Benigní hyperplazie prostaty (BHP) je (spolu s kataraktou) nejčastější benigní onemocněni, pro které podstupují muži invazivní operační zákrok. Přes intenzivní výzkumy (počátky sahají do 18. století) dosud nevíme, proč BHP vzniká. Existuje předpoklad, že benigní hyperplazii prostaty onemocní každý muž, který se dožije dostatečně vysokého věku se zachovanou androgenni produkcí. Pojem „vysoký věk" nebyl přesně definován, ale řada autorů se shoduje na věku mezi 70. - 75. rokem života. Poměrně komplikované je vyjádřit se i k „dostatečné androgenni stimulaci", neboť s rozvojem teorie o parciálním deficitu androgenů (PADAM) jako jevu, který provází andropauzu, je mimořádně komplikované definovat každému konkrétnímu jedinci správnou hladinu testosteronu (Ts). Vedle věku a hladiny androgenů se dnes domníváme, že BHP vzniká na podkladě kombinace několika jevů - inhibice apoptózy, androgenně - estrogenní nerovnováhy, vysoké androgenni stimulace, inaktivace kmenových buněk a neinhibované reakce prostatických fibroblastů., knign prostate hyperplasia (BPH) is (together with cataracta) the most frequent benign disease, for which the men undergo the invasive surgical operation. Despite intensive reasearches (the beginnings go back to 18th century) we still do not know, why BPH occurs. There exists a presumption that benign prostate hyperplasia affects every man, who reaches sufficiently high age with preserved androgene production. The notion of „high age" has not been defined precisely, but many authors agree on the age between 70-75 years. It is quite complicated to explain also „sufficient androgen stimulation", since along with the development of theory of parúal androgen deficit (PADAM) as a phenomenon accompanying the andropause, it is extremely complicated to define correct testosterone level (Ts) for each individual. Onemocněni srdce a cév patří mezi časté důvody úmrti a to navzdory zlepšení diagnostiky, léčení i znalosti etiopatogeneze. V Evropě umírá 10 % nemocných s akutním infarktem myokardu a nestabilní angínou pectoris do 90 dnů od jejich manifestace. Sledujeme-li je rok, pak tvoří úmrtí 17 %. Tato čísla vedou k rozvoji vědeckého výzkumu v oblasti protektivnich postupů. Jako možný postup se jeví substituce estrogeny například u žen v menopauze (HRT - hormone replacement therapy). Právě nízká hladina estrogenu u mužů je zkoumána jako možný důvod vyšší incidence kardiovaskulárních onemocnění (estrogeny vznikají aromatizací z testosteronu) oproti ienám ve skupině do 50 let (1). V úvahách o hormonálním deficitu starší a staré populace (u mužů především androgennim - PADAM - parciální deficit androgenů, u žen estrogenním) se nabízí další otázka - není účelné podávat mužům „ženské protektivni hormony", které sníží riziko onemocnění srdce a cév? Mají estrogeny protektivní vliv na kardiovaskulární systém? Je rozdíl v cestě podání a mechanizmu syntézy substitučních látek?, Cardiovascular diseases count among common causes of death despite the improvement of their diagnostics, treatment and knowledge of etiopathogenesis. Ten percent of patients with acute myocardial infarction and unstable angina die to 90 days after the disease manifestation in Europe. During one year foUow-up the mortality represents 17 %. This values lead to scientific research development in the area of protective methods. One of them seems to be estrogen replacement, e.g. in menopausal women (HRT - hormone replacement therapy). The low estrogen level in men is investigated as a possible cause of higher incidence of cardiovascular diseases (estrogens are pro¬ duced from testosterone via aromatization) in comparison with women under 50 years of age. [1] Considering a hormone dehciency in elderly and old population (in men mainly partial androgen deficiency - PADAM, in women estrogen deficiency) an other question arises - is it effective to administer to men „women protective hormones", which decrease a risk of cardiovascular diseases?
