Retrospektivní studie analyzovala novotvary následující po primárních bronchogenních karcinomech (BCA) a primární novotvary před BCA u přežívajících, hlášených v Národním onkologickém registru ČR v letech 1976–2010. Z 166 239 hlášených BCA u mužů se vyskytlo s jiným novotvarem 16 622 BCA, tj. 10 %, z toho 4 395 (2,6 %) primárních a 12 227 (7,4 %) následných BCA; podobně z 37 619 hlášených BCA u žen bylo s jiným novotvarem 5 322 BCA tj. 14,1%, z toho 1 022 (2,7 %) primárních a 4 300 (11,4 %) následných BCA. Průměrná doba vzniku následného novotvaru byla 4,1 roku u mužů a 3,9 roku u žen. Z následných novotvarů (po BCA) bylo asi 23 % trávicích a 14 % močových cest, dále u mužů 14 % dýchacích cest, 14 % kůže a 11 % prostaty, u žen 11 % rodidel, 11 % kůže a 11 % prsů. Z primárních novotvarů (před BCA) bylo u mužů asi 38 % kůže, 13 % močových, 13 % trávicích a 10 % dýchacích cest, u žen 27 % kůže, 21 % rodidel, 17 % prsů a 9 % trávicích a 5 % močových cest. Mezi roky 1976 a 2010 roční počet primárních BCA vzrostl z 65 na 119, podíl jejich stádií (ST) I, II klesl v pětiletých intervalech z 53 % na 22 %, podíl ST III, IV vzrostl z 20 % na 64 %; počet následných BCA vzrostl z 23 na 1 089, jejich ST I, II klesla z 33 % na 20 % a ST III, IV vzrostla z 29 % na 63 %. Podrobnější rozbor omezilo vyšší zastoupení neznámých stádií. Z údajů lze odhadovat asi 1 155 novotvarů ve ST III, IV (19 % u mužů, 22 % u žen ze všech následných ZN) zjištěných po primárních BCA, s měsíčním průměrem tří nemocných v průběhu 34 let. Četnější ST III, IV z 4 966 vybraných následných nádorů byla u ZN vaječníků (47 %), dalších ZN plic (muži 45 %, ženy 52 %), hrtanu (muži 45 %, ženy 52 %), dutiny ústní (muži 41%, ženy 39 %), žaludku (muži 28 %, ženy 42 %), kolorekta (muži 33 %, ženy 37 %), děložního hrdla (28 %), prsů (25 %) a pankreatu (muži 23 %, ženy 26 %). Zejména následným pokročilým stádiím by měla předcházet dispenzární doporučení a časná intervence. Omezené zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících nejen v pneumologii., The neoplasms following primary lung cancer (LC) and primary neoplasms before LC analyzed a retrospective study in survivors, notified in the National Czech Cancer Registry between 1976 and 2010. A total 16 622 LC in males associated with other neoplasms, presented 10 % of 166 239 newly registered LC in males, of which were 4,395 (2.6 %) primary and 12,227 (7.4 %) subsequent LC; a total 5,322 LC in females, presented 14.1 % of 37,619 newly registered LC in females, of which were 1,022 (2.7 %) primary and 4,300 (11.4 %) subsequent LC. The average interval of occurrence of subsequent neoplasms was 4.1 years in males and 3.9 years in females. Among subsequent neoplasms (after LC) were about 23 % cancers of digestive and 14% urinary tract, in males 14% cancers of respiratory tract, 14 % skin and 11 % prostate, in females 11% cancers of female genital organs, 11 % skin and 11% breast. Among primary neoplasms (before LC) were about 38 % cancers of skin, 13 % urinary, 13 % digestive and 10 % respiratory tract in males; 27 % cancers of skin, 21 % female genital organs, 17% breast, 9 % digestive and 5 % urinary tract in females. The yearly number of primary LC has increased since 1976 to 2010 from 65 to 119 cases, their representation at the early clinical stages decreased in 5-year periods from 53% to 22 %, at the advanced stages increased from 20 % to 64 %; the number of subsequent LC increased from 23 to 1089 cases, their early clinical stages decreased from 33 % to 20 %, the advanced stages increased from 29 % to 63 %. The higher proportion of LC at the unknown stages limited detailed analysis. In our study were estimated about 1,155 neoplasms at the advanced stages (19 % in males and 22 % in females of all subsequent neoplasms) diagnosed after primary LC, which presented a monthly average of three cancers during 34 years. The most frequented advanced stages of 4,966 selected subsequent cancers were cancers of ovary (47 %), other lung (males 45 %, females 52 %), larynx (males 45 %, females 52 %), oral cavity (males 41 %, females 39 %), stomach (males 28 %, females 42%), colorectum (males 33%, females 37 %), cervix uteri (28 %), breast (25 %) and pancreas (males 23 %, females 26 %). Mainly subsequent advanced stages must be prevented by the follow-up guidelines and early interventions. Úvod: Odběr ledvin z dárce s nezvratnou zástavou oběhu (DCD – donor after circulatory death) je důležitou součástí transplantačních programů v celosvětovém měřítku. V České republice byla první transplantace ledvin z DCD úspěšně provedena v roce 2002. Autoři diskutují vlastní a literární zkušenosti s tímto programem. Snahou je zvýšit potenciál odběru ledvin z DCD v České republice. Metoda: Od roku 2002 do roku 2015 bylo v plzeňském Transplantačním centru odebráno celkem 44 ledvin z DCD. Technikou byl „in situ“ odběr pomocí „double baloon triple lumen“ katetru při zachování 5–10 min. „no-touch“ intervalu. Metoda hypotermické pulzatilní perfuze byla použita k testování viability odebraných ledvin. Transplantováno bylo celkem 28 ledvin příjemcům o průměrném věku 51,1 roku (26–73 let). Dle maastrichtských kritérií bylo 16 (57,1 %) ledvin ve skupině II, 8 (28,6 %) ve skupině III a 4 (14,3 %) ve skupině IV. Výsledky: 30denní pooperační mortalita byla 0 a morbidita 10,7 tj. 14,3 % (N=4). Primární afunkce ledviny byla přítomna u 2 (7,1 %), opožděný nástup funkce pak u 5 (17,9 %) nemocných. Jeden rok, 5 a 10 let po transplantaci žije 100 %; resp. 86,4 % a 76,7% nemocných a ve stejných intervalech je funkčních 92,9; 69,6 a 61,9 % transplantovaných ledvin. Dlouhodobé výsledky jsou plně srovnatelné s transplantacemi ledvin od dárců se smrtí mozku. Závěr: DCD jsou významným zdrojem ledvin pro transplantace. Transplantace ledvin z DCD je metodou logisticky, ekonomicky a personálně náročnou s velmi dobrými dlouhodobými výsledky., Introduction: Kidney procurement from donors after circulatory death (DCD) is an important part of worldwide transplantation programmes. The first kidney transplantation from DCD was successfully performed in the Czech Republic in 2002. Method: Forty four kidneys from DCD were procured in the Transplant Centre of Pilsen between 2002 and 2015. We used the technique of “in situ“ procurement with the double balloon triple lumen catheter and 5−10 minutes of the no-touch interval. The method of pulsatile hypothermic perfusion was used to test the viability of the kidneys. Twenty eight recipients with mean age 51.1 (26−73) years were transplanted. Sixteen (57.1%) kidneys were from the 2nd, 8 (28.6%) from the 3rd and 4 (14.3%) from the 4th category according to the Maastricht criteria. Results: 30-day mortality and morbidity rates were 0 and 10.7% i. e.14.3% respectively (N=4). Primary non-function was presented in 2 (7.1%), and delayed graft function in 5 (17.9%) cases. One, five and ten years of recipient and graft survival rates were 100%, 86.4% and 76.7%; and 92.9%, 69.6% and 61.9%, respectively. The long-term results are fully comparable with kidneys transplanted from donors after brain death. Conclusion: DCD are an important source for kidney transplantation. Cíl: Cílem práce je charakterizovat klinické a elektrofyziologické nálezy dvou typů ulceromutilující dědičné neuropatie CMT2B a HSN1 u tří českých rodin s molekulárně geneticky objasněnou příčinou. Soubor: Popisujeme tři rodiny s dědičnou senzitivní neuropatií s celkem 16 postiženými. Metodika: Na základě neurologického a elektromyografického vyšetření, výskytu podobných obtíží u příbuzných byla stanovena diagnóza senzitivní, převážně axonální neuropatie. Následně bylo provedeno sekvenování genů SPTLC1 a RAB7 v rodinách A, B, C a u dalších 24 nepříbuzných pacientů s klinickým podezřením na dědičnou senzitivní neuropatii. Výsledky: V rodině B byla nálezem mutace p.C133Y v SPTLC1 genu prokázána hereditární senzitivní neuropatie typ 1 (HSN1). V rodinách A a C byla nálezem mutací p.L129F a p.V162M v genu RAB7 prokázána choroba Charcot‑Marie‑Tooth typ 2B (CMT2B). Všechny tři mutace již byly dříve popsány a fenotyp odpovídá popisu pacientů z jiných zemí. DNA vyšetření dalších 24 nepříbuzných pacientů však příčinu onemocnění neobjasnilo. Závěr: Ulceromutilující dědičné neuropatie CMT2B a HSN1 jsou v české populaci, podobně jako v jiných zemích vzácné, ale klinické projevy jsou jasně poznatelné, pokud jsou zohledněny a správně získány genealogické údaje – rodinná anamnéza. Tyto tři popsané rodiny jsou dosud jediné známé v ČR s objasněnou příčinou dědičné senzitivní neuropatie v důsledku mutací v RAB7 a SPTLC1. Objasnění příčiny ulceromutilující senzitivní neuropatie má význam nejen pro upřesnění genetické a klinické prognózy a pro cílenou genetickou prevenci, ale u pacientů s mutacemi v SPTLC1 genu možná i pro cílenou terapii se substitucí L‑serinem, která bude testována v klinické studii (Boston, USA), Aim: The goal was to clinically and electrophysiologically characterize two types of ulceromutilating hereditary neuropathy CMT2B and HSN1 in three Czech families where molecular genetic cause was confirmed. Patients: We describe three families, overall 16 affected patients, with hereditary sensory neuropathy. Methods: The diagnosis of sensory, predominantly axonal neuropathy was done on the basis of neurological and electrophysiological examination. Sequencing of the SPTLC1 and RAB7 genes was done in families A, B, C and 24 unrelated patients with clinical suspicion for HSN. Results: Hereditary sensory neuropathy type 1 (HSN1) caused by the p.C133Y mutation in the SPTLC1 gene was confirmed in family B and the Charcot-Marie-Tooth type 2B (CMT2B) caused by p.L129F and p.V162M mutations in the RAB7 gene was confirmed in families A and C. All three mutations have been previously described. DNA examination of 24 unrelated patients did not reveal the cause of their disease. Conclusion: As in other countries, ulceromutilating hereditary neuropathies CMT2B and HSN1 are rare in the Czech population. However, clinical manifestations are clearly recognizable if correctly obtained genealogical data – family history – is properly taken into account. The three families described here are the only known families with hereditary neuropathies caused by mutations in RAB7 and SPTLC1 in the Czech Republic. Clarification of the cause of ulceromutilating sensory neuropathy is crucial for genetic and clinical prognosis, including targeted genetic prevention, but possibly also for an L-serin therapy in SPTLC1 mutation patients to be tested in a clinical study (Boston, USA)., and D. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. CONCLUSION: Since the introduction of ART, the frequency of oral mucosal lesions is minimal in patients with HIV infection. and M. Šembera, V. Radochová, R. Slezák
BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. It is also important to duly revise the entire bowel to avoid an early reccurence due to multiple gallstones. Key words: biliary ileus − diagnosis − treatment, and O. Vyčítal, V. Liška, J. Geiger, V.
