Bio-degradable stents are be made of different synthetic polymers (like polylactide or polyglycolide) or their co-polymers (polydioxanone). They can be used for treating benign stenoses of the small and large intestine, particularly in Crohn’s disease. Endoscopic introduction of bio-degradable stents into small and large intestinal stenoses is feasible and relatively simple. Initial results are encouraging and the complication rate is low. However, there are still some difficulties that need to be overcome. The rate of early stent migration is still rather high (up to one third of patients). This might be solved by changes in the shape or rigidity of the stents as well as by further improvement in the design. Úvod: Nízkomolekulární inhibitory tyrozinkináz receptoru pro epidermální růstový faktor (EGFR) představují moderní, účinné preparáty užívané k léčbě pacientů s pokročilým nemalobuněčným karcinomem plic (NSCLC). Aktivační mutace genu EGFR predikují dobrý efekt léčby EGFR tyrozinkinázovými inhibitory. Cílem této studie bylo zmapování výskytu mutací genu EGFR u pacientů s NSCLC v České republice. Metodika: Celkem 486 pacientů s pokročilým stadiem NSCLC (stadium IIIB a IV) neskvamózního histologického typu bylo vyšetřeno na přítomnost mutace genu EGFR na třech pneumoonkologických pracovištích. Porovnání zastoupení EGFR mutace dle pohlaví, věku a kouření bylo provedeno pomocí Fisherova-exaktního testu. Výsledky: Mutace genu EGFR byla prokázána u 74 (15,2 %) pacientů. Mutace byly prokázány u 25 (8,8 %) mužů vs. u 49 (24,3 %) žen (p < 0,001), u 12 (6,7 %) kuřáků vs. u 20 (11,2 %) exkuřáků vs. u 38 (37,3 %) nekuřáků (p < 0,001) a u 32 (15,7 %) pacientů do 65 let věku vs. u 42 (14,9 %) pacientů nad 65 let věku (p = 0,898). Závěr: Mutace genu EGFR byly prokázány u 15,2 % vyšetřených pacientů, nejčastěji u žen a nekuřáků. Dosažené výsledky jsou dobře srovnatelné s doposud publikovanými daty., Introduction: Low-molecular-weight tyrosine kinase inhibitors for epidermal growth factor receptor (EGFR) are modern, effective agents used to treat patients with advanced non-small-cell lung carcinoma (NSCLC). Activating EGFR gene mutations predict a good effect of treatment with EGFR tyrosine kinase inhibitors. The goal of this study was to map the occurrence of EGFR gene mutations in patients with NSCLC in the Czech Republic. Methods: A total of 486 patients with advanced stage of NSCLC (stage IIIB and IV) of nonsquamous histological type were investigated for the presence of EGFR gene mutations at three pneumo-oncology centres. A comparison of distribution of the EGFR mutation according to sex, age, and smoking status was performed by means of Fisher's exact test. Results: EGFR gene mutation was demonstrated in 74 (15.2%) patients. Mutations were shown in 25 (8.8%) men vs. 49 (24.3%) women (p < 0.001), in 12 (6.7%) smokers vs. 20 (11.2%) ex-smokers vs. 38 (37.3%) non-smokers (p < 0.001), and in 32 (15.7%) patients under 65 years of age vs. 42 (14.9%) patients over 65 years of age (p = 0.898). Conclusion: EGFR gene mutations were shown in 15.2% of the patients examined, most commonly in female patients and non-smokers. Cystosarcoma phyllodes je málo častý typ nádoru prsu. Jsou popsány jak benigní, tak i maligní formy tohoto nádoru. Většina studií se shoduje na tom, že benigní tumory jsou dominantní (asi 80–85 %) a asi pouze 15–20 % představuje maligní formu. Ze všech typů nádorů prsu představuje cystosarcoma phyllodes pouze asi 1 %, přesto se jedná o nejčastější non-epiteliální tumor prsu. V kazuistice popisujeme případ mladé ženy s diagnózou cystosarcoma phyllodes, u které byl použit v rekonstrukci prsu DIEP lalok., Cystosarcoma phyllodes is an uncommon type of breast tumour. Both benign and malignant types of this tumour have been described. The majority of studies agree that benign tumours are predominant (approximately 80-85%) and the malignant form only accounts for 15-20%. Of all types of breast cancers, cystosarcoma phyllodes only accounts for about 1%; however, it is the most frequent non-epithelial breast tumour. Melanocytární léze vykazují značnou morfologickou heterogenitu. Jejich diagnostika je náročná i přes současné využití moderních histopatologických a imunohistochemických metod. Významným přínosem v diagnostice melanocytárních lézí je zavedení molekulárně genetické metody fluorescenční in situ hybridizace s použitím čtyřbarevné sondy značící geny CCND1, RREB1, MYB a centromeru chromozomu 6. V letech 2008–2012 bylo v naší laboratoři úspěšně metodou fluorescenční in situ hybridizace vyšetřeno 160 vzorků různých typů melanocytárních lézí, 44 névů a 116 melanomů. Změny asociované s diagnózou melanomu byly prokázány u 116 vzorků (72,5 %), normální nález u 41 vzorků (25,6 %), ve dvou případech bylo rozhodnutí na hranici hodnotitelnosti metody (1,2 %) a v jednom případě se jednalo o vyšetření recidivy melanomu v jizvě, který byl svým nálezem atypický (monozomie chromozomu 6) (0,6 %). Na základě dosažených výsledků byla stanovena senzitivita metody 96,6 % a specifita metody 91%. Tyto výsledky ukazují, že fluorescenční in situ hybridizace je vhodnou metodou pro zajištění přesné a rychlé diagnostiky nejednoznačných melanocytárních lézí. Právě rychlost a správnost diagnózy je u vysoce maligních melanomů zásadní a má neoddiskutovatelný vliv na prognózu a úspěšnost léčby pacienta., Melanocytic lesions exhibit significant morphological heterogenity, making their diagnosis difficult, although both sophisticated histopathological and immunohistochemical methods are used. The introduction of molecular genetic method flourescence in situ hybridization (FISH) has contributed significantly to the diagnosing of melanocytic lesions. Between 2008 and 2012, we studied a total of 160 samples of different types of melanocytic lesions, including 44 nevi and 116 melanomas, using a four-color probe mix for CCND1, RREB1 and MYB genes and for the centromere of chromosome 6. A positive result was found in 116 melenoma samples (72.5 %) and a negative result in 41 samples (25.6 %). A borderline value was found in two samples (1.2 %), and an atypical finding (monosomy of chromosome 6) was seen in a melanoma recurrence in one case (0.6 %). Sensitivity and specificity were 96.6 % and 91 %, respectively. These results show that FISH is a suitable and relatively low-cost method for accurate, rapid diagnosis of ambiguous melanocytic lesions.
