Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years. Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Užívání hormonální antikoncepce je považováno za rizikový faktor vzniku hluboké žilní trombózy a plicní embolie. Od zavedení hormonální antikoncepce v 60. letech 20. století byl u jejích uživatelek pozorován častější výskyt těchto komplikací. Nejnovější studie pak ukázaly dvakrát vyšší riziko vzniku žilní trombózy při užívání kombinovaných preparátů obsahujících progestiny třetí generace (desogestrel, gestoden, norgestimate) ve srovnávání s progestiny druhé generace (levonorgestrel a norgestrel), ačkoli progestiny třetí generace neprokázaly žádný klinicky významný benefit. Přičemž riziko vzniku hluboké žilní trombózy je ještě vyšší u neperorálních forem hormonální antikoncepce (vaginální kroužek, transdermální náplast)., Since the early 1960s, it has been well documented that combined hormonal contraceptives increase the risk of venous thrombosis. Second and third generation oral contraceptives (OCs) differ in their progestin component. Third generation OCs contain desogestrel, gestodene or norgestimate while second generation OCs contain norgestrel or levonorgestrel. In spite of the fact that the third generation OCs have not shown any clinically significant benefit over the second generation OC, multiple studies and two meta‑analyses show that the third generation OC containing desogestrel are associated with approximately doubled risk of venous thrombosis. The newer generation of oral formulations, as well as non‑oral contraceptives (transdermal and vaginal), were recently studied for thrombotic risk. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J.
