Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Inversion of uterus is very rare and serious complication of childbirth. It is a situation with very high maternal morbidity and mortality, whitch can lead to developement of very serious periparthal bleeding, DIC, neurogenic and haematologic shock. V předložené kazuistice je zachycena práce rodinného terapeuta s rodinou, kde matka trpí bolestmi zad a dítě (3,5) je chronicky nemocné. Trpí kašlem nejasného původu. Autorka podrobně popisuje průběh šesti sezení, během nichž se v rodině postupně měnila atmosféra. Konflikty, které pár značně zatěžovaly, měly své kořeny v minulosti obou rodin a v mylných nevyslovovaných předpokladech na obou stranách. Narativní terapie s respektem ke struktuře a genderové vyváženosti, kterou se autorka učila v rámci čtyřletého výcviku v institutu rodinné terapie psychosomatických poruch v Liberci, byla zde hlavním nástrojem léčby. Léčba se nemůže zaměřit přímo na symptom dítěte. Jde o symptom celého rodinného systému. Zjednodušeně řečeno, bylo třeba, aby se všichni mohli zase volně nadýchnout., In the presented casuistry there is given a description of a family therapist’s work with a family, where the mother suffers from backache and the child (3.5) is chronically ill suffering from cough of unknown origin. The author describes in detail the course of six sessions, during which the family atmosphere was gradually changing. The conflicts that significantly burdened the couple, had their roots in the past of both families and in wrong untold assumptions on both sides. Narrative therapy with respect to the structure and gender balance, which the author learned in the four-year training in the family therapy institute of psychosomatic disorders in Liberec, was the main tool of treatment. V tomto textu se zabývám formami zavádění PBSP do individuální práce s klientem v rámci běžné klinicko-psychologické praxe. Reflektuji v něm svou zkušenost s metodou, kterou od roku 2001 praktikuji v její základní podobě, tedy formou pravidelných, dlouhodobých, malých skupin. Vývoj terapeutické technologie PBSP posledních let však umožnil její širší využití také v individuální terapii. Zatímco v prvních desetiletích vývoje metody postupoval terapeutický proces v průběhu sezení rychle a často dramaticky „od těla ke slovu“ (bezprostřední prací s pocity ve speciálních cvičeních), což vyžadovalo téměř bezpodmínečně velmi specificky pojaté prostředí skupiny, dnešní přístup sleduje pozvolnější, jemnější i bezpečnější směr – „od slova k tělu“ (od rozhovoru k tělovým projevům, které význam slova evokuje). K možnostem i atmosféře individuálního psychoterapeutického uspořádání se tak PBSP dostala mnohem blíže. Naléhavá otázka však zůstala: jak vložit do dyády terapeut – klient METODU? Metodu, která není jen funkcí terapeutova myšlení, o něž se klient nemusí v posledku mnoho zajímat, ale která naopak vyžaduje jeho aktivní spoluúčast a nemalou kompetenci, která má velmi nezvyklou vnější podobu, navíc patinu zvláštního slangu?, The topic of the paper is: how to set up PBSP into one to one session format within an ordinary clinical psychology practice. I reflect my own professional experience with the method I have been using for years in its basic group form. However, recent developents of PBSP technology have facilitated it‘s use also in individual psychoteprapy. In the first decades of PBSP developement, the process of therapy was running on ahaed fast, often dramatically, due to body work that had been carried out immediately in special exercises. The current approach has become much slower, subtle and safe and therefore it fits better for the individual setting conditions. However, the basic question stayed remained: how to introduce A METHOD into dyad therapist – client?? Hlboká infiltrujúca endometrióza (DIE) je chronické progresívne estrogén dependentné ochorenie, ktoré môže postihovať aj štruktúry gastrointestinálneho traktu. Ochorenie je zvyčajne dlhodobo asymptomatické, respektíve sa prejavuje veľmi nešpecifickými príznakmi. Jeho neskorý záchyt môže viesť k ireverzibilným zmenám na jednotlivých štruktúrach. Táto kazuistika popisuje raritný prípad rozsiahlej hlbokej infiltrujúcej endometriózy zadného kompartmentu malej panvy, ktorá vyústila do stavu obštrukčného ilea.
