In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Because of this it’s very important that non-medical health staff will be enough educated. It is that sufficient awareness and practical experience that can affect the rate of inclination to ageist attitudes. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Conclusions: e work shows that in the sample survey, the majority of students appearing middle inclination to the myths of old age. V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The choice of researched sample was intentional. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. Závěry: Výzkumné studie (n = 34) a jeden systematický přehled ukazují pozitivní vliv metody klokánkování na nedonošeného novorozence umístěného na Jednotce intenzivní péče v oblastech biopsychosociálních. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Cíl studie: zhodnocení efektivity cerkláže dle McDonalda ve Fakultní nemocnici Ostrava v období 1/2007 – 3/2014. Typ studie: retrospektivní studie Název a sídlo pracoviště: Porodnicko–gynekologická klinika, Fakultní nemocnice Ostrava Metodika: retrospektivní analýza provedených cerkláží ve Fakultní nemocnici Ostrava a rozbor výsledků takto ošetřených gravidit. Kritériem úspěšnosti byl stanoven porod po ukončeném 34. týdnu gravidity. Závěr: ve sledovaném období bylo indikováno devět cerkláží – 7 terapeutických a 2 záchranné. Výkony byly provedeny mezi 18. a 25. týdnem gravidity. Tři pacientky již měly v minulosti cerkláž provedenu. 57 % takto ošetřených těhotných porodilo po ukončeném 34. týdnu gravidity. Pouze v jednom případě (11,1 %) byla cerkláž neúspěšná a těhotenství skončilo potratem. Průměrná doba mezi provedením cerkláže a porodem činila 69 dnů., Objective: To evaluate the efficacy of cerclage according to McDonald at the University Hospital Ostrava in the period 1/2007 - 3/2014. Design: A retrospective study Obstetrics and Gynecology: Obstetrics - Gynecology, University Hospital Ostrava Methods: A retrospective analysis of cerclage performed at the University Hospital Ostrava and analysis of the results thus treated pregnancies. The criterion for success was determined after childbirth completed 34 weeks of preg - nancy. Conclusion: In the reporting period, nine indicated cerclage - 7 of therapeutic and 2 emergency. The procedures were performed between the 18th and 25th week of pregnancy. Three patients have had in the past cerclage performed. 57% of the treated pregnant women gave birth after 34 completed weeks of gestation. Only in one case (11.1 %) cerclage was unsuccessful and the pregnancy ended in a miscarriage.
