Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Cíl: Cílem práce bylo shromáždit dostupná klinická, radiologická a patologická data o kalcifikujícím pseudotumoru nervové osy (CAPNON). Soubor: Soubor je tvořen kazuistikou a současně přehledem všech pacientů publikovaných v anglické literatuře, vyhledaných v databázi PubMed pomocí klíčových slov: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi a fibro‑osseous lesion. Zahrnuty jsou taktéž všechny citace případů v nalezených publikacích. Výsledky: Včetně naší pacientky jsme v literatuře nalezli celkem 72 pacientů. Intrakraniálně bylo lokalizováno 62 % těchto lézí, v páteřním kanále 31 % a v kranio‑cervikálním přechodu zbylých 7 %. Průměrný věk výskytu je 45,6 (rozmezí 2–83) let. U intrakraniální lokalizace byla tato léze nalezena supratentoriálně v 74 % a infratentoriálně v 26 %. Z nejčastějším projevů intrakraniálních CAPNON jsme nalezli epilepsii v 33,3 %, bolesti hlavy v 24 %, nebo některý z příznaků zadní jámy v 16,7 %. U pacientů s epilepsii byl CAPNON nalezen v temporálním laloku v 50 %. U CAPNON nalezených v páteřním kanále byla dominantním projevem bolest v 82 %, následovaná poruchou chůze v 27 %. V kranio‑cervikálním přechodu byla dominantním příznakem bolest v 80 %. Závěr: CAPNON je velmi vzácnou, benigní, pomalu rostoucí lézí centrální nervové soustavy. Ve většině případů ji nalezneme intrakraniálně s lehkou převahou u mužského pohlaví. Původ této léze není zcela jasný. Diagnóza CAPNON by měla být zvažována setkáme‑li se s nálezem solidně kalcifikované léze při vyšetření CT mozku a hypodenzní léze v T1 a T2 obraze při vyšetření MR. Léčebnou modalitou je radikální odstranění této léze. Incidentální léze musí být monitorovány, jelikož mohou růst a stát se symptomatickými. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Aim: The aim of this study was to summarize clinical, radiological and pathological data on calcifying pseudoneoplasm of the neural axis (CAPNON). Methods: A case report of a female patient with CAPNON is presented. In addition, all CAPNON patient case histories published in English language are included. Articles were identified via PubMed searches using the following key words: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi and fibro‑osseous lesion. All cases referenced within the identified publications are also included in this review. Results: Seventy two cases were identified, including our case report. Of these, 62% were intracranial, 31% spinal and 7% were located in the cranio‑cervical junction (CCJ). The mean age of the CAPNON patients was 45.6 (range 2–83) years. 74% of intracranial CAPNON were supratentorial and 26% were infratentorial. Symptoms of intracranial CAPNON included epilepsy in 33.3%, headaches in 24% and posterior fossa symptoms in 16.7%. The majority of patients with epileptic seizures had lesions in the temporal lobe (50%). Pain was the dominant symptom in 82% and gait disorder in 27% of spinal CAPNON cases. Pain was the dominant symptom (80%) in CCJ CAPNON. Conclusion: CAPNON is a rare, benign, slowly‑growing lesion of the central nervous system. In the majority of cases, location is intracranial and there is a slightly higher prevalence in men. The origin of CAPNON remains unclear. Cíl: Zjistit prevalenci celiakie u dospělých pacientů s diabetes mellitus 1. typu (DM1T). Vliv nově zahájené léčby celiakie na kompenzaci diabetu a body mass index (BMI) pacientů. Chování protilátek proti tkáňové transglutamináze (atTG) po roce od zahájení léčby. Metodika: Retrospektivní hodnocení cíleného screeningu na celiakii v období 1. 1. 2007–1. 7. 2011 u 465 dospělých pacientů s DM1T v Diabetologickém centru I. interní kliniky LF UK a FN Plzeň (80 % všech pacientů s DM1T). Při pozitivitě atTG-A (popř. atTG-G) byla indikována enterobiopsie. U pacientů s nově indikovanou bezlepkovou dietou byl porovnán HbA1c a BMI v průběhu roku před stanovením diagnózy celiakie vs období 1 roku po 6 měsících od zahájení léčby (3–4 kontroly). Po roce od zahájení léčby byla kontrolována hladina atTG. Ke statistice byl použit párový t-test. Výsledky: Prevalence všech forem celiakie v souboru byla 10,5 %. 1,1 % pacientů (5 ze 465) mělo diagnostikovanou celiakii v dětství. Nově byla pozitivita atTG zachycena u 9,5 % (44 ze 465) pacientů – indikováni k enterobiopsii. Z toho 3 pacienti s atTG > 300 kIU/l odmítli enterobiopsii, u těchto pacientů je celiakie vysoce pravděpodobná a pro malkomplianci nešlo hodnotit vliv bezlepkové diety na BMI a HbA1c. 22 pacientů mělo potenciální formu celiakie (negativní histologie). Pozitivní nález z enterobiopsie byl zjištěn u 19 pacientů (4,1 %). Z podsouboru 22 pacientů (nově indikována bezlepková dieta), bylo nutné vyřadit další 3 pacienty (vývoj HbA1c a BMI byl ovlivněn prvozáchytem DM1T). Charakteristika podsouboru: 9 žen a 7 mužů, průměrný věk 38 ? 12 let, trvání diabetu 21 ? 13 let, celiakie diagnostikována 20,7 ? 13 let od záchytu DM1T. Během sledovaného období nedošlo ke statisticky významné změně bazální hodnoty HbA1c 67 ? 11,4 vs 69 ? 13,9 mmol/mol po sledování a významně se zvýšil BMI z 25,4 ? 4,2 na 25,9 ? 4,3 (p < 0,01). Po roce přetrvávala pozitivita atTG u 47 % (9 z 19) pacientů. Závěr: V souboru dospělých pacientů s DM1T byla zjištěna celková prevalence celiakie 10,5 %. Po zahájení léčby nedošlo k významné změně HbA1c, došlo k významnému vzestupu BMI. Pozitivita atTG přetrvávala po roce u 47 % pacientů., Objective: To assess the prevalence of celiac disease in adult patients with type 1 diabetes mellitus (T1DM). Influence the new started treatment of celiac disease on glycemic control and body mass index (BMI) of the patients. Prevail the anti-transglutaminase antibody (atTG) positivity one year after commencement of the therapy. Methods: A retrospective assessment of celiac disease targeted screening in 465 adult T1DM patients at Diabetes Center, 1st Medical Department, University Hospital in Pilsen (80 % of all T1DM patients) from 1. 1. 2007 until 1. 7. 2011. Enterobiopsy was indicated in case of atTG-A (or atTG-G) positivity. In patients with newly started gluten-free diet, HbA1c and BMI within a year after diagnosis of celiac disease were compared to a year period six months after treatment commencement (3–4 visits), atTG was evaluated one year after treatment beginning. Paired T-test was used for statistical evaluation. Results: The prevalence of all forms of celiac disease in the studied group was 10.5 %. Celiac disease diagnosed in childhood was found in 1.1 % patients (5/465). Positivity of atTG was newly observed in 9.5 % (44/465) patients. Three patients with atTG > 300 kIU/l refused the enterobiopsy examination. Celiac disease is highly plausible. The influence of gluten-free diet on BMI and HbA1c could not be evaluated due to the lack of compliance. 22 patients had a potential form of celiac disease (negative histology). Positive enterobiopsy was found in 19 patients (4.1 %). Another 3 patients had to be excluded from the subgroup of 22 patients (newly indicated gluten-free diet) as the HbA1c values and BMI were affected by the primary diagnosis of T1DM. Subgroup characteristics: 9 women and 7 men, mean age 38 ? 12 years, diabetes duration 21 ? 13 years, celiac disease diagnosed 20.7 ? 13 years since first diagnosis of T1DM. No statistically significant change in HbA1c (67 ? 11.4 vs 69 ? 13.9 mmol/mol) was observed in the studied period, however and a significant change of BMI from 25.4 ? 4.2 to 25.9 ? 4.3 (p < 0.01) was found. The atTG positivity prevailed in 47 % (9/19) of patients after one year. Conclusion: A total prevalence of the celiac disease in the group of adult T1DM patients was 10.5 %. No significant change in HbA1c occurred following treatment, a significant change of BMI was observed. Cíl: Naším záměrem bylo zjistit diagnostické charakteristiky Montrealského kognitivního testu (MoCA) v tréninkové podobě pro lepší administraci u pacientů s Alzheimerovou nemocí (AN) poprvé v českém písemnictví. <p align="">Soubor a metodika: Záznamový formulář jsme obohatili o instrukce pro vyšetřovaného, instrukce k administraci a návod ke skórování. Obsah a skórování zůstaly zachovány a odpovídají předchozí české verzi. Upravili jsme návod k administraci. Zpětný překlad záznamového archu a návodu do angličtiny byl zkontrolován, schválen a nazván kanadským tvůrcem jako tzv. tréninková verze MoCA‑CZ1. Tímto testem jsme vyšetřili 30 pacientů s již diagnostikovanou mírnou demencí způsobenou Alzheimerovou nemocí podle nových doporučení NIA‑AA (78 ? 6 let, 13 ? 3 let vzdělání, 56 % žen) a 30 kognitivně normálních seniorů (NOS) podobného věku, vzdělání a pohlaví. <p align="">Výsledky: Oproti standardní verzi obsahují záznamové archy tréninkové české podoby MoCA‑CZ1 navíc další informace, s nimiž předpokládáme snadnější a spolehlivější provádění a vyhodnocování testu. Celkové skóry MoCA‑CZ1 byly významně nižší u pacientů s mírnou demencí způsobenou AN (18 ? 5 bodů) než u NOS (26 ? 3 body) (p < 0,00001). Celkové skóry NOS nezávisely na věku, pohlaví a vzdělání. Při porovnávání obou skupin podle analýzy receiver operating curve byl zjištěn optimální hraniční skór ≤ 23 bodů se senzitivitou 87 % a specificitou 84 %. Plocha pod křivkou byla 0,92. <p align="">Závěry: Tréninková MoCA‑CZ1 je volně dostupná na http://www.pcp.lf3.cuni.cz/adcentrum/testy.html. Výsledek testu může přispět k dřívějšímu záchytu mírné demence způsobené AN, a to časnou detekcí kognitivního deficitu. Při používání tréninkové verze MoCA‑CZ1 doporučujeme používat hraniční skór ≤ 23 bodů., Aim: Our aim was to determine diagnostic characteristics of the Montreal cognitive assessment (MoCA) using its training version for better administration in patients with Alzheimer’s disease (AD) for the first time in Czech literature. <p align="">Patients and methods: We complemented the test form with instructions for the patient, and with administration and scoring instructions. The content and the scoring were preserved from the previous Czech version. A separate Czech administration instruction manual was refined. English back translation of the data collection form and the manual was checked and approved by the Canadian author who also named it the training version. We examined 30 patients previously diagnosed with mild dementia due to Alzheimer’s disease according to the new NIA‑AA recommendations (78 ? 6 years, 13 ? 3 years of education, 56% women) and 30 cognitively normal seniors (NOS) of similar age, education and gender by MoCA‑CZ1. <p align="">Results: The training version of MoCA‑CZ1 contains additional information. We assume that this ensures reliability, and facilitates administration and scoring of the test. Total MoCA‑CZ1 scores were significantly lower in patients with AD (18 ? 5 points) than in NOS (26 ? 3 points) (p < 0.00001). Total scores of NOS did not correlate with age, gender or education. Analysis of receiver operating curve comparing the two groups found optimal cut off score of ≤ 23 points with sensitivity of 87% and specificity of 84%. Besides acknowledging the obvious biological characteristics, primatologists needed to also inves - tigate the psychological and social similarities between the human animal and the two Pan species. Jane Goodall pioneered the close-range and long-term observations of wild chimpanzees in their natural habitats. Likewise, Harold Coolidge and Frans de Waal contributed to un - derstanding and appreciating bonobos as a r esult of their ground-breaking research. These studies clearly attest to a s triking contrast between the aggressive chimpanzee and the peaceful bonobo. Cíl: Autoři prezentují tříleté klinické a radiologické výsledky po implantaci interspinózní rozpěrky pro laterální lumbální stenózu páteřního kanálu. Soubor a metodika: Po dobu tří let sledovali 42 pacientů s 62 implantovanými interspinózními náhradami. Indikací k implantaci byla jednostranná nebo oboustranná laterální stenóza páteřního kanálu s degenerací meziobratlové ploténky. Pacienti byli vyšetřeni podle algoritmu, který zahrnoval: neurologické vyšetření, RTG – prosté a dynamické snímky, MR vyšetření a v případě kontraindikace MR bylo provedeno CT vyšetření. Klinický nález byl hodnocen pomocí vizuální analogové škály (VAS), Oswestry Disability Indexem (ODI) a Odom kritériemi. Na RTG snímcích byl hodnocen rozsah pohybů, distrakce v oblasti zadní hrany meziobratlové ploténky a postavení implantátu. Po šesti měsících a po dvou letech bylo provedeno pooperační MR vyšetření a byla hodnocena distrakce foramen intervertebrale v předozadním (AP) a kraniokaudálním rozměru a míra degenerace meziobratlové ploténky ve srovnání s předoperačním nálezem. Výsledky: Po implantaci interspinózní rozpěrky do bederní páteře došlo ve tříletém sledovacím období k poklesu skóre ve VAS škále v průměru o 2,7 bodů a ODI o 24,5 %. Byla prokázána distrakce v oblasti zadní části disku a distrakce v AP rozměru foramen intervertebrale. Závěr: Po zavedení interspinózní rozpěrky došlo ke zlepšení ODI a VAS parametrů, byla zjištěna statisticky významná distrakce v oblasti zadní části meziobratlové ploténky a AP rozměru foramen intervertebrale. Nebyly zaznamenány žádné peri‑ či pooperační komplikace., Aim: The authors present clinical and radiological results of a 3‑year follow‑up of patients after implantation of interspinous spacers for lateral stenosis. Cohort and methodology: We followed 42 patients with 62 implanted interspinous spacers for a period of three years. The indication for implantation was unilateral or bilateral lateral stenosis with degenerative disc disease. Patients were examined according to an algorithm involving: neurological examination, X‑ray – plain and dynamic images, MRI, patients contraindicated for MRI underwent CT examination. Clinical findings were assessed using a VAS score, ODI and Odom criteria. X‑rays were performed and the range of motion, distraction in ventral and dorsal areas of the intervertebral disc and foramen intervertebrale and the position of the implant were evaluated. Six months and two years post‑surgery, patients underwent postoperative MRI examination and intervertebral foramen distraction in anteroposterior and craniocaudal diameter, and the assessment was compared with preoperative findings. Results: Implantation of interspinous spacers in the lumbar spine in a 3-year follow‑up period was followed by the mean improvement of 2.7 points on VAS score, and of 24.5% on ODI. Statistically significant difference in the distraction of the intervertebral foramen in the dorsal part of the intervertebral disc was proven by pre‑ and post‑operative MRI. Conclusion: Implantation of a spacer led to improved ODI and VAS parameters, distraction in the region of the intervertebral foramen, especially in the AP diameter, and in the dorsal part of an intervertebral disc. Cíl: Cílem studie je představit dlouhodobé výsledky chirurgické léčby supratentoriálních kavernózních malformací mozku na Neurochirurgické klinice 1. LF UK a ÚVN‑VFN. <p align="">Soubor a metodika: Retrospektivně jsme do studie zařadili pacienty se supratentoriálním, kortiko‑subkortikálně uloženým kavernomem léčené v letech 2000–2012. Hodnotili jsme epidemiologická data pacientů, radiologický nález, klinický obraz onemocnění a operační výsledky včetně komplikací. <p align="">Výsledky: Iniciálním příznakem byl epileptický záchvat (u 49 % kavernomů), cefalea (22 %), fokální neurologický deficit (19 %) a 15 % bylo náhodně nalezených. Známky recentního krvácení na MR byly přítomny u 27 % pacientů. Operovali jsme celkem 145 pacientů s 158 kavernomy. Dvacet pět lézí bylo léčeno konzervativně. K chirurgické komplikaci došlo v 8 %. U dvou pacientů (1,4 %) se trvale zhoršil stav v přímé souvislosti s operací. Pooperační výskyt epileptického záchvatu byl signifikantně vyšší ve skupině s rannou infekcí či pooperačním hematomem (p < 0,05). <p align="">Závěr: Mikrochirurgická resekce hemisferálního kavernomu je relativně bezpečná operace s minimální morbiditou a mortalitou. Pooperační hematom nebo ranná infekce může mít epileptogenní potenciál., Aim: The aim of the study is to present surgical outcome of treatment of supratentorial cavernous malformation of the brain at the Department of Neurosurgery, Charles University and the Central Military Hospital in Prague. <p align="">Material and methods: We retrospectively enrolled patients diagnosed between 2000 and 2012 with supratentorial, cortico‑subcortically located cavernoma. We analysed epidemiological and radiological data, clinical presentation and surgical results including complications. <p align="">Results: Initial symptoms included epileptic seizure (49%), headache (22%) and focal neurological deficit (19%); 15% of cavernomas were found incidentally. Radiological signs of recent haemorrhage on MR scans were found in 27% patients. We performed surgery in 145 patients with 158 cavernous malformations. Twenty five lesions were treated conservatively. Surgical complications occurred in 8% of patients. One patient died and one had permanent neurological deficit attributable to surgery. Postoperative seizure rate was significantly higher in a group with wound infection or postoperative hematoma (p < 0.05). <p align="">Conclusion: Microsurgical resection of lobar cavernoma is relatively safe procedure with minimal morbidity and mortality. V terapii meningeomů na prvním místě přetrvává chirurgická léčba. Cílem chirurgické léčby je úplné odstranění nádoru, které často znamená úplné vyléčení pacienta. Prognózu pacienta určuje především radikalita resekce, dále také anatomické umístění a histologický typ nádoru dle WHO klasifikace. Některé meningeomy, zejm. meningeomy baze lební, které často utiskují nebo obrůstají velké cévy a hlavové nervy, a dále potom meningeomy, které histologicky odpovídají WHO st. III, jsou velmi obtížně léčitelné. Nezanedbatelným faktorem je i celkový stav a interní komorbidity. Autoři uvádějí nejčastější typy obtížně chirurgicky řešitelných meningeomů a popisují zkušenosti svého pracoviště s chirurgickým odstraněním, další léčbou a sledováním pacientů s těmito nádory., Surgical treatment is still considered as the method of choice for the treatment of meningiomas. The goal of treatment is to completely surgically remove the tumor; complete excision often results in a permanent cure of the patient. The patient prognosis is mainly determined by the completeness of the resection but anatomical location and histological type of the tumor according to the WHO classification also play an important role. Some meningiomas, especially the skull base meningiomas, often involving large vessels and cranial nerves, as well as WHO grade III meningiomas are very difficult to treat. Patient general health status and internal comorbidities represent another very important factor. The authors describe the most common types of surgically difficult-to-treat meningiomas and describe their experience with surgical removal and additional treatment and surveillance of patients with these tumors. Úvod: Tumory thalamu jsou relativně vzácné léze rostoucí ve vysoce funkčně důležité části mozku. Jsou častější v pediatrické populaci. Jejich chirurgické odstranění je náročné s rizikem značné morbidity. Relativně benigní charakter mnoha těchto lézí vybízí k pokusu o jejich radikální odstranění. Chirurgický přístup musí být velice pečlivě plánován, obvykle s použitím moderních diagnostických metod, jako je DTI (Diffusion Tensor Imaging). Lokalizace a projekce tumoru v thalamu hraje při výběru přístupu důležitou roli. Materiál a metodika: Od roku 2005 do roku 2014 bylo na Neurochirurgické klinice FN Brno operováno celkem 13 pacientů s tumorem thalamu, průměrný věk 20,6 roku (1–64 let). Jednalo se o 10 pacientů mužského a tři ženského pohlaví. Další 15letý pacient s pilocytárním astrocytomem je observován. Transkortikální přístup byl použit 7×, transkalózní přístup 4×, transsylvijský přístup 2× a supracerebellární infratentoriální 1×. Výsledky: Totální resekce (GTR) byla provedena v šesti případech, subtotální resekce ve čtyřech případech a parciální resekce ve třech případech. Jednalo se o osm pilocytárních astrocytomů, jeden subependymální obrovskobuněčný astrocytom, jeden difúzní astrocytom GII a tři glioblastomy. Dva starší pacienti s glioblastomem zemřeli šest a devět měsíců po operaci. Ostatní pacienti přežívají se střední délkou sledování 5 let. Neurologický stav po operaci byl dobrý kromě jednoho staršího pacienta s glioblastomem, u kterého se rozvinula těžká hemiparéza a fatická porucha. Závěr: Pokud je zvolen správný chirurgický přístup, thalamické tumory mohou být celkem bezpečně radikálně odstraněny. Volba přístupu je dána projekcí tumoru. Menší tumory, které nedosahují povrchu thalamu, mohou být observovány nebo bioptovány, pokud máme podezření na maligní povahu léze. Následná onkologická léčba je vyhrazena pro vysokostupňové gliomy., Introduction: Thalamic tumours are relatively rare lesions growing in a highly functional part of the brain. They are more frequent in paediatric population. Their surgery is challenging and associated with a risk of significant morbidity. Relatively benign nature of many of these tumours suggests that radical resection should be attempted. The approach has to be very carefully planned, sometimes with help of modern diagnostic methods such as DTI. Location and projection of the tumour in the thalamus plays an important role in selecting an approach. Material and methods: During 2005–2014 we operated on 13 patients with thalamic tumours, mean age 20.6 years (1–64 years). These were 10 males and three females. Another 15-year-old male patient is being observed. Transcortical approach was used 7×, transcallosal 4×, transsylvian 2× and supracerebellar infratentorial 1×. Results: Gross total resection was achieved in six cases, subtotal in four and partial in three. There were eight pilocytic astrocytomas, one subependymal giant cell astrocytoma, one diffuse astrocytoma G II and three glioblastomas. Two older patients with glioblastomas died six and nine month after operation. All other patiens are still alive with the mean follow-up 5 years. Neurological status after the operation was good except for one older patient with glioblastoma who developed severe hemiparesis and aphasia. Conclusion: Thalamic tumours might be safely radically resected if correct approach is used. The choice of approach is based on the projection of the tumour. Smaller tumours that are not reach thalamic surface might be followed up or biopsied if there is a likelihood of their malignant nature. Oncological treatment should be reserved to malignant tumours. Úvod: V roce 2016 to bude 60 let od doby, kdy Nissen poprvé publikoval svoji operační metodu 360° fundoplikace kolem distálního jícnu a její výsledný efekt. V současnosti je Nissenova fundoplikace pravděpodobně nejčastěji používanou metodou k chirurgické léčbě gastroezofageálního refluxu (GER) a hiátové hernie (HH). Incidence HH je v západní populaci mezi 25–50 %. Současná prevalence GER je v Evropě kolem 4–10 %, v Severní Americe 5–7 %, ale v některých studiích je tato prevalence udávána dokonce kolem 10–20 %. Metody: Na Chirurgické klinice 2. LF UK a ÚVN bylo v období 2010 až 2015 provedeno 157 laparoskopických operací pro hiátovou hernii nebo refluxní nemoc jícnu (GERD). Většina nemocných byla indikována pro symptomatickou HH s/bez GERD, jen minimum nemocných bylo indikováno pro GERD bez HH. V souboru bylo 73 mužů a 84 žen s průměrným věkem 53,9 roku v rozmezí 20–88 let. Z tohoto počtu mělo 11 nemocných syndrom hrudního žaludku. U 109 nemocných (69,4 %) byla provedena 360° fundoplikace dle Nissena (NFP) a u 48 nemocných (30,6 %) byla provedena 270° fundoplikace dle Toupeta (TFP). Po operaci byli nemocní ambulantně kontrolováni v intervalu 3 měsíce, 6 měsíců a 12 měsíců a dále po roce. Prospektivně byly sledovány příznaky pyrózy, dysfagie, možnosti či nemožnosti odříhnutí, meteorismu nebo výskyt recidivy, získaná data byla zkoumána retrospektivně. Pro vyhodnocení získaných dat byla použita metoda deskriptivní statistiky a explorační jednosměrné analýzy. Výsledky: V období 2010 až 2015 bylo provedeno 157 laparoskopických operací pro hiátovou hernii nebo GERD. V souboru se nevyskytlo úmrtí ve vztahu k provedené operaci. Tři měsíce po operaci došlo jak u pacientů po NFP, tak u pacientů po TFP k výraznému poklesu až vymizení symptomů refluxu, příznaků pyrózy a epigastrické bolesti. Pooperační dysfagické obtíže vymizely ve skupině po NFP do 4–7 týdnů, u nemocných po TFP do 3–5 týdnů. V intervalu 2–5 let po operaci došlo celkem u 36 (22,9 %) nemocných k recidivě obtíží. U 22 nemocných (14 po NFP, 8 po TFP), u kterých byla prokázána endoskopicky nebo RTG vyšetřením recidiva refluxu bez recidivy kýly, byla zahájena léčba inhibitory protonové pumpy s velmi dobrým efektem. U 14 nemocných s anatomickou recidivou hiátové hernie s/bez refluxu (8 po NFP, 6 po TFP) byla provedena laparoskopicky rehiátoplastika a refundoplikace dle Nissena. Závěr: Laparoskopická chirurgická antirefluxní léčba GERD a HH je bezpečná metoda s velmi nízkou perioperační morbiditou a s minimální mortalitou. NFP nebo TFP ve stejné míře dlouhodobě obnovují mechanickou bariéru proti regurgitaci žaludečního obsahu do jícnu se signifikantním efektem na vymizení příznaků GERD a HH a s výrazným zlepšením kvality života nemocných. U obézních nemocných je před operací nutná redukce hmotnosti. U morbidně obézních nemocných s HH indikovaných k bariatricko-metabolické operaci je doporučováno provést hiátoplastiku současně s bariatrickým výkonem., Introduction: In 2016 60 years will have elapsed from the first publication by Nissen about his surgical method of 360° fundoplication around distal esophagus and its final effect. Nissen’s fundoplication is currently the most common surgical method for the gastroesophageal reflux (GER) and hiatal hernia (HH) probably. Incidence of HH ranges between 25–50% in the western population. Current prevalence of GER ranges between 4–10% in Europe and 5–7% in North America, but some studies report even 10–20%. Methods: From 2010 to 2015, 157 laparoscopic procedures for HH or gastroesophageal reflux disease (GERD) were done at the Department of Surgery, the Second Faculty of Medicine, Charles University and Central Military Hospital in Prague. Most of the patients were operated because of symptomatic HH with/without GERD. Only very few patients were operated only for GERD without HH. The group of the patients included 73 men and 84 women with the mean age 53.9 years in the range 20–88 years. 11 patients of the group were operated because of the upside-down stomach syndrome. 109 (69.4%) patients underwent Nissen’s 360° fundoplication (NFP) and 48 (30.6%) patients underwent Toupet’s 270° fundoplication (TFP). The patients were followed after 3 month, 6 month, 12 month and then once a year after the surgery. Symptoms such as heartburn, dysphagia, belching, bloating or recurrence of HH or GERD were collected prospectively and analyzed retrospectively. Methods of descriptive statistics and explorative unidirectional analysis were used. Results: From 2010 to 2015, 157 laparoscopic procedures were done for HH or GERD. No death occurred in the group of the patients in connection with the surgery. Similar rates of considerable improvement or resolution of the symptoms such as the reflux, heartburn, and epigastric pain were observed after three months from the surgery among patients after NFP or TFP. Postoperative dysphagia resolved in 4–7 weeks in the group after NFP, and in 3–5 weeks in the group after TFP. Disease recurrence was observed in 36 (22.9%) patients in 2 to 5 years after the surgery. Recurrence of only the gastroesophageal reflux without HH was observed in 22 patients (14 after NFP; 8 after TFP), shown by endoscopy or x-ray assessment, with a satisfactory response to PPI medication. Laparoscopic rehiatoplasty and refundoplication according to Nissen were performed in 14 patients with HH recurrence with/without the reflux (8 after NFP; 6 after TFP). Conclusion: Laparoscopic antireflux surgery of GERD and HH is safe and is associated with very low perioperative morbidity and minimal morbidity. Both NFP and TFP have a similar effect on restoration of the mechanical gastroesophageal barrier against gastric reflux, with significant effect on resolution of GERD and HH symptoms and with a marked improvement of the patient’s quality of life. As regards morbid obese patients, weight reduction is needed before the surgery. Hiatoplasty done at the same time with the bariatric procedure is recommended in morbid obese patients with HH indicated for the bariatric-metabolic surgery., and M. Kasalický, E. Koblihová
