Cíl: Cílem výzkumného šetření bylo získat informace o protektivních a rizikových faktorech stravování a stravovacích návyků ve vztahu k prevenci kolorektálního karcinomu, a to u záměrně vybrané skupiny adolescentů a dospělých. Výsledky byly porovnávány mezi oběma skupinami a s údaji publikovanými v odborných časopisech. Metodika: Pro výzkum bylo zvoleno kvantitativní dotazníkové šetření za pouţití nestandardizovaného dotazníku vlastní tvorby hodnotícího způsob stravování u obou skupin respondentů. Šetření probíhalo ve dvou záměrně vybraných středních školách a ve čtyřech podnicích v Hradci Králové v období října aţ prosince 2012. Výsledky: Celkem bylo do výzkumného šetření zařazeno 343 respondentů, z toho 123 adolescentů (36 %) a 220 dospělých (64 %). Statistické testování prokázalo, ţe se dospělí stravují kvalitněji neţ adolescenti, přesto však obě skupiny konzumují ovoce a zeleninu v nedostatečném mnoţství a naopak často konzumují červené maso, masné výrobky a potraviny upravené smaţením. Špatné stravovací návyky dosvědčuje i prevalence nadváhy a obezity; 20,4 % dotázaných adolescentů a 63,6 % dospělých má vyšší neţ normální váhu. Třetina adolescentů se navíc stravuje v řetězcích s rychlým občerstvením. Závěr: Zdravá výţiva a vhodné stravovací návyky hrají klíčovou roli v prevenci mnoha onemocnění. Spolu s adekvátní pohybovou aktivitou a nekouřením zásadně sniţují riziko vzniku nejen kolorektálního karcinomu, ale i dalších nádorových i nenádorových onemocnění. Přesto však způsob stravování není ze zdravotního hlediska příznivý a přibývá osob s nadměrnou hmotností., Aim: The aim of the research was to get information about protective and dangerous factors of nutrition and eating habits in relation to the prevention of colorectal cancer. It focuses on intentionally chosen groups of adolescents and adults. The results were compared between both groups of respondents and with information published in professional periodicals. Methods: Quantitative research based on non-standardized questionnaires of own formulation evaluating a way of alimentation of both groups of respondents was chosen. The research took place in two chosen secondary schools and in four companies in Hradec Králové in a period from October to December 2012. Results: The total amount of 343 respondents participated in the research, namely 123 adolescents (36 %) and 220 adults (64 %). Statistical testing has shown that the eating habits of adults are of a higher quality than the eating habits of adolescents. Nevertheless, both groups consume insufficient amount of fruit and vegetables. On the other hand, they frequently consume red meat, meat products and fried food. Bad eating habits are proven even by the quantity of overweight and obesity occurrence; 20,4 % of interviewed adolescents and even 63,6 % of interviewed adults are of a higher than normal weight. Moreover, one third of adolescents eat in fast-foods. Conclusion: Healthy nutrition and correct eating habits play a key role in the prevention of many diseases. Together with adequate physical activity and no smoking they reduce essentially the risk of occurrence not only of colorectal cancer but also of other cancerous and noncancerous diseases. Cieľ: Cieľom práce bolo posúdenie výskytu stresu, stresorov, stratégií zvládania stresu a prediktorov stresu u sestier poskytujúcich paliatívnu starostlivosť (PS). Metodika: V rámci štúdie bol použitý štandardizovaný dotazník – Inventár copingových stratégií a dotazník zameraný na posúdenie stresu a stresujúcich situácií. Pri spracovaní údajov boli použité deskriptívne štatistické metódy a lineárna regresná analýza. Výsledky: Prieskumu sa zúčastnilo 68 sestier poskytujúcich PS s priemerným vekom 37,9 rokov a priemernou dĺžkou praxe v PS 8,22 rokov. Celkovo sme zaznamenali iba zriedkavý výskyt stresu v sledovanej vzorke sestier, pričom reakcie na stres sme zaznamenali najmä v emocionálnej a fyzickej oblasti. Najvýznamnejšími stresormi boli smrť a umieranie pacienta, pracovná záťaž (hlavne nedostatok personálu), pracovné prostredie a rodina pacienta. Sestry v stresových situáciách využívali viac príklonové stratégie zvládania stresu – najmä riešenie problému a sociálnu oporu. Signifikantnými prediktormi stresu boli v tejto vzorke pasívne stratégie zvládania (sebaobviňovanie) a mladší vek. Tieto premenné vysvetľovali 26,6 % variability stresu u sestier poskytujúcich PS. Záver: Ako optimálne sa javia programy zamerané na znižovanie vulnerability sestier, efektívnejšie využívanie aktívnych stratégií zvládania stresu a účinnejšiu komunikáciu s rodinou pacienta., Aim: The aim of this study is to assess the occurrence of stress, stressors, and stress coping strategies and stress predictors in nurses providing palliative care (PC). Methods: The study used a standardized questionnaire – Coping Strategies Inventory and a questionnaire aimed to assess stress and stressful situations. Data were processed by descriptive statistical methods and a linear regression analysis. Results: Sixty eight nurses working in PC at the average age of 37.9 years and the average term of practice in PC of 8.22 years participated in the survey. Overall, we only observed rare occurrence of stress in the study sample of nurses, and stress and we have noted responds to stress particularly in the physical and emotional area. The highest stressors were patient’s death and dying, workload (particularly staff shortages), working environment and relations with patient’s family. Nurses rather used active stress coping strategies in stressful situations, especially problem-solving and social support. Passive stress-coping strategies (self-criticism) and younger age were significant predictors of stress in this sample group. These variables explained 26.6% of stress variability in nurses working in PC. Conclusion: Training focused on decreasing vulnerability of nurses, more efficient using of active stress-coping strategies, and more efficient communication with patient’s family seems to be an appropriate solution to reduce stress., Mária Sováriová Soósová, Jana Sušinková, Mária Cenknerová, and Literatura

Cíl: Cílem našeho příspěvku bylo zjistit vztah mezi konstrukty kvality života související se zdravím Health Related Quality of Life, používaný ve standardní zkratce HRQoL a subjektivní pohody Subjective well-being, používaný ve standardně zavedené zkratce SWB, u pacientů s chronickým onemocněním, konkrétně bronchiálním astmatem. Zaměřujeme se na diskriminační validitu obou konstruktů. Metodika: Výzkumný soubor tvořilo 316 pacientů, kteří byli vyšetřeni na plicní ambulanci ve Fakultní nemocnici Ostrava. HRQoL byla hodnocena prostřednictvím zkrácené verze Dotazníku kvality života astmatiků (Mini Asthma Quality of Life Questionnaire, Mini AQLQ), SWB prostřednictvím Indexu osobní pohody (Personal Wellbeing Index, PWI). Ke zjištění závislosti mezi vybranými proměnnými byl použit Pearsonův korelační koeficient a lineární regresní analýza. Diskriminační validitu jsme zkoumali prostřednictvím zkoumání rozdílů v HRQoL a SWB z hlediska vybraných klinických proměnných. Rozdíly v HRQoL a SWB byly zjišťovány vícefaktorovou analýzou rozptylu (ANOVA). Výsledky: Korelace mezi HRQoL (operacionalizované prostřednictvím Mini AQLQ) a SWB (operacionalizované prostřednictvím PWI) jsou pozitivní a středně silné. Percepce kontroly astmatu byla identifikovaná jako signifikantní prediktor obou konstruktů, avšak vytvářela vyšší míru variability v rámci HRQoL. Závěr: SWB a HRQoL představují související, avšak odlišné konstrukty. Výsledky podporují chápání HRQoL jako konstruktu založeném na větší míře klinických proměnných než je tomu u SWB., Aim: The goal of the study was to determine the relationship between the health-related quality of life (usually abbreviated as HRQoL) and subjective well-being (usually abbreviated as SWB) of patients with chronic disease, namely bronchial asthma. The focus was on discriminative validity of both constructs. Methods: The research sample consisted of 316 patients examined at the Pulmonary Clinic of the University Hospital Ostrava. HRQoL was assessed by a short version of the Asthma Quality of Life Questionnaire (Mini AQLQ) and SWB was assessed by the Personal Wellbeing Index (PWI). Pearson’s correlation coefficient and linear regression analysis were used for evaluation of dependencies between selected variables. Discriminative validity was explored by examination of differences between HRQoL and SWB relevant to selected clinical variables. Differences in HRQoL and SWB were detected by multi-factor analysis of variance (ANOVA). Results: The correlations between HRQoL (assessed by the Mini AQLQ) and SWB (assessed by the PWI) are positive and moderate. Perception of asthma control was identified as a significant predictor of both constructs. However, it produced greater variance in the context of HRQoL. Conclusion: SWB and HRQoL are related but distinct constructs. Přehled problematiky: Magnézium je čtvrtým nejčastějším kationtem v lidském organizmu a po draslíku druhým kationtem v intracelulárním prostoru. Účastní se jako kofaktor enzymů metabolických reakcí, tvorby a utilizace energie a je nezbytný k udržení elektrolytové rovnováhy. Tělo dospělého člověka obsahuje asi 0,043 % (535-730 mmol) magnézia, což je v přepočtu asi 22-30 g (60 % v kostěném skeletu, 40 % intracelulárně ve svalovině a měkkých tkáních, 1 % (7,5-10 mmol) je obsaženo v extracelulární tekutině). Denní potřeba magnézia se odhaduje na 10-20 mmol, tj. 200-400 mg. Ve vztahu k podané dávce pak platí, že čím vyšší podaná dávka, tím nižší procento rezorbce ze střeva. Z doporučené denní dávky 15 mmol (365 mg) se rezorbuje pouze okolo 1/3. Význam magnézia v těhotenství: Důsledkem manifestního nedostatku magnézia může být: 1. zvýšené riziko potratu - předčasný porod (inkompetence hrdla děložního, předčasný odtok vody plodové), 2. placentární insuficience a hypotrofizace plodu, 3. rozvoj gestózy u matky, 4. zvýšené riziko astmatu a ekzému u dětí. Magnézium sulfát, podávaný těhotným ohroženým předčasným porodem (23+0 -30+0), snižuje riziko dětské mozkové obrny u přežívajících novorozenců. Souhrn: Nedoporučuje se používat intravenózní magnézium sulfát v indikaci léčby hrozícího předčasného porodu déle jak 5-7 dní. Tato omezení se nijak nevztahují na perorální užívání magnézia., Objectives: Magnesium is the fourth most common cation in the human body, and the second cation to potassium in the intracellular space. It participates as a cofactor of enzymes in the metabolic reactions, creation and utilization of energy and is necessary for the maintenance of electrolyte balance. The body of an adult contains about 0.043% (535-730 mmol) of magnesium, which is the equivalent of about 22-30 g (60% in the bony skeleton, 40% intracellular in muscles and soft tissue, 1% (7.5-10 mmol) in extracellular fluid). The daily requirement of magnesium is estimated at 10 to 20 mmol, i.e. 200-400 mg. As for dosage, the higher the dose, the lower the percentage of absorption from the intestine. Of the recommended daily dose of 15 mmol (365 mg), only about a third is absorbed. The importance of magnesium in pregnancy: implications of a manifest lack of magnesium may include: 1. increased risk of miscarriage – preterm labor (cervical incompetence, premature rupture of membranes); 2. placental insufficiency and fetal hypotrophy; 3. development of maternal gestosis; 4. increased risk of asthma and eczema in neonates. Magnesium sulfate given to pregnant women with threatened preterm labor (23+0 – 30+0) reduces the risk of cerebral palsy in surviving infants. Summary: In cases of threatened preterm labor it is not recommended to administer magnesium sulfate intravenously for more than 5-7 days. Cíl práce: Aktuální pohled na těhotné s roztroušenou sklerózou (RS), analýza výsledků těhotenství těchto nemocných ve FN Motol v letech 2003-2011. Typ studie: Retrospektivní analýza Soubor a metodika: Analýza 76 těhotenství pacientek s relaps-remitentní formou roztroušené sklerózy – posouzení vlivu gravidity na aktivitu onemocnění (použitá analgezie a laktace ve vztahu ke vzniku akutní ataky) a naopak vlivu onemocnění na průběh těhotenství (komplikace, vedení porodu a perinatální výsledky). Výsledky: Akutní ataka v graviditě se v souboru 76 žen vyskytla u 4 z nich (5,3 %, RR 0,08), do půl roku po porodu jsme akutní ataku zaznamenali u 17 žen (22,4 %, RR 0,46). Ataku mělo 18,4 % kojících žen (9/49) ve srovnání s 33,3 % žen (8/24), které nekojily, p = 0,2375 (OR = 0,45, 95 % CI 0,15-1,37). Epidurální analgezii k porodu mělo 13,6 % žen (3/22) s akutní atakou, 25,9 % žen (14/54) s atakou po porodu bylo bez analgezie, p = 0,3648 (OR = 0,45,95 % CI 0,12-1,76). Zhoršení choroby jsme neprokázali (EDSS 1,4, resp. EDSS 1,6). 75 % těhotenství bylo ukončeno nekomplikovaným vaginálním porodem. Výskyt závažných těhotenských komplikací nebyl zvýšen a perinatální výsledky byly srovnatelné s běžnou populací. Závěr: V souladu s recentními studiemi prokazujeme v našem souboru pokles aktivity choroby v graviditě s nárůstem počtu akutních atak po porodu. Ke zhoršení či progresi onemocnění však během 12 měsíců po porodu nedochází. Kojení ani použití epidurální analgezie nemá na vznik akutní ataky vliv, těhotenství probíhají ve většině případů fyziologicky, vedení porodu a novorozenecké výsledky se neliší od běžné populace. Gravidita je tedy pro stabilizované pacientky s relaps-remitentní formou RS bezpečná., Objective: Current view on pregnant women with multiple sclerosis (MS), the analysis of pregnancy outcomes of these women with MS in the Motol Hospital in the years 2003-2011. Design: Retrospective analysis Methods: Analysis of 76 pregnant women with MS – to assess the impact of pregnancy on the activity of the disease (breastfeeding and used analgesia in relation on the relapse rate post partum) and the impact of the disease on the course of pregnancy (pregnancy complications, differences in the management of labor, and perinatal outcomes). Results: Relapse during pregnancy occurred in 4 out of the 76 women (5.3% relapse rate – RR 0.08). 17 women experienced a post partum relapse in the first six months after delivery (22.4%, RR 0.46). 18.4% breastfeeding women (9/49, 18.4%) had post partum relapse in comparison with 33.3% women, who did not breastfeed (8/24, 33.3%), p = 0.2375 (OR=0.45, 95% CI 0.15-1.37). 13.6% women (3/22, 13.6%) with post partum relapse used epidural analgesia (EDA), 25.9% women (14/54, 25.9%) with relapse did not used it, p = 0.3648 (OR = 0.45, 95% CI 0.12-1.76). Pregnancy did not influence the progress of disability (EDSS 1.4 or EDSS 1.6). 75% women had uncomplicated vaginal delivery, the incidence of serious pregnancy complications was not increased, and the perinatal outcomes are comparable with the general population. Conclusion: Consistent with recent studies, we evaluated decreasing relapse rate during pregnancy. In the post partum period the relapse rate has increased, however the pregnancy did not influence the disability progress in 12 months postpartum. Neither breastfeeding nor epidural analgesia correlated with presence of post partum relapses. In most cases, the pregnancies in patients with MS were physiological, the method of delivery and the overall perinatal outcomes are comparable with the general population. Competencies develop and changes throughout a persons life, they can gain or lose, going through various age stages. Their development does not end in youth, but continue on through the adult life. The ability to thing and reflect those thoughts specifically come forward in the center of structure of competency, which grows at the same time as the individual matures. One of the competency types is health competency. Health competency is a relatively new concept; it is not sufficiently researched. Aim of the study was to determine the factors of an adult individual health competency. 827 respondents participated in the study, in the processing of data was used SPSS. Was used factor analysis, analysis of variance with ANOVA and KruskalWallis test and Pearsons correlation. It was found that health competency is affected by several factors. These are: health education, health behavior, and the value of the environment. Each of the sets was distributed to key factors. The acute destructive pneumonias (ADP) occupy up to 80% of the total number of pneumonias. They require constant improvement of treatment strategy. Nowadays the use of surfactants is a part of most treatment protocols. The aim was to study the features of the solid phase bronchoalveolar lavage in children with the ADPs in the dynamics of complex treatment with exogenous surfactant. Material and methods: We examined 39 patients of contaminated surgery. We identified 2 groups of patients. The patients of first group (n=27) had pulmonary pleural form of ADP, the second group (n=12) had pulmonary form of ADP. All patients got classical treatment and the earlier draining of pleural cavity. We used as an antiseptic reamberin 1.5% by 10 ml/kg and endobronchially injected exogenous surfactant Bl in dose12 mg/kg body weight a day, 6 mg/kg every 12 hours. All the children were made a bronchoscopy to obtain BAL to study the crystallization properties. The solid phase of BAL was studied by method of cuneal dehydration. Results: All facies before treatment were divided into two groups according to classification of facies of biological fluids. Only the facies of the second and the third types were detected there. It was revealed that the sizes of the zones of the facies were different in the comparison groups before treatment and after. And the level of crystalline structures and amorphous aggregates were different in the groups with different degrees of inflammation. Conclusion: So, we can assume that the change in surfactant system is characterized by changes in the morphological structure of solids phases of BAL. Aim: The aim of this paper is to develop an evidence-based clinical protocol for the use of gum chewing in postoperative care to reduce the risk of paralytic postoperative ileus in patients who have undergone abdominal surgery. Methods: A clinical question (For adult patients who have undergone abdominal surgery, does the use of gum chewing postoperatively reduce the risk of paralytic postoperative ileus in comparison with the usual care regimen?) was formulated, a population and setting were defined, and the databases OVID Medline®, CINAHL, and PubMed were searched for relevant material. Six data-based research articles were chosen for review: four randomized controlled trials and two meta-analyses. Finally, the articles were critically appraised to generate evidence on which to base the clinical protocol. Results: The development team created a detailed description of the clinical protocol as well as a protocol algorithm to assist clinicians in determining patient eligibility. In addition, protocol implementation and evaluation plans were proposed.
