Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. Cíl: Cílem studie bylo vyhledat poznatky týkající se NANDA International ošetřovatelské diagnózy Porucha polykání (00103) se zaměřením na výskyt této ošetřovatelské diagnózy u různých skupin pacientů a její stanovení prostřednictvím znaků uvedených v klasifikačním systému dle NANDA International. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system. Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. Cíl: Cílem studie bylo zhodnotit míru subjektivně vnímaných potíží osob s poruchou čichu. Výsledky porovnat s 2 standardizovanými olfaktometrickými testy. Materiál: Celkem bylo vyšetřeno 134 osob (59 mužů a 75 žen) průměrného věku 53 let. Se sinonazální poruchou čichu bylo vyšetřeno 58 osob (průměrný věk 50 let). 21 osob s poúrazovou poruchou čichu (průměrný věk 39 let), 19 osob s povirovou poruchou čichu (průměrný věk 58 let) a 36 osob s jinou etiologií poruchy čichu (průměrný věk 54 let). Metodika: Subjektivní hodnocení závažnosti čichové poruchy a její vnímání v každodenním životě bylo zjišťováno formou dotazníku. K vyšetření čichu byl použit Test parfémovaných fi xů (Odorized Markers Test – OMT) a Sniffi n’Sticks test (16 složkový identifi kační test). Výsledky: Subjektivně udalo potíže s čichem 134 osob, které byly zařazeny do studie. Potíže s čichem trápí pacienty ve 42 % výrazně, ve 43 % mírně a 15 % osob poruchu čichu nevnímá vůbec. V každodenním životě vnímá poruchu čichu 62 % respondentů. Závěr: Ženy trápí potíže s čichem výrazněji než muže. Souvislost mezi mírou vnímání tíže poruchy čichu a vlivem na kvalitu života nebyla zjištěna. Výstup studie bude sloužit k vytvoření dotazníku pro hodnocení kvality života osob s poruchou čichu., Aim: Th e aim of study was to determine the degree of subjectively perceived problems in people with smell disorders. Th e results were compared with two standardized smell identifi cation tests. Methods: Th e total of 134 persons (59 men and 75 women) aged 53 years on average were examined. 58 persons with sinonasal disease (aged 50 years on average), 21 persons aft er head trauma (aged 39 years on average), 19 persons with smell loss due to upper respiratory tract infection (aged 58 years on average), and 36 persons with other etiology of smell disorders (aged 54 years on average). Subjective assessment of severity of olfactory disorders and its perception in daily life was examined by questionnaire. Th e sense of smell was tested by Odorized Markers Test (OMT) and Sniffi n’Sticks test (16 – items identifi cation test). Results: All 134 persons who were included to the study reported diffi culty with smell. Th e problems with smell were perceived in 42 % of patients strongly, 43 % mildly and 15 % of patients did not smell anything at all. 62 % of respondents realized their smell disorder in everyday life. Conclusion: Women suff ered from problems with smell more strongly compared to men. Th e relationship between the level of perception of severity of smell disorders and the eff ect on life was not confi rmed. Cíl: Hodnocení komunikačních a vybraných psychických oblastí (napětí, obavy, podráždění, deprimace) dispenzarizovaných nemocných se zhoubnými nádory hrtanu a slinných žláz, pomocí standardizovaných dotazníků kvality života. Východiska: Zhoubná nádorová onemocnění hlavy a krku se v četnosti výskytu řadí na šesté místo mezi zhoubnými nádory na celém světě. Otázka léčby a následné péče je velmi aktuální zvláště z hlediska rizika mutilujících léčebných výkonů a jejich vlivu na kvalitu života. Metody: Do výzkumu byli zařazeni aktivně dispenzarizovaní nemocní ORL kliniky se zhoubnými nádory hrtanu a slinných žláz. Vlastní šetření absolvovalo 113 respondentů, výzkum probíhal od ledna do prosince 2011. Pro sběr dat byly využity dva standardizované dotazníky kvality života (EQRTC QLQ-30, EQRTC QLQ-H&N35). Dále informace z nestandardizovaného dotazníku a zdravotnické dokumentace. Použité statistické metody – dvouvýběrový Wilcoxonův test a analýza rozptylu. Výsledky: U pacientů s nádorovým onemocněním hrtanu s tracheostomickou kanylou a bez ní byl statisticky významný rozdíl v komunikaci. Stádium nemoci dle klasifi kace TNM nemělo vliv na vybrané psychické oblasti u respondentů s nádorovým onemocněním hrtanu a slinných žláz. Závěry: Závažnost obtíží u dispenzarizovaných respondentů závisí na lokalizaci a velikosti nádoru, terapeutické strategii, věku pacienta, polymorbiditě a dalších faktorech. Trvalá tracheostomická kanyla má vliv na komunikaci respondentů s nádory hrtanu. V našem souboru respondentů jsme neprokázali vliv stádia nemoci dle klasifi kace zhoubných novotvarů TNM, na vybrané oblasti psychiky (napětí, obavy, podráždění, deprimace)., Aim: Assessment of communication and selected psychological areas (e. g. tension, concerns, irritation, depression) related to the dispensarized patients with malignant laryngeal and salivary glands tumors using standardized questionnaires assessing quality of life. Background: The head and neck malignant tumors rank sixth in frequency among malignant tumors worldwide. The question of treatment and follow-up care is very current especially in terms of the risk of mutilating surgical procedures and their impact on quality of life. Methods: In the research actively dispensarized patients of ENT clinic with malignant tumors of the larynx and salivary glands were included. 113 respondents took part in the research, which was conducted from January to December 2011. Standardized (EQRTC QLQ-30, EQRTC QLQ-H&N35) and non-standardized questionnaires assessing quality of life and medical records were used for data collection. Statistical methods such as double selection Wilcoxon test and analysis of dispersion were used. Results: There was a statistically signifi cant diff erence in communication between patients with tracheostomy cannula and without it. Stage of the illness according to TNM classifi cation did not infl uence selected psychological areas of the respondents with cancer of the larynx and salivary glands. Conclusions: The seriousness of the diffi culties of the dispensarized patients depends on localization and size of the tumor, therapeutic strategy, age of the patient, polymorbidity and other factors. Permanent tracheostomy cannula has an impact on communication of the respondents with laryngeal tumors. Cíl: Cílem práce je analyzovat literární přehled faktorů pracovní spokojenosti sester ve vztahu k setrvání sester na pracovišti. Metodika:Zdroje dat - Elektronické databáze: MEDLINE, Wiley Library Online, PubMed, Bibliographia medica Čechoslovaca. Periodika – Kontakt, Profese on-line, Ošetřovatelství a porodní asistence, Ošetrovateľstvo: teória, výskum, vzdelávanie a odborné recenzované publikace. Klíčová slova byla zadávána v databázích publikovaných od roku 1990 nebo později, které zkoumaly pracovní spokojenost sester a její vliv na setrvání sester v práci. Literární výsledky jsou prezentovány v tabulkovém formátu ke komparaci jednotlivých studií. Výsledky: Z celkového počtu 42 sdělení nebo abstraktů, bylo do tohoto přehledu zahrnuto 29 studií, převáţně kvantitativních studií. Byly zjištěny následující prediktory pracovní spokojenosti sester, které ovlivňují jejich záměr setrvat v zaměstnání: (1) pracovní vztahy, (2) pracovní odměny, (3) styl vedení, vztah a podpora manaţera, (4) organizační podpora, stav pracovního prostředí, (5) fyzická a psychická reakce na práci. Závěr: Manaţeři ošetřovatelské péče sehrávají významnou roli v udrţení pracovní spokojenosti sester. Znalosti o proměnných pracovní spokojenosti sester mohou pomoci managementu zdravotnických zařízení implementovat strategie k udrţení spokojenosti sester. Pozornost by měla být směrována zvláště k posílení interpersonálních vztahů a potřebné kapacitě sester v zařízení, k udrţení kvality péče o pacienta., Aim: The aim of this work was to perform a literature review of nurses’ job satisfaction factors with respect to their retention in the workplace. Methods: Data sources – electronic databases: MEDLINE, Wiley Online Library, PubMed, Bibliographia medica Čechoslovaca; journals: Kontakt, Profese on-line, Ošetřovatelství a porodní asistence, Ošetrovateľstvo: teória, výskum, vzdelávanie; and peer-reviewed publications. The key words were entered into the databases published from 1990 or later that have examined the job satisfaction of nurses and its impact on their retention in work. Literature results are presented in a tabular format for the comparison of individual studies. Results: Out of a total of 42 articles or abstracts, 29 – mostly quantitative – studies were included in this review. The following predictors of nurses’ job satisfaction that influence their intention to remain in the workplace were identified: (1) working relationships, (2) job rewards, (3) leadership style, relationship and support of a manager, (4) organizational support, the state of the work environment, and (5) physical and psychological reactions to work. Conclusion: Nursing care managers play an important role in maintaining job satisfaction of nurses. Awareness of the variables of nurses’ job satisfaction can help healthcare facility managers implement strategies to maintain nurses’ job satisfaction. Objective: To determine risk factors in different pathogenic subtypes of ischemic stroke in Tashkent hospital registry and to compare with the data of other stroke registers. Design: Prospective observational study. Participants: 300 adults aged 18 years or over who suffered first-ever acute ischemic stroke, of them, the data of 100 patients with atherothrombotic, lacunar and cardioembolic strokes were analyzed. Main outcome measures: Mathematically significant differences and not significant differences but interesting from other point of view. Categorical variables are expressed as frequencies and percentages. Differences were regarded as statistically significant when the P value was less than 0.05 (P<0.05). Results: Of 100 patients with acute first-ever ischemic stroke at the median age of 62.63±4.68 (interquartile range 25-92) years, 57% were men and 43% were women. Depending on aetiology and stroke subtype, all the patients were divided into three groups: 42 patients with atherothrombotic stroke, 41 – with lacunar stroke, and 17 patients with cardioembolic stroke. Atherothrombotic stroke developed often in people over 60 years old, while lacunar stroke prevailed at the younger age. Stroke cases increased in people over 51 years old. Although men were more likely to suffer strokes than women, stroke incidence was equal in both sexes with increasing of age (over 60 years old), especially in atherothrombotic stroke, testifying to that women live longer. Combination of cerebral atherosclerosis and hypertension was the most significant risk factor for stroke. Atherosclerosis was the main aetiological factor for ischemic stroke, independently of its pathogenic heterogeneity. Diabetes mellitus was an independent risk factor for all stroke subtypes, which in most cases worsened the European Medical, Health and Pharmaceutical Journal ISSN 1804-5804 course and outcome of stroke. Hypertension was the leader aetiological factor for lacunar stroke. In these patients, we Cardioembolic stroke developed mainly because of cardiac thrombosis and valve pathology, which were often associated with chronic diseases of other organs, worsening the course and outcome of stroke. Despite of high prevalence of myocardial infarction in men, women were more likely to develop cardioembolic stroke. Conclusions: This study found simultaneous influence of many risk factors (multifactorial disease) in the development of ischemic stroke. The more course and influence of the underlying disease and risk factor, the higher stroke risk and severity, especially in the absence or inadequate previous therapy of underlying disease before the stroke index. We have noted a high prevalence of under-diagnosis of vascular risk factors at levels of primary and secondary care. We recommend for improved public awareness of vascular risk factors and better diagnostic and treatment facilities aimed at addressing those factors at levels of primary and secondary healthcare. In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination.
