Acenocoumarol is a vitamin K antagonist that is used for the treatment of acquired and congenital, both arterial and venous, thrombotic diseases. Its use is complicated by the narrow therapeutic range. Bleeding following oral anticoagulation, despite rare, remains the major complication. Most cases of hemorrhagic episodes usually require short hospitalization and transfusion, while surgical drainage of the hematoma is not recommended. However, in cases that conservative treatment isn’t successful, surgical intervention remains an option. The Purpose of the study: define the indications and contraindications to, combined operation in locally invasive tumor of the rectum. The Material and methods of the study: we have analysed results combined operation in cancer of the rectum with germination in genital organs, performed in department of coloproctology in National Oncological Scientific centre during 20052009 years. Under observation were 118 women at age from 21 to 68 years. This before 45 years 26 patients, from 46 to 59 years 54 patients, 60 and above years. The Results and their discussion: Postoperative complications suppurativeinflammatory character appeared beside 36 patients (30,5%), most of all after abdomeno perineal extirpation of rectum (35,5%) and abdominoanal resection of the rectum (33,3%). In lesser extend after front resection of the rectum and after operation Hartman (28,5% and 22,7%). The General lethality has formed 3,4%, have died after combined operation 4 patients from 118 operated patients. The Conclusion: thereby, brought data evident that such important factors, as frequency of the origin relapse (28%), 5year probability of survival (37,1%), under combined interference and operation of the standard volume in the cancer of the rectum practically the same., H. Neexistuje jedna obecná defi nice seniorské křehkosti. Někteří autoři ji popisují jako symptom, jiní jako komplexní syndrom, nebo jako nahromadění funkčních defi citů u seniora. Dle odborných publikací se křehkost vyskytuje až u 25 procent seniorů (Holmerová, 2007, s. 100). Rozvoj seniorské křehkosti může být mimo jiné podmíněn multimorbiditou, nežádoucími účinky léků, pohybovou inaktivitou, malnutricí, osamělostí či depresí (Kalvach, 2008, s. 122). Cílem práce bylo vyhledání a analýza odborných zahraničních článků týkající se křehkosti seniorů a následně získané poznatky využít v diagnostice NANDA International. Pro získání relevantních dat byly využity elektronické databáze Pubmed, Medline, ProQuest a Springer a vyhledávač Google. Byla využita specifi cká vyhledávací strategie. Na základě diagnostického algoritmu dle NANDA International byly identifi kovány diagnostické prvky a stanoveny ošetřovatelské diagnózy. Ošetřovatelská diagnostika dle NANDA International se jeví jako vhodná při poskytování ošetřovatelské péče u křehkých seniorů., There is no general definition of frailty in the elderly. Some authors describe it as a symptom, others as a complex syndrome, or as an accumulation of functional defi cits in older adults. According to professional publications, frailty occurs in up to 25 percent of the elderly (Holmerová, 2007, p. 100). Developing of frailty in older adults may be subject to multimorbidity, undesired eff ects of medication, physical inactivity, malnutrition, loneliness or depression (Kalvach, 2008, p. 122). The aim of the article is to search for and analyse foreign scientifi c articles related to frailty in the elderly and to use gained knowledge in NANDA International diagnostics. To obtain the relevant data, electronic databases Pubmed, Medline, ProQuest, Springer and Google were used. Published articles concerning frailty in the elderly were found by a specifi c search strategy. Diagnostic elements were identifi ed and nursing diagnoses were determined according to the diagnostic algorithm by NANDA International. Cíl: Přesnost screeningových škál pro vyhledávání pacientů s rizikem vzniku dekubitů byla u nás dosud zkoumána jen okrajově. Cílem bylo navázat na pilotáž zabývající se inter-rater reliabilitou (shodou mezi posuzovateli) Škály Bradenové (ŠB) a zjistit shodu mezi dvěma posuzovateli u většího vzorku respondentů. Metodika: Dva posuzovatelé nezávisle na sobě hodnotili 60 pacientů hospitalizovaných s neurologickým onemocněním pomocí ŠB. Celková skóre (max. 23 bodů) byla dichotomizována (riziko versus absence rizika) dle doporučeného cut-off skóre (18 bodů). Inter-rater reliabilita byla vyjádřena pomocí koeficientu kappa (κ), a to pro dichotomizovaná celková skóre i pro skóre v jednotlivých položkách škály. Výsledky: Koeficient κ byl pro dichotomizovaná celková skóre 0,564 (95% konfidenční interval [KI] = 0,182; 0,946). V jednotlivých položkách škály byl κ následující: „smyslové vnímání“ 0,557 (95% KI = 0,306; 0,808); „vlhkost“ 0,454 (95% KI = 0,142; 0,766); „aktivita“ 0,613 (95% KI = 0,425; 0,801); „mobilita“ 0,091 (95% KI = – 0,127; 0,309); „výživa“ 0,283 (95% KI = 0,095; 0,471); a „tření a střih“ 0,554 (95% KI = 0,240; 0,868). Závěr: Inter-rater reliabilita dichotomizovaných celkových skóre byla střední úrovně (κ = 0,564). V položce „aktivita“ lze shodu mezi posuzovateli považovat za poměrně dobrou (κ = 0,613); v ostatních položkách však byla nižší. Jsou prezentovány faktory, které mohly tyto výsledky ovlivnit., Aim: The accuracy of the screening scales for identification of patients who are at risk of developing pressure sores has, so far, received only marginal attention in the Czech Republic. The aim was to extend a pilot study that focused on inter-rater reliability of the Braden Scale and to determine inter-rater reliability for a larger patient sample. Methods: Two raters performed independent assessments of 60 patients hospitalized with a neurological disease, using the Braden Scale. Total scores (max. 23 points) were dichotomized (at risk versus not at risk) using the recommended cut-off score (18 points). Using the kappa (κ) coefficient, inter-rater reliability was expressed for the dichotomized total scores as well as for scores obtained for each category of the scale. Results: For the dichotomized total results, the κ coefficient was 0.564 (95% confidence interval [CI] = 0.182; 0.946). For the individual categories, the κ coefficient was as follows: “sensory perception” 0.557 (95% CI = 0.306; 0.808); „moisture“ 0.454 (95% CI = 0.142; 0.766); „activity“ 0.613 (95% CI = 0.425; 0.801); „mobility“ 0.091 (95% CI = – 0.127; 0.309); „nutrition“ 0.283 (95% CI = 0.095; 0.471); and „friction and shear“ 0.554 (95% CI = 0.240; 0.868). Conclusion: For the dichotomized total scores, moderate inter-rater reliability (κ = 0.564) was obtained. Cíl: Cílem výzkumu bylo zjistit vztah reminiscenčních aktivit (RA) a depresivity u seniorů žijících v institucionální péči. Sledována byla míra depresivity u seniorů starších 65 let. Byl sledován rozdíl mezi skupinou experimentální a kontrolní po 50 - ti seniorech. Metodika: Pro sběr dat a vymezení depresivity respondentů byl použit screeningový nástroj Škála deprese pro geriatrické pacienty (GDS). Studiem zdravotní dokumentace byla sbírána data, jako je věk, výsledky kognitivního testu MMSE a data z oblasti psychofarmakologické medikace, jelikož je to významný faktor ovlivňující míru depresivity. Hodnocení depresivity proběhlo ve dvou měřeních. U experimentální skupiny proběhla měření před a po absolvování strukturovaných RA. U kontrolní skupiny byla měření provedena ve stejném časovém období, jako ve skupině experimentální, ale již bez RA. Výsledky: Prevalence depresivity v celém souboru respondentů byla 43 %. Depresivita v experimentální skupině byla prokázána u 20 % respondentů. Ve stejném časovém období u skupiny kontrolní byla zjištěna depresivita u 23 % respondentů. Po absolvování RA klesla depresivita u experimentální skupiny o 4 %. Ve zkoumaném souboru bylo statisticky potvrzeno, že RA snižují míru depresivity u seniorů v institucionální péči. Přítomnost depresivní symptomatologie po absolvování RA u experimentální skupiny byla nižší než u skupiny kontrolní. Závěr: Deprese je vážným a život ohrožujícím onemocněním, které se podílí na snížení kvality života seniorů a jejich rodin. Správná diagnostika a léčba deprese vede ke zlepšení kvality života seniorů a jejich rodin. RA jsou významnou psychosociální nefarmakologickou intervencí, která může výrazně přispět ke zlepšení kvality života., Aim: The aim of this research was to determine the relationship between reminiscence activities (RAs) and depression in seniors living in institutional care. The rate of depression was studied in seniors over 65 years of age. Differences between experimental and control groups, each comprising 50 individuals, were assessed. Methods: The Geriatric Depression Scale was used for data collection and identification of depression in the participants. Medical records were used to ascertain their age, Mini Mental State Examination results and psychopharmacological drug use, a significant factor affecting the rate of depression. Depression was evaluated by two measurements. In the experimental group, the measurements were carried out before and after completion of structured RAs. In the control group, the measurements were carried out at the same times but no RAs were provided. Results: The overall prevalence of depression in all participants was 43%. In the experimental and control groups, the rates were 20% and 23%, respectively. After completion of RAs, the rate decreased by 4% in the experimental group. RAs were confirmed to lower the rates of depression in seniors in institutional care. Depressive symptoms were less frequent after RAs in the experimental group than in the controls. Conclusion: Depression is a serious and lifethreatening disease contributing to impaired quality of life of both seniors and their families. Correct diagnosis and treatment of depression lead to improved quality of life of seniors and their families.
