Hyperparatyreóza (HPT) patří k onemocněním, jejichž incidence stoupá. Převážnou většinu primárních hyperparatyreóz (asi 80 %) tvoří adenomy. Karcinom příštítných tělísek je velmi vzácný. K lokalizaci adenomů příštítných tělísek se standardně používá ultrasonografie, scintigrafie, nejčastěji 99mTc-MIBI a CT. Při stanovení diagnózy primární hyperparatyreózy je operační výkon standardním léčebným postupem. Nezastupitelné postavení má peroperační ultrasonografie. V průběhu tří let bylo na našem pracovišti operováno celkem 21 pacientů s diagnózou adenom příštítného tělíska, který byl histologicky potvrzen u 19 pacientů. Náš malý soubor operovaných pacientů vykazuje úspěšnost 90,5 %., Hyperparathyreosis is one of the diseases whose incidence has been increasing. Vast majority of hyperparathyreosis (about 80 %) is made up by adenomas. Carcinoma of parathyroid glands is very rare. Ultrasonography, scintigraphy, most frequently 99mTc-MIBI and CT are standardly being used for localization of the adenomas of parathyorid glands. Operation is a standard medical method in the process of determinating the diagnosis of primary hyperparathyreosis. Peroperative ultrasonography has irreplaceable position. 21 patients with diagnosis of adenoma of parathyroid glands which was histologically confirmed in 19 patiens have been operated in our workplace during three years. Karcinom prsu je nejčastější nádorové onemocnění u žen. Každoročně postihuje více než 1 000 000 žen na celém světě. Z toho zhruba 11 % případů rakoviny prsu lze připisovat konzumaci alkoholu u žen. Cílem tohoto článku je vytvořit představu o podílu nákladů na léčbu karcinomu prsu, který lze přiřadit konzumaci alkoholu v ČR v roce 2007. Zjištěná výsledná částka, kterou je třeba vynaložit na léčbu pacientek pravidelně konzumujících alkohol, je 39 mil. Kč., Breast cancer is the most common cancer in women. Annually, it affects more than 1 million women worldwide. Thereof, some 11% of the breast cancer incidence can be attributed to alcohol consumption in women. The aim of this article is to create a notion of the proportion of the costs spent to treat breast cancer that can be ascribed to alcohol consumption in the Czech Republic in 2007. V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. CD200 and its receptor were recognized as having the multiple immunoregulatory functions. Their immunoregulatory, suppressive, and tolerogenic potentials could be very effectively exploited in the treatment of many diseases, e.g. Alzheimer disease, rheumatoid arthritis, and allergy to name only some. Many research projects are aimed to develop clinically valuable methods being based on the structure and function of these paired molecules. CD200/CD200R are highly conserved type I paired membrane glycoproteins that belong to the Ig superfamily containing a two immunoglobulin‑like domain (V, C). CD200 is broadly distributed in a variety of cell types, whereas CD200R is primarily expressed in myeloid and lymphoid cells. They fulfill multiple functions in regulating inflammation. The interaction between CD200/CD200R results in activation of the intracellular inhibitory pathway with RasGAP recruitment and thus contributes to effector cell inhibition. It was confirmed that the CD200R activation stimulates the differentiation of T cells to the Treg subset, upregulates indoleamine 2,3‑dioxygenase activity, modulates cytokine environment from a Th1 to a Th2 pattern, and facilitates an antiinflammatory IL‑10 and TGF‑β synthesis. CD200/CD200R are required for maintaining self‑tolerance. Karcinóm žalúdka je smrteľná choroba a je stále druhá najčastejšia príčina úmrtí na rakovinu na celom svete. Chirurgický zákrok s kompletnou resekciou tumoru, ktorá má kuratívny potenciál, zostáva hlavnou terapiou pre skoré štádiá. Bohužiaľ, väčšina pacientov s rakovinou žalúdka je diagnostikovaná v pokročilom štádiu, čím paliatívna systémová liečba zostáva jedinou liečebnou možnosťou. Najčastejšia kombinácia chemoterapie ako liečby prvej línie na liečbu pokročilého karcinómu žalúdka (PKŽ) zahŕňa zlúčeniny platiny a fluoropyrimidínov. Fluoropyrimidíny sú chrbtovou kosťou chemoterapeutických režimov pre liečbu karcinómu žalúdka. Teysuno® (S-1) je perorálny štvrtogeneračný fluoropyrimidín, v ktorom sa kombinuje tegafur, čo je prekurzor 5-fluorouracilu (5-FU) a dva biochemické modulátory: (1) 5-chlór-2,4-dihydroxypyridín, čo je silný, ale reverzibilný inhibítor dihydropyrimidíndehydrogenázy, ktorá bráni degradácii 5-FU, a (2), oxonát draselný, ktorý znižuje gastrointestinálnu (GI) toxicitu inhibíciou 5-FU fosforylácie v gastrointestinálnej sliznici. Je to nový liek, ktorý poskytuje výhodu pohodlného a bezpečného perorálneho podávania proti intravenóznemu fluoropyrimidínu v PKŽ., Gastric cancer is a lethal disease and continues to be the second leading cause of cancer death worldwide. Surgical resection remains the main treatment for early stages with complete resection having the potential for a cure. Unfortunately, most patients with gastric cancer are diagnosed in advanced stages, rendering palliative systemic therapy as the only choice of treatment. The most common chemotherapy combination as a first-line treatment in advanced gastric cancer (AGC) includes a platinum compound and fluoropyrimidine. Fluoropyrimidines have been the backbone in the chemotherapy regimens for the treatment of gastric cancer. Teysuno® (S-1) is a peroral fourth-generation fluoropyrimidine that combines tegafur, which is a prodrug of 5-fluorouracil (5-FU), and two biochemical modulators: 5-chloro-2,4-dihydroxypyridine, a powerful but reversible inhibitor of dihydropyrimidine dehydrogenase that prevents 5-FU degradation, and potassium oxonate, which reduces gastrointestinal (GI) toxicity by inhibition of 5-FU phosphorylation in the GI mucosa. Teysuno® (S-1) is considered, as a single agent as the standard of care in Japan for the adjuvant treatment of resected gastric cancer and in combination with cisplatin in the advanced setting based on level 1 evidence in Phase III trials. Chronická neinfekční onemocnění představují závažný problém soudobé společnosti. Jejich etiologie má komplexní charakter. Jsou výsledkem v čase se vyvíjející interakce genetických a vnějších faktorů, jejichž podstatná část je vázána na životní styl a chování jedince. Genetická složka může být životním stylem modifikována, do jisté míry však sama chování a životní styl determinuje. Celoživotní vliv prostředí na genom je z velké části zprostředkován epigeneticky. Epigenetické mechanismy řídí chování genů, jsou podkladem fenotypové plasticity. Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Prenatálně indukované změny epigenomu mohou být předány na další generace potomků. Významným induktorem těchto alterací jsou nutriční faktory a kouření. V souvislosti s environmentální modifikací genové exprese je studována zejména DNA metylace, post-translační modifikace histonů a regulační aktivita nekódující RNA. Potenciální reverzibilita epigenetických procesů otevírá nové možnosti pro prevenci komplexních chorob. Poznatky o interakci genů, chování a prostředí potvrzují opodstatněnost primárně preventivních opatření a mohou přispět ke změně postojů ke zdraví., Non-infectious diseases represent a serious health problem in current society. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases.
