- Description:
- CD200 and its receptor were recognized as having the multiple immunoregulatory functions. Their immunoregulatory, suppressive, and tolerogenic potentials could be very effectively exploited in the treatment of many diseases, e.g. Alzheimer disease, rheumatoid arthritis, and allergy to name only some. Many research projects are aimed to develop clinically valuable methods being based on the structure and function of these paired molecules. In this review, we would like to introduce CD200/CD200R functions in a clinical context., Drahomíra Holmannová, Martina Koláčková, Kateřina Kondělková, Pavel Kuneš, Jan Krejsek, Ctirad Andrýs, and Literatura 51

- Description:
- CD200/CD200R are highly conserved type I paired membrane glycoproteins that belong to the Ig superfamily containing a two immunoglobulin‑like domain (V, C). CD200 is broadly distributed in a variety of cell types, whereas CD200R is primarily expressed in myeloid and lymphoid cells. They fulfill multiple functions in regulating inflammation. The interaction between CD200/CD200R results in activation of the intracellular inhibitory pathway with RasGAP recruitment and thus contributes to effector cell inhibition. It was confirmed that the CD200R activation stimulates the differentiation of T cells to the Treg subset, upregulates indoleamine 2,3‑dioxygenase activity, modulates cytokine environment from a Th1 to a Th2 pattern, and facilitates an antiinflammatory IL‑10 and TGF‑β synthesis. CD200/CD200R are required for maintaining self‑tolerance. Many studies have demonstrated the importance of CD200 in controlling autoimmunity, inflammation, the development and spread of cancer, hypersensitivity, and spontaneous fetal loss., Drahomíra Holmannová, Martina Koláčková, Kateřina Kondělková, Pavel Kuneš, Jan Krejsek, Ctirad Andrýs, and Literatura 46

- Cíl: Zhodnocení zkušeností s celotělovou magnetickou rezonancí na 3T systému. Metodika: Vyhodnotili jsme celkem 279 vyšetření provedených na přístroji 3T (SKYRA, Siemens, Německo) provedených během 36 měsíců a posoudili jejich indikace a zejména možnost zobrazení skeletu, ale i parenchymových orgánů při indikacích mnoho četného myelomu, lymfomů a ostatních nádorových onemocnění. Výsledky: Nejčastější indikací zůstává myelom (228), u celkem 35 vyšetření lymfomů jsme získali kvalitní diagnostické zobrazení, které umožňuje posoudit nejenom postižení lymfatických orgánů včetně sleziny, ale i kostní dřeně a plic. Difuzní zobrazení bylo shledáno dostatečné diagnostické kvality, avšak výrazně prodlužuje celý diagnostický protokol. Závěr: Celotělové vyšetření na 3T MRI je plně diagnosticky využitelné při hodnocení stagingu i restagingu mnohočetného myelomu, lymfomů i ostatních nádorů jako je karcinom prostaty., Aim: Evaluation of the experience with whole body magnetic resonance on 3T system. Method: 279 examinations performed on 3T system (SKYRA, Siemens, Erlangen, Germany) performed during 36 months were assessed according indications, possibility of imaging of the bone marrow, organs of the abdomen and lungs, when the indications are multiple myeloma, lymphoma or the other tumors. Results: The most frequent indication remains multiple myeloma (228), in 35 patients was performed lymphoma imaging with reached sufficient diagnostic imaging enabling assessment of the bone marrow, lymph nodes, lungs and parenchymatous organs. Diffusion weighted imaging on 3T is sufficient, but increases the imaging time significantly. Conclusion: Whole body imaging on 3T MRI is fully diagnostic in the evaluation of staging or restaging in multiple myeloma, lymhomas or the other tumors., Boris Kreuzberg, Jiří Ferda, Jan Kastner, and Literatura

- Description:
- The subject of work is the development of Czech health care law in the past 20 years in the context of (1) Czechoslovak and Czech social change, (2) the development of international human rights interpretations related to health. The goal is evaluation of the Czech health care law in the individual developmental stages and their compliance with human rights related to health. Running away from the Czech health care law framework of human rights and its substitution by a positive concept of patients‘ rights is accompanied by efforts to reduce human rights protection mechanisms at the Constitutional Court („euroamendment“ of the Czech Constitution in 2001). Major changes in medical law in relation to human rights brings the current changes in health care reform (2011 - Health Reform I and II. Phase)., Petr Háva, Pavla Mašková, Tereza Teršová, and Literatura

- Description:
Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Poznatky o interakci genů, chování a prostředí potvrzují opodstatněnost primárně preventivních opatření a mohou přispět ke změně postojů ke zdraví., Non-infectious diseases represent a serious health problem in current society. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases. Knowledge of the interactions of genes, behaviour and environment confirm the validity of primary preventive measures and may contribute to changing attitudes to health., Šárka Kunzová, Drahoslava Hrubá, and Literatura

- Description:
- Závažným zdravotním problémem rozvinutých zemí jsou chronická neinfekční onemocnění, jejichž etiologie má komplexní charakter. V prevenci i léčbě těchto chorob hrají důležitou roli faktory životního stylu. Životní styl je charakterizován vzorci chování, které komplexně působí na psychické i fyzické zdraví jedince, ovlivňují vznik a rozvoj nemocí, jejich průběh, úspěšnost léčby a kvalitu života. Chování tvořící životní styl je formováno faktory environmentálními, psychosociálními i genetickými, které vzájemně interagují a mají potenciál modifikovat genetické riziko komplexních chorob ovlivněním genové exprese. Tyto poznatky korespondují s bio-psycho-sociálním pojetím zdraví a nemoci a akcentují význam chování a možností jeho změny., Chronic non-communicable diseases with the complete determinants are serious health problems in developed countries. Lifestyle factors are important in prevention and treatment of disease. Lifestyle is characterized by patterns of behaviour and plays a comprehensive role in both mental and physical health and the quality of life. Behavioural, environmental, psychological and genetic elements form the lifestyle, are interactive and can modify the hereditary risk of complex diseases influencing genetic expression. These findings correspond to the bio-psycho-social concept of health and illness and highlight the importance of health-related behaviour and its change., and Šárka Kunzová, Drahoslava Hrubá

Chování směřující ke zdraví a způsoby jeho ovlivnění mají věková specifika., Chronic non-infectious diseases are a major health problem in the developed countries. Lifestyle plays an important role in their complex aetiology. Affecting the behaviour forming the lifestyle therefore has a strong preventive potential. Cognitive theories and behavioural models illustrate psychological factors participating in the behavioural change and enable to create effective preventive strategies that are consistent with the bio-psycho-social concept of health and disease. Behavioural interventions are used in both primary and secondary prevention, have effects on the individual and population levels, enhance the quality of life and reduce healthcare costs.
