Úvod: Pacienti se známkami krvácení do zažívacího traktu jsou z organizačních důvodů hospitalizováni na jednotkách intenzivní péče chirurgických pracovišť. V této skupině pacientů roste podíl těch, kteří užívají různé formy antitrombotické terapie. Od nástupu endoskopických metod hemostázy je chirurgická léčba těchto nemocných spíše výjimkou a mortalita velmi nízká. Jejich léčba je nákladná personálně, organizačně i finančně a otázkou je, zda by nebylo možné nárůst počtu těchto pacientů zastavit, nebo dokonce snížit. Metody: Retrospektivní analýza pacientů hospitalizovaných pro akutní krvácení do zažívacího traktu na našem pracovišti se zaměřením na užívání různé formy antitrombotické terapie jako rizikového faktoru. Výsledky: Během 24 měsíců bylo na našem pracovišti hospitalizováno celkem 251 pacientů s akutním krvácením do zažívacího traktu. 107 z nich užívalo antiagregační, antikoagulační nebo kombinovanou terapii. Ani jeden z pacientů neužíval žádnou formu gastroprotekce. Závěr: Pacientů akutně krvácejících do zažívacího traktu je mnoho a jejich počet roste zejména ve skupině nemocných užívajících různé formy antitrombotické terapie. Navzdory všeobecně platným guidelines nejsou tito pacienti mnohdy vybaveni žádnou formou gastroprotekce. Dodržením standardů by mohlo být možné tento nepříznivý trend pozitivně ovlivnit. Klíčová slova: krvácení do GIT − antikoagulační/antiagregační terapie, Introduction: Patients with signs of acute bleeding into the gastrointestinal tract are hospitalized at intensive care units of surgical departments for organizational reasons. There is a growing proportion of those treated with various forms of antithrombotic therapy. From the onset of endoscopic methods of haemostasis, mortality is low and surgical treatment of these patients is rarely required. Their treatment takes up a lot of human and financial resources. The question remains, whether it would be possible to stop the volume growth or even reduce the number of these patients. Methods: A retrospective analysis of patients hospitalized for acute gastrointestinal bleeding at our department focused on the use of various forms of antithrombotic therapy as a risk factor. Results: A total of 251 patients were hospitalized with acute bleeding into the digestive tract during 24 months at our department. 107 of these were receiving antiplatelet, anticoagulant or combination therapy. None of the patients used any form of gastroprotection. Conclusion: The number of patients acutely bleeding into the digestive tract is high and rising particularly within the group using various forms of antithrombotic therapy. Despite the existence of accepted guidelines, these patients are often not equipped with any form of gastroprotection. Úvod: Perkutánní punkční cholecystostomie je považována za výkon v nouzi při selhávání konzervativní léčby těžké akutní cholecystitidy u starých a kriticky nemocných pacientů. Otázkou je, nakolik je v současnosti toto řešení aktuální či obsoletní. Metody: Retrospektivně byla analyzována data pacientů, kteří podstoupili v období 1/2010−1/2015 punkční cholecystostomii pod kontrolou počítačové tomografie (CT). Sledována byla data pacientů, úspěšnost a komplikace výkonu, krátkodobé a dlouhodobé výsledky. Výsledky: Punkční cholecystostomie pod CT navigací byla na Chirurgické klinice 2. LF UK a FNM za sledované období provedena u 30 pacientů. V souboru bylo 21 žen (70 %), 9 mužů (30 %), průměrný věk 78 let (SD?12,3), medián 82 let (34–93 let). Indikací výkonu byla těžká akutní cholecystitida, respektive empyém žlučníku nereagující na konzervativní léčbu u rizikových pacientů aktuálně neúnosných pro operační výkon. Primárně byla punkce úspěšná u 23 pacientů (77 %), selhala u 7 pacientů (23 %) s nutností opakované punkce u jedné pacientky a urgentní cholecystektomie u 6 nemocných. Průměrná doba hospitalizace byla 16,5 dne (SD?8,2), medián 15 dnů (7–49 dnů). Celková 30 denní mortalita byla 17 %, mortalita v souvislosti s primární indikací k výkonu byla 10 %. Recidivu onemocnění jsme zachytili u 3 pacientů (10 %). U jednoho byla řešena opakovanou drenáží, u druhého konzervativní terapií a u třetího akutní cholecystektomií. U jednoho pacienta (3 %) byla provedena odložená laparoskopická cholecystektomie bez komplikací. Závěr: Perkutánní cholecystostomie pod CT kontrolou u pacientů neúnosných k operaci představuje efektivní a bezpečnou terapeutickou možnost., Introduction: Percutaneous cholecystostomy is considered to be an emergency treatment option when conservative treatment of acute cholecystitis fails in elderly and critically ill patients. The question is: to what extent is this technique still up-to-date or obsolete. Methods: We retrospectively reviewed data of patients who underwent a computer tomography (CT) guided percutaneous cholecystostomy between 1/2010−1/2015. We analyzed the patient data, the success rate, complications of the procedure, short- and long-term outcomes. Results: 30 patients undergoing CT-guided percutaneous cholecystostomy at the Department of Surgery, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital during the study period were enrolled. The study group included 21 females (70%) and 9 males (30%) with mean age of 78 years (SD?12.3), median 82 years (range 34−93 years). Percutaneous cholecystostomy was indicated for patients with severe cholecystitis/empyema of the gallbladder not responding to conservative therapy who were poor candidates for operative cholecystectomy. Of these, 23 patients (77%) were successfully treated with initial percutaneous cholecystostomy whereas 7 patients (23%) experienced treatment failure – one was subsequently successfully treated with repeated percutaneous cholecystostomy and six underwent emergency cholecystectomy. The mean length of stay was 16.5 days (SD ?8.2), median 15 days (7–49 days). The total 30-day mortality was 17%, and indication-related mortality was 10%. Three patients (10%) had a recurrence. One patient required repeated percutaneous drainage, the second recovered on conservative treatment and the third patient underwent acute cholecystectomy. Only one patient (3%) underwent delayed laparoscopic cholecystectomy without complications. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Úvod: Dekubity u paraplegiků představují náročný problém vedoucí k opakovaným hospitalizacím, mnohonásobným operacím a potenciálně devastujícím komplikacím Metody: Přehledový článek. Výsledky: Uvádíme možnosti rekonstrukčních operací ischiadických, sakrálních a trochanterických dekubitů a vlastní zkušenost s lalokovými plastikami takových dekubitů u pacientů operovaných v průběhu tří let. Hlavními sledovanými ukazateli kvality po resekci rekta pro karcinom jsou operační radikalita, pooperační komplikace, počet recidiv a přežití. Poruchy funkce rekta, které postihují 25–50 % nemocných, zůstávají v pozadí. Tyto poruchy zahrnují vysokou frekvenci stolice, urgenci, evakuační poruchy, inkontinenci a sexuální a urologické dysfunkce. Funkční poruchy po resekčních výkonech na rektu jsou v anglofonní literatuře označovány akronymem „LARS“ (low anterior resection syndrome). V článku jsou v přehledu rozebrány změny anorektální fyziologie po nízké resekci rekta, etiopatogeneze a rizikové faktory LARS, způsoby konstrukce neorektálního rezervoáru, možnosti prevence a terapie LARS. Samostatně jsou v přehledu rozebrány sexuální a močové dysfunkce., The most important quality parameters of rectal cancer surgery are oncological radicality, postoperative complications, recurrence rate and survival. Rectal dysfunction following low anterior resection occurs in up to 25–50% of patients. Despite its high frequency, however, it does not receive enough attention. Rectal dysfunction after surgery includes frequent bowel movements, urgency, incomplete evacuation, incontinence or sexual and urinary dysfunctions. The complex of symptoms is collectively referred to as the low anterior resection syndrome – LARS. In this review, we discuss the alterations in anorectal physiology after low anterior resection, the etiology and risk factors of LARS, different types of neorectal reservoir construction, and various options for prevention and treatment of LARS. Úvod: Vysoká incidence kolorektálního karcinomu představuje v České republice aktuální a populačně závažný zdravotnický problém. Polovina všech nemocných je věku nad 70 let. Možnosti chirurgické i nechirurgické léčby v této skupině závisejí na faktorech, jen minimálně vyjádřitelných zavedenou onkologickou a anesteziologickou klasifikací (cTcNcM, ASA). Cílem práce je ukázat vliv věku na využití léčebných modalit do protokolu léčby a na její výsledky a na jiné možnosti hodnocení únosnosti k léčbě. Metody: Analýza údajů z databáze NOR z pětiletého období 2008–2012 zpracované Institutem biostatistiky a analýz MU. Výsledky: Ve všech sledovaných parametrech byl prokázán rozdíl mezi nemocnými pod a nad 70 let v neprospěch starších. Ve věku pod 70 let nebylo léčeno 11,2 % nemocných, ve věku na 70 let 25,2 %. Závěr: K rozhodování o způsobu vedení léčby včetně volby operace by mohla přispět klasifikace pomocí komplexního geriatrického vyšetření., Introduction: High incidence of colorectal cancer in the Czech Republic is an actual and demographically significant health issue. Half of all of the patients is older than 70 years. Both surgical and non-surgical treatment options in this group of patients depend on factors that are difficult to measure only by current oncological and anesthesiological classifications (cTcNcM, ASA). The objective of this paper is to measure the impact of age on the use of various treatment modalities within the protocol and their results, and also to suggest alternative options for therapy tolerance assessment. Methods: Analysis of data over a five-year period from the NOR database prepared by the Institute of Biostatistics and Analyses, Masaryk University. Results: In all parameters a difference was demonstrated between patients below the age of 70 and those above the age of 70 years. Older patients were disadvantaged. Only 11.2% of patients younger than 70 years were not treated, whereas 25.2% over the age of 70 years were not treated. Úvod: Tři jednonukleotidové polymorfismy (SNP) v NOD2/CARD15 genu (3020insC, R702W, G908R) jsou pro Crohnovu chorobu významným rizikovým faktorem. Jejich diagnostické a terapeutické využití v klinické praxi je však doposud limitované. Cílem studie byla analýza možné asociace těchto polymorfismů s rizikem reoperací a posouzenísouvislosti s fenotypem onemocnění. Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Anamnestická a klinická data, včetně chirurgické léčby byly retrospektivně získány ze zdravotnické dokumentace a prospektivně doplňovány podle protokolu studie. Analyzovány byly souvislosti mezi vybranými genovými polymorfismy, nutností reoperací a fenotypem nemoci. Výsledky: U 24 pacientů (32 %) se našla alespoň jedna sledovaná varianta v NOD2/CARD15 genu. 25 pacientů (33 %) bylo reoperováno, 51 (67 %) představovalo kontrolní skupinu. Trend vyšší četnosti reoperací ve skupině s genovou mutací nedosáhl statistické významnosti (p=0,2688). Dva ze čtyř homozygotů (50 %) nesoucích variantu 3020insC byli opakovaně operováni. Neprokázali jsme signifikantní rozdíly mezi soubory s wild formou a mutovanou formou NOD2/8CARD15 genu s ohledem na věk v době diagnózy (p=0,4356), formu onemocnění (p=0,6610) a lokalizaci (p=0,4747) podle Montrealské klasifikace. Závěr: Polymorfismy v NOD2/CARD genu signifikantně neovlivnily četnost reoperací, homozygoti nesoucí mutaci 3020insC genu však představovali vysoce rizikovou skupinu. Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. Úvod: Rozsah nádoru a kvalita jeho chirurgického odstranění patří k hlavním prognostickým faktorům v léčbě karcinomu rekta. Počet pooperačních lokálních recidiv závisí na úplnosti odstranění mezorekta – TME (total mesorectal excision) – a dosažení bezpečných resekčních okrajů bez nádoru. Provedení a kvalita operace záleží na předoperační diagnostice a detekci rizikových míst na rektu a mezorektu, na chirurgické schopnosti výkon správně provést a na patologickém vyšetření, hodnotícím úplnost odstranění nádoru v parametrech resekčních okrajů včetně cirkumferentního (CRO) a kompletnost mezorektální excize. Cílem práce bylo zavést a standardizovat metodu hodnocení kvality chirurgického výkonu − mezorektální excize – v léčbě karcinomu rekta hodnocením resekčních okrajů včetně CRO a kompletnosti excize. Metody: Studie měla dvě části. V první, retrospektivní části multicentrické studie byly v sestavě 288 pacientů analyzovány jednotlivé dílčí parametry diagnostiky, operací a histologického vyšetření karcinomu rekta, identifikována kritická místa a připraven jednotný protokol sledování. Ve druhé, prospektivní části studie byla v sestavě 600 pacientů tato data sledována parametricky a byla hodnocena kvalita TME ve vztahu k onkologickému výsledku léčby. Výsledky: V předoperační diagnostice se zvýšil podíl pacientů, kterým byl proveden restaging po neoadjuvantní léčbě z 60,0 % na 81,7 %. Podíl resekátů, u nichž nebyla hodnocena kvalita excize mezorekta, se snížil z 52,9 % v retrospektivní části na 22,8 % v části prospektivní. Podíl kompletních excizí mezorekta se zvýšil z 22,6 % na 26,0 %, podíl téměř kompletních excizí se zvýšil z 10,1 % v retrospektivní části na 24,0 % v prospektivní části studie. Závěr: Zavedení parametrického sledování do rutinní praxe zvýšilo kvalitu předléčebné a předoperační diagnostiky, vyšetření resekátu patologem a vlivem zpětnovazebné informace také kvalitu chirurgického výkonu., Introduction: Tumour size and the quality of its complete surgical removal are the main prognostic factors in rectal cancer treatment. The number of postoperative local recurrences depends on whether the mesorectum has been completely removed – total mesorectal excision (TME) – and whether tumour-free resection margins have been achieved. The surgery itself and its quality depend on the accuracy of preoperative diagnosis and detection of risk areas in the rectum and mesorectum, on the surgeon’s skills, and finally on pathological assessment evaluating whether complete tumour excision has been accomplished including circumferential margins of the tumour, and whether mesorectal excision is complete. The aim of our study was to implement and standardize a new method of evaluation of the quality of the surgical procedure – TME – in rectal cancer treatment using an assessment of its circumferential margins (CRO) and completeness of the excision. Methods: The study consisted of two parts. The first, multi-centre retrospective phase with 288 patients analysed individual partial parameters of the diagnosis, operations and histological examinations of the rectal cancer. Critical points were identified and a unified follow-up protocol was prepared. In the second, prospective part of this study 600 patients were monitored parametrically focusing on the quality of the TME and its effect on the oncological treatment results. Results: The proportion of patients with restaging following neoadjuvant therapy increased from 60.0% to 81.7% based on preoperative diagnosis. The number of specimens missing an assessment of the mesorectal excision quality decreased from 52.9% in the retrospective part of to the study to 22.8% in the prospective part. The proportion of actually complete TMEs rose from 22.6% to 26.0%, and that of nearly complete TMEs from 10.1% to 24.0%. Conclusion: The introduction of parametric monitoring into routine clinical practice improved the quality of pre-treatment and preoperative diagnosis, examination of the tissue specimen, and consequently improved quality of the surgical procedure was achieved., and J. Perineální hernie je kýla prostupující defektem v pánevním dnu. Primární hernie jsou extrémně vzácné, častěji se jedná o onemocnění sekundární po operacích v pánvi, zejména po abdominoperineálních resekcích či exenteracích pánve. Jako nejvýznamnější rizikové faktory pro vznik onemocnění jsou uváděny poruchy hojení perineální rány a neoadjuvantní chemoradioterapie u nemocných s maligním onemocněním. Lze očekávat nárůst výskytu tohoto onemocnění v souvislosti s využíváním laparoskopických přístupů a ELAPE. I přes značný odstup od první popsané operace chybějí soubory pacientů, studie i doporučené postupy řešení. Autoři prezentují kazuistiku 66letého nemocného se sekundární perineální hernií po předchozí laparoskopické abdominoperineální resekci rekta pro adenokarcinom, u něhož byla provedena plastika kýly z abdominálního přístupu a k uzávěru defektu bylo využito stopkovaného laloku m. rectus abdominis., Perineal hernia is defined as a protrusion of intra-abdominal viscera through a defect in the pelvic floor. Primary hernias are extremely rare; secondary (postoperative) hernias following pelvic surgery, especially abdominoperineal resection or pelvic exenteration, are more common. Impaired perineal wound healing and neoadjuvant chemoradiotherapy in cancer patients are defined as main risk factors. A growing incidence of this complication is expected in connection with an increasing use of laparoscopic approaches and ELAPE. Despite the considerable time since the first described secondary perineal hernia in literature, patient series, prospective studies and treatment guidelines are still missing. The authors present a case of a 66-year-old man with secondary perineal hernia following a previous laparoscopic abdominoperineal resection for rectal cancer undergoing a surgical intervention. Časné pooperační komplikace hojení anastomózy představují pro pacienta významné riziko zvýšení morbidity, mortality i zhoršení dlouhodobých výsledků. Existuje řada opatření redukujících rizika vzniku těchto komplikací včetně peroperačního hodnocení kvality kolorektální anastomózy. Postupy hodnocení spolehlivosti anastomózy se vyvíjely od mechanického testování přes endoskopickou vizualizaci střevního lumen až po moderní techniky hodnotící mikroperfuzi perianastomotických tkání. Mechanické testování kolorektální anastomózy a částečně i endoskopické techniky prokazatelně do jisté míry redukují riziko pooperačních komplikací hojení kolorektální anastomózy. Moderní postupy hodnotící mikrocirkulaci, založené na fluorescenční angiografii pomocí indocyaninové zeleně, mají potenciál zásadně zlepšit posouzení bezpečnosti kolorektální anastomózy a významně přispět k rozhodnutí o dalším postupu včetně vynechání protektivní ileostomie., Early postoperative anastomotic complications have serious clinical implications for the patient’s morbidity, mortality as well as long-term results. A number of measures can be undertaken to reduce the risk of anastomotic complications, including intraoperative colorectal anastomotic integrity assessment. Methods used to assess anastomotic reliability have gradually developed from basic mechanical techniques, direct visual endoluminal inspection, to microperfusion assessment of perianastomotic tissue. Moderate benefit in terms of reduced postoperative anastomotic complications has been shown with mechanical patency testing and partly with intraoperative endoscopic visualization of colorectal anastomoses.
