Autoimunitní forma pankreatitidy patří mezi specifickou formu onemocnění slinivky břišní s významným podílem autoimunitní složky v etiopatogenezi. V současné době existují 2 formy onemocnění, které jsou definované klinicky i histomorfologicky a jsou laboratorně popsány. Přes řadu společných rysů histomorfologických se obě formy zásadně odlišují přítomností tzv. endoteliálních granulárních lézí (GEL) u typu 2, resp. absencí GEL u typu 1. Abundantní přítomnost gamaglobulinu a imunoglobulinu G4 charakterizuje 1. formu nemoci, podobně jako častá přítomnost IgG4 pozitivních extrapankreatických lézí. Subtyp 2 bývá typicky spojen s nálezem idiopatických střevních zánětů, především ulcerózní kolitidy. Oba typy autoimunitní pankreatitidy se vyznačují rychlou odpovědí na terapii steroidy. Vzhledem k různým diagnostickým kritériím autoimunitní formy pankreatitidy byl v roce 2011 zveřejněn diagnostický konsenzus sjednocující stávající kritéria a vycházející z klinických symptomů, biochemických nálezů, nálezů při použití zobrazovacích metod, histomorfologie žlázy a pozitivní odezvy na podání steroidů. Stálým problémem zůstává včasná diferenciální diagnostika mezi fokální formou autoimunitní pankreatitidy a karcinomem hlavy pankreatu. MRI/CT, MRCP a v Japonsku ERCP, s cílenou biopsií žlázy pod EUS kontrolou, jsou doporučeny jako metody volby., The autoimmune type of pancreatitis represents the specific disease of pancreas, with significant contribution of autoimmune processes in its etiopathogenesis. Currently, there are two proved subtypes of this particular pancreatopathy, which are defined clinically, histomorphologically and serologically. They have many histomorphological signs in common, but differ in the presence of so-called granulocytic epithelial lesions (GEL), which are absent in subtype 1. The subtype 1 is characterized by the presence of gammaglobulines, esp. immunoglobuline G4 and IgG4 positive extrapancreatic lesions. The subtype 2 is typically associated with the inflammatory bowel diseases, esp. ulcerative colitis. But the common characteristic of both subtypes is the fact response to applied steroid treatment. Due to diverse diagnostic criteria in the past, in 2011 the consensus for the diagnosis of autoimmune pancreatitis was announced. It is based on clinical symptoms, biochemical results, the results got by using of imaging methods, histomorphology and positive response to steroid treatment. The matter to be solved is the question of early differential diagnosis between focal autoimmune pancreatitis and adenocarcinoma of pancreatic head. Gastrointestinální stromální tumory (GIST) jako nejčastější mezenchymální tumory trávicího traktu vycházejí z Cajalových intersticiálních buněk. Ke konfirmaci morfologické diagnózy jsme použili panel imunohistochemických protilátek CD117, desmin, S100 a DOG1. Při hodnocení biologických vlastností jsme vycházeli z Miettinenovy klasifikace pro GIST, rozdělující tumory do tří skupin podle rizika agresivního chování. Klasifikace tumoru sleduje tři parametry: lokalizaci, velikost a mitotický index. Mitotický index je definován jako počet mitóz v nádorové tkáni na celkové ploše 5 mm2. Provedli jsme srovnání metod odběru tenkojehlové, navigované aspirační cytologie – EUS FNA a klešťové endoskopické biopsie či resekátu nádorové tkáně z pohledu patologa se zaměřením na stanovení všech potřebných morfologických a prognostických parametrů této neoplázie. Naše závěry a zkušenosti ukazují, že výsledky cytologického vyšetření jsou pro diagnostiku GIST srovnatelné s bioptickým vyšetřením. Hlavní limitací cytologické metody je nemožnost v 75 % případů stanovit mitotický index z důvodů malého množství nádorové tkáně. Tento nedostatek lze nahradit vyšetřením nukleárního proteinu asociovaného s buněčnou proliferací Ki-67, který se dle literárních zdrojů ukazuje jako významný prognostický parametr progrese onemocnění., Gastrointestinal stromal tumors (GIST) belong to the most common mesenchymal tumors of digestive tract deriving from Cajal interstitial cells. A panel of immunohistochemical antibodies CD117, desmin, S100 and DOG1 is used to confirm the morphological diagnosis. When evaluating the biological characteristics we used a Miettinen classification for GIST which divides tumors into three groups according to a risk of aggressive behaviour. This classification observes three parameters – tumor localisation, its size and mitotic index. Mitotic index is defined as a number of mitoses in the tumor tissue on the overall area of 5 mm2. We performed a comparison of sampling methods – biopsy forceps, resection versus endoscopic ultrasound-guided fine-needle aspiration cytology (EUS FNA) from the point of view of a pathologist, focusing on the assessment of all necessary morphological and prognostic parameters of neoplasia. The study conclusions and experience show that when diagnosing GISTs, results of cytological examination of a sample using EUS FNA are comparable to those using forceps endoscopic biopsy. The main limitation of EUS FNA method is that in 75 % of cases it is impossible to state the mitotic index due to insufficient amount of tumor tissue. Gastrointestinální stromální nádory (GISTy) jsou mezenchymální nádory gastrointestinálního traktu (GI), pocházející z intestinálních Cajalových buněk převážně ze žaludku a tenkého střeva. V posledním desetiletí došlo k rozvoji cílené molekulární léčby mnoha solidních nádorů a s objevem onkogenních mutací kináz u GISTů došlo k rychlému pochopení molekulárně biologické povahy těchto nádorů a přispělo k zavedení inhibitorů tyrozinkináz do klinické praxe i u tohoto typu nádorů. Proto v současné době hraje významnou úlohu molekulární genotypizace těchto lézí, především z hlediska prognózy, léčebné odpovědi, respektive primární nebo sekundární rezistence na cílenou léčbu., Gastrointestinal stromal tumors (GISTs) are mesenchymal tumors of the gastrointestinal tract (GI) arising from the interstitial cells of Cajal, primarily of the stomach and small intestine. During the past decade came up to progress of molecular targeted therapies for many solid tumors and in association with discovery of onkogenic kinase mutations in GISTs to rapid evolution in understanding of molecular biology features of these tumors and to implementation tyrosine kinase inhibitors in clinical practice. Úvod: Tři jednonukleotidové polymorfismy (SNP) v NOD2/CARD15 genu (3020insC, R702W, G908R) jsou pro Crohnovu chorobu významným rizikovým faktorem. Jejich diagnostické a terapeutické využití v klinické praxi je však doposud limitované. Cílem studie byla analýza možné asociace těchto polymorfismů s rizikem reoperací a posouzenísouvislosti s fenotypem onemocnění. Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Anamnestická a klinická data, včetně chirurgické léčby byly retrospektivně získány ze zdravotnické dokumentace a prospektivně doplňovány podle protokolu studie. Analyzovány byly souvislosti mezi vybranými genovými polymorfismy, nutností reoperací a fenotypem nemoci. Výsledky: U 24 pacientů (32 %) se našla alespoň jedna sledovaná varianta v NOD2/CARD15 genu. 25 pacientů (33 %) bylo reoperováno, 51 (67 %) představovalo kontrolní skupinu. Trend vyšší četnosti reoperací ve skupině s genovou mutací nedosáhl statistické významnosti (p=0,2688). Dva ze čtyř homozygotů (50 %) nesoucích variantu 3020insC byli opakovaně operováni. Neprokázali jsme signifikantní rozdíly mezi soubory s wild formou a mutovanou formou NOD2/8CARD15 genu s ohledem na věk v době diagnózy (p=0,4356), formu onemocnění (p=0,6610) a lokalizaci (p=0,4747) podle Montrealské klasifikace. Závěr: Polymorfismy v NOD2/CARD genu signifikantně neovlivnily četnost reoperací, homozygoti nesoucí mutaci 3020insC genu však představovali vysoce rizikovou skupinu. Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. Cholangiokarcinom (CC) je vzácný maligní nádor vznikající z cholangiocytů. Prognóza CC bývá většinou nepříznivá. Zásadní podíl na této skutečnosti má pozdní diagnóza nádoru. Současná incidence CC v České republice činí asi 1,4 na 100 000 obyvatel za rok; u méně než 30 % pacientů s CC lze identifikovat některý ze známých rizikových faktorů, nejčastěji primární sklerozující cholangiitidu. Naději na delší dobu přežití mají pacienti s časně diagnostikovaným, chirurgicky řešitelným CC, u kterých dosahuje 5letá doba přežití asi 20–40 %. Pro časnou diagnózu CC mají zásadní význam zobrazovací metody a histopatologické zhodnocení, zatímco časně diagnostický význam onkomarkerů je omezený. Racionální časná diagnostika CC spočívá v účelném využití diferencovaných výhod jednotlivých zobrazovacích metod – optimální se jeví MRI s DSA, EUS je citlivou metodou pro identifikaci malignit v oblasti jaterního hilu nebo distálního choledochu, zatímco MRCP (magnetic resonance cholangiopancreatography) spíše při zobrazení patologických změn biliárního stromu, ERCP (endoscopic retrograde cholangiopancreatography) umožňuje odběr materiálu pro histopatologické vyšetření. Přínosem jsou i novější diagnostické metody, např. IDUS – intraduktální ultrasonografie biliárních cest anebo SPY-GLASS, umožňující vyšetřit žlučové cesty přímým pohledem s možností odběru cíleného bioptického vzorku. Specificitu i senzitivitu histologického a cytologického vyšetření lze zvýšit využitím molekulárně cytogenetické metody FISH, tj. fluorescenční in situ hybridizací, u níž specificita dosahuje 97 %., Cholangiocarcinoma (CC) is a rare malignant tumour arising from cholangiocytes, and its prognosis is usually unfavourable, mostly as a result of late diagnosis of the tumour. The current incidence of cholangiocarcinoma in the Czech Republic is 1.4/100,000 inhabitants per year; in less than 30 % of patients with CC, one of the known risk factors can be identified, most frequently, primary sclerosing cholangitis. Only patients with early diagnosed and surgically amenable cholangiocarcinoma are likely to have a longer survival time; in their case, survival for more than five years has been achieved in 20 % to 40 %. From the perspective of the need for early diagnosis of CC, a significant part is played by imaging and histopathologic evaluation; the early diagnostic significance of oncomarkers is limited. The rational early diagnosis of CC consists in effective use of differentiated advantages of different imaging modalities – MRI with DSA appears to be the optimal method, endosonography is a sensitive method for the identification of malignancy in the hepatic hilum or distal common bile duct, MRCP (magnetic resonance cholangiopancreatography) is used to display pathological changes in the biliary tree, ERCP (endoscopic retrograde cholangiopancreatography) allows material removal for histopathological examination. Other new approaches are also beneficial, such as IDUS – intraductal ultrasonography of biliary tract or SPY-GLASS, enabling examination of the bile ducts by direct view with the possibility of taking targeted biopsies.
