Úvod: Léčba intratékálním baklofenem (ITB) kontinuálně pomocí pumpových systémů je důležitou léčebnou modalitou u nemocných s těžkou spasticitou, která již nezabírá na perorální léky nebo když se objeví výrazné nežádoucí účinky. Tato metoda je relativně velmi bezpečná, přesto se ale mohou vyskytnout komplikace spojené s touto léčbou. Nejčastěji se objevují komplikace spojené s implantací systému. Jde zejména o problémy s katétrem. Rotace pumpy je extrémně vzácnou komplikací a její výskyt je méně než 0,3 %. Kazuistika: U 74leté ženy s těžkou spasticitou po chronickém míšním poranění ve výši Th9 byla úspěšné provedena implantace baklofenové pumpy (Medtronic, Synchromed II). Pacienka měla bezprostřední úlevu se snížením spasticity již na dávce baklofenu 100 ?g/den. Na první ambulantní kontrole se opakovaně nezdařilo doplnit rezervoár pumpy. Bylo vysloveno podezření na její rotaci. Pacientka byla okamžitě odeslána na rentgenologické vyšetření, kde bylo potvrzeno přetočení pumpy o 180 stupňů. Tentýž den byla pacienta v lokální anestezii operována, pumpa byla otočena a důkladně zafixována stehy. Vzhledem k rychlé revizi se u pacientky nerozvinul syndrom z náhlého odnětí baklofenu při prázdném rezervoáru pumpy. Závěr: Rotace pumpy je vzácná komplikace léčby ITB. Je nutné okamžité vyřešení tohoto problému, neboť hrozí rozvoj abstinenčního syndromu z náhlého odnětí baklofenu. Léčbu ITB je třeba provádět v centrech se specializovanou péčí o tyto pacienty. Despite generally favorable outcomes and relative safety of this method, complications associated with ITB can be observed. The most common problems include surgery‑related complications resulting from system implantation, including catheter failure. Pump rotation is a rare complication with incidence less than 0.3%. Case report: Seventy‑four‑years‑old female suffered from spasticity after chronic spinal cord injury at T9 level. e underwent successful implantation of ITB pump (Medtronic, Synchromed II) with prompt spasticity relief on baclofen dose of 100 ?g/day. During the first outpatient appointment, it was difficult to puncture and fill the reservoir with baclofen. Plain x‑ray revealed rotation of the pump with reservoir in downright position. On the same day, the patient underwent emergency surgical revision with correction of the pump position and refill of the reservoir. No baclofen withdrawal syndrome or other complications occurred and the patient recovered well. Conclusion: Pump rotation is a very unusual and rare complication of ITB treatment. Still's disease is an inflammatory disorder of unknown etiology. First-line therapy is based on corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs) but the frequency of relapses and corticosteroid-induced adverse events are limiting factors. The efficacy of intravenous tocilizumab (TCZ) has been shown at a dose of 8 mg/kg but the corticosteroid-sparing effect of intravenous low-dose TCZ followed by subcutaneous (SC) injection in the course of the disease has been poorly investigated. We report the case of a 28-year old Caucasian woman presenting a relapse of Still's disease eleven months after diagnosis under treatment with 6 mg of methylprednisolone. TCZ at a dose of 4 mg/kg every 2 weeks was combined with 32 mg of methylprednisolone, followed by 162 mg SC every 3 weeks. Evolution was rapidly favourable with a decrease in corticosteroid doses. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months. Úvod: Bolesti v pravém podbřišku jsou nejčastějším důvodem hospitalizace na chirurgických pracovištích obecně. Zároveň akutní apendicitida je nejčastěji se vyskytující náhlá příhoda břišní. Včasná indikace k apendektomii je jedinou metodou léčby, obvykle s nekomplikovaným průběhem a krátkou délkou hospitalizace. V rámci diferenciálně-diagnostické rozvahy je nutné pátrat i po dalších symptomech, zejména dlouhodobých, při akutní exacerbaci často ustupujících do pozadí. Kazuistika: Prezentujeme případ 38leté pacientky, která byla přijata pro akutní bolesti břicha v pravém podbřišku. Klinický i laboratorní nález nasvědčoval pro typicky probíhající akutní apendicitidu, byla proto indikována k apendektomii. Při operaci byl nalezen hemorhagický výpotek v pánvi, na apendixu makroskopicky netypický nález pro akutní zánět. Překvapivý byl nález ztluštění stěny terminálního ilea a zvětšení pravých adnex. Nález si vyžádal rozšíření operačního výkonu na ileocekální resekci a pravostrannou adnexektomii. Definitivní diagnózu stanovilo histologické vyšetření potvrzující endometriózu apendixu, terminálního ilea a pravého ovaria. Dlouhodobou dispenzarizací nemocné byla následně zjištěna prekanceróza, resp. mikroadenokarcinom hrdla děložního. Závěr: Stanovení diagnózy endometriózy apendixu předoperačně je prakticky nemožné. V rámci diferenciální diagnostiky lze na ni pomýšlet u fertilních žen s chronickými gynekologickými obtížemi neobjasněné etiologie. Diagnostickou, resp. léčebnou metodou volby je laparoskopie, která dovolí přehlédnutí celé peritoneální dutiny, včetně malé pánve, provedení apendektomie a eventuálně excizi podezřelých ložisek z endometriózy. Definitivní stanovení diagnózy je doménou histopatologického vyšetření. Při průkazu endometriózy apendixu nebo jakékoli jiné lokalizace je nezbytné předání pacientky do péče gynekologa., Introduction: In general, pain in the lower right abdomen is the most frequent reason for hospital surgical admissions, acute appendicitis representing the most common cause of operation for acute abdomen. Timely appendectomy remains the only treatment in the early stages of inflammation and is usually uncomplicated, requiring only a short hospital stay. A differential diagnostic analysis necessitates a search for other, particularly long-term symptoms that might be driven to the background in cases of acute exacerbation. Case report: The case report presents a 38-year-old female patient who was admitted for lower right abdominal pain. Clinical examination and a blood test both suggested typical acute uncomplicated appendicitis, and therefore the patient underwent appendectomy. Haemorrhagic peritoneal fluid and nodularity of the appendix not typical for appendicitis was found. Oedema of the terminal ileum and a right adnexal tumour were a surprising finding. A more extensive surgical procedure involving ileocaecal resection and right-side adnexectomy was finally performed with regard to the intraoperative finding. The definitive diagnosis of appendiceal endometriosis, endometrial mass in the terminal intestine and ovarian endometriosis was established by histological evaluation. Long-term follow-up revealed microadenocarcinoma of cervix uteri. Conclusion: It is generally very difficult to confirm appendiceal endometriosis before operation, and revealing primary appendiceal endometriosis is virtually impossible. It is advisable to consider endometriosis in fertile women with chronic abdominal pain of unclear aetiology and gynaecological symptoms in their personal history. The best diagnostic and therapeutic method, respectively, is laparoscopy enabling exploration of the entire peritoneal cavity including the minor pelvis, and performing appendectomy as well as excision of suspicious endometrial lesions. Úvod: Klivus je centrální struktura baze lební v těsné blízkosti mozkového kmene a životně důležitých cév. K frakturám klivu dochází většinou při vysokoenergetických úrazech, jednou z nejčastějších komplikací bývá nazální likvorea. Kazuistika: Představujeme případ muže ve středním věku, u kterého se po běžném úrazu hlavy rozvinula nazální likvorea. CT vyšetření ukázalo masivní pneumocefalus a frakturu klivu v oblasti sinus sphenoidalis. Provedli jsme endoskopický endonazální výkon a defekt úspěšně uzavřeli. Diskuze: Pneumatizace sfenoidální dutiny vykazuje velkou interindividuální variabilitu. V našem případě se jedná o extrémně vzdušný sinus v kombinaci s gracilním klivem, což bylo predisponujícím faktorem pro zlomeninu. Traumatická likvorea představuje riziko likvorové hypotenze a vzniku meningitidy. Klasický chirurgický přístup je mikroskopický transnazální. Endoskopie je moderní miniinvazivní variantou. Závěr: Endoskopické endonazální ošetření traumatické likvorey u zlomenin baze lební v oblasti klivu je metodou volby. Clival fractures are usually caused by a high-energy trauma. Cerebrospinal fluid leak is one of the most common complications. Case report: A middle-aged male sustained a mild head trauma, followed by a nasal cerebrospinal fluid leak. CT scan revealed the massive pneumocephalus and the fracture of the clivus in the posterior wall of the sphenoidal sinus. We performed an endoscopic endonasal surgery to seal the defect. Discussion: Pneumatization of a sphenoidal sinus shows high variability. In the presented case, extreme pneumatization of the sinuses was combined with the gracile clivus, which was the predisposing factor for fracture. Traumatic cerebrospinal fluid leak carries the risk of intracranial hypotension and meningitis. Microscopic transseptal management is the classical surgical approach, while endoscopy provides the modern miniinvasive option. Conclusion: Endoscopic endonasal treatment of traumatic cerebrospinal fluid leak is the treatment of choice for the clival fractures. Key words: skullbase fracture – neurosurgery – endoscopy, and M. Je popsána resekce tumoru s částí stěny duté žíly i resekce metastázy v oblasti pánve. Gastrointestinální stromální tumory (GISTy) představují většinu mezenchymálních nádorů zažívacího traktu. Extragastrointestinální tumory (EGISTy) jsou nádory, které se v histologickém a imunohistochemickém obraze GISTům podobají, ale nacházejí se mimo trávicí trubici, nemají žádnou spojitost se stěnou žaludku či střeva. V naší kazuistice představujeme případ 68letého pacienta s EGISTem vycházejím z mezenteria.
