Background: There is emerging molecular evidence concerning the modification of miR expression pattern after the treatment with chemopreventive agents. We investigated whether a potential chemopreventive agent called "Procont" deriving from a biological system has a positive effect on the DMBA induced dysregulation of let7 a, miR21 and miR146a miRNAs. Materials and Methods: CBA/CA H2k inbred mice were fed by Procont for 7 days before the intraperitoneal injection of DMBA. After one week of the beginning of Procont diet we analized the let7a, miR21, miR146a gene expression in vital organs of mice. Results: The Procont feeding significantly decreased all of the investigated miRNA expression compared with either the mice on a normal diet in the control group or mice were exposed to DMBA alone. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. Autor předkládá kritický rozbor současné situace v poskytování pracovnělékařských služeb v České republice. Společnost pracovního lékařství České lékařské společnosti J.E.Purkyně vyvinula v období několika posledních let maximální komplexní úsilí pro zajištění takové pozice pracovního lékařství mezi lékařskými obory i mezi poskytovateli zdravotnických služeb, jaká je obvyklá ve vyspělých evropských zemích. Součástí tohoto úsilí byla i harmonizace vzdělávání v pracovním lékařství, mimo jiné za účelem zajištění přiměřeného standardu poskytování služeb podnikům. Tyto snahy, ač podpořené a zdůvodněné stanovisky a doporučeními mezinárodních i tuzemských institucí (ICOH, UEMS, WHO, MPSV), nenašly žádoucí odezvu na MZ ČR. Výsledkem je proto problematická právní úprava postgraduálního vzdělávání lékařů v pracovním lékařství a absence povinného vzdělávání zejména praktických lékařů v pracovním lékařství. Právní úprava pracovnělékařských služeb, jejíž přijetí bylo po desítky let předpokládáno v ustanovení § 40 zákona č. 20/1966 Sb., nebyla realizována ani po vstupu ČR do EU. Od 1. dubna 2012 zákonem č.373/2011 Sb., o specifických zdravotních službách, jenž je součástí tzv. zdravotnické reformy, má být vyřešena i právní úprava pracovnělékařských služeb. Vládní návrh tohoto zákona v důvodové zprávě konstatuje, že je nezbytné, aby pracoviště, na nichž se vykonávají práce druhé rizikové kategorie, posuzoval lékař se znalostí pracovních podmínek a obecně problematiky bezpečnosti a ochrany zdraví při práci (BOZP) - každoročně je totiž zaznamenáván výskyt nemocí z povolání i u těch osob, které vykonávají práci zařazenou do kategorie druhé, přičemž podíl těchto nemocí z povolání na celkově přiznaných nemocech z povolání činí cca 25 až 26%. Přesto paradoxně zákon umožňuje, aby pracovnělékařské služby v plné šíři mohl poskytovat všeobecný praktický lékař nebo pracovní lékař, přičemž absolvování „akreditovaného kurzu zaměřeného na pracovnělékařské služby“ je považováno za vhodné, nikoliv za obligatorní (od roku 1995 do roku 2011 absolvovalo obdobné kurzy dobrovolně 422 praktických lékařů). Zajišťování kvality je šancí pro zlepšení přijatelnosti pracovních lékařů v podnicích/smluvních lékařů podniků a rovněž výzvou pro pracovní lékařství. Kompetenci pracovních lékařů lze klientům znázornit výrazněji a srozumitelněji pomocí prokazatelného zajištění kvality - potřeba zajištění zdraví zaměstnanců při práci je tak pro podnik snadněji pochopitelná. Možné konkurenční výhody pro lékaře samotné i ochrana před neoprávněnou kritikou hovoří rovněž pro zprůhlednění kvality a účinnosti prováděných činností. During the last several years the Society of Occupational Medicine of the Czech Medical Association of Jan Evangelista Purkyne has exerted maximum and complex efforts to ensure such a position of occupational medicine both among medical fields, as well as among health care service providers, as is customary in developed European countries. A part of this efforts has also been harmonization of education in occupational medicine, inter alia, for the purpose of ensuring adequate standard service provision to businesses. In spite of being supported and substantiated by statements and recommendations of international and domestic institutions (ICOH, UEMS, WHO, MPSV), these ambitions did not find desirable reaction from the Ministry of Health of the Czech Republic). Therefore, it resulted in a questionable legal regulation of postgraduate education of physicians in occupational medicine and in absence of mandatory education of especially general practitioners in the field of occupational medicine. The legal regulation of occupational medicine services, the adoption of which was anticipated in the provision of Section 40 of the Act no. 20/1996 Coll., was neither implemented after accession of the CR to the EU. Since April 1, 2012 the legal regulation of occupational medicine services should be tackled along with the Act no. 373/2011 Coll., on Specific Medical Services, which is a part of the so-called Health care reform. The government bill of this act states in the explanatory memorandum that it is crucial to have the workplace, where the second risk category works are being performed, assessed by a physician with the knowledge of working conditions and generally with the knowledge of occupational safety and health (OSH) related issues - as annual occurrence of occupational diseases is recorded also in those persons who perform the work ranked in the second category, while the share of these occupational diseases in the overall number of acknowledged occupational diseases amounts to approx. 25 to 26%. Yet, paradoxically, the act allows that occupational medicine services are provided within full scope by a general practitioner, or an industrial physician and at the same time the passing of "accredited occupational medicine services course" is considered to be suitable, however not mandatory (since 1995 to 2011, similar courses were passed voluntarily by 422 general practitioners). Assurance of quality is a chance for improvement of acceptability of industrial physicians in businesses/contractual physicians, as well as a challenge for industrial medicine. The competence of industrial physicians can be illustrated in a more pronounced and comprehensible manner by means of provable assurance of quality - the need to ensure health of employees at work is therefore more easily understandable for the business. The prospective competitive advantage for physicians themselves, as well as protection against unauthorized criticism, also speaks for making more transparent the quality and efficiency of performed activities. V souvislosti s ochranou lidských práv zaručených Úmluvou o ochraně lidských práv a základních svobod se v poslední době čím dál častěji diskutuje otázka, zda přístup k asistované reprodukci také spadá pod rámec ochrany poskytovaný Úmluvou. Příspěvek přináší přehled rozhodnutí Evropského soudu pro lidská práva, která se problematikou asistované reprodukce zabývaly. These cases are concerning very controversial ethical aspects of biomedical developments in the field of medical techniques of artificial procreation and reshape the content (meaning) of Article 8 of European Convention of Human Rights. This paper provides short overview of cases of European Court of Human Rights involving “right to access to assisted reproduction” and focuses especially on the latest developments in this field (case S.H. and Others v. Austria - Application no 57813/00) . Although the technology of medically assisted procreation had been available in Europe for several decades, many of the issues to which it gave rise, remained the subject of debate. There is no consensus towards the use of IVF treatment in general and even less clear are trends in some particular questions (f.e. allowing gamete donation). To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Toxoplasmóza je jedna z nejčastějších zoonóz u nás. Patří mezi onemocnění, která mohou při primoinfekci v graviditě ohrozit vývoj plodu či způsobit mnohočetná poškození, často manifestovaná až po narození dítěte. Závažnost postižení, jeho variabilita a pravděpodobnost vzniku závisí na době, kdy se těhotná žena nakazila. V ČR máme k dispozici několik diagnostických metod. Základem je sérologické vyšetření (protilátky celkové i jednotlivých tříd, avidita IgG, komparativní imunoblot), prováděné z krve či plodové vody. Možný je i přímý průkaz DNA Toxoplasma gondii pomocí PCR z krve, plodové vody či po porodu z placenty. Prenatálně se provádí izolační pokus na myši (inokulací plodové vody). Pomocnou metodou je také ultrazvuk či MRI v průběhu těhotenství, se zaměřením na případnou fetopatii. Po narození je dítě z rizikové gravdity podrobně vyšetřeno a dlouhodobě sledováno. Terapii lze podat prenatálně i postnatálně, o jejím efektu se vedou diskuze. Základními preparáty jsou spiramycin, pyrimethamin a sulfadiazin (doplněny kyselinou folinovou). V článku jsou probrány indikace k jednotlivým vyšetřením a doporučeny nejvhodnější postupy v diagnostice, léčbě a také prevenci tohoto onemocnění u těhotných. Text se podrobně věnuje i interpretaci možných výsledků a obsahuje vyjádření k preventivnímu vyšetření těhotných žen u nás., Toxoplasmosis is one of the most common zoonoses in the Czech Republic. Toxoplasma infection during pregnancy can affect fetal development and cause multiple damages that often do not manifest until after the birth. The severity of pediatric disability, its variability and the probability of occurrence depend on the time of maternal infection. Serological diagnosis of toxoplasmosis in pregnant women is performed in two steps: first, total anti-Toxoplasma antibodies are detected, most commonly by the complement fixation test (CFT) combined with IgG ELISA. In the case of a negative result, the woman can be retested 1-3 months later. Low CFT titers show latent infection with a minimal risk of primary infection during pregnancy. When CFT titers of 1:64 or higher are obtained, the second step follows. Positivity in the IgM and IgA tests and low IgG avidity can confirm a recent infection. The diagnosis of fetal infection is based on the detection of anti-Toxoplasma antibodies, and of IgM antibodies in particular, and/or on the detection of Toxoplasma in amniotic fluid by PCR or the mouse test. Other helpful methods are fetal ultrasound or MRI focusing on fetopathy. Each child born from a high-risk pregnancy is thoroughly examined and followed up on a long-term basis. The detection of neonatal anti-Toxoplasma IgM antibodies and a neonatal IgG profile differing from the maternal IgG profile in comparative Western blot are the major markers of congenital infection. Toxoplasma infection can be treated prenatally and postnatally and the therapeutic outcomes are still being considered. The first-line options for congenital toxoplasmosis are spiramycin, pyrimethamine and sulfadiazine with folinic acid. The article discusses the indications for screening and recommends best practices for the diagnosis, treatment and prevention of toxoplasmosis during pregnancy. Článek pojednává o principech lékařské etiky a je volným pokračováním první části článku, ve kterém se autorky také zabývaly principy lékařské etiky, ovšem z jiné (širší) perspektivy. Principy lékařské etiky můžeme rozdělit na primární principy, sekundární principy a alternativní principy (alternativy, modifikace). Primární principy (základní principy, tzv. čtyři principy lékařské etiky, principy prvního řádu) jsou základními pilíři lékařské etiky. Determinují etické uvážení v této oblasti. Zahrnují principy – neškodění (nonmaleficence), konání dobra (beneficence), autonomie a spravedlnosti. K sekundárním principům náleží: pravdomluvnost, mlčenlivost, věrnost, poctivost a důvěryhodnost. Alternativami (modifikacemi) stěžejních principů jsou: lékařská etika založená na ctnostech, lékařská etika vycházející z péče o nemocné a lékařská etika vycházející z kazuistického přístupu., The text deals with the principles of medical ethics and it is free following of the first part of the article in which the authors were interested in the principles of medical ethics but from the other (wider) view. The principles of medical ethics can be devided into primary principles, secondary principles and alternative principles (alternatives, modifications). The primary principles (basic principles, so called four principles of medical ethics, the principles of the first class) are the basic columns of medical ethics. They determine ethics meaning in this area. They comprise these principles – harmlessness (nonmaleficence), doing goodness (beneficence), autonomy and justice. Secondary principles comprise: truthfulness, secrecy, faithfulness, morality and credibility. The text called „The Principles of Medical ethics – 1st part“ is the beginning „enter“ to problem of medical ethics principles. The principles have their irreplaceable place in medical ethics. It isn´t possible to talk about principles without a connection with norms (rules), institutions and limits. A concept norm started to use for signing what is morally right until 19th century. Norms work in the society as regulators of peoples´ actions containing orders, prohibitions or permissions. Moral norms will never stop mens´ exceeding. Of course, there can be given reasons and justification with re-calling to the values like (freedom, life, health…) or there can be general principles (dignity, justice…). Only reasonable and justified moral norms can establish another expectations in human subjects. The text is oriented on principles of medical ethics. The doctor naturally has to know four basic principles of medical ethics. He has to know theoretical and practical level of these principals. Holding norms and principles are necessary part of doctor´s practice. Tento článek pojednává o problému cetérů nedaří zastavit. Přitom důsledky obezity jsou značné: zhoršování zdravotního stavu a ekonomické ztráty. V poslední 40 letech došlo ke znatelnému pokroku ve vývoji teoretických poznatků o obezitě. Významným posunem je to, že obezita začala být nahlížena jako komplexní a dynamický problém, který je výsledkem působení mnoha faktorů, z nichž mnohé byly a stále jsou opomíjeny (zejména socioenvironmentální faktory) a přetrvává zažitý biomedicínský přístup, který neřeší problém jako celek a nepřináší tak žádné zlepšení. Je tedy nutné, aby tvůrci politik začaly tyto nové poznatky reflektovat, aby došlo k revizi dosavadních postupů řešení problému obezity a aby byl problém řešen systémově, tj. cestou víceúrovňové a víceoborové spolupráce se zapojením širokého okruhu aktérů i ze strany soukromého a občanského sektoru., This article deals with a problem of worldwide obesity epidemic, whose growing trend over considerable endeavour of many participants goes badly to a stop. Withal consequences of obesity are considerable: worsening of health and economic waste. In last 40 years it has come to a considerable progress in the development of theoretical knowledge in obesity. There is a significant shift: the obesity has begun to be regarded as a complex dynamic problem, which results from the coincidence of many factors, from which many of them have been neglected (especially socio-environmental factors), and there still persists the biomedical approach, which does not solve the problem as a whole and therefore brings no improvement.
