Cíl: Některé kategorie migrantů ze třetích zemí mají v České republice omezený přístup ke zdravotní péči. Cílem této studie je popsat a analyzovat zkušenosti migrantů ze třetích zemí s jejich přístupem ke zdravotní péči a využíváním alternativních strategií k zajištění zdraví. Metody: Kvalitativní design za využití semistrukturovaných rozhovorů. Bylo realizováno dvacet čtyři rozhovorů s migranty ze třetích zemí různých národností a s různými typy povolení k pobytu na území České republiky. Výsledky: Migranti v praxi čelí četným problémům v přístupu ke zdravotní péči. Byly identifikovány hlavní bariéry přístupu ke zdravotní péči: omezený právní nárok na veřejné zdravotní pojištění, nedůvěra a špatná informovanost pojištění, nedůvěra a špatná informovanost zdravotnických pracovníků, přístup komerčních zdravotních pojišťoven založený na tržních principech, jazyková bariéra a nedostatek znalostí cizinců o českém zdravotnictví. Tyto překážky byly příčinou následujících alternativních strategií migrantů k zajištění zdraví: kontaktování nebo využívání lékařů v zemích původu, platby v hotovosti, odkládání zdravotní péče, návštěvy známých lékaře nebo lékařů s migračním původem a samoléčba. Závěr: Některé kategorie migrantů ze třetích zemí čelí formálním a neformálním překážkám využívání českého zdravotnického systému, které jsou příčinou alternativních strategií zajišťování zdraví. Ty mohou vést k nepříznivým účinkům na jejich zdraví. Tato studie ukazuje, že v souladu s mezinárodními lidskými právy je potřeba implementovat změny v rámci veřejné ev. zdravotní politiky a zajistit tak přístup ke zdravotní péči pro všechny legální migranty ze třetích zemí v České republice., Background: Some categories of third countries´ migrants in the CR have restricted access to healthcare. The aim of this study is to describe and analyse migrants´ experiences of access to health- -care. Methods: Qualitative design using semi-structured interviews with twenty four migrants in the Czech Republic. Results: Some categories of third countries‘ migrants face formal and informal barriers to the Czech healthcare system, which leads to alternative health-seeking strategies such as visiting doctors in countries of origin (so called salmon bias), payments in cash, postponing the health care, visiting the known physicians or physicians with migration background and self-medication. Czech health and social services have undergone significant changes related to the reform of public administration including the transfer of certain social and health services to the competence of municipalities and regions. Regions also became legally responsible for social services ́ planning process including their registration and since 2015 financing. The findings in the article are based on a qualitative methodological approach, which was used as a combination of public policy documents analyses and primary data analysis. The empirical research was based on focus groups when respondents represented persons caring for the elderly and disabled people as well as providers of health-social services in the Central Bohemian Region. Based on the survey we can state that some types of services are underdeveloped, particularly the domiciliary services, social services for older children and adults with disabilities, respite care and other support services for caregivers. The great problem is the absence of interconnection between health and social services in the region. From the viewpoint of service providers and caregivers the level of municipalities is perceived as closer to the clients than regional level, thus service providers and caregivers aim their demands to municipalities more frequently. and Kotrusová Miriam, Dobiášová Karolína, Angelovská Olga
