Background: In evidence-based education, experts are supposed to possess skills and content knowledge. Scientific skills provide the tools and ways of thinking that enable researches to build the robust conceptual frameworks needed to gain expertise. Science education should not only provide broad content knowledge base but also develop analytical thinking, understanding of scientific research processes, and inspire curiosity of researchers. Objectives: is paper discusses the key results of subjective evaluation of attained outcomes of a 2,5-year project focused on enhancing research competences in a faculty of health sciences. Methods: e data was gathered from project members by four consequent semi-structured online questionnaires. Total number of respondents was 78 (N = 78) with 75% response rate. e data was analysed by a summative content analysis combining quantitative and qualitative analyses. Results: e findings present evident progress in personal achievements, team working, and scientific practice in the faculty. Based on identified needs the concrete measures supported learning and lead to enhancement in the target group’s research competences. Conclusions: Capacity building approach was successful in laying a robust foundation for consistent development in research-based practice both in academic and professional levels. 25. Evaluating the effectiveness of fructose-1,6-diphosphate in treating of ocular ischemic syndrome

The purpose of this study was to evaluate the efficacy and safety of FDP in patients with ocular ischemic syndrome. Material and methods. The material for this study is based on results of a comprehensive examination and treatment of 53 patients with a diagnosis OIS. The average age of the patients was 57,8 ± 6,82 year. 19 of them women, 34 men. 27 patients entered to the main group (1) which received standard therapy in combination with intravenous FDP (fructose 1,6-bisphosphate). 26 patients in the control group (2) received standard treatment. Results. In applying the FDP combined with comprehensive therapy in the main group resulted in increased of visual acuity by 32.8%, parameters of retinal sensitivity by 17.8%, reducing the area of scotomas compared with patients of the control group. Optical coherence tomography registered significant changes in the dynamics in patients of the main group - reducing the edema and restoration of RNFL and ONH. Recovery of visual function may have contributed neuroprotective activity of the drug FDP by a protective effect on nerve tissue, reducing the effects of hypoxic stress. Conclusions. The use of standard therapy in combination with FDP in the treatment of ocular ischemic syndrome has a positive effect on the course of the disease, thereby, increase of visual acuity, a decrease in sectoral loss in vision fields, the positive dynamics OCT parameters, improving hemodynamic parameters at Doppler imaging in dynamics., Dilbar Makhkamova, and Literatura

26. Extra-anatomické stenty jako alternativa nefrostomie

Extra-anatomické stenty jsou alternativou trvalé nefrostomie u subrenální obstrukce mo- čovodu, kde není možné zajistit derivaci moči ureterálními stenty. Jsou určeny zejména pro pacienty s  extraureterální blokádou ureteru inoperabilními nádory nebo závažnou formou retroperitoneální fibrózy. Hlavní výhodou je možnost zbavit pacienta nefrostomie včetně nutnosti trvale nosit sběrný močový sáček. Zavedení stentu není příliš obtížné. V současné době vyráběné extra-anatomické stenty již obsahují v jednom balíčku všechny nezbytné součásti pro jejich zavedení. Většina pacientů, kteří měli extra-anatomický stent zavedený, měla velmi limitovanou délku přežívání a stent nebylo nutné měnit. Cílem použití této speciální formy derivace moči bylo zlepšení kvality života a případně také zmírnění renální insuficience. Zhodnocení dosud publikovaných zkušeností je předmětem této práce., Extra-anatomic stents represent an alternative to the nephrostomic tube for patients with ureteric obstruction in whom the ureteral stenting failed. They are designed for patients with extramural obstruction of ureters caused by abdominal or pelvic tumors or severe form of retroperitoneal fibrosis. The main advantage of extra-anatomic stents is that they do not require use of nephrostomic tube and urine bag. The introduction is not difficult and all currently available extra-anatomic stents have all the necessary components for implementation included in the package. Quality of life improvement and the amelioration of renal insufficiency are the main goals for insertion of the extra-anatomic kidney stent. 28. Fotoselektivní vaporizace prostaty (PVP) s laparoskopickou divertikulektomií (LD) močového měchýře

Úvod: Pseudodivertikl močového měchýře bývá většinou spojen se subvezikální obstrukcí (SO). Dříve byla nejčastějším výkonem otevřená prostatektomie s divertikulektomií (D). Nověji bylo možno řešit situaci endoskopicky, dvoudobě. V první době TURP, v druhé době laparoskopická divertikulektomie (LD). Zde prezentujeme možnost provést výkony v jedné době pomocí fotoselektivní vaporizace prostaty zeleným laserem (PVP). Materiál: Od 1/2011 do 7/2015 jsme provedli 18 LD. Jednalo se o muže ve věku 62,4±10,0 (36,0–75,1). Velikost divertiklu byla 62,2±22,5 (26–120) mm. Třikrát byl řešen samotný divertikl, dvakrát společně s laparoskopickou radikální prostatektomií a 13x zároveň řešena BPH. U čtyř proveden v první době TURP, LD v druhé době. U devíti (jsou dále hodnoceni) byla LD kombinována v jedné době s PVP. V rámci předoperačních vyšetření užita jako standard 3D CT cystografie či lépe CT vylučovací urografie (zobrazen i vztah močovodů k divertiklu). Výsledky: Devět mužů ve věku 67,0±5,1 (57,3– 75,1) let. BMI 28,3±4,1 (23,0–35,0). Velikost prostaty dle transabdominální sonografie byla 43,4±14,2 (30–80) ml. Hodnota PSA 2,7±1,8 (0,5–6,5) ng/ml. Výkon je zahájen v celkové anestezii v endoskopické (litotomické) poloze. Antibiotická profylaxe rutinně nepodávána. Je provedeno PVP. K PVP byl použit přístroj HPS (u prvního) resp. 7x XPS Green Light Laser (vodou chlazené MOXY® vlákno). Aplikovaná energie při PVP 191,6±107,3 (84–458) kJ. Na závěr endoskopie provedena sondáž močovodu na straně divertiklu (ev. obou ureterů dle lokalizace divertiklu). Poté je změněna poloha na Trendelenburgovou. Ze čtyř portů je provedena transperitoneální divertikulektomie extravezikálním přístupem. Následovala laparoskopická divertikulektomie. Velikost divertiklu 61,5±19,8 (26–90) mm. Užity čtyři transperitoneální porty (2x5 a 2x10 mm). Čas operace 166,5±44,7 (90–255). Ureterální katétr odstraňován první pooperační den, měchýřová cévka je šestý pooperační den. Nebyly pozorovány postoperační komplikace. U prvního pacienta byl po roce proveden TUR sklerózy hrdla. Závěr: Řešení subvezikální obstrukce s divertikulektomií močového měchýře není v praxi pří- liš častý výkon. Kombinace PVP a LD patří mezi moderní mininvazivní metody a v klinické praxi ji v současné době hodláme dávat jednoznačnou preferenci před ostatními metodikami. Nesmyslné je to pouze z pohledu financování pomocí DRG systému, kdy je výhodnější provedení výkonu ve dvou dobách., Introduction: Pseudodiverticulum of urinary bladder is usually associated with Bladder Outlet Obstruction (BOO). Trdaditionally the most common surgical management was open prostatectomy with diverticulectomy (D). More recently, two-stage endoscopic procedures have been usedFirstly, TURP,followed by laparoscopic diverticulectomy (LD). We present the option of a single operation using green laser photoselective vaporization of the prostate (PVP) and LD. Material: From 1/2011 to 7/2015, 18 LDs were performed. The mean age was 62.4±10.0 (36.0 to 75.1). The size of diverticulum was 62.2±22.5 (26–120) mm. In three cases only the diverticulum was treated, twice with laparoscopic radical prostatectomy and 13x combined with BPH treatment. In 4 cases TURP was performed before LD. In 9 cases LD was combined simultaneously with PVP As a part of the preoperative examination 3D CT cystography or better CT IVU (for better imagining of the relationship between ureters and diverticulum) were used. Results: 9 men, aged 67.0±5.1 (57.3 to 75.1) years. BMI 28.3±4.1 (23.0 to 35.0). Prostate volume measured by transabdominal sonography was 43.4±14.2 (30–80) ml. PSA 2.7±1.8 (0.5 to 6.5) ng / ml. The procedure was carried out under general anaesthesia in endoscopic (lithotomy) position. No routine antibiotic prophylaxis. PVP was performed, in the fi rst case by Green Light Laser® HPS and 7x by XPS Green Light Laser (cooled MOXY® fi bre). Delivered energy by PVP was 191.6±107.3 (84–458) kJ. At the end of endoscopy stenting of the ureter on the side of the diverticulum (or both ureters according to localization the diverticulum) was performed. The patient was re-positioned to Trendelenburg. Laparoscopic diverticulectomy was performed using 4 ports (2x5 and 2x10 mm) by extravesical transperineal approach. The size of the diverticulum was 61.5±19.8 (26–90) mm. The mean time of operation was 166.5±44.7 (90–255). Ureteral catheter was removed fi rst day after surgery, bladder catheter the sixth postoperative day. No postoperative complications were observed. The fi rst patient underwent TUR incision for sclerosis of bladder neck after one year. Conclusion: Resolution of subvesical obstruction with urinary bladder diverticulectomy is not a common procedure in daily practice. The combination of PVP and LD is a modern minimally invasive method. We currently intend to give it a clear preference in clinical practice to the other methods. 29. Glokalizace ayahuasky v ČR a možnosti jejího terapeutického využití v místních podmínkách

Z ayahuasky, klíčového prvku tradiční domorodé medicíny peruánské Amazonie, se stal globální fenomén. Sezení, během něhož je užívána tato psychoaktivní látka, lze v současné době absolvovat nejen v prostředí jihoamerického deštného pralesa, ale také v České republice. Vzhledem k tomu, že je v ČR ayahuasca pro svůj obsah N, N-dimethyltriptaminu dle nařízení vlády č. 463/2013 Sb., O seznamech návykových látek, nelegální, je tato substance užívána v rámci subkultury. S cílem poprvé zmapovat tento aktuální fenomén jsme provedli pilotní on-line anonymní dotazníkové šetření, ve kterém jsme se mimo jiné zaměřili i na možnosti terapeutického využití této psychoaktivní látky v místních podmínkách. Analýza dat získaných od 70 respondentů ukázala, že ayahuasca je užívána ve všech krajích ČR (nejvíce v Praze) a to zejména z léčebných důvodů. Došlo tak k přizpůsobení tradičního užívání ayahuasky lokálním podmínkám a se vzrůstající informovaností lze předpokládat další rozšíření tohoto sociokulturního jevu. Při dodržení tradičních bezpečnostních zásad má ayahuasca terapeutický potenciál vhodný pro léčbu drogové závislosti a jiných civilizačních chorob., Ayahuasca, a key element of traditional indigenous medicine of the Peruvian Amazon, has become a global phenomenon. Sessions, during which this psychoactive substance is used, can be currently attended not only in the South American rainforest, but also in the Czech Republic. According to the Government Regulation No 463/2013 on schedules of addictive substances, ayahuasca in the Czech Republic for the presence of N, N-dimethyltryptamine is on the list of illegal drugs. However, the substance is used within the subculture. In order to investigate the current phenomenon, we conducted a pilot anonymous online survey. The investigation focused on the possibilities of therapeutic application of this substance under local conditions. The data provided by 70 respondents show that ayahuasca is used in all Czech regions (mostly in Prague), especially for the therapeutic purpose. The tradition of drinking ayahuasca has been adapted to local conditions, and it is expected to increase in future.
