- Východiska: Od zdravotní péče se očekává, že bude poskytována v té nejvyšší kvalitě. K dosažení vysoké úrovně kvality ošetřovatelské perioperační péče je nutný její kontinuální monitoring, měření a následné vyhodnocování indikátorů kvality, které se propojují v management kvality. Management perioperační péče je vymezen jako péče o klienta, která mu je poskytovaná bezprostředně před operací, v průběhu a bezprostředně po skončení operace. Pro monitoring kvality poskytované péče je za vhodný nástroj považován ošetřovatelský audit. Ten slouží pro přezkoumání stavu zavedených procesů a vede především k prevenci rizik. Kvalita perioperační péče se soustřeďuje na bezpečnost a ochranu zdraví jak pacientů, tak personálu operačních sálů. Cíl: Cílem příspěvku je představit vytvořený kontrolní list, určený pro audity ošetřovatelské perioperační péče. Příspěvek popisuje metodiku tvorby kontrolního listu a postup, jakým byl validován. Dílčím cílem je představit výsledky získané externími audity za použití navrženého kontrolního listu. Metody: Prvním krokem ve výzkumném rámci byla rozsáhlá rešerše legislativy a relevantní literatury v databázích Scopus a Medline za období 2010–2015. Následně byl navržen kontrolní list pro ošetřovatelské audity na operačních sálech. Ten byl dále evaluován pilotním auditem a expertními rozhovory s odborníky z praxe. Reliabilita i validita kontrolního listu byla ověřena pěti provedenými audity ošetřovatelské perioperační péče a jejich porovnáním. Audit byl uskutečněn v průběhu celého perioperačního procesu, tedy od počátku operace po její ukončení. Následně byl kontrolní list znovu diskutován společně s výsledky auditů na jednotlivých pracovištích. Výsledky: Studie ověřila použitelnost a validitu kontrolního listu a taktéž přinesla zpětnou vazbu o úrovni ošetřovatelské péče pěti nemocnic akutní péče. Nejzávažněji byly vnímány odchylky v procesech hygienicko- -epidemiologického režimu. V průběhu všech auditů byly zaznamenány minimálně tři nežádoucí události. Závěry: Hodnocení auditů byla předložena managementu nemocnic jako podklad pro plánované zdokonalování ošetřovatelských postupů v rámci operačních traktů. Kontrolní list dále projde kvantitativním ověřením v rámci nemocnic akutní péče České republiky., Background: It is expected that health care will be provided at the highest quality. To achieve a high level of quality in nursing perioperative care is necessary the continuous monitoring, measurement and subsequent evaluation of quality indicators. Perioperative care is defined as care for a patient before, during and immediately aer the end of the operation. An audit is appropriate instrument for the monitoring of quality of care. Audits inspect the status of the established processes and lead to the prevention of risks. The quality of perioperative care focuses on the safety and health protection as patients so staffs of operating theatres. Aim: The aim of the paper is to present the checklist, designed for audits of nursing perioperative care. is paper describes a methodology of making the checklist and the procedure was validated. Another aim is to present the results of the external audits, when the proposed checklist was used. Methods: The first step in the research framework was extensive search of relevant legislation and literature in the databases Scopus and Medline for the period 2010–2015. Then the checklist for nursing audits on the operating theatres was designed. e checklist was evaluated by a pilot audit and expert interviews with experts. Reliability and validity of the checklist was verified by the five audits of nursing perioperative care and with comparison of the audit´s results. The audit was realized in the course of the entire perioperative process, i.e. from the beginning of an operation to finishing of the surgery. Subsequently the checklist was again discussed together with the results of the audits on the individual departments. Results: The study has verified the applicability and validity of the checklist and brought a feedback about the level of nursing perioperative care from five acute care hospitals. Perturbations in processes of hygiene-epidemiological regime were the most serious. Least three adverse events were logged in the course of all audits. Conclusions: The evaluation was submitted to the management audits of hospitals as the basis for the planned improvement of nursing practice within the operating trac. - a1_Based on light and scanning electron microscopical studies, eight species (five adult and three larval) of nematodes belonging to the Ascaridida, Oxyurida and Enoplida were collected from fishes of the Okavango River, Botswana, namely Falcaustra similis Moravec et Van As, 2004, Atractidae gen. sp. (only female) (both Cosmocercoidea), Cucullanus sp. (only female) (Seuratoidea), Cithariniella longicaudata sp. n., Synodontisia annulata sp. n. (both Oxyuroidea), Contracaecum sp. third-stage larvae, third-stage larvae of Galeiceps sp. (both Ascaridoidea) and Eustrongylides sp. fourth-stage larvae (Dioctophymatoidea). The new species Citharinella longicaudata (type host Schilbe intermedius Rüppel) is mainly characterised by the shape and size of cephalic papillae and the spicule 108 µm long, and Synodontisia annulata (type host S. intermedius) by the shape of cephalic papillae, body length of gravid females (4.88-5.33 mm) and a short spicule (66 µm long). The female specimen of Cucullanus sp. from Tilapia sparmanni Smith markedly differs from congeners parasitising inland fishes in Africa by the elongate pseudobuccal capsule and by the excretory pore far posterior to the oesophago-intestinal junction; apparently, it belongs to an undescribed species. Galeiceps larvae parasitising fishes are described for the first time. Cithariniella gonzalezi Van Waerebeke, Chabaud, Bain et Georges, 1988 is considered a junior synonym of C. khalili Petter, Vassiliadès et Troncy, 1972, and the previous records of Cithariniella citharini Khalil, 1964 from Synodontis spp. in Egypt concern, in fact, Cithariniella khalili Petter, Vassiliadès et Troncy, 1972., a2_The specimens of Cithariniella reported by Koubková et al. - Atrioventrikulární nodální re-entry tachykardie (AVNRT) je nejčastější formou supraventrikulární tachykardie (SVT), která se vyskytuje v těhotenství a po porodu. Nefarmakologická léčba zahrnuje použití dobře tolerovaných vagových manévrů (Valsalvův manévr, masáž karotid, ponoření obličeje do chladné vody). Adenosin a betablokátory jsou léky první linie k terminaci epizody SVT. Léčba těchto pacientek vyžaduje úzkou mezioborovou spolupráci gynekologa, internisty a intenzivisty. Ideálně by supraventrikulární dysrytmie měly být vyřešeny ještě před vlastní graviditou, například radiofrekvenční ablací. Symptomatická léčba malou dávkou betablokátoru je možná i v průběhu těhotenství., Atrioventricular nodal re-entrant tachycardia (AVNRT) is the most common supraventricular tachycardia (SVT) detected during pregnancy and labour. Non pharmacological treatment including vagal manoeuvres such as Valsalva manoeuvre, carotid massage and facial ice immersion are well tolerated and aid in diagnosis. Adenosine and beta-blocking agents are the first line drugs of choice to terminate an episode of SVT. The treatment requires interdisciplinary cooperation. Ideally, management of supraventricular arrhythmias should start before conception, for example by radiofrequency ablation. 468. Surgical removal of the epidermal inclusion cysts with squeeze technique: case report

- Epidermal inclusion cysts are common benign cutaneous cysts which arise from hair follicles. These cysts usually present as asymptomatic, small, smooth, firm, round, slow growing swellings on hair bearing areas such as scalp, face, neck and trunk. Epidermal inclusion cysts are easily diagnosed by their clinical features. However, trichilemmal cyst, dermoid cyst, neurofibroma, hemangioma, lipoma and liposarcoma should also be kept in mind in differential diagnosis of cutaneous cystic lesions. As malignant transformation of epidermal inclusion cysts has been reported, histopathological evaluation of epidermal inclusion cysts is mandatory in order to rule out malignancy. Moreover, giant epidermal inclusion cysts with ulceration, rapid growth, resistance to treatment, recurrence and fistula drainage may have malignant potential. Therefore, epidermal cysts should be surgically removed. There are several different types of surgical techniques to remove cutaneous cysts. A proper surgical technique should facilitate the complete removal of the cyst wall to prevent recurrence. In addition, it should provide minimal scarring and a low wound infection rate. Epidermal inclusion cysts can be easily removed surgically with squeeze technique. In this technique, the cyst is squeezed out through a small incision using both index fingers. Thus, the cyst is not ruptured. The squeeze technique allows the cyst capsule to remain intact. Therefore, the risk of wound infection, recurrence and scar formation is minimized. - Many aspects of surgical treatment of intestinal malrotation in children remain to be debatable. In the opinion of the majority of the specialists, surgical treatment is required after the diagnosis taking into account serious complications of intestinal malrotation. Purpose. The purpose of this research was to conduct an analysis of surgical tactics and operative treatment method for isolated and associated intestinal malrotations in children. Material and methods. We observed 123 children at the age of one day to 15 years with malrotation during the period of 2002 to 2013. Results. We presented the data from observing 123 children at the age of one day to 15 years with various clinical-anatomic forms of intestinal malrotation over from 2002 to 2013. In 62 patients (50.4%), the evidences of the high intestinal obstruction were prevalent, while 61 (49.6%) showed signs of low intestinal obstruction. 116 patients (94,3%) were given operative intervention: radical – 95(81,9%) and palliative – 21 (18,1%). In 56 % of the cases, various simultaneous surgeries were required. There are proposed differential approaches in relation to anatomic form of malrotation and possibility of the fixation of large intestine in the physiological position. Conclusion. The results obtained from the operative treatment are presented. - Článek se zabývá problematikou odmítání transfuze krve ze strany členů náboženské společnosti Svědkové Jehovovi. Danou otázku přitom zkoumá nejen z pohledu aktuální právní úpravy v zákoně o zdravotních službách (dříve vyslovených přáních) a vybrané judikatury, ale také z širšího právněfilozofického a politického pohledu. Dochází k závěru, dle něhož formálně právní úprava sice preferuje vůli pacienta, nicméně tento přístup může mít z obecnějšího společenského hlediska i některé sporné důsledky. Proto je nakonec přijat názor, že transfuzi je nutné poskytnout i proti vůli pacienta., The article deals with the issue of the blood transfusion rejection by the member of the religious community Jehovah's Witnesses. The issue is examined not only from the view of the recent legal provisions (living will) and the available judicature, but also from the broader philosophical and political view. It concludes that the legal provisions prefer the patient's autonomy but this approach can generally also have some controversial consequences.
