Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. The risk of high-grade lesions and carcinoma is known to correlate with specific high-risk HPV genotypes. The distribution of HPV types varies between countries and little is known about HPV distribution in our country. Therefore, the purpose of this study was to determine the range and frequency of HPV genotypes in studied group of women in Montenegro. HPV genotypes were determined using the method of enzyme restriction of PCR products amplified with group-specific primers MY09/MY11 and restricted with seven different restriction endonucleases. Out of the total number of women HPV infection was found in 1/5 of participants (20%). Genotyping performed in HPV DNA positive women shows that the HPV genotype 16 is dominant and present in more than 1/3 of the participants (36.8%). The second most frequent HPV infection is with HPV genotype 58 and it is found in 10.5% of participants. HPV 31 and HPV 6 infections are present in 7.9% of women, while infections with other genotypes were demonstrated individually by 2.6%. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. The study has shown that the gentlest method of surgical treatment is drilling by HoYag laser, which is least likely to injure the tissue of ovaries, and also this method is most effective in enhancing and preserving ovarian reserve., Vasiliy Simrok, Daria Simrok-Starcheva, and Literatura
Cíl: Sémontův repoziční manévr se v praxi užívá k léčbě pacientů s benigním paroxyzmálním polohovým vertigem zadního polokruhového kanálku. Cílem studie bylo zhodnotit interindividuální a intraindividuální variabilitu při provádění tohoto manévru a definovat parametry, které jeho provedení nejvíce ovlivňují. Metody: Tři zkušení terapeuti provedli manévr oboustranně u 10 zdravých jedinců. Inerciální měřicí jednotka složená z akcelerometru a gyroskopu získávala informace o trajektorii pohybu hlavy v prostoru a její rychlosti. Úhlové změny byly zaznamenány v souřadnicovém referenčním systému. Z odchylek od uvažované roviny polokruhového kanálku byla vyhodnocena data o přesnosti provedení manévru. Výsledky: Parametry ovlivňujícími přesnost manévru jsou výška probanda (p = 0,0252), fáze pohybu (p < 0,0001) a dále terapeut a strana pohybu, přičemž tyto faktory vykazují vliv především ve své interakci. Byl hodnocen vztah fáze pohybu k výšce probandů (p = 0,0130), terapeuta k fázi pohybu (p = 0,0001), terapeuta k výšce probandů (p < 0,0252). Největší zaznamenaný rozsah odchylky od senzorické roviny byl v rozmezí –37,17° až 31° se standardní odchylkou 16,6°. Závěry: Získaná data poukazují na překvapivě vysokou variabilitu při provádění Sémontova manévru ať v porovnání mezi jednotlivými měřeními u jednoho terapeuta nebo v porovnání mezi terapeuty. Analýza vlivu těchto odchylek na úspěšnost terapie benigního polohového paroxyzmálního vertiga a bližší analýza ovlivňujících parametrů by měly být předmětem dalšího výzkumu., Aim: Sémont liberatory manoeuvre is used in the treatment of posterior semicircular canal in patients with benign paroxysmal positional vertigo. The aim of the study was to determine the inter-individual and intra-individual variability in the processing of the manoeuvre and to define the parameters that affect it the most. Methods: Three experienced therapists applied the manoeuvre bilaterally in 10 healthy probands. Inertial measurement unit consisting of the accelerometer and gyroscope recorded the trajectory and speed. Angle changes were placed in the coordinate reference system. Accuracy of the manoeuvre was evaluated based on the deviations from the projected sensory plane. Results: Parameters affecting the accuracy of the manoeuvre included height of a proband (p = 0.0252), phase of the movement (p < 0.0001), therapist and the side of the movement. The effect of these factors is the most pronounced when combined. We assessed interactions of the phase of the movement and the height of the probands (p = 0.0130), the therapist and the phase of movement (p = 0.0001), the therapist and the height of the probands (p < 0.0252). The largest magnitude of deviation from the sensory plane was in the range of –37.17° to 31° with a standard deviation of 16.6°. Conclusions: The data highlight high variability in the implementation of the Sémont liberatory manoeuvre, whether measurements by a single therapist or inter-individually between the therapists are compared. Analysis of the impact of these deviations on therapeutic efficacy in patients with benign paroxysmal positional vertigo and detailed analysis of influencing parameters should be the subject of further research. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients.
