V této kazuistice je uváděn případ pacienta hospitalizovaného na Neurochirurgické klinice ve FN Brno. Použitá zbraň je mezi replikami historických zbraní velmi raritní a statistiky poranění dlouhou předovkou neuvádějí., Missile brain injuries are very serious injuries, survival of which depends on early diagnosis and neurosurgical intervention. The choice of a weapon is an important aspect. In this case report, we describe a case treated at the Department of Neurosurgery, University Hospital Brno. The weapon causing this injury was a quite rare replica of a historical weapon and injury statistics do not report injuries caused by a longrifle. - Úvod: Observační, prospektivní a neintervenční projekt SOLOSTAR měl za cíl získat informace z klinické praxe v České republice o spokojenosti pacientů s používáním předplněného inzulinového pera SoloStar. Metodika: 1 805 pacientů s diabetem 1. nebo 2. typu bylo po zahájení používání pera SoloStar sledováno 3–4 měsíce. Spokojenost pacientů s perem SoloStar a s jeho jednotlivými parametry byla posuzována ve skupině všech pacientů a v podskupinách rozdělených dle demografických parametrů včetně zdravotního omezení. Pacienti také porovnávali pero SoloStar s pery používanými před vstupem do projektu. Výsledky: 98,3 % pacientů ohodnotilo celkovou spokojenost s perem SoloStar jako „výbornou“ a „dobrou“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Na celkovou spokojenost s perem SoloStar nemělo statisticky významný vliv žádné z demografických kritérií, a to ani zrakový handicap či omezení manuální zručnosti. Parametry pera SoloStar byly u více než 90 % pacientů hodnoceny jako „výborné“ a „dobré“. Lépe byly některé parametry hodnoceny zejména pacienty bez předchozí zkušenosti s aplikací inzulinu. Pacienti, kteří používali jiný typ aplikátoru před zařazením do projektu, byli více spokojeni s perem SoloStar než s předchozím aplikátorem a většina z nich ohodnotila používání pera SoloStar (93,1 %) a aplikaci inzulinu perem SoloStar (83,5 %) v porovnání s původním perem jako „mnohem jednodušší“ a „jednodušší“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Závěr: Používání pera SoloStar je v běžné klinické praxi v České republice spojeno s vysokou spokojeností pacientů, a to včetně pacientů se zdravotním omezením. Pacienti navíc pero SoloStar upřednostňovali před inzulinovými pery používanými před vstupem do projektu. Methodology: 1 805 patients suffering from type 1 or 2 diabetes who began using the pen SoloStar were observed for 3 to 4 months. Satisfaction of patients with SoloStar and with its particular features was evaluated in the group of all patients and in subgroups defined by demographic parameters including the type of handicap. Patients also compared SoloStar with the pens used before the entry into the survey. Results: 98.3 % of patients rated overall satisfaction with the pen SoloStar as “excellent” and “good” (the highest and second highest rating on a 5-point scale). Demographic criteria, including visual handicap and manual dexterity handicap, did not have any statistically significant effect on the rating of the overall satisfaction with SoloStar. Features of SoloStar were rated as “excellent” and “good” by more than 90% of patients. Higher rating of some of the features of SoloStar was awarded mostly by patients without prior experience with insulin application. Patients who used other insulin applicators before entering into the survey were more satisfied with the pen SoloStar than with the previously used applicator and most of them rated the use of SoloStar (93.1 %) and insulin application with SoloStar (83.5 %) as “much easier” and “easier” (the highest and second highest rating on a 5-point scale) in comparison with the previously used pen. Conclusion: The use of the pen SoloStar in a clinical practice setting in the Czech Republic is associated with high levels of satisfaction of patients, including handicapped patients. In addition, patients preferred SoloStar over insulin pens used before the entry into the survey. Výzkumný soubor tvořilo 51 osob se smíšenou úzkostně depresivní poruchou a 50 osob se z ávislostí na alkoholu., Our paper analyses the issues of shyness as emotiona l personality trait for which the fear of negative judgement is typical, accompanied by emo tional discomfort. The most significant emotion when focusing on our own failure s and functioning is shame. The shyness which becomes part of negative self-image has adverse impact on individual’s life satisfaction and also has close relationship t o mental disorders. To assess the degree of shyness the Czech translation of "Questionnaire of shyness" by Lynne Henderson and Philip Zimbardo (2002) may be used. Ve srovnání s jinými glykosaminoglykany má sulodexid větší antitrombotické vlastnosti při současně nižším riziku krvácení a na rozdíl od heparinů je možné jeho podávání jak parenterálně, tak perorálně. Preparát má prokázány mnohočetné farmakodynamické účinky. Klinickými studiemi je dobře doložena efektivita preparátu u pacientů s klaudikacemi. Evidence účinku je také u nemocných s nejtěžšími případy chronické žilní nedostatečnosti, včetně pacientů s aktivními bércovými vředy. V případě pozitivního výsledku probíhajících studií bude možné jako další indikaci přidat sekundární prevenci žilní trombózy., Sulodexide is a compound belonging to the group of glycosaminoglycans (together with heparins). In comparison with other glycosaminoglycans, sulodexide has stronger antithrombotic properties and a lower risk of bleeding, and unlike heparin, it can be administered both parenterally and orally. Multiple pharmacodynamic effects of the preparation have been demonstrated. Clinical studies have documented the effectiveness of the drug in patients with claudication. There is evidence of its effect also in patients with the most severe cases of chronic venous insufficiency, including patients with active leg ulcers. 639. Surgical and non-surgical therapy of obstructive sleep apnea syndrome in children

- Interventions of paediatric obstructive sleep apnea syndrome are complex, varied and multidisciplinary. The goal of the treatment is to restore optimal breathing during the night and to relieve associated symptoms. Evidence suggests that the surgical intervention with removal of the tonsils and adenoids will lead to significant improvements in the most incomplicated cases, as recently reported from a meta-analysis. However, post-operative persistence of this syndrome in paediatric population is more frequent than expected, which supports the idea of the complexity of this syndrome. Adenotomy alone may not be sufficient in children with OSAS, because it does not address oropharyngeal obstruction secondary to tonsillar hyperplasia. Continuous positive airway pressure can effectively treat this syndrome in selected groups of children, improving both nocturnal and daytime symptoms, but poor adherence is a limiting factor. For this reason, CPAP is not recommended as first-line therapy for OSAS when adenotonsillectomy is an option. It is now being investigated the incorporation of nonsurgical approaches for milder forms and for residual OSAS after surgical intervention. Althought adeno-tonsillar hypertrophy is the most common for OSAS in children; obesity is emerging as an equally important etiological factor. Therefore an intensive weight reduction program and adequate sleep hygiene are also important lifestyle changes that may be very effective in mitigating the symptoms of this syndrome. Pharmacological therapy (leukotriene antagonists, topical nasal steroids) is usually use for mild forms of OSAS and in children with associated allergic diseases. - Our case-based review focuses on limb salvage through operative management of Charcot neuroarthropathy of the diabetic foot. We describe a case, when a below-knee amputation was considered in a patient with chronic Charcot foot with a rocker-bottom deformity and chronic plantar ulceration. Conservative treatment failed. Targeted antibiotic therapy and operative management (Tendo-Achilles lengthening, resectional arthrodesis of Lisfranc and midtarsal joints, fixation with large-diameter axial screws, and plaster cast) were performed. On the basis of this case, we discuss options and drawbacks of surgical management. Our approach led to healing of the ulcer and correction of the deformity. Two years after surgery, we observed a significant improvement in patient's quality of life.
