Cíl: Cílem studie je představit dlouhodobé výsledky chirurgické léčby supratentoriálních kavernózních malformací mozku na Neurochirurgické klinice 1. LF UK a ÚVN‑VFN. <p align="">Soubor a metodika: Retrospektivně jsme do studie zařadili pacienty se supratentoriálním, kortiko‑subkortikálně uloženým kavernomem léčené v letech 2000–2012. Hodnotili jsme epidemiologická data pacientů, radiologický nález, klinický obraz onemocnění a operační výsledky včetně komplikací. <p align="">Výsledky: Iniciálním příznakem byl epileptický záchvat (u 49 % kavernomů), cefalea (22 %), fokální neurologický deficit (19 %) a 15 % bylo náhodně nalezených. Známky recentního krvácení na MR byly přítomny u 27 % pacientů. Operovali jsme celkem 145 pacientů s 158 kavernomy. Dvacet pět lézí bylo léčeno konzervativně. K chirurgické komplikaci došlo v 8 %. U dvou pacientů (1,4 %) se trvale zhoršil stav v přímé souvislosti s operací. Pooperační výskyt epileptického záchvatu byl signifikantně vyšší ve skupině s rannou infekcí či pooperačním hematomem (p < 0,05). <p align="">Závěr: Mikrochirurgická resekce hemisferálního kavernomu je relativně bezpečná operace s minimální morbiditou a mortalitou. Pooperační hematom nebo ranná infekce může mít epileptogenní potenciál., Aim: The aim of the study is to present surgical outcome of treatment of supratentorial cavernous malformation of the brain at the Department of Neurosurgery, Charles University and the Central Military Hospital in Prague. <p align="">Material and methods: We retrospectively enrolled patients diagnosed between 2000 and 2012 with supratentorial, cortico‑subcortically located cavernoma. We analysed epidemiological and radiological data, clinical presentation and surgical results including complications. <p align="">Results: Initial symptoms included epileptic seizure (49%), headache (22%) and focal neurological deficit (19%); 15% of cavernomas were found incidentally. Radiological signs of recent haemorrhage on MR scans were found in 27% patients. We performed surgery in 145 patients with 158 cavernous malformations. Twenty five lesions were treated conservatively. Surgical complications occurred in 8% of patients. One patient died and one had permanent neurological deficit attributable to surgery. Postoperative seizure rate was significantly higher in a group with wound infection or postoperative hematoma (p < 0.05). <p align="">Conclusion: Microsurgical resection of lobar cavernoma is relatively safe procedure with minimal morbidity and mortality. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Case study documents maladaptation to MS diagnosis and prolonged maladaptive coping, diffusion and influence of affective components on inhibition of cognitive abilities and mental health status, the contribution of stress to development of „pseudo-relapses“ of MS, psychosocial problems and quality of life. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Repeated neuropsychological assessment was a part of a comprehensive diagnostic process and treatment of MS and, at the end of observation, in chronic-progressive phase of MS, was complemented with cognitive-behavioural therapy aimed at reducing burden of disease and improving the quality of life., and A. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem., Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. Užívání hormonální antikoncepce je považováno za rizikový faktor vzniku hluboké žilní trombózy a plicní embolie. Od zavedení hormonální antikoncepce v 60. letech 20. století byl u jejích uživatelek pozorován častější výskyt těchto komplikací. Nejnovější studie pak ukázaly dvakrát vyšší riziko vzniku žilní trombózy při užívání kombinovaných preparátů obsahujících progestiny třetí generace (desogestrel, gestoden, norgestimate) ve srovnávání s progestiny druhé generace (levonorgestrel a norgestrel), ačkoli progestiny třetí generace neprokázaly žádný klinicky významný benefit. Přičemž riziko vzniku hluboké žilní trombózy je ještě vyšší u neperorálních forem hormonální antikoncepce (vaginální kroužek, transdermální náplast)., Since the early 1960s, it has been well documented that combined hormonal contraceptives increase the risk of venous thrombosis. Second and third generation oral contraceptives (OCs) differ in their progestin component. Third generation OCs contain desogestrel, gestodene or norgestimate while second generation OCs contain norgestrel or levonorgestrel. In spite of the fact that the third generation OCs have not shown any clinically significant benefit over the second generation OC, multiple studies and two meta‑analyses show that the third generation OC containing desogestrel are associated with approximately doubled risk of venous thrombosis. The newer generation of oral formulations, as well as non‑oral contraceptives (transdermal and vaginal), were recently studied for thrombotic risk. Capsular bag distension syndrome is a very rare complication of cataract surgery. Most cases occur after uneventful phacoemulsification with continuous curvilinear capsulorhexis and implantation of an intraocular lens in the capsular bag. The entity presents with reduction of visual acuity and myopic shift in the early-to-late postoperative period. Characteristic findings include the distension of the capsular bag due to the accumulation of milky material and the forward displacement of the intraocular lens. We present two cases with an unusually delayed presentation of 6 and 8 years respectively following phacoemulsification, and describe their successful management with Nd:YAG laser posterior capsulotomy. and P. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient.
