Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Monoklonální gamapatie nejasného významu (MGUS) je definována jako klinicky němý bezpříznakový stav, při kterém nejsou splněna diagnostická kritéria mnohočetného myelomu (MM), Waldenströmovy makroglobulinémie, AL-amyloidózy nebo jiného zhoubného lymfoproliferativního onemocnění. Dlouhodobým sledováním bylo prokázáno, že jde o možnou prekancerózu s přechodem v maligní monoklonální gamapatií v přibližně 1 % pozorování za rok, pravděpodobně všechny případy MM vznikají transformací z MGUS. V rámci přehledného sdělení se autor snaží odpovědět na otázku, zda je v současnosti možné nějakým způsobem transformaci MGUS v MM zabránit nebo ji alespoň zpomalit. Odpovědí u MGUS pacientů je zatím jednoznačné NE. Vzhledem k mnohem vyšší míře rizika transformace u nemocných s asymptomatickým mnohočetným myelomem než je tomu u pacientů s MGUS, budou další analýzy CMG (Česká myelomová skupina) zaměřeny na tuto skupinu nemocných s cílem vytipovat nejrizikovější skupinu nemocných, a tu poté podrobit časné a pokud možno netoxické léčbě., Monoclonal gammopathy of undetermined significance (MGUS) is defined as a clinically silent symptomless condition where not met diagnostic criteria for multiple myeloma, Waldenström macroglobulinemia, AL-amyloidosis or other malignant lymphoproliferative disease. Long-term monitoring has shown that it is possible to premalignant progression to malignant monoclonal gammopathy in approximately 1% of cases a year, probably all cases of MM arise transformation from MGUS. Within an easy communication, the author tries to answer the question whether it is currently possible to somehow transform MGUS in MM to prevent or at least slow down. The response in MGUS patients is not for now. Úvod: Scintigrafie je optimální metodou pro molekulární zobrazování, je tedy vhodná pro vyšetřování pacientů s me-tabolickými poruchami. Diabetická kardiomyopatie se klinicky projevuje jako srdeční selhání bez přítomnosti koronární aterosklerózy, hypertenze či chlopenní vady. Její příčina není zcela objasněna, nejspíše je multifaktoriální s podílem poruchy buněčného metabolismu včetně inzulinové rezistence, poškození mikrocirkulace či endoteliální dysfunkce, autonomní neuropatie a intersticiální fibrózy. Materiál a metody: Scintigrafie umožňuje sledovat řadu patofyziologických mechanismů, které se mohou na rozvoji diabetické kardiomyopatie podílet. V experimentu se jedná především o poruchy buněčného metabolismu. Ze substrátů lze použít glukózu, mastné kyseliny, acetát. Scintigrafické metody jsou také vhodné pro diagnostiku poruchy mikrocirkulace s následnou poruchou prokrvení myokardu. K tomu slouží zejména perfuzní radiofarmaka nejvíce používaná v rutinní klinické praxi. Pro vyšetřování poruch autonomní inervace se používají metabolická analoga noradrenalinu nebo přímé značení katecholaminů. Výsledky: U diabetiků je narušena metabolická plasticita myokardu, snižuje se schopnost utilizace glukózy se zvýšeným hromaděním mastných kyselin. To má za následek poruchu funkce myocytů. Jednak dochází ke snížení produkce energie se snížením kontraktility, jednak se zvyšuje lipotoxicita vedoucí až k apoptóze s následnou fibrózou myokardu. Prokázaná porucha prokrvení myokardu se vyskytuje u 20 - 40 % asymptomatických diabetiků. Rovněž se u diabetiků prokázala snížená vasodilatační kapacita i bez postižení epikardiálních tepen. U diabetiků byla rovněž prokázaná změna regionální distribuce sympatické inervace v myokardu levé komory. Závěr: Scintigrafické metody mají velký diagnostický potenciál. Mohou umožnit včasné zahájení efektivní léčby. V klinické praxi se však musí používat uvážlivě a cíleně zejména u symptomatických a vysoce rizikových pacientů., x, Otto Lang, and Literatura
Kolorektální karcinom je druhým nejčastějším maligním onemocněním ve vyspělých zemích. Vzhledem k tomu, že proces kancerogeneze sporadického karcinomu probíhá relativně pomalu, je možné tomuto onemocnění úspěšně předcházet. Národní screening kolorektálního karcinomu byl v České republice zahájen v roce 2000 a od této doby prochází kontinuálním vývojem s cílem zvýšení efektivního pokrytí cílové populace. V současnosti je k dispozici dvouetapový screeningový program (test na okultní krvácení do stolice a v případě pozitivity kolonoskopie), nebo jednoetapový program (primární screeningová kolonoskopie). Kvalita programu je hodnocena pomocí indikátorů kvality – zejména záchytu adenomů (adenoma detection rate, ADR) a podílu totálních kolonoskopií (cecal intubation rate, CIR). Díky screeningovému programu dochází v ČR k dlouhodobé stabilizaci incidence a mírnému poklesu mortality na kolorektální karcinom. K dalšímu snižování incidence a mortality onemocnění je nutná vyšší účast cílové populace za pomocí adresného zvaní cílové populace a mediální kampaně., Colorectal cancer is the second most common malignant disease in developed countries. Given that the process of sporadic cancer carcinogenesis is relatively slow, it is possible to successfully prevent this disease. Czech National Colorectal Cancer Screening Program was launched in 2000 and since that time is in continual development in order to increase the effective coverage of the target population. It is currently available as two-step screening program (stool test for occult blood and colonoscopy) or one-step program (primary screening colonoscopy). Quality of the program is evaluated using performance indicators – namely adenoma detection rate (ADR) and cecal intubation rate (CIR). The screening program in the Czech Republic enables long-term stabilization of incidence and a slight decrease in mortality from colorectal cancer. OBJECTIVES: The purpose of this study was to evaluate the in vitro sealing ability of three repair materials. Mineral trioxide aggregate (MTA; Group A), calcium phosphate cement (CPC; Group B), and light cured glass ionomer cement (GIC; Group C) when used to repair the perforation created in the pulpal floor of fifty extracted human permanent molars. MATERIALS AND METHODS: Preparation of access openings and furcation perforations were done, and the teeth divided into five experimental groups (A, B, C) including two controls (D, E) with ten samples in each group randomly. Following the repair procedure, the pulp chambers and access openings were filled with composite resin and immersed in 2% methylene blue solution for 48 hours. The teeth were sectioned longitudinally and the linear dye penetration measured under a stereomicroscope. RESULTS: The comparison of the linear length of micro-leakage (mm) among the experimental groups revealed no significant difference (p = 0.332). On calculating the percentage of depth of leakage to the total length of the perforation, it was observed that the mean leakage was 35.5% in Group A, 53.6% in Group B and the highest, 87.5% in Group C. The mean of leakage percentage was statistically significant by Kruskal-Wallis test (p = 0.003). The results indicated that the dye penetration used as furcation perforation repair material was least with mineral trioxide aggregate. Comparing the depth of penetration of dye, 50% of the Group A samples showed less than 25% of depth penetration. While 40% of Group B cases had more than 50% dye penetration. In our study, all Group C teeth had > or = 50% dye penetration. CONCLUSIONS: The present study indicated that GIC had the greatest dye penetration followed by CPC and MTA. BACKGROUND: The male-female ratio at birth (M/F: male births divided by total births), which is anticipated to approximate 0.515, has been shown to exhibit latitude gradients and secular trends. METHODS: Annual national data for male and female live births for the 15 countries that comprise the former Soviet Union were obtained from the World Health Organisation for the period 1980-2009 (115,167,569 total live births) and analysed with contingency tables. Spearman correlation was also carried out to compare percentage annual gross domestic product growth (GDP%--downloaded from the World Bank) and M/F. In this context, GDP% is used as a measure for economic hardship or wellbeing within the populace. RESULTS: There have been overall highly significant secular increases in M/F (p < 0.0001) in the countries and regions investigated. M/F is significantly lower in the three more northern regions (Russian Federation, Baltic States and Central Asia. M/F 0.51324, 0.51335-0.51314) than the two more southern regions (Southern Caucasus and Eastern Europe. M/F 0.51654, 0.51635-0.51672). There was a male excess of 113,818 live births.There was a significant positive correlation between GDP% and M/F for Armenia, Azerbaijan and Uzbekistan. There was a significant negative correlation in Estonia. CONCLUSION: Previous studies have shown that improving socioeconomic conditions increase M/F, and the converse has also been demonstrated. This is a potential influence in this geographical area since this region has relatively recently emerged from communist rule and experienced an overall economic upturn, but is only partially supported using GDP%. Another factor may be the selective termination of female pregnancies. The latitude gradient parallels that of neighbouring Europe but no theory has been put forward to convincingly explain this finding to date. and V. Grech
BACKGROUND: Latitude gradients and secular trends in Europe and North America have been found in the male-female ratio at birth (M/F: male births divided by total births) which approximates 0.515. METHODS: Annual national data for Yugoslavia and the post-Yugoslavia States for male and female live births were obtained from the World Health Organisation and analysed with contingency tables. RESULTS: This study analysed 22,020,729 live births. There was a increasing trend in M/F prior to the breakup of the former Yugoslavia (1950-1990, p = 0.002), followed by a decreasing trend after 1990 (p = 0.02). A latitude gradient was also noted, with more males being born in southern, warmer latitudes (p < 0.0001). There was an overall excess of 42,753 male births based on an anticipated M/F of 0.515. CONCLUSION: M/F is decreasing in this region, similar to the rest of Europe and North America. A latitude gradient is also present with more males being born in warmer (more Southern) latitudes (p < 0.0001), even in this small region and over the short time-frame studied. and V. Grech
Cíl studie: Sledování trendů koncentrací olova v krvi (plumbémie) u pracovníků vystavených zvýšené zátěži olovem během let 2005 - 2012. Hlavním účelem studie bylo posoudit efektivnost zavedených preventivních opatření na snížení expozice olovem (snížení plumbémie). Typ studie: Observační prospektivní studie Materiál a metody: Do studie bylo zařazeno 236 pracovníků bateriového průmyslu. Podle vstupních hodnot plumbémií z roku 2005 jsme pracovníky rozdělili do čtyř skupin (< 200 μg/l, 200 – 299 μg/l, 300 – 399 μg/l, ≥ 400 μg/l). Plumbémie byla stanovena metodou bezplamenové atomové absorpční spektrometrie. Pro statistické vyhodnocení byl použit párový t-test. Výsledky: Nejvýznamnější pokles v plumbémii mezi lety 2005 – 2012 byl zaznamenán ve skupině pracovníků s nejvyšší vstupní hodnotou plumbémie ≥ 400 μg/l (průměrný pokles na 37 % k roku 2012, t(63) = 29.105, p < 0.001). Významné poklesy byly zjištěny i u všech ostatních skupin pracovníků – se vstupní hodnotou plumbémie 300 – 399 μg/l (průměrný pokles na 41 %, t(70) = 32.704, p < 0.001), 200 – 299 μg/l (průměrný pokles na 42 %, t(67) = 34.208, p < 0.001) a pod 200 μg/l (průměrný pokles na 51 %, t(32) = 11.663, p < 0.001). Závěr: U všech skupin pracovníků vystavených zvýšené zátěži olovem jsme sledovali významné snížení plumbémie během let 2005 – 2012. Tato studie ukazuje, že zavedená preventivní opatření jsou vysoce efektivní., Objective: Monitoring trends in blood lead concentrations (plumbemia) of lead-exposed workers during years 2005 - 2012. Determining the effectiveness of preventive measures on reducing lead exposure (decreasing plumbemia) was investigated. Design: Observation prospective study Material and Methods: The study was performed on 236 workers of a battery industry factory in period 2005 – 2012. Workers were divided into 4 groups based on their initial plumbemia in the year 2005 (< 200 μg/l, 200 – 299 μg/l, 300 – 399 μg/l, ≥ 400 μg/l). Plumbemia was measured using graphite furnace atomic absorption spectrometry. Paired t-test was used for statistic interpretation. Results: The most significant decrease in plumbemia between years 2005 – 2012 was measured in the group of workers with initial plumbemia ≥ 400 μg/l (average decrease to 37 % by the year 2012, t(63) = 29.105, p < 0.001). Significant decreases were measured also in other groups of workers with initial plumbemia 300 – 399 μg/l (average decrease to 41 %, t(70) = 32.704, p < 0.001), 200 – 299 μg/l (average decrease to 42 %, t(67) = 34.208, p < 0.001) and under 200 μg/l (average decrease to 51 %, t(32) = 11.663, p < 0.001). Conclusion: In all groups of lead-exposed workers we found significant decrease in plumbemia during years 2005 – 2012.
