Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Because of this it’s very important that non-medical health staff will be enough educated. It is that sufficient awareness and practical experience that can affect the rate of inclination to ageist attitudes. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Conclusions: e work shows that in the sample survey, the majority of students appearing middle inclination to the myths of old age. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
We studied 52 patients with epilepsy with the average age of 36.2±14.7 years old. Of them, 38 patients had idiopathic epilepsy, 14 patients had symptomatic epilepsy. Our study has shown that epilepsy is accompanied with increased levels of autoantibodies to NF-200, GFAP, S100, MBP, DNA, GABA and dopamine receptors, testifying to the important role of autoimmune disturbances in the pathogenesis of epilepsy. More severe attacks are accompanied by worsening of neuroimmune dysregulation. Východiska: Praxe založená na důkazech je velmi užitečná v každé profesi, v porodní asistenci nevyjímaje. Nedostatečná informovanost o využití praxe založené na důkazech může vést k nekvalitní a neplnohodnotné péči o klientku. Cíl: Hlavním cílem bylo zjistit informovanost porodních asistentek o Evidence Based Midwifery. Mezi dílčí cíle patřilo zjištění, zda porodní asistentky znají pojem Evidence Based Midwifery a zkratku EBM a zda s Evidence Based Midwifery ve své praxi pracují. Metody: Byl vytvořen nestandardizovaný dotazník. Celkem obsahoval 15 otázek. V úvodu dotazníku byly respondentky seznámeny se záměrem výzkumu a byl jim předložen informovaný souhlas. Respondentkami byly porodní asistentky z České republiky pracující na gynkologických, porodnických lůžkových a ambulantních pracovištích. Rozdáno bylo celkem 160 dotazníků. Návratnost činila 93,8 %. Celkem se pracovalo se 150 dotazníky. Výzkumné šetření probíhalo v měsících září–listopad roku 2013. Výsledky: Zkratku EBM uvedlo, že nezná celkem 100 (66,7 %) respondentek. Znalost pojmu Praxe založená na důkazech potvrdilo celkem 46 (30,7 %) respondentek. Zájem o studijní materiál věnující se praxi založené na důkazech v porodní asistenci má 93 (62,0 %) respondentek, zájem o školící kurz o EBMid uvedlo 110 (73,4 %) respondentek. Závěry: Cíle výzkumného šetření byly splněny. Úroveň informovanosti porodních asistentek o Evidence Based Midwifery jsme stanovili jako nedostatečná (méně než polovina respondentek nezná pojem EBMid). Většina respondentek s Evidence Based Midwifery ve své praxi nepracují. Bylo by vhodné informovat porodní asistentky o využití EBMid v praxi a poté provést další výzkum, díky kterému bychom mohli zhodnotit zlepšení dané situace či nikoliv., Background: Evidence-based practice is very useful in every profession, in midwifery exception. Lack of awareness about the use of evidence-based practice can lead to poor quality and fiduciary care of the client. Objective: To determine the awareness of Midwives of Evidence Based Midwifery, determine whether midwives are familiar with the concept of Evidence Based Midwifery and acronym EBM and whether they practice Evidence Based Midwifery in their work. Methods: Respondents were midwives working in the gynecological-obstetric wards and clinics in the Czech Republic selected by purposive sampling, there were 150 respondents. e research was carried out by a non-standardized questionnaire. Results: 100 respondents don´t know EBM: 93 respondents are interested in the study material about EBMid and 110 respondents are interested in a training course. Conclusions: e objectives of the research were met. e level of Midwives`awareness of Evidence Based Midwifery was detected. Východiska: Systematické review (SR) představuje v kontextu s přístupem a filozofií Evidence-Based Healthcare (EBHC) nejvýznamnější a nejhodnotnější typ studie v rámci úrovně vědeckých důkazů. Hallux valgus (HV) patří mezi nejčastější deformity v oblasti přednoží. SR, zabývající se vlivem operace HV na chůzi, kvalitu života, spokojenost a současně i bolest, ve světovém písemnictví chybí. Cíle: Hlavním cílem tohoto propedeuticky zaměřeného článku bylo poukázat na význam SR v rámci přístupu EBHC a přiblížit rigorózní a systematickou metodiku jeho tvorby příkladem dílčích výsledků SR zaměřeného na hodnocení efektu operační korekce deformity HV. Metodika: Nejprve jsme formulovali review otázku a specifikovali kritéria pro zařazení a vyloučení primárních studií do SR. Protokol SR jsme vypracovali pomocí softwaru, vyvinutého Joanna Briggs Institute pro tvorbu SR. Systematické vyhledávání publikovaných i nepublikovaných primárních studií zahrnovalo 3 kroky: 1. Iniciální vyhledávání v Medline, Embase a Cinahl; 2. sekundární vyhledávání v 13 databázích; 3. prostudování referenčních seznamů relevantních studií. Vyhledávali jsme randomizované kontrolované studie (RCT) bez časového omezení a ve všech jazycích. Výsledky: Celkem jsme nalezli 2412 studií a odstranili 767 duplikátů. Na základě dvoufázového hodnocení relevance a kritického hodnocení metodologické kvality studií pomocí standardizovaného hodnotícího nástroje JBI-MAStARI bylo zařazeno 27 RCT. V současné době probíhá extrakce výsledků pomocí standardizovaného nástroje JBI-MAStARI a příprava dat na meta-analýzu. Dílčí výsledky ukázaly nejednotnost použitých metod k hodnocení bolesti a spokojenosti pacientů po operaci HV v zahrnutých RCT. Závěry: SR představují velmi významné pojítko mezi vědeckými důkazy a jejich implementací do klinické praxe. Hallux valgus (HV) is a common forefoot deformity. In the current literature, there is no SR which has reviewed the effect of HV surgery on gait, pain or function in the global evidence. Aim: Main aim of this propedeutically focused paper was to highlight the importance of SR within EBHC approach and explain rigorous and systematic methods of its development by example of partial results of the SR focusing on evaluation of the effect of the hallux valgus surgery. Medhods: Firstly, we formulated an answerable review question and specified inclusion and exclusion criteria. Protocol of the SR was prepared using soware developed by Joanna Briggs Institute for SR preparation. Systematic search of published and unpublished studies included three steps: 1. Initial search in Medline, Embase and Cinahl; 2. secondary search in 13 databases; 3. hand-search in the reference lists of relevant papers. We have searched randomized controlled trials (RCT) in any date and language. Results: We found 2412 papers and deleted 767 duplicates. en a paper retrieval according to inclusion and exclusion criteria and critical appraisal of methodological quality were done using standardized critical appraisal instruments JBI-MAStARI. 27 RCT were included into SR. Currently, we are extracting data using JBI MAStARI and preparing them for meta-analysis. Partial results showed heterogeneity of used method for evaluation of pain and patient satisfaction aer HV surgery in included RCT. We aimed to investigate in detail the structural mutations after influence of actellic insecticide at different doses and different temperature conditions. We tested effects of single introduction of different doses of actellic: 48, 96, 192, and 384 mg/kg. Cytogenetic effect of actellic in bone marrow cells at different temperature regimens was studied. Following actellic administration, animals were being in two temperature regimes: normal (18-20°C) and high (37-38°C). After treatment with actellic, animals were kept within 1, 2, 4 and 8 hours at high temperature (37-38°C), i.e. in a thermostat (TV-80) with access of air. In experiments, the animals were divided into seven groups. The animals of the 1st group were administered actellic at a dose 384 mg/kg and kept under normal temperature until slaughtering. The animals of the 2nd group after administration of actellic were immediately placed in an environment with a temperature of 37-38°C. Animals of the 3rd, 4th, 5th, and 6th groups after treatment with actellic were transferred to conditions with high temperature, respectively, for 1, 2, 4 and 8 hours. The 7th group of mice was under normal temperature conditions without treatment with actellic. Our results have shown that in normal temperature conditions single exposure of actellic (384 mg/kg) did not affect the genetic apparatus of somatic cell, evidenced by the frequency of chromosome aberrations in bone marrow cells that were within the control levels. Východiska: Implementace mezinárodní klasifikace NIC v podmínkách intenzivní péče v ČR. Cíl: Identifikace intervencí NIC Klasifikačního systému, které všeobecné sestry označí jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Metody: Kvantitativní analýza četnosti intervencí NIC. Dosáhla-li konkrétní intervence NIC 75% hranice (po sečtení v kategorii denní a týdenní péče), byla identifikována jako intervence NIC, která je použitelná minimálně jednou týdně v klinické péči prostředí intenzivní péče. 386 záznamových archů, ve kterých všeobecné sestry označovaly frekvence použití vybraných 184 intervencí Klasifikačního systému NIC. Získaná data byla získána z klinické praxe intenzivní péče u 12 poskytovatelů zdravotnických služeb v České republice (ČR). Ke statistickému porovnání získaných dat z jednotlivých klinických pracovišť byl použit Pearsonův chi kvadrát, na hladině významnosti 5 % (p ≤ 0,05). Výsledky: 46 intervencí z vybraných intervencí NIC, bylo identifikováno jako intervence, použitelné minimálně jednou týdně v klinické péči prostředí intenzivní péče ČR. U 14 intervencí NIC nebyly shledány signifikantní rozdíly v označení použitelnosti jednou týdně na pracovištích ARO a JIP interního a chirurgického typu. U 32 intervencí NIC byly shledány signifikantní rozdíly v označení použitelnosti jednou týdně v klinické praxi mezi výše uvedenými pracovišti. Použitelnost těchto intervencí NIC minimálně jednou týdně častěji označily všeobecné sestry na pracovišti ARO. Závěry: Ze 184 mapovaných intervencí Klasifikačního systému NIC bylo identifikováno 46 intervencí, které všeobecné sestry označily, jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Tyto intervence NIC podstoupí expertní validaci., Background: Implementation of the International Classification of NIC in conditions intensive care in the Czech Republic. Aim: The aim of this enquiry was the choice of interventions NIC of the classification system, which are marked by nurses as usable minimaxy once a week in the clinical practice of intensive care. This contribution maps the posibility of the usage of chosen interventions of classification system Nursing Interventions Classification NIC, which will be used for the future implementation in the intensive care sector. Methods: Reaches the specific interventions NIC 75% threshold for counting records in the category of daily and weekly care has been identified as an intervention NIC, which is applicable at least once a week in clinical care intensive care environment. The quantitative analysis of 386 records with 184 interventions NIC in the clinical practice, by 12 health service proveders in the Czech Republic. Pearson‘ s chi quadrat (p≤0,05) was used for the statistic comparison of the data from the individual clinical workplaces. Results: By the quantitative analysis was confirmed 46 interventions NIC of Classification system which possible usage in clinical intensive care minimally once a week in the CR. In comparison with the data distribution at individual clinical workplaces, there were not found any significant differences in minimal weekly usage indication at 14 NIC intervention of the classification system. There were found some differences in usage marking in 32 interventions NIC classification system by nurses at some workplaces. Higher frequency of presence was recorded by Anesthesiologic Resuscitation ward nurses. Conclusions: Analysis from 184 intervention NIC confirms the possibility 46 of serviceability of classification system NIC in our country. There are interventions, which usage in clinical care workplaces Anesthesiologic Resuscitation ward, Intensive Care Unit surgery minimally once a week.
