Východiska: Implementace mezinárodní klasifikace NIC v podmínkách intenzivní péče v ČR. Cíl: Identifikace intervencí NIC Klasifikačního systému, které všeobecné sestry označí jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Metody: Kvantitativní analýza četnosti intervencí NIC. Dosáhla-li konkrétní intervence NIC 75% hranice (po sečtení v kategorii denní a týdenní péče), byla identifikována jako intervence NIC, která je použitelná minimálně jednou týdně v klinické péči prostředí intenzivní péče. 386 záznamových archů, ve kterých všeobecné sestry označovaly frekvence použití vybraných 184 intervencí Klasifikačního systému NIC. Získaná data byla získána z klinické praxe intenzivní péče u 12 poskytovatelů zdravotnických služeb v České republice (ČR). Ke statistickému porovnání získaných dat z jednotlivých klinických pracovišť byl použit Pearsonův chi kvadrát, na hladině významnosti 5 % (p ≤ 0,05). Výsledky: 46 intervencí z vybraných intervencí NIC, bylo identifikováno jako intervence, použitelné minimálně jednou týdně v klinické péči prostředí intenzivní péče ČR. U 14 intervencí NIC nebyly shledány signifikantní rozdíly v označení použitelnosti jednou týdně na pracovištích ARO a JIP interního a chirurgického typu. U 32 intervencí NIC byly shledány signifikantní rozdíly v označení použitelnosti jednou týdně v klinické praxi mezi výše uvedenými pracovišti. Použitelnost těchto intervencí NIC minimálně jednou týdně častěji označily všeobecné sestry na pracovišti ARO. Závěry: Ze 184 mapovaných intervencí Klasifikačního systému NIC bylo identifikováno 46 intervencí, které všeobecné sestry označily, jako použitelné v klinické praxi intenzivní péče minimálně jednou týdně. Tyto intervence NIC podstoupí expertní validaci., Background: Implementation of the International Classification of NIC in conditions intensive care in the Czech Republic. Methods: Reaches the specific interventions NIC 75% threshold for counting records in the category of daily and weekly care has been identified as an intervention NIC, which is applicable at least once a week in clinical care intensive care environment. The quantitative analysis of 386 records with 184 interventions NIC in the clinical practice, by 12 health service proveders in the Czech Republic. In comparison with the data distribution at individual clinical workplaces, there were not found any significant differences in minimal weekly usage indication at 14 NIC intervention of the classification system. There were found some differences in usage marking in 32 interventions NIC classification system by nurses at some workplaces. Higher frequency of presence was recorded by Anesthesiologic Resuscitation ward nurses. Conclusions: Analysis from 184 intervention NIC confirms the possibility 46 of serviceability of classification system NIC in our country. There are interventions, which usage in clinical care workplaces Anesthesiologic Resuscitation ward, Intensive Care Unit surgery minimally once a week. V devadesátých letech minulého století dochází k rozvoji laparoskopické chirurgie. Cestu ukázali chirurgickým oborům gynekologové. Vývoj metody započal simultáně na třech místech. V USA, Francii a Německu. V roce 1989 provedl Reich v USA první LAVH. Querleu v roce 1991 první laparoskopickou lymfadenektomii a v roce 1992 kombinoval Dargeant Schautovu radikální vaginální hysterektomii s laparoskopickým přístupem. Systematická pánevní a alternativně paraortální lymfadenektomie je součástí radikálních laparoskopických výkonů. Stanovili jsme novou nomenklaturu parametrií, snažíme se šetřit autonomní inervaci. Zpočátku byli operatéři autodidakté, v poslední době jsme svědky standardizace indikací a techniky. Bezpečnost výkonu a komfort operatéra zvyšuje vyspělá technika, 3D zobrazení, vysokofrekvenční bipolární technika, konstrukce trokarů omezujících možnost cévních poranění. Postupně byl téměř na všech specializovaných pracovištích opuštěn vaginální přístup a kompletní operace je prováděna pouze laparoskopicky pod mezinárodně srozumitelným označením TRLH (totální, radikální, laparoskopická hysterektomie). Systematicky se budeme ve svém sdělení věnovat anatomickým aspektům, aplikaci trokarů, základním preparačním krokům a postupům, které omezí možné komplikace, zejména ze strany močového měchýře a ureterů. Zmíníme nutné technické vybavení, základy bezpečné práce s vysokofrekvenční koagulací. Na závěr probereme pooperační péči., In the nineties we experienced a massive development of laparoscopic surgery. Gynecologists were the first who paved the way. The method was simultaneously developed in three countries - the United States, France and Germany. From the United States the vaginal laparoscopically assisted procedures have came to Europe. The French and German schools were based on the historical knowledge of the Schauta radical hysterectomy. In 1989 Reich was the first who operated the LAVH, followed by Querleu in 1992 with lymphadenectomy and Dargeant who combined the Schauta procedure with the laparoscopic lymph node dissection. It was the laparoscopy that taught us the proper functional anatomy. A new nomenclature of the parametries was defined. Careful attention is given to the preservation of the autonomous inervation. In the beginning all laparoscopic surgeons were self-taught. Nowadays a standardized laparoscopic techniques are implemented for a radical laparoscopic operations. The safety of this method is enhanced by using advanced technologies like 3D imaging, high frequence bipolar tools, safety trocars. More often the vaginal approach is being abandoned and the whole operation is performed laparoscopically. A generally accepted term for this operation is TRLH which stands for Total Radical Laparoscopic Hysterectomy. We will systematically describe the anatomical aspects of the dissection technique regarding the save preparation of the ureters and the bladder. Presented is the experience of the application of stable osteosynthesis by three Kirschner wires on diaphyseal fractures of the metacarpal bones at 74 patients. Indications for osteosynthesis was opened (6%), closed (94%), non-united, mal-united fractures and false joints of metacarpal bones diaphysis. Patients were given intraosseous anesthesia at distal epi-metaphysis area of radial bone. Described is a method of operation extra medullary osteosynthesis using three Kirschner wires. Also, method of wedge osteotomy for full recovery of hand function is described. Východiska: Validizační studie v ošetřovatelském procesu umožňují vyjádření míry platnosti komponent NANDA-I pro různé soubory osob a v různých oblastech péče. Bolest jako ošetřovatelská diagnóza v péči o rodící ženu je jedním z nejčastějších problémů, které porodní asistentky v praxi posuzují, řeší a hodnotí. Validované definující charakteristiky ošetřovatelských diagnóz jsou rozhodující pro správnou diagnostiku typu a intenzity bolesti a pro navržení efektivní léčby. Cíl: Cílem práce bylo validizovat definující charakteristiky ošetřovatelské diagnózy 00132 Akutní bolest vybraným souborem expertek – porodních asistentek a zjistit, které definující charakteristiky (určující znaky, související faktory) porodní asistentky považují za hlavní a které definující charakteristiky za vedlejší. Metody: Pro obsahovou validizaci byl využit Fehringův model validity diagnostického obsahu (Diagnostic Content Validity Model). Expertky – porodní asistentky splňovaly kritérium bakalářského stupně vzdělá- ní v oboru porodní asistence a klinické praxe na oddělení porodního sálu. Výzkum probíhal současně ve Fakultní nemocnici Olomouc, v Nemocnici Šternberk a v Nemocnici Uherské Hradiště. Měřící nástroj pro posouzení významnosti diagnostických prvků byl sestrojený pomocí jednotlivých definujících charakteristik ošetřovatelské diagnózy z NANDA-I, Taxonomie II. Výsledky: Z celkového počtu 22 definujících charakteristik (18 určujících znaků, 4 související faktory) považují porodní asistentky za hlavní (majoritní) 9 definujících charakteristik. Mezi hlavní definující charakteristiky (určující znaky) s váženým skóre (VS) vyšším než 0,75 porodní asistentky zařadily: úlevová poloha z důvodu vyhnutí se bolesti (VS 0,82); expresivní chování (VS 0,77); pozorované známky bolesti (VS 0,80); slovní vyjádření bolesti (VS 0,84); změny dýchání (VS 0,80); změny krevního tlaku (VS 0,82); změny srdeční- ho rytmu (VS 0,86). Mezi hlavní související faktory zařadily tyto charakteristiky: biologické faktory – porodní mechanismus (VS 0,94) a faktory psychogenní (VS 0,80). Závěry: Z výsledků tohoto šetření vyplývá, že dotazované porodní asistentky považují z celkového nabízeného počtu 22 definujících charakteristik (18 určujících znaků a 4 související faktory) 9 za hlavní – majoritní pro celkové posouzení stavu klientky – rodičky přicházející na porodní sál k porodu., Background: Validation studies allow expression of the degree of validity of components NANDA-I for different sets of people and in different areas of care. Pain is one of the most common problems that midwives in practice assess, evaluate and solve. Validated defining characteristics of the nursing diagnoses are crucial for correct diagnosis of the type and intensity of pain, and for proposal of effective treatment. Aim: The aim of the study was to validate defining characteristics of the nursing diagnosis Acute pain 00132 by a selected file of experts-midwives, and see which characteristics they consider major and minor. Methods: The study followed Fehring Diagnostic Content Validity Model. Experts-midwives met the criterion of bachelor degree in midwifery and clinical practice in the delivery room. The research was conducted simultaneously in Olomouc region. Measuring tool for assessing the significance of diagnostic elements was constructed using individual defining characteristics of the nursing diagnoses of NANDA-I Taxonomy II. Results: Out of the 22 defining characteristics considered by experts-midwives nine characteristics were described as major with weighted score (WS) greater than 0,75: antalgic position due to the avoidance of pain (WS 0,82); expressive behavior (WS 0,77); observed signs of pain (WS 0,80); verbal expression of pain (0,84); changes in breathing (0,80); changes in blood pressure (WS 0,82); changes in heart rhythm (WS 0,86). Among the major related factors were selected the following characteristics: biological factors - birth mechanism (WS 0,94) and psychogenic factors (WS 0,80). In recent years there has been a noticeable "rejuvenation" of pelvic organ prolapse. Inconsistency of the pelvic floor muscles, including the omission of sexual organs, is extremely common pathology, observed almost a third of women of reproductive age. The search for effective, convenient methods of contraception for this category of patients is an important problem of modern gynecology. We proposed a method of transvaginal voluntary surgical contraception, produced in conjunction with surgical treatment of descent and prolapse of the vaginal walls. Studied the nearest and long-term results of surgery in 50 women to which, during the surgical treatment of genital prolapse at the same time was performed transvaginal occlusion of the fallopian tubes. Control groups consisted of 30 women to which in the first step before surgical correction of pelvic organ prolapse have been performed minilaparotomy and voluntary surgical sterilization (VSS). Our method consists in penetrating into the abdominal cavity through the anterior vaginal vault, downgrading the fallopian tubes with a hook of Ramathibodi and tubal sterilization by Pomeroy method. Intra - and postoperative complications were not observed. In the late postoperative periods - the effectiveness of the method was 100%. Ciel štúdie: Alzheimerova choroba (AD) je progresívna a ireverzibilná neurodegeneratívna porucha, ktorá vedie k demencii a smrti. Dochádza k masívnej strate neurónov, najmä v hipokampe a asociačných oblastiach neokortexu. Vysoká koncentrácia cirkulujúceho homocysteínu (Hcy) je považovaná za nezávislý rizikový faktor vzniku cievnej mozgovej príhody. Alzheimerova choroba (AD) sa bežne vyskytuje ako dôsledok mozgovej ischémie. Nedávne prospektívne štúdie ukázali, že hyperhomocysteinémia (hHcy) je silný, nezávislý rizikový faktor pre rozvoj demencie a AD. Molekulárne mechanizmy, ktoré zodpovedajú za rozvoj týchto patológii nie sú úplne objasnené. V práci sme skúmali účinok hHcy na zmeny MAPKs (mitogénom aktivované proteínkinázy) dráhy v neurónových bunkách v korelácii s neurodegeneratívnymi zmenami v mozgu zvierat. Typ štúdie: Experimentálna. Názov a sídlo pracoviska: Univerzita Komenského v Bratislave, Jesseniova lekárska fakulta v Martine, Ústav histológie a embryológie, Malá Hora 4, 036 01 Martin, Slovensko Materiál a metódy: Pre naše experimenty sme použili dospelé laboratórne potkany (samce) kmeňa Wistar. Zvieratá boli rozdelené do dvoch skupín. V prvej skupine bol počas 14 dní subkutánne podávaný Hcy v dávke 0,45 μmol/g/zviera dvakrát denne. Druhá skupina bola bez ošetrenia Hcy. Po uplynutí tejto doby bola prvej aj druhej skupine navodená globálna ischémia štvorcievnym podväzom, a to 15minútová ischémia s reperfúziou 72 hodín. Výsledky: Výsledky ukázali, že ischemický inzult v kombinácii s indukovanou hHcy vedie k zvýšeniu smrti nervových buniek v mozgovej kôre. Analýza proteínov MAPKs Western blotom naznačuje, že IR aj hHcy ovplyvňujú hladinu MAPK/EKR a MAPK/p38 proteínov, ktoré sú spojené s prežívaním/smrťou nervových buniek. Záver: Tieto zistenia naznačujú, že ischemické poškodenie v kombinácii s indukovanou hHcy má u potkanov neurodegeneratívny účinok. Naše výsledky tiež poukazujú na to, že tento kombinovaný model poškodenia by mohol viesť k progresii AD-podobným patologickým črtám u laboratórnych zvierat., Objective: Alzheimer’s disease (AD) is a progressive and irreversible neurodegenerative disorder combined with massive neuronal loss, mainly in the hippocampus and association regions of the neocortex that results in dementia and death. A high circulating concentration of homocysteine (Hcy) is an independent risk factor for stroke. Commonly, Alzheimer’s disease (AD) is frequently occurring as a result of stroke. A recent prospective study showed hyperhomocysteinemia to be a strong, independent risk factor for dementia and AD. The molecular mechanisms underlying these mechanisms are not fully understood yet. We investigated the effect of hHcy (hyperhomocysteinemia) on associated changes in MAPK/ERK (Mitogen Activated Protein Kinase/extracellular signal-regulated kinases) pathways in neuronal cells correlated with neurodegeneration. Design: Experimental. Settings: Commenius University, Jessenius Faculty of Medicine, Department of Histology and Embryology, Mala Hora 4, 036 01 Martin, Slovakia Material and methods: For our experiments we used adult rats (male) Wistar. Animals were divided into two groups. The first group was treated by subcutaneous injection 0,45 μmol/g of Hcy in duration of 14 days before experiment. After this period both groups underwent global forebrain ischemia by 4-vessels occlusion. Thus, 15 minutes of ischemia was followed by reperfusion period of 72 hours. Results: The results showed that IR after induced hHcy may attenuate the neural cell death in forebrain cortex. Further Western blot study of MAPKs suggested that IR as well as hHcy affect number and level of MAPK/ERK and MAPK/p38 proteins, which are associated with survival/death of neural cells. 182 Klinická biochemie a metabolismus 4/2015 nútovej ischémie ihneď nasledovala 72 hodinová reperfúzia. Zvieratá s indukovanou hHcy Zvieratá v tejto skupine dostávali počas 14 dní pred experimentom subkutánne Hcy v množstve 0,45 μmol/g/zviera dvakrát denne podľa Mattého a kol. [13]. Zvieratá boli rozdelené do rovnakých dvoch podskupín A a B ako bolo uvedeno (Hcy-C a Hcy-IR-72h rep skupiny), pričom množstvo zvierat v skupinách, všetky podmienky počas operácie, celého trvania ischémie i reperfúzie, ako aj odobratie tkanív boli totožné s predchádzajúcou skupinou zvierat. Po ischémii, resp. po reperfúzii boli zvieratá usmrtené dekapitáciou a odobraté mozgové tkanivo bolo použité na prípravu homogenátov na izoláciu bielkovín, resp. na histologickú analýzu. Príprava homogenátu proteínov Tkanivá mozgov sa ponorili do 3 ml homogenizačného roztoku [0.32 mol.m-3 sacharóza + HEPES (4-(2-Hydroxyethyl)piperazine-1-ethanesulfonic acid); pH = 7.4 v pomer 1 : 10].
