association between serum levels of sol-end and clinicopathological features in CRC patients. Multivariate regression analysis showed the LN R (hazard ratio, 2.54; 95% CI , 1.46–4.34; p < 0.001), to be significant independent factors to estimate local recurrence in stage III CRC patients. Toxoplasmóza je jedna z nejčastějších zoonóz u nás. Patří mezi onemocnění, která mohou při primoinfekci v graviditě ohrozit vývoj plodu či způsobit mnohočetná poškození, často manifestovaná až po narození dítěte. Závažnost postižení, jeho variabilita a pravděpodobnost vzniku závisí na době, kdy se těhotná žena nakazila. V ČR máme k dispozici několik diagnostických metod. Základem je sérologické vyšetření (protilátky celkové i jednotlivých tříd, avidita IgG, komparativní imunoblot), prováděné z krve či plodové vody. Možný je i přímý průkaz DNA Toxoplasma gondii pomocí PCR z krve, plodové vody či po porodu z placenty. Prenatálně se provádí izolační pokus na myši (inokulací plodové vody). Pomocnou metodou je také ultrazvuk či MRI v průběhu těhotenství, se zaměřením na případnou fetopatii. Po narození je dítě z rizikové gravdity podrobně vyšetřeno a dlouhodobě sledováno. Terapii lze podat prenatálně i postnatálně, o jejím efektu se vedou diskuze. Základními preparáty jsou spiramycin, pyrimethamin a sulfadiazin (doplněny kyselinou folinovou). V článku jsou probrány indikace k jednotlivým vyšetřením a doporučeny nejvhodnější postupy v diagnostice, léčbě a také prevenci tohoto onemocnění u těhotných. Text se podrobně věnuje i interpretaci možných výsledků a obsahuje vyjádření k preventivnímu vyšetření těhotných žen u nás., Toxoplasmosis is one of the most common zoonoses in the Czech Republic. Toxoplasma infection during pregnancy can affect fetal development and cause multiple damages that often do not manifest until after the birth. The severity of pediatric disability, its variability and the probability of occurrence depend on the time of maternal infection. Serological diagnosis of toxoplasmosis in pregnant women is performed in two steps: first, total anti-Toxoplasma antibodies are detected, most commonly by the complement fixation test (CFT) combined with IgG ELISA. In the case of a negative result, the woman can be retested 1-3 months later. Low CFT titers show latent infection with a minimal risk of primary infection during pregnancy. When CFT titers of 1:64 or higher are obtained, the second step follows. Positivity in the IgM and IgA tests and low IgG avidity can confirm a recent infection. The diagnosis of fetal infection is based on the detection of anti-Toxoplasma antibodies, and of IgM antibodies in particular, and/or on the detection of Toxoplasma in amniotic fluid by PCR or the mouse test. Other helpful methods are fetal ultrasound or MRI focusing on fetopathy. Each child born from a high-risk pregnancy is thoroughly examined and followed up on a long-term basis. The detection of neonatal anti-Toxoplasma IgM antibodies and a neonatal IgG profile differing from the maternal IgG profile in comparative Western blot are the major markers of congenital infection. Toxoplasma infection can be treated prenatally and postnatally and the therapeutic outcomes are still being considered. The first-line options for congenital toxoplasmosis are spiramycin, pyrimethamine and sulfadiazine with folinic acid. The article discusses the indications for screening and recommends best practices for the diagnosis, treatment and prevention of toxoplasmosis during pregnancy.
