The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Úvod: Cílem prospektivní randomizované studie bylo porovnání tahové cerkláže a hákovité dlahy při léčbě AC luxací, především z hlediska funkčních a radiologických výsledků. Metoda: Soubor tvořilo 80 pacientů s akutní akromioklavikulární (AC) luxací typu 3, 4 a 5 Rockwoodovy klasifikace. Diagnóza byla stanovena na základě klinického (defigurace a instabilita) a rtg vyšetření (AP a zátěžové rtg). Tahovou cerkláží (TC) bylo ošetřeno 40 pacientů a hákovitou dlahou (HD) rovněž 40 pacientů. Hodnocení bylo prováděno v průběhu jednoho roku po operaci na základě rtg snímků a Constant score. Výsledky: Constantovo skóre 3 měsíce od operace činilo průměrně u TC 84 bodů a u HD 88 bodů. Po 1 roce od operace byl výsledek shodně 93 bodů u obou skupin. V souboru HD se hodnota skóre zvýšila mezi 2 a 4 týdny od operace z 56 na 78 bodů. V 71 případech bylo pooperační postavení AC kloubu a implantátu hodnoceno jako správné. U 6 případů jsme nalezli malpozici Kirschnerových drátů a ve 3 případech horizontální rozšíření AC kloubu. Redislokace v rozsahu 50–100 % šíře kosti akromia byla na rtg snímku patrná u 4 pacientů (10 %) ze skupiny TC a u 5 pacientů (13 %) ze skupiny HD. Viditelnou osteolýzu distální plochy akromia jsme nalezli u 83 % pacientů s HD. Komplikace jsme zaznamenali u 30 % pacientů s TC a u 5 % pacientů s HD. Závěr: Hákovitá dlaha i tahová cerkláž jsou srovnatelnými metodami při léčbě AC luxace hodnocené radiologicky a klinicky po 3 měsících a 1 roce od operace. Hákovitá dlaha má nižší počet komplikací a umožňuje časnější plnou zátěž a hybnost než TC. U TC doporučujeme odstranění implantátu za 8 týdnů, dobu 6 týdnů považujeme za příliš krátkou pro zhojení měkkých tkání. U HD je vhodné odstranění do 3 měsíců vzhledem k subakromiálnímu dráždění a tlakové osteolýze., Introduction: The aim of the prospective randomized study was to compare tension wire cerclage and hook plate in the treatment of AC dislocation, primarily from the viewpoint of functional and radiological results. Method: The cohort comprised 80 patients with acute acromioclavicular (AC) dislocation of types 3, 4 and 5 of Rockwood classification. The diagnosis was based on the clinical (disfiguration and instability) and radiographic examination (AP and stress radiograph). Forty patients were treated with tension band wiring (TBW) and another 40 with a hook plate (HP). Evaluation was performed during one year after the surgery based on radiographs and the Constant score. Results: The mean Constant score 3 months after the surgery was 84 points for TBW and 88 points for HP. One year after the surgery, the result was the same in both groups: 93 points. In HP group the score increased from 56 to 78 points between 2 and 4 weeks from the surgery. In 71 cases the postoperative position of the AC joint and implant was assessed as correct. Malposition of Kirschner wires was recorded in 6 cases and horizontal widening of the AC joint in 3 cases. Redislocation of up to 50−100% of the width of acromion was shown by radiograph in 4 TBW patients (10%) and in 5 HP patients (13%). A visible osteolysis of the distal surface of acromion was found in 83% of patients with HP. Complications were recorded in 30% of TBW patients and in 5% of HP patients. Conclusion: Based on radiological and clinical results assessed 3 months and 1 year after the surger, the hook plate and tension band wiring are comparable treatment methods for AC dislocation. The hook plate is associated with a lower complication rate and allows earlier full weight bearing and mobility than tension wire cerclage. In TBW we recommend to remove the implant after 8 weeks; 6 weeks are in our view too short a period for the healing of soft tissues. Konečné produkty pokročilé glykace (AGEs) hrají významnou roli v patogenezi diabetu i diabetických komplikací. Jejich hromadění v oční čočce odráží celkový stav glykace v organizmu. Nedávno byl na trh uveden nový konfokální biomikroskop ClearPath DS-120, který umožňuje rychlé a neinvazivní stanovení AGEs v oční čočce na základě autofluorescence (LAF) některých AGEs. V naší pilotní studii jsme vyšetřili LAF u 69 diabetiků a 49 zdravých kontrol a srovnali se současně měřenou kožní autofluorescencí (SAF) na předloktí pomocí AGE-Readeru. Bylo pozorováno významné zvýšení LAF (DM1T: 0,27 ? 0,09; DM2T: 0,22 ? 0,06; kontroly: 0,17 ? 0,04 AU; p < 0,0001) i SAF (DM1T: 2,0 ? 0,4; DM2T: 2,3 ? 0,6; kontroly: 1,8 ? 0,3 AU; p < 0,0001) u pacientů s diabetem oproti zdravým kontrolám. Ve všech skupinách byl popsán významný vztah mezi LAF a SAF (DM1T: r = 0,53; p < 0,005; DM2T: r = 0,37; p < 0,05; kontroly: r = 0,30; p < 0,05). Naopak závislost LAF ani SAF na HbA1c nebyla přesvědčivá, což asi souvisí s tím, že hodnota glykovaného hemoglobinu dostatečně neodráží mnohaletý stav glykace v organizmu. Stanovení oční autofluorescence možná přináší robustní marker dlouhodobé kompenzace diabetu predikující riziko budoucích komplikací, avšak definitivní potvrzení takové hypotézy si vyžádá četnější a delší klinické studie., Advanced glycation end-products (AGEs) play a crucial role in the pathogenesis of diabetes and its complications. Their accumulation in the lens reflects total glycation rate in the human body. Recently, a new confocal biomicroscope ClearPath DS-120 quickly measuring lens autofluorescence (LAF) has been developed. Our pilot study included 69 patients with diabetes and 49 healthy controls, in all subject LAF was measured and compared with skin autofluorescence (SAF) assessed by AGE-Reader. Both LAF (T1DM: 0,27 ? 0,09; T2DM: 0,22 ? 0,06; controls: 0,17 ? 0,04 AU; p < 0,0001) and SAF (T1DM: 2,0 ? 0,4; T2DM: 2,3 ? 0,6; controls: 1,8 ? 0,3 AU; p < 0,0001) was significantly higher in patients with diabetes. In all groups significant relationship between LAF and SAF was observed (T1DM: r = 0,53, p < 0,005, T2DM: r = 0,37, p < 0.05; controls: r = 0,30, p < 0,05). On the contrary, LAF and SAF relationship with glycated hemoglobin (HbA1c) was rather poor, since HbA1c cannot wholly reflect long-term glycation process. Lens autofluorescence could be a robust marker of long-term diabetes control predicting future complication risks. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Typ studie: retrospektivní. Cíl: Cílem studie bylo zjistit úspěšnost porodů u žen s císařským řezem v anamnéze na našem pracovišti. Rovněž bylo cílem určit rizikové faktory pro úspěšnost porodu po předchozím císařském řezu a míru rizika, kterou tyto pacientky podstupují. Metodika: Výzkumný soubor tvořilo 1 060 porodů, které na našem pracovišti proběhly ve sledovaném období. Z toho 103 žen mělo císařský řez v anamnéze a 70 z nich se pokusilo porodit vaginálně. Výsledky: Z 1 060 porodů bylo 19,2 % sekcí, ze 70 TOLAC (trial of labour after caesarean – pokus o spontánní porod po císařském řezu) bylo 54,3 % úspěšných (porodily vaginálně), ze 13 indukcí po sekci bylo úspěšných 23,1 %. Jako prediktivní faktor pro úspěšný TOLAC byl prokázán vaginální porod v anamnéze (p = 0,00), BMI < 35 (p = 0,04) a trend naznačil věk rodičky < 25 let (p = 0,05). Byl prokázán signifikantní rozdíl v krevní ztrátě (měřené odhadem) mezi pokusem o vaginální porod po sekci a bez sekce v anamnéze (p = 0,000), pokusem o vaginální porod po sekci a primární sekcí po sekci (p = 0,000) a mezi úspěšným a neúspěšným pokusem o vaginální porod po sekci (p = 0,000). Byl rovněž prokázán signifikantní rozdíl Apgar(ové) skóre v 1. a 5. minutě u dětí z úspěšných a neúspěšných pokusů o vaginální porod po sekci. Tyto výsledky byly dány do kontextu s doporučenými postupy z USA, Kanady, Spojeného království, Německa, Rakouska a Austrálie a Nového Zélandu. Závěr: Studie poskytla informace, se kterými je možné seznámit pacientku plánující pokus o vaginální porod po sekci na našem pracovišti. We also wanted to determine the predictive factors for successful vaginal birth and the amount of risk which the vaginal birth after caesarean brings to the patient. Metodics: There were 1,060 births in our study, that took place in our clinic at the specified time. Out of this set there were 103 women, who had caesarean delivery in medical history and 70 of those tried to deliver vaginally. Results: Out of 1,060 births there were 19.2 % caesarean sections, from 70 patients who tried to give birth vaginally after caesaren (TOLAC – trial of labour after caesarean) 54.3 % succeeded (the rest gave birth per sectionem caesaream), out of 13 induced labours after caesarean there were only 23.1 % successful. We determined following predictive factors for successful TOLAC: at least one vaginal birth in medical history (p = 0.00), BMI under 35 (p = 0.04) at the time of birth and age under 25 (p = 0.05). We found significant difference in the blood loss between following groups: TOLAC vs deliveries without caesarean in medical history (p = 0.000), TOLAC vs. elective repeat sections (p = 0.000) and successful vs. unsuccessful TOLAC (p = 0.000). We also found significant difference in Apgar score in first and fifth minute between the group of successful TOLAC and unsucessful TOLAC. These results were compared with the current guidelines in UK, USA, Canada, Australia and New Zealand, Germany and Austria. Conclusion: This study has brought us data, that should be provided to the patients who think about giving birth in our clinic and who have already caesarean section in medical history. Neuroendokrinné nádory (NEN) vychádzajú z buniek difúzneho neuroendokrinného systému. Produkujú peptidy alebo amíny, ktoré účinkujú ako hormóny alebo neurotransmitery. Incidencia NEN je pomerne nízka. Diagnostika a liečba týchto tumorov je multidisciplinárna. Cieľom práce bola analýza súboru pacientov s dobre diferencovaným neuroendokrinným nádorom tráviaceho traktu. Zaradení boli pacienti sledovaní od roku 1998 do roku 2013, ktorí mali histologicky potvrdený dobre diferencovaný, nízko alebo stredne malígny neuroendokrinný nádor gastrointestinálneho traktu. Do súboru bolo zaradených 97 pacientov. Z toho 34 (35,1 %) mužov a 63 (64,9 %) žien. Zistili sme, že v liečbe pacientov so stanovením diagnózy po roku 2005 sa signifikantne menej využíva interferón a signifikantne častejšie sa využíva endoskopická a PRRT liečba. Identifikovali sme najvhodnejší diskriminant pre rozdelenie pacientov z hľadiska výskytu metastáz v čase stanovenia diagnózy podľa hladiny 5-HIAA na 6,8 mg/24 hod, podľa hladiny CgA na 70 ng/ml. Identifikovali sme nasledovné rizikové faktory pre celkovú mortalitu: pečeňové metastázy, hnačky, flush, lokalizácia primárneho tumoru v tenkom čreve, vysoké hodnoty CgA a 5-HIAA v čase stanovenia diagnózy (5-HIAA > 520,52 mg/24 hod, CgA > 174,5 ng/ml). Ako pozitívny prognostický faktor sme identifikovali chirurgickú liečbu. They produce peptides or amines that act as hormones or neurotransmitters. Incidence of NENs is relatively low. Diagnostic work-up and treatment requires a multidisciplinary team approach. The aim of this study was an analysis of data from patients with well-differentiated neuroendocrine neoplasmas of gastrointestinal tract. The study included patients followed up from 1998 to 2013 with histologically confirmed well-differentiated digestive neuroendocrine neoplasm with low or intermediate malignant potential. 97 patients were included; 34 men (35.1 %) and 63 women (64.9 %). In patients being diagnosed after 2005 interferon treatment is significantly less used than endoscopic and peptide receptor radionuclide therapy. We have identified more appropriate discriminant values of 5-HIAA and chromogranin A (6.8 mg/24 hours; 70 ng/ml) for predicting the presence of metastases at the time of diagnosis. We have identified following risk factors for overall mortality: liver metastases, presence of diarrhea, flush, small bowel primary tumor, high values of CgA and 5-HIAA at the time of diagnosis (5-HIAA > 520.52 mg/24 hours, CgA > 174.5 ng/ml). Surgical treatment was found to be a positive prognostic factor. Úvod: Recidíva pectus excavatum je najzávažnejšou neskorou komplikáciou primárnej korekcie. V indikovaných prípadoch je možné pacientovi odporučiť rekorekciu modifikovanou Ravitchovou technikou (HMRR), alebo minimálne invazívnu korekciu (MIRPE). Práca sa zameriava na vyhodnotenie dostupných operačných techník u dospelých pacientov a adolescentov s recidívou pectus excavatum. Metódy: Autori retrospektívne vyhodnotili výsledky svojej práce. V období od júna 2006 do októbra 2014 odoperovali na Klinike hrudníkovej chirurgie 126 výkonov pre pectus excavatum. HMRR bolo metódou voľby v 51 prípadoch a MIRPE v 75. Korekcia pre recidívu pectus excavatum bola indikáciou pre 12 (9,5 %) výkonov u 11 pacientov. Tí absolvovali šesť korekcií technikou HMRR a 6 technikou MIRPE. HMRR ako primárny výkon podstúpilo 10 pacientov a MIRPE jeden. Medián veku súboru bol 23,5 (17−44) roku, medián odstupu od prvej operácie bol 9,5 (2−31) roku. Muži tvorili 75 % operantov. Výsledky: Napriek potrebe torakoskopickej adheziolýzy a implantácie dvoch dláh u všetkých pacientov sa MIRPE vyznačovala kratším operačným časom (120 vs. 172 min) a menším počtom komplikácií (16,7 % vs. 50 %). V skupine s HMRR sme zaznamenali tri komplikácie (ranový seróm, migráciu dlahy do perikardu s masívnym hemoperikardom, oddelenie priamych brušných svalov od sterna) a v skupine s MIRPE jednu (symptomatický fluidotorax). U pacienta s migráciou dlahy vznikla ďaľšia recidíva pectus excavatum. Nezaznamenali sme rozdiel v dľžke pooperačnej hospitalizácie. Záver: Napriek limitovaným skúsenostiam považujeme MIRPE v indikovaných prípadoch za bezpečnú a efektívnu techniku pri riešení recidívy u dospelých pacientov a adolescentov., Introduction: Recurrent pectus excavatum is the most serious late complication after primary repair. Redo open repair (Highly Modified Ravitch Repair, HMRR) or minimally invasive repair of pectus excavatum (MIRPE) are usually performed in indicated cases. This paper focuses on the evaluation of available redo surgical techniques in adult and adolescent patients with recurrent pectus excavatum. Methods: 126 operative corrections, predominantly in adult patients, were performed by the authors between June 2006 and October 2014. HMRR was the method of choice in 51 cases, and MIRPE in 75 cases. Recurrent pectus excavatum was the indication in 12 repair procedures (9.5%) in 11 patients. Prior repairs included HMRR in 10 patients and MIRPE in one case. Both HMRR and MIRPE were indicated as redo procedure in six cases. The median age in the redo group was 23.5 (17−44) years and the median interval between the primary correction and the redo procedure was 9.5 (2−31) years. The male to female ratio was 3:1. Results: The use of MIRPE resulted in shorter operation time (120 vs. 172 min). There was no difference in the length of postoperative hospitalisation. Three complications (50%) were recorded in the HMRR group (wound seroma, intrapericardial bar migration with hemopericardium, displaced rectus abdominis muscle) and one (16.7%) occurred in the MIRPE group (symptomatic fluidothorax). Conclusion: Despite our limited experience with both techniques in the treatment of recurrent pectus excavatum we believe that MIRPE should be regarded as a safe and effective technique for the redo repair in adolescent and adult patients., and M. Úvod: Rozsah nádoru a kvalita jeho chirurgického odstranění patří k hlavním prognostickým faktorům v léčbě karcinomu rekta. Počet pooperačních lokálních recidiv závisí na úplnosti odstranění mezorekta – TME (total mesorectal excision) – a dosažení bezpečných resekčních okrajů bez nádoru. Provedení a kvalita operace záleží na předoperační diagnostice a detekci rizikových míst na rektu a mezorektu, na chirurgické schopnosti výkon správně provést a na patologickém vyšetření, hodnotícím úplnost odstranění nádoru v parametrech resekčních okrajů včetně cirkumferentního (CRO) a kompletnost mezorektální excize. Cílem práce bylo zavést a standardizovat metodu hodnocení kvality chirurgického výkonu − mezorektální excize – v léčbě karcinomu rekta hodnocením resekčních okrajů včetně CRO a kompletnosti excize. Metody: Studie měla dvě části. V první, retrospektivní části multicentrické studie byly v sestavě 288 pacientů analyzovány jednotlivé dílčí parametry diagnostiky, operací a histologického vyšetření karcinomu rekta, identifikována kritická místa a připraven jednotný protokol sledování. Ve druhé, prospektivní části studie byla v sestavě 600 pacientů tato data sledována parametricky a byla hodnocena kvalita TME ve vztahu k onkologickému výsledku léčby. Výsledky: V předoperační diagnostice se zvýšil podíl pacientů, kterým byl proveden restaging po neoadjuvantní léčbě z 60,0 % na 81,7 %. Podíl resekátů, u nichž nebyla hodnocena kvalita excize mezorekta, se snížil z 52,9 % v retrospektivní části na 22,8 % v části prospektivní. Podíl kompletních excizí mezorekta se zvýšil z 22,6 % na 26,0 %, podíl téměř kompletních excizí se zvýšil z 10,1 % v retrospektivní části na 24,0 % v prospektivní části studie. Závěr: Zavedení parametrického sledování do rutinní praxe zvýšilo kvalitu předléčebné a předoperační diagnostiky, vyšetření resekátu patologem a vlivem zpětnovazebné informace také kvalitu chirurgického výkonu., Introduction: Tumour size and the quality of its complete surgical removal are the main prognostic factors in rectal cancer treatment. The number of postoperative local recurrences depends on whether the mesorectum has been completely removed – total mesorectal excision (TME) – and whether tumour-free resection margins have been achieved. The surgery itself and its quality depend on the accuracy of preoperative diagnosis and detection of risk areas in the rectum and mesorectum, on the surgeon’s skills, and finally on pathological assessment evaluating whether complete tumour excision has been accomplished including circumferential margins of the tumour, and whether mesorectal excision is complete. The aim of our study was to implement and standardize a new method of evaluation of the quality of the surgical procedure – TME – in rectal cancer treatment using an assessment of its circumferential margins (CRO) and completeness of the excision. Methods: The study consisted of two parts. The first, multi-centre retrospective phase with 288 patients analysed individual partial parameters of the diagnosis, operations and histological examinations of the rectal cancer. Critical points were identified and a unified follow-up protocol was prepared. In the second, prospective part of this study 600 patients were monitored parametrically focusing on the quality of the TME and its effect on the oncological treatment results. Results: The proportion of patients with restaging following neoadjuvant therapy increased from 60.0% to 81.7% based on preoperative diagnosis. The number of specimens missing an assessment of the mesorectal excision quality decreased from 52.9% in the retrospective part of to the study to 22.8% in the prospective part. The proportion of actually complete TMEs rose from 22.6% to 26.0%, and that of nearly complete TMEs from 10.1% to 24.0%. Conclusion: The introduction of parametric monitoring into routine clinical practice improved the quality of pre-treatment and preoperative diagnosis, examination of the tissue specimen, and consequently improved quality of the surgical procedure was achieved., and J.
