The article is focused on the key problems of long-term care provided to disabled seniors above 65 years of their age and disabled children in the Czech Republic from informal carers' point of view. The research is based on 25 semi-structured interviews with informal carers. The results show a serious shortage of (social and health care) services generally as well as for informal carers alone, shortage of useful information and medical devices. In some cases, insufficient coordinative and informative role of general practitioners and paediatricians is also a problem. V předložené kazuistice je zachycena práce rodinného terapeuta s rodinou, kde matka trpí bolestmi zad a dítě (3,5) je chronicky nemocné. Trpí kašlem nejasného původu. Autorka podrobně popisuje průběh šesti sezení, během nichž se v rodině postupně měnila atmosféra. Konflikty, které pár značně zatěžovaly, měly své kořeny v minulosti obou rodin a v mylných nevyslovovaných předpokladech na obou stranách. Narativní terapie s respektem ke struktuře a genderové vyváženosti, kterou se autorka učila v rámci čtyřletého výcviku v institutu rodinné terapie psychosomatických poruch v Liberci, byla zde hlavním nástrojem léčby. Léčba se nemůže zaměřit přímo na symptom dítěte. Jde o symptom celého rodinného systému. Zjednodušeně řečeno, bylo třeba, aby se všichni mohli zase volně nadýchnout., In the presented casuistry there is given a description of a family therapist’s work with a family, where the mother suffers from backache and the child (3.5) is chronically ill suffering from cough of unknown origin. The author describes in detail the course of six sessions, during which the family atmosphere was gradually changing. The conflicts that significantly burdened the couple, had their roots in the past of both families and in wrong untold assumptions on both sides. Narrative therapy with respect to the structure and gender balance, which the author learned in the four-year training in the family therapy institute of psychosomatic disorders in Liberec, was the main tool of treatment. Příspěvek nabízí pohled na tradiční pojem „psychosomatických rodin“ z hlediska jeho současné platnosti a užitečnosti. Poukazuje na výzkumné a klinické zkušenosti s častějším výskytem psychosomatických onemocnění u rodin s určitými poruchovými vlastnostmi struktury a interakce, v nichž tělesný projev nemoci plní účelnou, krátkodobě takovou rodinu ochraňující, dlouhodobě ji v její porušenosti ustalující a vývojově ji poškozující úlohu. Výsledkem má být připomenutí si vlastností takto rizikovějších rodin s výhledem na diagnostické rozpoznávání a porozumění funkci takového psychosomatického dění a zacílení terapeutické pomoci ke zdravějším způsobům zvládání rodinného stresu, než poskytuje takto probíhající patogenní „vyrovnávání účtů“ vzájemných potřeb v rodinném soužití., The paper presents a review of the traditional concept of „psychosomatic families“ from the viewpoint of its present validity and usefulness. It refers to research and clinical pieces of experience with more frequent occurrence of psychosomatic diseases in families that exhibit certain dysfunctional traits of structure and interaction, in which the somatic symptom fulfils an effective function in momentarily protecting the family from breakdown, however, in the long run, solidifying its ongoing malfunction and developmental impairment.
