Nejzhoubnější gynekologický nádor, karcinom ovaria, je příčinou více než 50 % úmrtí v této skupině nádorů. Více než 60 % případů je diagnostikováno až v pokročilých stadiích s výrazně sníženou pravděpodobností přežití pacientek. Diagnostické nástroje pro efektivní časnou detekci nebo screening zatím neexistují. Terapeutické možnosti (chirurgická operace, chemoterapie) jsou nedostatečně efektivní, neboť karcinom ovaria se vyznačuje silnou tendencí k recidivě spojenou s chemorezistencí. Jedním z nejdéle a nejrozsáhleji používaným markerem pro diagnostiku primárních nádorů i recidivy je CA125/MUC16. Tento glykoprotein je znám již od 80. let 20. století, ale teprve nejnovější studie odhalují jeho biologické funkce v karcinogenezi a interakcích s buňkami imunitního systému. V tomto článku se blíže podíváme na jeho biologický význam a především na současný stav použití CA125 jako významného diagnostického markeru u karcinomu ovaria. V diagnostice primárních nádorů se totiž začínají objevovat nové markery, navíc je význam monitoringu CA125 z hlediska detekce recidivy zpochybňován rozsáhlými klinickými studiemi, které nenalezly přínos pro dlouhodobé přežití pacientek. Také potenciální význam CA125 v imunoterapii karcinomu ovaria se nezdá být tak velký, jak se předpokládalo., The most fatal gynecologic malignancy, ovarian cancer, causes more than 50% deaths in this tumor group. Most of cases (>60%) are diagnosed in advanced stages with poor 5-year survival prognosis. Diagnostic tools for an effective early detection or screening have not been found yet. Treatment possibilities (surgery, chemotherapy) are insufficient while high tendency to recurrence and chemoresistance occurs. CA125/MUC16 has been one of the most extensively used markers for a detection of primary tumors, or recurrence for a long time since its discovery in 80´s. However, the structure and biological functions of CA125 have been discovered relatively recently. CA125 may play an important role in carcinogenesis and interactions with cells of immune system. We reviewed the known biological functions and particularly the current state of using this marker as the diagnostic tool in ovarian cancer. Recently, many new markers emerged ambitiously to replace CA125; moreover, the importance of monitoring CA125 for the recurrence detection has also been questioned in large clinical trials. These studies have not found an impact for overall survival/mortality of patients. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus. Cíl: Zhodnocení zkušeností s celotělovou magnetickou rezonancí na 3T systému. Metodika: Vyhodnotili jsme celkem 279 vyšetření provedených na přístroji 3T (SKYRA, Siemens, Německo) provedených během 36 měsíců a posoudili jejich indikace a zejména možnost zobrazení skeletu, ale i parenchymových orgánů při indikacích mnoho četného myelomu, lymfomů a ostatních nádorových onemocnění. Výsledky: Nejčastější indikací zůstává myelom (228), u celkem 35 vyšetření lymfomů jsme získali kvalitní diagnostické zobrazení, které umožňuje posoudit nejenom postižení lymfatických orgánů včetně sleziny, ale i kostní dřeně a plic. Difuzní zobrazení bylo shledáno dostatečné diagnostické kvality, avšak výrazně prodlužuje celý diagnostický protokol. Závěr: Celotělové vyšetření na 3T MRI je plně diagnosticky využitelné při hodnocení stagingu i restagingu mnohočetného myelomu, lymfomů i ostatních nádorů jako je karcinom prostaty., Aim: Evaluation of the experience with whole body magnetic resonance on 3T system. Method: 279 examinations performed on 3T system (SKYRA, Siemens, Erlangen, Germany) performed during 36 months were assessed according indications, possibility of imaging of the bone marrow, organs of the abdomen and lungs, when the indications are multiple myeloma, lymphoma or the other tumors. Results: The most frequent indication remains multiple myeloma (228), in 35 patients was performed lymphoma imaging with reached sufficient diagnostic imaging enabling assessment of the bone marrow, lymph nodes, lungs and parenchymatous organs. Diffusion weighted imaging on 3T is sufficient, but increases the imaging time significantly. Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. The standard treatment according to NCCN is concomitant chemoradiotherapy with a platinum derivative (category 1) or radiotherapy concomitantly with biological therapy with cetuximab, or surgical approach, followed by, according to histology, adjuvant radiotherapy or chemoradiotherapy (1). Recently, HPV positivity has also been taken into consideration concerning indications for treatment. This applies to patients with a better prognosis, but no standard treatment has been determined so far. Cetuximab is a chimeric IgG1 monoclonal antibody that targets the extracellular domain of the EGFR. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%).
