Cíl: Cílem pilotní studie bylo zjistit, zda dochází v průběhu klokánkování ke snížení srdeční a dechové frekvence u nedonošených novorozenců, zjistit zda klokánkování ovlivní periferní saturaci krve kyslíkem a délku hospitalizace nedonošených novorozenců na Jednotce intenzivní a resuscitační péče o novorozence. Dále zjistit, zda klokánkování ovlivňuje interakci matka - dítě. Metodika: Metodou studie byl experiment. Technikou sběru dat k posouzení efektu klokánkování bylo měření fyziologických funkcí. Interakce mezi matkami a jejich dětmi byly měřeny dotazníkem - Maternal Postnatal Attachment Scale (MPAS). Ke statistickému zpracování byl využit t-test pro dva výběry, neparametrický Wilcoxonův test, chí test a Fisherův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z fyziologických funkcí byl signifikantně potvrzen vzestup tělesné teploty. Při porovnání intervenční a kontrolní skupiny před klokánkováním nebyl v interakcích matek k jejich dětem zaznamenán žádný signifikantní rozdíl. Před propuštěním (resp. po klokánkování) byl zájem o dítě u matek klokánkovaných dětí významně vyšší (p = 0,0354) oproti matkám, které své dítě neklokánkovaly. Ve všech položkách MPAS byla úroveň připoutání ke klokánkovanému dítěti před propuštěním vyšší než u neklokánkovaných novorozenců. Závěr: Klokánkování prokázalo pozitivní efekt na nedonošené novorozence v oblasti vzestupu tělesné teploty, zvýšeného zájmu o potřeby novorozence a silnějšího připoutání matky k novorozenci., Objective: The pilot study aimed at determining whether kangaroo mother care decreases heart and respiration rates in preterm newborns, affects saturation of peripheral oxygen and length of newborns’ hospital stay at a neonatal intensive care unit, and influences mother-child interaction. Methods: The method of study was an experiment. To assess the effect of kangaroo mother care, physiological functions were measured. Mother-child interaction was assessed using the Maternal Postnatal Attachment Scale (MPAS). Statistical analyses were carried out with a two-sample t-test, non-parametric Wilcoxon test, chisquare test and Fischer’s exact test. The statistical tests were evaluated at a significance level of 5%. Results: Of the physiological functions studied, only body temperature was confirmed to be significantly increased. There was no significant difference in mother-child interaction prior to kangaroo care between the study and control groups. After kangaroo care (i.e. prior to hospital discharge), interest in their babies was significantly higher (p=0.0354) in mothers providing kangaroo care than in the controls. In all MPAS items, pre-discharge levels of maternal bonding were higher in the group of children provided with kangaroo care than in the controls. Cíl: Cílem studie bylo ověřit využitelnost ošetřovatelských intervencí NIC v ošetřovatelské praxi domácí péče a porovnat záznam aktuálních ošetřovatelských intervencí evidovaných v ošetřovatelské dokumentaci agentury domácí péče s navrhovaným souborem ošetřovatelských intervencí NIC. Metodika: Pilotní soubor o velikosti 110 ošetřovatelských intervencí a 2150 ošetřovatelských aktivit byl sestaven podle taxonomie NIC (Nursing Intervention Classification), jeho ověřování proběhlo u 30 klientů agentury domácí péče v roce 2011. Analýzou ošetřovatelské dokumentace 200 klientů stejné agentury byly pak sledovány reálné intervence a aktivity sester zaznamenané v ošetřovatelském plánu. Výsledky: Z pilotního souboru intervencí NIC využily sestry celkem 39 ošetřovatelských intervencí pro 221 potvrzených ošetřovatelských diagnóz NANDA-I. Nejvíce intervencí (73 %) bylo indikováno pro ošetřovatelské diagnózy Zhoršená pohyblivost, Riziko infekce, Riziko deficitu tělesných tekutin a Riziko porušení kožní integrity. Mezi nejčetnější ošetřovatelské intervence, které sestry ze souboru NIC vybíraly, patřily: Hospodaření s energií, Prevence pádu, Péče o ránu, Učení: předepsané činnosti/cvičení a Podpora cvičení. Ošetřovatelské činnosti reálně vykonávané sestrami u klientů ADP a zaznamenané v ošetřovatelském plánu (44 různých ošetřovatelských intervencí, 1406 provedených výkonů) se lišily od ošetřovatelských intervencí, které byly potvrzeny při pilotním ověřování. Pojmenování ošetřovatelských intervencí sestrami agentury domácí péče nereflektovalo terminologii NIC, proto nebyla data vzájemně porovnávána. Závěr: Používání jednotné ošetřovatelské terminologie je důležité pro přesnou a jednoznačnou komunikaci ošetřovatelských profesionálů, pro popis a evidenci práce sester a doložitelné vykazování ošetřovatelských činností., Aim: The aim of this study was to verify the utilization of nursing interventions NIC in home care and to compare the report of current nursing interventions recorded in nursing documentation in home care agency with proposed set of nursing interventions NIC. Methods: The sample of 110 nursing interventions and 2150 nursing activities was compiled according to taxonomy NIC (Nursing Intervention Classification), its verification was carried out in 30 home care agency clients in 2011. Using the analysis of nursing documentation in 200 clients of the same agency, were then monitored real interventions and activities by nurses recorded in nursing care plan. Results: From the pilot set of NIC interventions nurses have used 39 of nursing interventions for 221 confirmed nursing diagnoses NANDA-I. The most of interventions (73%) were indicated for nursing diagnoses: Impaired physical mobility, Risk for infection, Risk for deficient fluid volume and Risk for impaired skin integrity. Among the most frequent nursing interventions, which nurses have selected from NIC file, were: Energy management, Fall prevention, Wound care, Teaching: prescribed activities/exercise. Nursing activities actually performed by nurses in home care clients and recorded in nursing care plan (44 different nursing interventions, 1406 performed procedures) differed from nursing interventions that were confirmed during the pilot verification. Naming of nursing interventions by the nurses in home care agency did not reflect the NIC terminology, thus the data were not mutually compared. The article represents an overview of the development of Czech health promotion in the last two decades. Current “state of art” and health promotion in international context are mapped. Major behavioral risk factors are described and analybesity in comparison to EU countries. Their impact on the Czech population health is discussed in terms of relevant diseases. Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. Cíl. Cílem práce je zhodnotit výskyt pozdních komplikací a reintervencí po endovaskulární léčbě subrenálního aneuryzmatu břišní aorty stentgraftem. Materiál a metoda. Od dubna 1996 do prosince 2011 bylo na našem pracovišti léčeno 270 pacientů s aneuryzmatem břišní aorty implantaci stentgraftu, z toho bylo 233 mužů a 37 žen ve věku od 49 do 91 let. Věkový průměr byl 68,7 let. Nejméně rok po léčbě byli sledováni 204 pacienti, u nich byl hodnocen výskyt pozdních komplikací (ruptura, migrace stentgraftu, endoleak, ischemické komplikace a zánětlivé komplikace) a reintervencí. Výsledky. V souboru 204 nemocných bylo zjištěno 25 pozdních komplikací, tj. 12, 3% (25/204), a bylo provedeno 21 reintervencí, tj. 10,3 % (21/204). Z 25 komplikací byl zjištěn endoleak I. typu 5krát, endoleak II. typu se zvětšeným vakem aneuryzmatu 3krát, endoleak III. typu 4krát, endotension 3krát, migrace stentgraftu 4krát, ischemické komplikace 5krát a infekce lkrát. Endoleaky I. typu a migrace byly řešeny implantaci prodlužujícího segmentu a migrace u jednoho nemocného chirurgickou konverzí. Endoleaky III. typu byly léčeny implantaci stentgraftu a endoleaky II. typu podvazem nebo embolizací zdroje. Endotension byla řešena perkutánní punkcí vaku aneuryzmatu s aspirací jeho obsahu. U trombózy raménka byla provedena lkrát trombektomie, 2krát byl našit femoro-femorální zkřížený bypass, 1krát byla léčba konzervativní a 1krát byla provedena amputace končetiny. Infekce stentgraftu byla řešena chirurgickou konverzí. Závěr. Endovaskulární léčba aneuryzmatu břišní aorty je v našem souboru spojena s výskytem pozdních komplikací a reintervencí v 12,3 %, resp. v 10,3 %, což se shoduje s literárními údaji. Většinu těchto pozdních komplikací lze řešit endovaskulárně., Aim. To evaluate the incidence of late complications and reinterventions after endovascular treatment of subrenal abdominal aortic aneurysm with stentgraft. Material and method. From April 1996 to December 2011 we treated 270 patients with abdominal aortic aneurysm endovascularly with stentgraft implantation at our department. There were 233 men and 37 women aged from 49 to 91 years. The average age was 68.7 years. 204 patients were followed for at least a year after treatment. In these patients incidence of late complications (rupture, migration, endoleak, ischemic complications and inflammatory complications) and reintervention were evaluated. Results. In a group of 204 patients 25 late complications were found, i.e. 12.3% (25/204) and 21 reinterventions were performed, i.e. 10.3% (21/204). From 25 complications there were endoleak type I 5x, endoleak type II with sac enlargement 3x, endoleak type III 4x, endotension 3x, stentgraft migration 4x, ischemic complications 5x and infection 1x. Type I endoleaks and migrations were treated with cuff implantation and with surgical conversion in 1 patient with migration. Endoleaks III were managed by stentgraft implantation and type II endoleaks with sac enlargement by ligation or embolization of source. Endotension was solved by percutaneous puncture of the aneurysm sac with content aspiration. The stentgraft arm thromboses were managed by thrombectomy 1x, femo-femoral crossover bypass was created 2x, conservative treatment 1x and leg amputation was performed 1x. Stent graft infection was managed by surgical conversion. Conclusion. Endovascular treatment of abdominal aortic aneurysm is associated with the occurrence of late complications and reintervention in 12.3% and 10.3%, which correlates with literature. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Primární chirurgická léčba ovariálního karcinomu má zásadní význam jak stagingový, tak léčebný, umožňující u časných stadií rozlišit pacientky s velmi nízkým rizikem recidivy onemocnění, u kterých může být upuštěno od adjuvantní chemoterapie. V případě vyšších stadií je standardní léčebnou modalitou kombinace radikální operační léčby s adjuvantní chemoterapií. Zásadní vliv na celkové přežití u operovaných pacientek má velikost ponechaného nádorového rezidua. Cílem operační léčby by tak mělo být nulové (mikroskopické) reziduum. Extendované chirurgické výkony (resekční operace na kličkách střevních, splenektomie, resekce jater, odstranění peritonea bránice atd.) u pokročilých stadií onemocnění vyžadují multioborovou spolupráci. Z dostupných dat jasně vyplývá, že nejlepší výsledky operační léčby jsou u pacientek operovaných v onkogynekologických centrech., Completeness of the primary surgical staging in early-stage ovarian cancer has a strong impact on the prognosis and it could distinguish between low and high risk patients for the recurrence of the disease. The standard treatment of advanced-stage ovarian carcinoma includes radical cytoreductive surgery which aims at removing a visible tumor followed by platinum-based chemotherapy. Complete resection is achieved when no macroscopically visible residual tumor is left. The prognostic value of complete debulking on overall survival has been confirmed. In patients presenting with extensive metastatic tumor spread, complete tumor resection may require extensive upper abdominal surgery, including procedures such as diaphragm resection or peritonectomy, splenectomy, or partial liver resection. These extended surgical procedures require a multidisciplinary approach.
