Cieľ: Cieľom štúdie bolo analyzovať faktory ovplyvňujúce dentálnu hygienu, medzi ktoré patrí konzumácia sladkostí a sladených nápojov u školskej mládeže z hľadiska veku a pohlavia a zároveň analyzovať výskyt dentálnej hygieny počas dňa. Metodika: Súbor respondentov tvorilo 1187 žiakov (620 dievčat a 567 chlapcov) vo veku 11, 13 a 15 rokov v oblasti stredného Slovenska. Zber údajov prebiehal v období máj 2010 až jún 2010. Na zber údajov bol použitý štandardizovaný dotazník vytvorený pre potreby HBSC štúdie (Health Behaviour in School – aged Children). Výsledky boli vyhodnotené Chí-kvadrát testom. Výsledky: Sladkosti a kolu v jednotlivých vekových kategóriách konzumuje každý deň alebo viac krát za deň približne tretina respondentov. Výskyt umývania zubov viac ako raz denne je približne rovnaký vo všetkých vekových kategóriách od 53% u 11 ročných chlapcov do 64% u 15 ročných dievčat. Vo všetkých vekových kategóriách dievčatá udávajú častejšie umývanie zubov v porovnaní s chlapcami. Záver: Výsledky poukazujú na zvýšenú konzumáciu sladkostí a koly počas dňa u školskej mládež stredného Slovenska, čo môže spôsobovať zvýšenú kazivosť zubov. Dentálna hygiena je v našom súbore respondentov nedostačujúca. Na základe získaných výsledkov je možné konštatovať, že potreba zvyšovania úrovne vedomostí nielen detí, ale aj rodičov o zásadách správneho stravovania a o zásadách dentálnej hygieny je neodmysliteľnou súčasťou výchovy k zdraviu a preventívnych opatrení v školskom veku., Aim: The aim of the study was to analyse the factors affecting dental hygiene such as consumption of sweets and sweet soft drinks in school-aged pupils in terms of their age and gender and at the same time to analyse the occurrence of dental hygiene during the day. Methods: The set of respondents consisted of 1187 pupils (620 girls and 567 boys) at the age of 11, 13 and 15 years in the area of central Slovakia. The data collection was conducted in the period of May 2010 to June 2010. For data collection was used standardized questionnaire for HBSC studies (Health Behaviour in School–aged Children). The results were analysed by Chi-square test. Results: Approximately one third of respondents in each category consumes sweets and Coca-cola every day or several times a day. The occurrence of cleaning the teeth more than once a day was equally proved in all age groups ranging from 53% in 11 years old boys to 64% in 15 years old girls. The girls of all age groups indicate more frequent cleaning of teeth in comparison with boys. Conclusion: The results show increased consumption of sweets and Cocacola during the day in the school youth from central Slovakia, which may cause an increase in tooth decay. Dental hygiene in our set of respondents is insufficient. Autorky příspěvku seznamují s dílčími výsledky vlastní sociometricky zaměřené metody, kterou aplikovaly ve třídách druhého stupně jedné ZŠ v České republice. V příspěvku je věnována pozornost hlavně těm výstupům, které se vztahují ke skupince dětí s nadváhou a obezitou. Pro adolescentní děti je prostředí školy a školní třídy důležitou součástí sociální reality, která značně ovlivňuje utváření jejich sebedůvěry, sebevědomí, identity a z těchto hledisek ovlivňuje po řadu kritických let kvalitu jejich života. Některé klinicky zaměřené zahraniční publikace tuto realitu reflektují a okolnostem života dětí s nadváhou a obezitou ve školním prostředí věnují pozornost (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Skidmore, Yarnell, 2004). Vzhledem k malému počtu dětí s nadváhou a obezitou byly výsledky v diskusi a v závěrečné části příspěvku shrnuty jen popisným způsobem., The authors of this article acquaint with the partial results of their own sociometric focused method, that they applied on the second stage of one elementary school in the Czech Republic. Attention is paid mainly to those outcomes, that relate to a group of children with overweight and obesity. For adolescent children are school environment and school classes important part of social reality, that greatly affects the formation of self-confidence, self-esteem, identity and affects many aspects of their quality of life during these crucial years. Even some clinically oriented foreign publications reflect this reality and pay attention to these circumstances of these children´s life (Kiess et al., 2009; Pallan et al., 2012; Taylor et al., 2005; Reilly et al., 2012; Skidmore, Yarnell, 2004). Hymenální atrézie - potažmo její důsledky - je vzácnou příčinou bolestí v podbřišku u dospívajících dívek, snadno zaměnitelnou za chirurgickou etiologii obtíží. Gynekologické vyšetření dívek a žen před chirurgickou intervencí je nezbytnou součástí diagnostického algoritmu, v případě atrézie hymen tímto lze v některých případech předejít abdominálnímu operačnímu řešení obtíží. Tato kazuistika demonstruje překvapivý nález v dutině břišní u 15leté dívky s bolestmi břicha, primárně chirurgem operované pro diagnózu „akutní apendicitida“. Kromě klinického obrazu jsme v tomto případě byli svědky i poměrně nezvyklého nálezu laboratorního - extrémně vysoké hodnoty CA 125., Hymenal atresia – and its results in fact - is the rare cause of pelvic or abdominal pain in pubescent girls, which could be changed for surgical cause of ailment easily. Gynecologic examination should be a part of a diagnostic algorithm in women and young girls before every surgical intervention - we can avoid an abdominal operation in case of hymenal atresia. This case report demonstrates a surprising finding in 15 years old girl, which was primarily operated by surgeon for diagnosis „acute appendicitis“. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Lichen sclerosus (LS) je onemocnění neznámé etiologie, postihující především postmenopauzální ženy. LS patří mezi nejčastější non-neoplastické epiteliální změny na vulvě, má typický makroskopický vzhled a klinicky je charakterizován intenzivním pruritem, případně dyspareunií. Mezi rizika LS patří tendence k jizvení a vznik dlaždicového karcinomu v terénu LS. Léčba LS je obyčejně dlouhodobá, opakovaná a jejím základem je lokální aplikace silného kortikosteroidu. Při adekvátní konzervativní léčbě je chirurgické řešení indikováno zřídka. Pacientky s LS je vhodné sledovat na specializovaném pracovišti., Lichen sclerosus (LS) is a disease of unknown ethiology, affecting mainly postmenopausal women. LS belongs to the most frequent non-neoplastic epithelial disorders of the vulva, it has a typical macroscopic pattern, and it is characterized with an intensive pruritus or dyspareunia. Patients with LS have a risk of scarring of external genitalia and risk of developing squamous cell carcinoma of the vulva. Treatment of LS is usually long-term, repeated, and it is based on local potent corticosteroids. Surgical treatment is rarely indicated in the management of LS. Onemocnění lidským papilomavirem (HPV) patří mezi jednu z nejběžnějších sexuálně přenosných infekcí a HPV působí spektrum nemocí, od genitálních bradavic ke karcinomům. Důležitým cílem očkovacích programů v mnoha zemích je snížení incidence karcinomu děložního hrdla použitím kvadrivalentní (Silgard/Gardasil) nebo bivalentní (Cervarix) HPV (human papillomavirus) vakcíny. Randomizované kontrolované studie prokazují robustní protilátkovou odpověď a vysokou účinnost HPV vakcinace také u mužů. V roce 2009 Úřad pro kontrolu potravin a léčiv (FDA) v USA schválil kvadrivalentní HPV vakcínu u mužů k prevenci anální prekancerózy a karcinomu. Podrobné finanční modely, které předcházely tomuto rozhodnutí, ukázaly, že pokud se do modelů zahrne výskyt onemocnění u mužů, záleží na ceně vakcíny, pokrytí očkované populace a dalších faktorech, zda HPV vakcinace mužů může být finančně efektivní. V každém případě cílená vakcinační strategie zvláště pro muže, kteří mají sex s muži, by měla podstatné benefity., The disease of the Human Papillomavirus (HPV) belongs to one of the most prevalent sexually transmitted infections and HPV causes a spectrum of diseases, ranging from genital warts to cancer. The important goal of immunization programs in many countries is the reduction of the incidence of cervical cancer using either the quadrivalent (Silgard/Gardasil) or the bivalent (Cervarix) HPV (human papillomavirus) vaccine. Randomized controlled trials demonstrate robust antibody responses and high efficacy also in men. In 2009 the Food and Drug Administration in the USA approved the quadrivalent HPV vaccine for young men to prevent anal precancerosis and cancer. Detailed cost effective modeling has preceded this decision showing that when the burden of disease in men is included in the models then, depending upon vaccine price, coverage of a vaccinated population, and other factors male vaccination can become cost effective. Cieľ: Cieľom výskumu bolo u detí s bronchiálnou astmou zistiť vplyv ochorenia a stupňa kontroly astmy na kvalitu života dieťaťa a kvalitu života jeho rodičov. Metodika: Na zber empirických údajov sme použili štandardizované dotazníky na kvalitu života – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) a Paediatric Asthma Caregiver´s Quality of Life Questionnaire (PACQLQ) a na hodnotenie kontroly astmy – Asthma Control Questionnaire (ACQ). Výsledky boli analyzované pomocou metód deskriptívnej a induktívnej štatistiky. Výsledky: Vo vzorke detí (n = 72) sme zistili, že 27 (37,5 %) detí malo astmu pod kontrolou a 28 detí (38,9 %) bolo v pásme nekontrolovanej astmy. Celkové skóre ACQ (1,24 ±1,07) potvrdilo stredný stupeň kontroly astmy. Celkové skóre dotazníka PAQLQ u detí bolo 5,34 ±1,16, čo indikuje, že deti s astmou vyjadrujú dobrú kvalitu života. U detí ochorenie viac vplývalo na aktivity ako na emocionálne prežívanie a výskyt symptómov. Celkové skóre dotazníka PACQLQ u rodičov bolo 4,53 ±1,07, u rodičov nebol zaznamenaný rozdiel v doménach emocionálne prežívanie a aktivity. Výskum potvrdil, že stupeň kontroly astmy štatisticky významne ovplyvňuje kvalitu života dieťaťa aj jeho rodičov, pričom čím je vyššia kvalita života dieťaťa, tým je vyššia kvalita života rodičov. Záver: Cieľom komplexného manažmentu liečby je dosiahnuť čo najoptimálnejší stupeň kontroly astmy, a tým vytvoriť predpoklady k vyššej kvalite života dieťaťa a jeho rodičov. Využívaním posudzovacích nástrojov na hodnotenie kvality života je možné identifikovať závažný dopad choroby na každodenný život detí aj rodičov., Aim: The aim of the research was to find out the impact of the disease and the degree of asthma control on quality of life in children with bronchial asthma and quality of life in their parents. Methods: To collect empirical data we have used standardized questionnaires concerning quality of life – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) and Paediatric Asthma Caregiver’s Quality of Life Questionnaire (PACQLQ) and for the assessment of asthma control we have used questionnaire Asthma Control Questionnaire (ACQ). The results were analysed using the methods of descriptive and inductive statistics. Results: In the sample of children studied (n = 72) we have found that 27 children (37.5%) have their disease under control and 28 children (38.9%) were in the range of uncontrolled asthma. Total ACQ score (1.24±1.07) confirmed medium degree of asthma control (partially controlled asthma). Total score of PAQLQ questionnaire in children (5.34±1.16) indicates that children with asthma represent good quality of life. Bronchial asthma in children had more significant influence on activities than on emotions and incidence of symptoms. Total score of PACQLQ questionnaire in parents was 4.53 ±1.07 and we have not registered differences between domains of emotions and activities. The research has proved that degree of asthma control has statistically significant influence on the quality of life of the child and his parents, whilst the higher the quality of life of the child, the higher the quality of life of parents. Conclusion: The aim of comprehensive management of asthma therapy is to achieve the most optimal level of asthma control and thus to establish conditions for higher quality of live of the child and his parents. Cíl: Cílem studie bylo zjistit potencionální riziko hypertenze u dětí ve věku 14 – 15 let dvojím standardním měřením krevního tlaku a zmapovat moţný vztah mezi hodnotami krevního tlaku dětí a jejich vybranými sociodemografickými faktory a tělesnými parametry. Metodika: Výzkumný soubor tvořilo 143 ţáků 8. a 9. tříd základních škol v Ostravě zapojených do projektu Zdravá škola. Data byla získána měřením fyziologických funkcí a antropometrických dat a doplňujícími otázkami na vybrané rizikové faktory kardiovaskulárních chorob. Výsledky: Měření krevního tlaku (TK) proběhla opakovaně, při druhém měření byla prokázána odlišnost naměřených hodnot TK mezi pohlavími, kdy u chlapců byly hodnoty TK vyšší neţ u děvčat. Bylo potvrzeno, ţe na hodnoty TK má vliv pohybová aktivita, a to zejména u dětí s vysokými hodnotami BMI. U normálních hodnot BMI korelace mezi hodnotou krevního tlaku a pohybovou aktivitou nebyla statisticky významná. Z rizikové skupiny 14 dětí s rodinnou predispozicí hypertenze bylo evidováno 29 % dětí s průměrnou hodnotou TK v pásmu pro 95. percentil (hypertenzní hodnoty) vzhledem k danému věku, pohlaví a výšce. Závěr: Výskyt vysokého TK byl četnější u dětí s nadváhou a obezitou. Pohybová aktivita ovlivňovala hodnotu krevního tlaku, zvláště pak právě u dětí s nadváhou a obezitou, kdy hodnoty TK u těchto dětí, které necvičily, byly vyšší neţ u dětí cvičících. Pravidelným měřením a správným zhodnocením TK můţe sestra včas zachytit děti a adolescenty s rizikovými parametry., Aim: The aim of the thesis is to examine potential risk of hypertension in children aged 14 - 15 years by two standard measurements of blood pressure and to find out possible relation between values of blood pressure in children and their selected socio-demographic factors and physical parameters. Methods: The research was carried out on 143 pupils from elementary schools in Ostrava, which participate in the project „Zdravá škola“(Healthy School). The data were obtained from measuring of physiological functions, survey of anthropometric data and further questions focused on particular risk factors for cardiovascular diseases. Results: The second measurement revealed a difference in values of blood pressure between sexes, when boys had higher blood pressure than girls. The research confirmed that blood pressure is influenced by physical activity, especially in children with high value of BMI. In children with lower values of BMI, the correlation between the value of blood pressure and physical activity was not statistically relevant. From a high-risk group of 14 children with genetic predisposition, 29% of children had average value of blood pressure in range of 95. percentile (hypertension) given age, sex and height. Conclusion: The occurrence of high blood pressure is common in children who suffer from overweight and obesity. The value of blood pressure is influenced by physical activity, especially in overweight and obese children, when value of blood pressure in the children who did not practice, were higher in children than exercisers. Objective: To prevent the possible functional complications after gastrectomy, to improve the quality of life and the process of early rehabilitation. Material and Methods: The study included 165 patients with gastric cancer of stage III (T3N12M0), which, depending on the method of recovery divided into 2 groups: Group 1 n = 80 patients who, after an extensive gastrectomy had reservoir formed, II group n = 85 patients who had gastrectomy performed without forming a reservoir. Results: There were no incidence of reflux esophagitis and dumping syndrome observed in the main group, while in the control group, these complications were respectively 61.2% and 25.8%. Weight gain in the main group was observed in 83.7% of patients, in the control group in 4.7%. Assessment of physical activity on the Karnovsky scale in the study group was 85.0%, while 62.0% in the control group. Within 3 months, physical rehabilitation was observed in 90.0% of patients of the main group, while in the control group it was 25.7%. Threeyear survival rate in the main group was 71.4+0.4%, while in the control group, 57.2+0.3% (P> 0.05). Conclusions: Creation of a stomach replacing reservoir after gastrectomy is adequate way in ridding patients from possible complications.
