Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed. Úvod: Byly srovnány dvě různé metody izoelektrické fokusace v agarosovém gelu pro detekci oligoklonálních IgG pásů v likvoru a séru: komerční metoda s imunofixací (Sebia) a „home-made“ metoda s použitím přístroje Multiphor II následovaná afinitním imunoblottingem. Byla posouzena shoda mezi metodami i hodnotícími, pokud jde o přítomnost intrathékální syntézy IgG, zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace, počet intrathékálně syntezovaných IgG pásů a počet pásů v likvoru a v séru. Výsledky: Pro posouzení shody jsme použili statistiku kappa. Pokud jde o přítomnost intrathékální syntézy IgG, byla shoda velmi dobrá (kappa 0,870 až 1,000 mezi metodami a 0,947 a 0,920 mezi hodnotícími, což představuje 0 až 5 neshodně hodnocených nálezů z celkového počtu 114). V zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace byla shoda méně vyjádřená (kappa 0,389 až 0,596 mezi metodami); při použití „home-made“ metody byla shoda mezi hodnotícími horší (kappa 0,478) než v případě komerční imunofixační metody (kappa 0,791). Shoda v počtu intrathékálně syntezovaných IgG pásů byla jen mírná, shoda v počtu oligoklonálních IgG pásů v likvoru a v séru byla slabá. Závěry: Oběma metodami lze spolehlivě detekovat oligoklonální IgG a metody lze považovat za rovnocenné. Pro klasifikaci typu nálezu však byla shoda mezi hodnotícími lepší při použití komerční imunofixační metody. Poměrně špatná reprodukovatelnost počítání oligoklonálních IgG pásů by měla být známa klinickým lékařům, protože porovnávání počtu pásů s sebou přináší potenciální riziko zavádějících interpretací., Background: We compared two different agarose isoelectric focusing methods for detection of oligoclonal IgG bands in cerebrospinal fluid and serum: commercial method with immunofixation (Sebia) and home-made method using Multiphor II apparatus followed by affinity immunoblotting. Interobserver agreement for both methods was tested concerning the presence of intrathecal IgG synthesis, the detailed isoelectric focusing pattern type, the number of CSF-restricted oligoclonal IgG bands, and the number of oligoclonal IgG bands in CSF and in serum. Findings: Using kappa statistics for evaluation of agreement, we found there was very good agreement concerning the presence of intrathecal IgG synthesis (kappa 0.870 to 1.000 between methods, and 0.947 and 0.920 between observers, respectively, representing 0 to 5 out of 114 samples classified differently). The agreement was less pronounced when international consensus classification of isoelectric focusing patterns into 5 different types was taken into account (kappa 0.389 to 0.596 between methods); using home-made method, the interobserver agreement regarding pattern type was worse (kappa 0.478) than using commercial Sebia method (kappa 0.791). There was moderate agreement on the number of CSFrestricted oligoclonal IgG bands, and mostly poor agreement on the number of oligoclonal IgG bands in CSF and serum. Conclusions: Both methods were capable to detect oligoclonal IgG reliably, and neither method could be evaluated as superior to the other. However, better interobserver agreement regarding to the pattern type was obtained using commercial Sebia immunofixation method. The local Chow Kit market is the largest wet market in the city of Kuala Lumpur. It is very close to the biggest government hospital in the city centre. However, the level of cleanliness in this area is always questionable and a matter of concern. The aim of this study was to identify the prevalence of T. gondii oocyst in water samples used by hawkers in that market and tissue cysts in rats’ brains captured from the same area. Water samples were taken to the parasitology laboratory at the National Universtiy of MalaysiaUniversity and a sugar flotation concentration method was used. Supernatant microscopical examination was then performed. A total of 752 slides were screened for the presence of T. gondii oocyst. A hundred rats wandering in the same area were also captured by the hawkers using mousetraps. After each animal was sacrificed, and an electric microtome was used to cut out serial sections 5μ thick from the rat brains. The de-waxed tissue sections were stained by the progressive Haematoxylin and Eosin (H&E) stain for microscopical examination. A total of 1000 slides were screened under a light microscope to detect the presence of T. gondii brain cysts. All the water samples were found to be negative for T. gondii oocyst. Out of the 100 rats captured, three rats were found to possess T. gondii cysts in their brains. Water samples reflect minimal or no solid food contamination, while the 3% of positive brain cysts influence the researchers to broaden their investigations for future projects., Amal R. Cíl. Prospektivně zhodnotit možnosti detekce a charakterizace ložiskových lézí jater hepatocytárního původu při vyšetření magnetickou rezonancí po podání hepatospecifické kontrastní látky Gd-EOB-DTPA. Metoda. U 78 pacientů (67 žen, 11 mužů) s arteriálně hypervaskularizovaným jaterním ložiskem (jiným než hemangiom) nalezeným na předchozím zobrazovacím vyšetření bylo provedeno vyšetření jater magnetickou rezonancí s podáním hepatospecifické kontrastní látky Primovist (Gd-EOB-DTPA, Bayer Schering Pharma, Berlín, Německo). Všechna vyšetření byla provedena na MR přístroji firmy General Electric Signa 1,5T nativně, následovalo dynamické kontrastní zobrazení po podání kontrastní látky Primovist v arteriální, portální fázi opacifikace jater, ve fázi ekvilibria a za 20 minut. Byla hodnocena velikost, signál a charakter opacifikace, homogenita a případná přítomnost centrální jizvy. Ložiska vykazující akumulaci kontrastní látky v hepatospecifické fázi vyšetření byla považována za ložiska hepatocytárního původu. Ložiska izointenzní nebo hyperintenzní v hepatospecifické fázi byla v necirhotickém terénu považována za ložiska fokální nodulární hyperplazie, pokud jejich nativní signál a chování při dynamickém kontrastním vyšetření byl s ložisky fokální nodulární hyperplazie slučitelný Ložiska vykazující akumulaci kontrastní látky v hepatospecifické fázi vyšetření, která však byla převážně nižší ve srovnání s okolním parenchymem jater, byla v necirhotickém terénu považována za pravděpodobné adenomy Ložiska bez známek akumulace kontrastní látky v hepatospecifické fázi vyšetření byla považována v necirhotickém terénu za pravděpodobné metastázy. Výsledky. Sto jedna ložisek (u 67 pacientů) bylo označeno za ložiska fokální nodulární hyperplazie. 14 ložisek (u čtyř pacientů: 1, 1, 4 a 8 ložisek) bylo označeno za pravděpodobné adenomy, u tří pacientů byla provedena kontrola na MR bez progrese (po 24, 42, resp. 17 měsících). Jedno ložisko v necirhotickém terénu u muže bylo označeno za pravděpodobný hepatom, méně pravděpodobně adenom (histologický potvrzen adenom). Mnohočetná (desítky) ložiska u jednoho z pacientů - jaterní adenomatóza, histologický ověřeno. Dvě ložiska (u dvou pacientů) v cirhotickém terénu byla označena za pravděpodobné hepatomy U tří pacientů s onkologickým onemocněním v anamnéze byla prokázána vícečetná metastatická ložiska. Závěr. Podání hepatospecifické kontrastní látky Primovist (Gd-EOB-DTPA) významně zvyšuje potenciál magnetické rezonance pro specifikaci ložiskových lézí jater hepatocytárního původu, zvláště užitečné je pro diagnostiku ložisek fokální nodulární hyperplazie, kdy umožňuje s velkou pravděpodobností potvrdit tuto diagnózu bez nutnosti histologické verifikace. Velkým přínosem jejího podání je také odlišení hypervaskularizovaných metastáz od ložisek hepatocytárního původu., Aim. To prospectively evaluate the possibilities of detection and characterization of hepatocellular liver lesions by means of MRI using hepatospecific contrast agent Gd-EOB-DTPA. Method. A total of 78 subjects (67 females, 11 males) with known arterial hypervascularized liver lesion (other than haemangioma) were examined on MRI with administration of liver specific contrast agent Primovist (Gd-EOB-DTPA, Bayer Schering Pharma, Berlin, Germany). All studies were performed on General Electric Signa 1.5T system before and after Primovist contrast agent injection in arterial, portal venous, equilibrium phases, and at twenty minutes past injection. Size, signal, enhacement pattern, homogeneity, and presence of central scar were evaluated. Lesions with contrast agent accumulation in liver specific phase were considered to be of hepatocellular origin. Iso-/hyperintense lesion in liver specific phase was in non-cirrhotic liver considered to be focal nodular hyperplasia provided the features on precontrast and dynamic contrast studies were in keeping with this diagnosis. Lesion showing signal increase in liver specific phase, however to a lesser degree than normal non-cirrhotic liver parenchyma was considered to be adenoma. Lesion showing no enhacement in liver specific phase was diagnosed as metastasis. Results. 101 focal nodular hyperplasias (in 67 subjects) were diagnosed. 14 adenomas (in 4 subjects with 1, 1,4, and 8 lesions, respectively) were found; three subjects were available to MRI follow-up without progression (after 24, 42, and 17 months, respectively). A single lesion in non-cirrhotic liver was thought to be a hepatoma, less likely an adenoma (adenoma confirmed on biopsy). A single patient presented with tens of lesions due to liver adenomatosis (histologically verified). Two lesions in two patients with liver cirrhosis were diagnosed as hepa-tomas. Three oncological patients were diagnosed with liver metastases. Conclusion. Administration of liver specific contrast agent Primovist (Gd-EOB-DTPA) improves specification of hepatocellular liver lesions, particularly in cases of focal nodular hyperplasia where non-invasive diagnosis can be made with high accuracy. Diagnostika sclerosis multiplex (SM) sa opiera o dôkaz diseminácie ochorenia v priestore a v čase pri vylúčení iných neurologických ochorení, ktoré môžu byť svojím klinickým priebehom a rádiologickým nálezom SM veľmi podobné. Kým vytvoreniu kritérií na splnenie daných požiadaviek diagnostiky SM sa venuje velká pozornosť, kritériá pre diferenciálnu diagnostiku SM ostávajú v pozadí. Na druhej strane MR kritériá pre disemináciu SM v priestore dosahujú vysokú špecificitu (89%) u tých pacientov, ktorí mali vylúčené iné neurologické ochorenia. Pomerne často vidíme v MR nálezoch nešpecifické poškodenie bielej hmoty mozgu, pre ktoré nemáme jednoznačné vysvetlenie. Sú diagnostickým problémom, umožňujú MR podporu velkého počtu ochorení, a takýmito nálezmi sa doposiaľ užívané kritériá nezaoberajú. Zároveň sa pri MR vyšetrení stretávame s nálezmi demyelinizačných lézií, ktoré svojou veľkosťou a uložením v bielej hmote mozgu pripomínajú SM, ale napr. vek pacienta alebo priebeh jeho klinických ťažkostí nezapadajú do vzorca niektorej z foriem SM (obr. 1A,B). V kontexte uvedeného môžu byť MR nálezy netypické pre SM alebo typické pre iné ochorenia nápomocné v procese diferenciálnej diagnostiky a manažmente pacientov s podozrením na SM. Európska skupina expertov nazvaná MAGNIMS (Magnetic resonance neťwork in multiple sclerosis) definovala na základe vedeckých výskumov a dôkazov práve takéto skupiny ochorení a pre praktickú názornosť i zdôraznenie ich významu sú označené červenou zástavkou., Diagnosis of Multiple Sclerosis (MS) relies on the evidence of disease dissemination in time and space at the exclusion of other neurological diseases that may clinically or radiologically mimic MS. While the establishment of criteria to meet the requirements of the diagnosis of MS is given múch attention, the criteria for differential diagnosis of MS remain in the background. On the other hand, MRI criteria for dissemination in space is of high specificity (89%) in those patients who had ruled out other neurological diseases. Quite often we see in the MRI findings a nonspecific white matter damage, for which we háve no clear explanation. There are a diagnostic problems with it, enabling MRI support for a large number of diseases and šuch findings háve not yet been engaged in the criteria used. Simultaneously on MRI seans we meet with the findings of demyelinating lesions suggestive of MS with their measurement and localisation in the white matter, but for example the age of a patient or his clinical course does not fit the pattern of anyforms of MS (Fig. 1A,B). In the context written above, the MRI findings atypical for MS or typical for other neurological diseases may be helpful in the process of diíferential diagnosis and management of patients with suspected MS.
