Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Pneumatizace okcipitální kosti a krčních obratlů je vzácný nález a při jeho náhodném zobrazení radiologickými metodami může vyvolat rozpaky. Nejčastěji je tento stav spojován s dysfunkcí Eustachovy trubice a často se opakujícím Valsalvovým manévrem. V této práci popisujeme náhodný nález kraniocervikální pneumatizace u pacienta s úrazem na kole a následnými polykacími problémy a potížemi s mluvením. Pro vznik kraniocervikální pneumatizace u tohoto pacienta se jeví klíčové opakované rýmy a záněty paranazálních dutin a rovněž záliba v potápění. Klinické potíže, trvající krátkodobě, jsme si vysvětlili současným nálezem pneumorrhachis., Pneumatization of the occipital bone and cervical vertebrae is observed infrequently and its incidental discovery by radiological imaging methods may be confusing. Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases. V krátkém sdělení dokumentujeme u pacienta s gliomem postihujícím precentrální gyrus relativitu výsledku zobrazení pyramidové dráhy pomocí DW traktografie a diskutujeme o faktorech, které mohou ovlivnit zobrazení dráhy. Dle našeho názoru se v dostupné literatuře nevěnuje dostatečná pozornost volbě oblasti zájmu trasování, přičemž právě ta je klíčovým determinantem výsledku., In our short report, we show the relative nature of pyramidal tract representation using Diffusion Tensor Imaging in a patient with a precentral gyrus glioma and we discuss the factors that may affect tract reconstruction. We believe that, so far, inadequate attention has been paid in the published literature to the region of interest selection even though this is the key determinant of the imaging outcome. Po dva roky trvajících bolestech bederní páteře s narůstajícími klaudikacemi dolních končetin byl u 61leté ženy diagnostikován rozsáhlý tumor v oblasti těla L3 a retroperitonea. Nádor byl totálně odstraněn a provedena náhrada těla L3 a stabilizace. Po operaci přetrvávaly výrazné bolesti, oslabení dolních končetin a instabilita stoje a chůze. Po léčebné rehabilitaci došlo k vymizení bolesti, ústupu nestability i parézy levé dolní končetiny. Vývoj pooperačního stavu dokumentuje stabilometrické vyšetření., After 2-year period of persistent pain of lumbar spine with increasing intensity of lower extremities claudication a vast tumor of L3 vertebra extending to the retroperitoneum was diagnosed in a 61-year old woman. The total L3 vertebrectomy as well as complete tumor removal has been performed and situation was reconstructed and stabilized. Pronounced pain, lower extremities paresis and instability persisted after the surgery. The rehabilitation therapy led to the relief of the pain improvement of standing and gait instability and paresis.
