INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Introduction: It has been suggested that the prevalence of neurodegenerative diseases in small, isolated European communities might be higher than in the general population. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. Úvod: Kardiovaskulární (KV) onemocnění patří celosvětově k hlavním příčinám úmrtí. Časné odhalení rizikových faktorů a zvýšeného celkového KV rizika může být významným pomocníkem při snižování morbidity i mortality. Za optimální věk pro hodnocení KV rizika v populaci se doporučuje věk 40 let u mužů a 50 let u žen. V rámci prezentovaného průřezového šetření byl zhodnocen KV rizikový profil osob v těchto věkových kategoriích při vyšetření všeobecným praktickým lékařem. Metodika: Do průzkumu bylo zařazeno 1 812 osob, z toho mužů v průměrném věku 40 a žen v průměrném věku 50 let. U každého vyšetřeného byla zjištěna a do protokolu studie zaznamenána rodinná, osobní a farmakologická anamnéza, abúzus, fyzická aktivita. Byly změřeny základní antropometrické údaje, tepová frekvence, krevní tlak, zjištěny hodnoty sérového lipidogramu a glykemie. Vyšetřující lékaři zaznamenávali přítomnost nově zjištěných rizik KV onemocnění a stanovili celkové KV riziko dle tabulek SCORE. Bylo hodnoceno procento nemocných s již existujícím sledovaným onemocněním, procento zastoupení těch, kteří měli dobře kontrolované rizikové faktory, a dále procento pacientů, kterým byl nově zjištěn některý ze sledovaných rizikových faktorů. Hypotéza vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění byla testována za použití Pearsonova testu. Výsledky: Bylo zařazeno 961 mužů průměrného věku 42,9 ? 4,7 let a 851 žen průměrného věku 51,2 ? 3,6 let. V pásmu nadváhy se nacházelo 49 % mužů a 31 % žen, obézních bylo 32 % mužů a 31 % žen. Aktivně kouřilo 36 % mužů a 22 % žen. Prevalence diabetu 2. typu byla shodně 11 % u obou pohlaví. Arteriální hypertenze byla zjištěna u 43 % mužů a 45 % žen a dyslipidemie u 39 % mužů a 41 % žen. Farmakologicky léčeno pro kteroukoli z uvedených chorob bylo 48 % osob, z nich pouze 8 % dosahovalo dobré kontroly všech identifikovaných rizik. Nově byl zjištěn diabetes u 3 % mužů i žen, arteriální hypertenze u 7 % mužů a 5 % žen a dyslipidemie u 20 % mužů i žen. 62 % mužů a 65 % žen doporučili participující lékaři nefarmakologickou léčbu. U 53 % mužů a 51 % žen byla zahájena nová farmakoterapie. U mužů i u žen šlo nejčastěji o antihypertenzní medikaci ze skupiny ACE-inhibitorů (29 % u mužů a 24 % u žen) a hypolipidemikum ze skupiny statinů (29 % u mužů a 27 % u žen). Z analýzy vztahu mezi pozitivní rodinnou anamnézou a výskytem onemocnění u dané osoby bylo zjištěno, že tato souvislost existuje pro arteriální hypertenzi, diabetes mellitus 2. typu i dyslipidemii. Závěr: Provedené šetření dokumentuje vysokou prevalenci rizikových faktorů kardiovaskulárních onemocnění v populaci 40letých mužů a 50letých žen. Vysoké zastoupení nově zjištěných rizikových faktorů i nadpoloviční podíl osob, u nichž byla v rámci provedeného šetření zahájena farmakoterapie, jasně dokazují nezbytnost časného screeningu rizikových faktorů. Genetická determinace jednotlivých kardiovaskulárních rizikových faktorů odrážející se v rodinné anamnéze je významnou komponentou celkového rizika oběhových onemocnění, a musí být aktivně zjišťována a zohledňována při úvaze o volbě intenzity preventivních strategií., Background: Cardiovascular (CV) disease belongs to the most important mortality causes worldwide. Early identification of risk factors and increased CV risk may help decrease of morbidity and mortality. An optimal age for CV risk factors screening ages of 40 in males and of 50 in females have been identified. CV risk profile of persons in these age categories has been examined by general practitioners and recorded for the purposes of this cross-sectional survey. Methods: 1812 persons, males at average age of 40 years and females at average age of 50 years, were included into the survey. In each of the examined family and personal history including pharmacological were recorded into the study protocol as well as abusus and physical activity patterns. Basic anthropometrical parameters, e.g. heart rate, blood pressure, body mass index and laboratory measures including blood lipids and glycaemia. The participating physicians recorded newly identified risk factors CV disease and determined the global CVD risk according to the SCORE charts. Percentage of those already treated for any of the main CVD risk factors and, among these, also attaining the treatment goals. The percentage of patients with a newly identified CV risk factor was calculated. We also tested the hypothesis of a relationship between positive family history of any of the followed risk factors and its risk in the examined probands using Pearson´s test. Results: 961 males, average age of 42.9 ? 4.7 years, and 851 females , average age of 51.2 ? 3.6 years, were enrolled into the study. 49% of males and 31% of females were overweight and 32% men and 31% of women were obese. There were 36% of smokers among men and 22% among women. The prevalence of type 2 diabetes was 11% in males and also in females. Arterial hypertension was diagnosed in 43% of males and 45% of females while dyslipidemia was present in 39% of males and 41% of females. Pharmacological treatment of any of the above mentioned diseases was used in 48% of the probands, however, only 7% of them attained treatment goals of blood pressure, LDL-cholesterol and glycaemia. Type 2 diabetes was newly identified in 3% of both males and females, arterial hypertension in 8% of males and 5% of females and dyslipidemia was newly detected in 20% of probands of both genders. Non-pharmacological treatment was recommended to 62% of male and to 65% of female participants, respectively. Pharmacological treatment was initiated in 53% of males and 51% of females. In both genders this was mostly antihypertensive treatment with ACE inhibitors (29% of males and 24% of females) and lipid lowering therapy with a statin (29% of males, 27% females). The analysis of relationship between the positive family history of any of the followed diseases and their presence in the probands examined revealed significant increases of the risk for arterial hypertension, type 2 diabetes mellitus and dyslipidemia. Conclusion: The survey following CV risk profile in a cohort of 40 years old men and 50 years old women showed high prevalence of CV risk factors in these age categories in the Czech probands. The high frequency of modifiable risk factors and the need to initiate pharmacological treatment in more than one half of the examined population documents the need of early detection of risk. Úvod: Vysoká incidence kolorektálního karcinomu představuje v České republice aktuální a populačně závažný zdravotnický problém. Polovina všech nemocných je věku nad 70 let. Možnosti chirurgické i nechirurgické léčby v této skupině závisejí na faktorech, jen minimálně vyjádřitelných zavedenou onkologickou a anesteziologickou klasifikací (cTcNcM, ASA). Cílem práce je ukázat vliv věku na využití léčebných modalit do protokolu léčby a na její výsledky a na jiné možnosti hodnocení únosnosti k léčbě. Metody: Analýza údajů z databáze NOR z pětiletého období 2008–2012 zpracované Institutem biostatistiky a analýz MU. Výsledky: Ve všech sledovaných parametrech byl prokázán rozdíl mezi nemocnými pod a nad 70 let v neprospěch starších. Ve věku pod 70 let nebylo léčeno 11,2 % nemocných, ve věku na 70 let 25,2 %. Závěr: K rozhodování o způsobu vedení léčby včetně volby operace by mohla přispět klasifikace pomocí komplexního geriatrického vyšetření., Introduction: High incidence of colorectal cancer in the Czech Republic is an actual and demographically significant health issue. Half of all of the patients is older than 70 years. Both surgical and non-surgical treatment options in this group of patients depend on factors that are difficult to measure only by current oncological and anesthesiological classifications (cTcNcM, ASA). The objective of this paper is to measure the impact of age on the use of various treatment modalities within the protocol and their results, and also to suggest alternative options for therapy tolerance assessment. Methods: Analysis of data over a five-year period from the NOR database prepared by the Institute of Biostatistics and Analyses, Masaryk University. Results: In all parameters a difference was demonstrated between patients below the age of 70 and those above the age of 70 years. Older patients were disadvantaged. Only 11.2% of patients younger than 70 years were not treated, whereas 25.2% over the age of 70 years were not treated. Cíl: Lehké hypoglykemie ovlivňují zvládání diabetu a produktivitu pacientů i kvalitu jejich života. Údaje o výskytu hypoglykemií v běžném životě pacientů s diabetem 2. typu léčených inzulinovými analogy přitom nejsou dostatečné. Cílem průzkumu provedeného v České republice v rámci mezinárodního projektu GAPP2™ (Global Attitudes of Patient and Physicians) bylo ověřit incidenci lehkých hypoglykemií a jejich důsledky u pacientů s diabetem 2. typu léčených inzulinovými analogy. Metodika: Projekt GAPP2™ – Globální přístupy pacientů a lékařů (Global Attitudes of Patients and Physicians) je mezinárodní průřezovou studií provedenou on-line prostřednictvím internetu formou dotazníku u pacientů s diabetem 2. typu léčených inzulinovými analogy a u lékařů, kteří tyto pacienty ošetřují. Průzkum proběhl ve dvou vlnách celkem v 17 zemích světa. Do první vlny, která byla dokončena počátkem roku 2012, bylo zahrnuto 6 zemí, do druhé, dokončené v roce 2014, pak dalších 11 zemí včetně České republiky. Průzkum měl za cíl získat data o pohledech pacientů a lékařů na některé aspekty inzulinové léčby diabetu 2. typu a o přetrvávajících problémech v této oblasti v reálné každodenní praxi. V jedné části výzkumu byly sledovány lehké hypoglykemie při léčbě inzulinovými analogy. Byla zkoumána četnost výskytu lehkých hypoglykemií, reakce pacientů i lékařů na tyto hypoglykemie (úprava inzulinové léčby, selfmonitoring glykemie) a dále byl sledován dopad lehkých hypoglykemií na kvalitu života pacientů z pohledu pacientů i zdravotníků. Výsledky: Výsledky průzkumu ukázaly, že lehké hypoglykemie jsou u pacientů léčených inzulinovými analogy v ČR běžné a že pacienti v ČR se hypoglykemií obávají více a cítí se jimi v běžném životě více omezeni než pacienti v ostatních zemích průzkumu GAPP2™. Pacienti se snaží vyhýbat nočním hypoglykemiím i za cenu snížení nebo vynechání předepsané dávky inzulinu. Dále z výzkumu vyplývá, že lékaři nejsou o frekvenci a závažnosti hypoglykemií ze strany pacientů dostatečně informováni. Závěr: Lehké hypoglykemie jsou v ČR stejně běžné jako v ostatních zemích průzkumu GAPP2™, čeští pacienti se jich však obávají více, přičemž tento fakt zůstává často zdravotníkům skryt. Zvýšená obava pacientů z hypoglykemie ukazuje nepokrytý prostor v rámci edukace, který je třeba věnovat poučení, jak hypoglykemiím předcházet a jak je zvládat. Zvláštní důraz je přitom nutno klást na noční hypoglykemie. Zdravotníci by měli problematiku hypoglykemií s pacienty více probírat. Je důležité motivovat pacienty, aby zdravotníky o frekvenci a závažnosti hypoglykemií dostatečně informovali., Objective: Mild hypoglycaemia affects diabetes management, productivity and quality of life of patients. Data are scarce about the incidence of mild hypoglycaemia in daily life of type 2 diabetic patients treated with insulin analogues. The aim was to focus on the incidence of mild hypoglycaemia and its consequences in the international GAPP2™ survey (Global Attitudes of Patient and Physicians) conducted also in the Czech Republic. Methods: The GAPP2 ™ project – Global Attitudes of Patients and Physicians is an international cross-sectional study conducted online via Internet in a questionnaire form dedicated to type 2 diabetic patients treated with insulin analogues and physicians who treat these patients. The survey was realized in two steps including 17 countries. The first step was completed by six countries in 2012. The second step was terminated by additional eleven countries including the Czech Republic in 2014. The survey was designed to obtain data on some aspects of insulin therapy and persistent issues in daily practice from the views of patients and physicians. One part of survey was dedicated to mild hypoglycaemia occurring in treatment with insulin analogues. The incidence of mild hypoglycaemia and the reaction of patients and physicians on hypoglycaemia (including modification of therapy, self-monitoring) were investigated. Moreover, there was investigated the impact of mild hypoglycaemia on quality of life from the view of patients and physicians. Results: The results of the survey have shown that mild hypoglycaemia is common in patients with insulin analogues in the Czech Republic. In addition, patients in the Czech Republic are more afraid from hypoglycaemia and feel more limited in daily life in comparison to other countries in GAPP2 ™ survey. Patients try to avoid nocturnal hypoglycaemia through reduction or omission of prescribed dose of insulin. Furthermore, research has observed that physicians are poorly informed by patients about incidence and severity of hypoglycaemia. Conclusion: The incidence of mild hypoglycaemia is in the Czech Republic as common as in other countries involved in the GAPP2 ™ survey. However, the Czech patients are more afraid and this fact is often hidden before physicians. Increased fear of hypoglycaemia suggests inadequate education in terms of how to avoid and manage hypoglycaemia. Particular emphasis should be placed on nocturnal hypoglycaemia. Physicians should discuss more with patients about issue of hypoglycaemia. Úvod: Léčba inzulinovou pumpou (IP) je jedním ze způsobů intenzivní terapie inzulinem, určeným preferenčně pro pacienty s diabetem 1. typu. Cena léčby je vyšší než léčba klasickým bazálbolusovým a inzulinovým režimem pomocí opakované aplikace inzulinu dávkovačem. Cíl: Zhodnocení kvality monitorování terapie IP u pacientů s DM v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u níž bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2009–2013 záznam o diagnóze DM (E10–E16 podle MKN 10), nebo měly předepsanou jakoukoliv antidiabetickou terapii (ATC skupina A10). Za celé období 2009–2014 bylo v datech VZP identifikováno celkem 4 002 unikátních pacientů s diabetem, kteří byli v hodnoceném období léčeni IP. Incidence pro rok 2014 (nově léčení inzulinovou pumpou): všichni pacienti, kteří měli v roce 2014 vykázanou IP a zároveň v předešlém období 2009–2013 není záznam o vykázání IP. Prevalence pro rok 2014 (všichni léčení inzulinovou pumpou): všichni pacienti, kteří v období 2010–2014 mají alespoň jednou vykázanou inzulinovou pumpu a zároveň nezemřeli před rokem 2014. Parametry kontroly kvality (vyšetření HbA1c a spotřeba diagnostických proužků pro testování hladiny glukózy) u pacientů léčených IP byly podrobně hodnoceny pouze pro rok 2014, a to u všech pacientů léčených inzulinovou pumpou v roce 2014 po celý rok (tj. od 1. 1. 2014 do 31. 12. 2014), tedy celkem u 3 189 pacientů. Výsledky: V roce 2014 bylo celkem 247 incidentních pacientů a celkem 3 794 prevalentních pacientů. Nově byla IP nasazena téměř v 50 % u pacientů ve věkovém rozmezí 20–39 let. V roce 2014 byla průměrná frekvence vyšetření HbA1c 3,38/pacienta a nejméně jednou byl vyšetřen u 98,5 % pacientů. Průměrná spotřeba diagnostických proužků pro testování hladiny glukózy na jednoho pacienta byla 879 kusů. Závěr: Pilotní projekt hodnocení parametrů kvality monitorování terapie IP ukazuje, že pacienti léčení IP mají poměrně často kontrolovaný glykovaný hemoglobin (3,38 vyšetření u jednoho pacienta v roce 2014) a průměrně si měří glykemii 2–3 krát denně. The price of the treatment is higher than that of the conventional basalbolus and insulin regimens using repeated insulin application with a dose selector. Goal: Quality assessment of IP therapy monitoring in patients with DM in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the period of 2009–2013. Over the whole period of 2009–2014 there were overall n = 4 002 unique patients with DM identified in the VZP data, who were treated with IP within the assessment period. Incidence for the year 2014 (the newly treated with an insulin pump): all patients who had IP recorded in 2014 while in the preceding period of 2009–2013 they had no record of IP use. Prevalence for the year 2014 (all treated with an insulin pump): all the patients who for the period of 2010–2014 had at least once insulin pump use recorded and who did not die before 2014. Quality control parameters (HbA1c examination and consumption of glucose level test strips) for patients treated with IP were only assessed in detail for the year 2014, namely for all patients undergoing insulin pump treatment in 2014 throughout the year (i.e. from 1 January 2014 to 31 December 2014), i.e. in n = 3 189 patients in all. Results: In 2014 there were 247 incident patients and 3 794 prevalent patients. IP was newly introduced for almost 50% of the patients aged 20–39 years. In 2014 an average frequency of HbA1c examination equaled 3.38 per patient and 98.5% patients were examined for HbA1c at least once. An average consumption of glucose level testing strips per patient was 879 pieces. Conclusion: The pilot project of assessment of quality parameters for IP therapy monitoring shows that the patients treated with IP have glycated hemoglobin checked quite frequently (3.38 checkups per patient in 2014) and they measure their blood glucose 2–3 times a day on average.
