POEMS syndrom je vzácnou klinickou jednotkou s velmi různorodým chováním a klinickou manifestací. Patří do skupiny onemocnění známých jako plazmocelulární dyskrazie a je pro něj typický současný výskyt monoklonální gamapatie spolu s periferní neuropatií a ložiskovým osteosklerotickým postižením skeletu. Naše sdělení ukazuje na třech kazuistických případech odlišnou manifestaci této klinické jednotky společně s posouzením možných diagnostických a léčebných přístupů k jedincům s POEMS syndromem. Cílem je jednak přiblížení tohoto méně známého onemocnění ze skupiny monoklonálních gamapatií a také zejména praktický přístup k diagnostice jedinců se současným výskytem paraproteinu a periferní neuropatie., POEMS syndrome is a rare clinical entity with a very variable behaviour and clinical manifestation. It belongs to a group of diseases known as plasma cell dyscrasias and is characterised by a coincidence of monoclonal gammopathy with peripheral neuropathy and focal osteosclerotic bone lesions. Using three case reports, our paper presents different manifestations of this clinical entity as well as an assessment of possible diagnostic and therapeutic approaches to individuals with POEMS syndrome. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. It is easier to prevent a disease than to cure it. This postulate is a foundation stone of the contemporary medicine, furthermore its mission. The Chronic Kidney Diseases (CKD), amongst them the Chronic Pyelonephrites (CP) and the mass kidney reduction take an important place in human pathologies in general, and in particular in renal ones. The Chronic Pyelonephrites are chronic renal pathologies, which on one side are of various causes and on the other side are multi systemic. At the same time they tend, earlier or later, depending on their course, to bring the patient towards the Chronic Kidney Insufficiency in stage of uremia, consequently in need of substitution therapies e.g. dialysis, peritoneum dialysis or transplant. It is worthy to emphasize that from the prevention and correct cure of CP make profit the patients, the family, the state and in the last analyses the entire society, because in that way the budget expense destined for the fore going substitution cures, dialysis, peritoneum dialysis or transplant, is considerably reduced. The same should be mentioned in relation to the CP and the mass kidney reduction, speaking about our country, which are still at the first place as the very cause of Chronic Kidney Insufficiencies (CRI), later on advancing toward uremia and terminal uremia along with its grave consequences. Cíl studie: Posouzení významu celotělové magnetické rezonance (WB-MR) v odlišení monoklonální gamapatie nejistého významu (MGUS) od iniciální/asymptomatické formy mnohočetného myelomu (MM), ve zpřesnění stadia MM včetně podchycení extramedulární progrese, v diagnostice solitárního plazmocytomu (SP) a zhodnocení praktického přínosu stážovacího systému Durie-Salmon Plus (D-S Plus). Název a sídlo pracoviště: III. interní klinika, LF UP a FN Olomouc. Materiál a metody: Analyzovaný 87členný soubor sestával z 28 jedinců s MGUS, z 54 nemocných s MM vyšetřených při rozpoznání nemoci a z 5 jedinců se solitární formou plazmocytomu. WB-MR byla prováděna na přístroji Magnetom Avanti 1,5 T pomocí virtuální celotělové maticové cívky s postupnou akvizicí v 7 etážích a v sekvencích T2 STIR a T1. Na základě počtu ložisek a charakteru difuzních změn bylo určeno stadium nemoci podle klasifikace D-S Plus a srovnáno s výsledky standardního stratifikačního systému podle D-S. Statistické hodnocení bylo provedeno pomocí koeficientu Cohenovo kappa a McNemarova-Bowkerova testu (p < 0,05). Výsledky: U 17 z 28 jedinců s MGUS (61 %) byla pomocí IMWG (International Myeloma Working Group) a/nebo WB-MR splněna kritéria transformace v MM. U 4 ze17 jedinců (23 %) bylo zaznamenáno při použití stratifikačního systému D-S Plus vyšší stadium než podle systému D-S založeném na konvenční radiografii skeletu (KRS). Devět ze 14 jedinců (64 %) s MGUS v transformaci s negativitou KRS mělo pozitivní obraz při použití WB-MR. Charakter změn zjištěných při WB-MR vedl u 9 ze 17 nemocných s MM (53 %) k zahájení indukční chemoimunoterapie. Stratifikace podle D-S Plus rozčlenila 54 doposud neléčených nemocných s MM do stadií 1–3 (17 %, 33 % a 50 %), přičemž u 22 % došlo při srovnání s D-S standardním systémem k posunu do vyššího stadia choroby, u 9 % naopak k přeřazení do stadia nižšího. U 13 % nemocných s MM bylo pomocí WB-MR rozpoznáno extramedulární, především paraspinální, šíření myelomového procesu. Z 5 nemocných s diagnózou SP byla u 2 rozpoznána pomocí WB-MR multifokální forma plazmocytomu. Závěr: WB-MR je vysoce přínosná zobrazovací metoda s podstatně vyšší citlivostí než KRS, neboť umožňuje citlivé zhodnocení tíže a rozprostření projevů myelomové kostní nemoci, jejího extraoseálního šíření a rozpoznání skutečně solitární formy plazmocytomu. Významně obohacuje standardní diagnostická kritéria používaná v odlišení stabilní formy MGUS od fáze její maligní transformace do MM. Stratifikační režim podle D-S Plus osvědčil praktický potenciál k začlenění do standardního algoritmu diagnostických a stratifikačních metod u MGUS a MM. Klíčová slova: celotělová magnetická rezonance (WB-MR), monoklonální gamapatie nejistého původu (MGUS), mnohočetný myelom, solitární plazmocytom, stážovací systém Durie-Salmon Plus (D-S Plus)., Objective: The aim of the study was to assess the significance of the whole-body magnetic resonance imaging (WB-MR) in resolution of monoclonal gammopathy of undetermined significance (MGUS) and initial, asymptomatic form of multiple myeloma (MM), in specification of MM stage, including the detection of extramedullary progression, as well as the diagnostics of solitary plasmocytoma (SP) and the evaluation of practical usefulness of the Durie-Salmon Plus staging system (D-S Plus). Settings: 3rd Department of Internal Medicine, University Hospital, Medical Faculty of Palacky University, Olomouc. Material and Methods: The analyzed 87 patient cohort consisted of 28 MGUS individuals, 54 patients with newly diagnosed MM and 5 patients with SP. WB-MR was evaluated using Magnetom Avanto 1,5 T with the use of virtual whole body coil with sequential acquisition on 7 levels and 2 sequentions – T2 STIR and T1. Based on the number of lesions and the pattern of diffuse changes there was assessed the D-S Plus stage and compared with the results of the standard D-S stratification system. Statistical estimation was done using the Cohen kappa and McNemara-Bowker test at p < 0.05. Results: In the group of 28 MGUS individuals, there were 17 (61%) patients fulfilling with the use of IMWG (International Myeloma Working Group) and/or WB-MR, criteria of transformation into the MM. In 4/17 (23%) individuals, there was described more advanced stage when comparing the stratification system D-S Plus to D-S system, based on the conventional radiography of the skeleton (CRS). Nine out of 14 (64%) individuals with transforming MGUS with negative radiological assessment had positive findings on WB-MR. The character of WB-MR findings led in 9/17 (53%) of the patients with MM to the initiation of the induction treatment. Stratification according to D-S Plus divided the 54 newly diagnosed patients with MM into stages 1-3 (17%, 33% and 50%). In 22% there was a shift into a higher stage using D-S Plus in comparison with D-S, in 9% of the patients the shift led to downstaging. In 13% of the MM patients there was described extramedullary, especially paraspinal propagation of the tumour. In 2 of the 5 SP patients there was recognized multifocal form using WB-MR.Conclusions: WB-MR is a very contributive imaging method with substantially higher resolution than conventional radiography, because it is able to evaluate the grade and the extent of myeloma bone disease, extramedullary propagation as well as the true recognition of solitary form of plasmocytoma. It improves the standard diagnostic criteria using in differentiation of stable MGUS from the phase of malignant transformation into MM. The D-S Plus system proved practical potential for incorporation into the standard diagnostic and stratification algorithms in MGUS and MM. Bylo provedeno anonymní dotazníkové šetření zaměřené na zjištění přístupu ke zdraví u 484 učitelů různých stupňů škol. Soubor byl hodnocen na základě prevalence ovlivnitelných rizikových faktorů životního stylu a z účasti učitelů na preventivních prohlídkách. Ve srovnání s průměrnou českou populací vykazují učitelé lepší tělesnou hmotnost, jsou méně obézní (pouze 6,6 %), méně kouří (7 % pravidelných a 5,3 % příležitostných kuřáků), stravují se pravidelně (76,5 %) a mají více pohybové aktivity (medián – 5 hodin tělesné aktivity týdně v létě a 3 hodiny týdně v zimě). Učitelé mají také dobrou účast na preventivních prohlídkách – k praktickému lékaři pro dospělé chodí pravidelně 77,5 % učitelů, k praktickému zubnímu lékaři 92,1 % a k praktickému gynekologovi 90,6 % dotázaných žen. Poměrně riziková je pouze skupina vysokoškolských učitelů, kteří vykazují relativně vysokou prevalenci rizikových faktorů a nejnižší účast na preventivních prohlídkách. Důvodem však je pravděpodobně vyšší zastoupení učitelů do 35 let a vyšší počet mužů v této skupině ve srovnání s ostatními typy škol., This anonymous questionnaire survey was carried out in order to clarify the approach of teachers to their own health. A group of 484 teachers teaching on various school levels was studied. Their approach to personal health was assessed on the grounds of prevalence of certain lifestyle factors, namely preventable ones, and of their participation in preventive check-ups. As compared to the rest of Czech population teachers have a better body mass index, are less obese (in 6.6% only), smoke less frequently (7% of regular and 5.3% of occasional smokers), eat more regularly (76.5%), and engage in move physical activity (median of 5 hours per week during summer and 3 hours per week in winter). Teachers also have a better extent of participation in preventive check-ups. Regular check-ups by general practitioners for adults are absolved by 77.5% of teachers, visits to dentists by 92.1% and check-ups at gynaecologists by 90.6% of women teachers. Nevertheless, a partially higher risk was detected amongst university lecturers who had a relatively higher prevalence of risk factors and also a lower frequency of regular preventive check-ups. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. Cíl. Pseudomyxom peritonea je velmi vzácné semimaligní onemocnění. Záměrem našeho sdělení je pojednání o typickém nálezu, možnostech léčby a komplikacích. Metoda. Nedávné případy z našeho pracoviště ilustrují patognomické znaky nemoci v CT obrazu, obvyklý klinický nález, průběh onemocnění i svízelnost jeho léčby. Ve dvou našich kazuistikách je příčinou pseudomyxomu peritonea mucinózní cystadenom appendixu, v jednom appendikální low grade cystadenokarcinom, v posledním případě mucinózní cystadenokarcinom neznámého původu. U prvního pacienta se navíc objevila raritní komplikace - metastatická ložiska intrathorakálně. Závěr. Pseudomyxom peritonea je klinický syndrom, který je způsoben excesivní produkcí gelatinózního ascitu buňkami mucinózního cystadenomu či cystadenokarcinomu různého původu, nejčastěji se však jedná o tumory appendixu. Typický morfologický nález v zobrazovacích metodách určuje jejich významné místo v diagnostice, stagingu a monitorování výsledků léčby pseudomyxomu., Aim. Pseudomyxoma peritonei is an uncommon semimalignous disease. The aim of our statement is the discussion about the typical finding, the possibilities of treatment and complications. Method. The recent findings from our workplace illustrate pathological signs of the disease on CT-scan, usual clinical finding, the progression of the disease and the arduous of its treatment. In two of our cases is the cause of pseudomyxoma peritonei mucinous cystadenoma of the appendix, in one low grade mucinous cystadenocarcinoma and in the last case the mucinuos cystadenoma of unknown origin. There has appeared by the first patient a rare complication - intrathoracic implants lesions. Conclusion. Pseudomyxoma peritonei is a clinical syndrome that is caused by the excessive production of gelatinous ascites cells mucinous cystadenoma or cystadeno-karcinoma of different origin, but mostly these are the cases of appendix tumors. Karcinom prostaty je jedním z nejčastějších nádorových onemocněním mužů. S rostoucím počtem diagnostikovaných karcinomů prostaty roste i počet nutných operací. Radikální prostatektomie u lokalizovaného karcinomu prostaty je metodou volby. Je to jediný léčebný postup, jehož přínos byl potvrzen randomizovanou prospektivní studií. Pokud je provedena zkušeným operatérem, má excelentní onkologické i funkční výsledky a minimum komplikací. V současné době nelze jasně upřednostnit jeden operační přístup před jiným. Role radikální prostatektomie a pánevní lymfadenektomie u pokročilých onemocnění se stále hledá., Prostate cancer is one of the most common malignancies in men. The number of surgeries required increases with the increasing numbers of prostate cancers diagnosed. Radical prostatectomy is the method of choice for localized prostate cancer. It is the only therapeutic approach the benefit of which was confirmed in a randomized prospective trial. When performed by an experienced surgeon, it results in excellent oncological as well as functional outcomes and minimal complications. Currently, there is no clear evidence of the superiority of one surgical approach over another.
