Olovo je kumulativní jed, který je zvláště nebezpečný pro malé děti. I při nízké úrovni expozice, která byla dříve považována za bezpečnou, poškozuje vývoj mozku a nervového systému, což vede k poruchám chování a snížení inteligence dětí. Hlavním zdrojem expozice olovu je potrava. Pitná voda může představovat významný příspěvek expozice olovu ve starých budovách s původními olověnými rozvody vody. Cílem prezentované práce bylo získat základní informaci o expozici olovu z pitné vody ve vytipovaných školských zařízeních ve starých budovách v Pardubickém kraji a o možném zdravotním riziku pro děti. V letech 2013 a 2014 byly v součinnosti se Zdravotním ústavem vyšetřeny vzorky pitné vody ve 20 školských zařízeních. Vzorky byly odebrány jak ráno po noční stagnaci vody v potrubí, tak náhodně během dne. Zvýšený obsah olova ve srovnání s dodávanou vodou z vodovodu byl zjištěn ve většině zařízení. V téměř polovině zařízení byly nalezeny zvýšené koncentrace olova i ve vzorcích náhodně odebraných během dne při provozu školy a ve 3 zařízeních bylo zjištěno překročení současného hygienického limitu 10 µg/l. Při charakterizaci rizika vývojové neurotoxicity u dětí byla použita Metodika rozdílu expozic (MOE), doporučená EFSA. Výsledkem je konstatování, že i podlimitní obsah olova v pitné vodě ve školském zařízení může představovat sice malé, ale ne úplně zanedbatelné riziko nepříznivého ovlivnění nervového vývoje u dětí. Do péče o zdravé prostředí předškolních a školních zařízení by proto měla patřit i eliminace tohoto potenciálního rizika., Lead is a cumulative toxicant that is particularly harmful to young children. Even at lower levels of exposure that were previously considered safe, lead damages brain and nervous system development resulting in behavioural changes and decreased intelligence of children. The main source of lead exposure is food. An important contributor to lead exposure may be the tap water in old buildings, having the original lead plumbing. The aim of the present study was to obtain basic information about lead exposure from drinking water in selected educational facilities placed in old buildings in the Pardubice Region as well as the possible health risks for the children. Between 2013 and 2014, in collaboration with the Public Health Institute, samples of drinking water in 20 educational facilities were examined. The samples were taken both in the morning after overnight water stagnation in the pipes and randomly during the day. In most facilities the lead content was increased as compared to water regularly supplied in the mains. In almost half of the monitored facilities increased lead content was detected even in the samples taken during the day. In three facilities the lead content exceeded the current lead content limit 10 µg/l. To characterize the risk of developmental neurotoxicity in children, the methodology "margin of exposure" (MOE), recommended by EFSA, was used. The result is, that even a lower lead content in drinking water of school facilities than the regulatory limit value may pose a small but not entirely negligible risk of neurodevelopmental effects. Aim: The principal aim was to establish reference paediatric data for the serum homocysteine levels in Czech children and adolescents. Methods and Results: 144 children either healthy or not sufferig from acute or chronic inflammation, autoimmune disorders including rheumatic diseases, inflammatory musculoskeletal disorders, inflammatory bowel disease, diabetes mellitus, hypercholesterolemia, epilepsy, chronic renal failure, aged 0–19.9 years (0–6.9 years, n = 40; 7–10.9 y, n = 28; 11–15.9 y, n = 45; 16–19.9 y, n = 31) had their blood samples collected and the serum homocysteine level (S-homocysteine) was evaluated by chemiluminescence. A significant age dependence of the S-homocysteine levels was observed (R = 0.35, p < 0.01); with highest values of upper reference range in the 11–15.9 and 16–19.9 years’ group, respectively. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Autoři ve svém příspěvku uvádějí první výsledky šetření, které bylo zaměřeno na zjištění hmotností školních tašek chlapců a dívek 1. a 2. tříd základních škol v olomouckém regionu. Celkem byla zjištěna hmotnost u 502 školních tašek, změřeno 502 dětí (263 dívek a 239 chlapců). Z výsledků měření je zřejmé, že hmotnost tašky se zvyšuje s postupem žáků do vyšší třídy. Největší rozdíl mezi minimální a maximální hodnotou hmotnosti tašky byl zjištěn u 8letých dívek, činil 8,9 kg. Výsledky ukázaly, že procentuální podíl hmotnosti tašky doporučeným 10 % tělesné hmotnosti dítěte odpovídal pouze u 17,5 % dětí. 82,5 % dětí pak nosí aktovky těžké (10–20 %), přičemž 18,3 % dětí nosí aktovky o hmotnosti nad 20 % své tělesné hmotnosti, což už lze považovat za extrémně těžké. Nepřiměřená hmotnost školní tašky pak může ovlivňovat držení těla a výskyt bolesti zad., Effects of the weights of school bags carried by pupils from the first two classes of primary schools in the Olomouc region (502 schoolbags belonging to 263 girls and 239 boys) on the pupils’ posture were examined. The weight of the schoolbags of pupils from the 2nd year class was found to be higher than of pupils in the first year. The greatest difference between minimum and maximum weights (8.9 kilograms) was detected in the schoolbags of 8 year old girls. Only 17.5% of children carried bags that were within the recommended 10% of the child’s body weight. Omezení reklamy na tabákové výrobky je považováno za efektivní opatření pro snížení prevalence kouření v populaci. Tato reklama ovlivňuje děti a mladistvé, kteří jsou na ni citlivější než dospělí. Bez ohledu na propagovanou značku vedou marketingové aktivity výrobců ke zvyšování povědomí o kouření mezi nezletilými a ke zvýšení rizika nástupu kouření. I když v mnoha zemích, včetně České republiky, došlo k výraznému omezení reklamy na tabákové výrobky, stále dochází k propagaci v místě prodeje a také pomocí grafického zpracování obalů. Významné jsou také produkty cílené na ženy a dívky. Naopak omezování propagace tabákových výrobků, omezení míst prodeje, jednotné balení cigaret a zejména úplný plošný zákaz reklam snižují počet náctiletých, kteří začnou kouřit., Restriction of tobacco advertising is an effective strategy to decrease smoking prevalence. Marketing of tobacco products influences smoking rates among children and adolescents, and several studies have shown that youth are more sensitive to tobacco advertising than adults. Tobacco advertising of any form increases the awareness of smoking among children and increases the risk of smoking onset, regardless of the brand advertised. Many countries, including the Czech Republic, restricted tobacco advertising, however, advertising at point of sale and promotion through cigarette package design is still permitted. Important are marketing campaigns targeted at girls and women. Cíl: Součástí diagnostiky febrilní infekce močových cest (IMC) je ultrazvukové vyšetření a mikční cystouretrografie (MCU), které slouží k odhalení případných komplikací infekce a anomálií močových cest. Tak, jak se vyvíjí porozumění vzájemným vztahů mezi infekcí močových cest, jizvením a přítomností vezikoureterálního refluxu (VUR), mění se i doporučené vyšetřovací algoritmy. Materiál a metoda: Do studie bylo zahrnuto 184 pacientů, kteří podstoupili mikční cystouretrografii po první či opakované atace febrilní infekce močových cest. Byl sledován výskyt vezikoureterálního refluxu u obou skupin pacientů v korelaci s ultrazvukovým nálezem. Pacienti s vezikoureterálním refiuxem byli rozděleni do skupiny s nediktujícím (VUR I.-II. stupně) a dilatujícím (VUR III.-V. stupně) refiuxem. Výsledky: Celkově byl VUR zaznamenán u 39/184 (21 %) pacientů. Ve skupině pacientů po první atace febrilní IMC byl VUR zachycen u 23/156 (15 %) pacientů, ve srovnání s 16/28 (57 %) pacienty po opakovaných febrilních IMC (statisticky významný rozdíl, p < 0,0001). U 8/156 (5 %) pacientů po první atace febrilní IMC byl zjištěn dilatující VUR ve srovnání s 11/28 (39 %) pacienty s opakovanými febrilními infekcemi močových cest (statisticky významný rozdíl, p < 0, 0001). Závěr: V souladu s aktuálním doporučením American Academy of Paediatrics poukázaly výsledky naší studie na nízký výskyt VUR včetně dilatujícího VUR u pacientů po první atace febrilní IMC. Na základě našich zjištění nedoporučujeme provádět MCU plošně u všech pacientů ve věku 2-24 měsíců po první atace IMC., Aim: Ultrasound investigation and voiding cystouretrography (VCUG) are part of the diagnostic algorithm in children with febrile urinary tract infection (UTI) with the aim of depicting any complication of the infection and underlying anomalies of the urinary tract. As the understanding of the relationship between UTI, renal scarring and vesicoureteral reflux (VUR) evolves, new recommended imaging algorithms are being introduced. Methods: 184 patients that underwent VCUG after the first or recurrent febrile UTI were included in the study. The incidence of VUR in both groups have been evaluated and compared with ultrasound findings. Patients with VUR were divided into those with non-dilating (VUR grade I-II) and dilating (VUR grade III-V) reflux. Results: Overall VUR was diagnosed in 39/184 (21%) patients. VUR was seen in 23/156 (15%) patients in the group of patients after the first fibrile UTI, in comparison to 16/28 (57%) patients with recurrent fibrile UTI (statistically significant difference, p < 0,0001). 8/156 (5%) patients after the first febrile UTI had dilating VUR, compared to 11/28 (39%) patients after recurrent febrile UTI (statistically significant difference, p< 0,0001). Conclusion. In compliance with current recommendations of the American Academy of Paediatrics the results of this study have shown low incidence of VUR and dilating VUR in patients with a history of single febrile UTI.
