A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. Takotsubo cardiomyopathy is an important entity in the differential diagnosis of acute coronary syndrome. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years. Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V. Cíl práce: Prezentujeme kazuistiku pacienta, u kterého došlo k rozvoji oboustranného seminomu na podkladě intratubulární germinální neoplazie – neklasifikovatelný typ (IGCNU – intratubular germ cell neoplasia unclassified type). Kazuistika: Pacient s anamnézou seminomu levého varlete klinického stadia I a IGCNU po radikální orchiektomii a adjuvantní radioterapii byl odeslán k urologickému vyšetření pro suspektní tumor solitárního pravého varlete dle sonografického vyšetření. Indikovali jsme 3biopsii varlete, která prokázala přítomnost IGCNU. Následovala radikální orchiektomie, kde byl histologicky verifikován klasický seminom. Závěr: Intratubulární germinální neoplazie představuje vysoké riziko rozvoje invazivního germinálního tumoru varlete. Léčba zahrnuje radioterapii či chemoterapii cisplatinou. Alternativou je pečlivá dispenzarizace a pravidelné sonografické kontroly, zejména u pacientů se solitárním varletem, kteří chtějí zachovat fertilitu a endokrinní funkci. Biopsie kontralaterálního varlete u pacientů s germinálním tumorem k vyloučení IGCNU by měla být provedena v případě kryptorchizmu, poruchy spermiogeneze a atrofií varlete s objemem pod 12 ml, vhodná je rovněž u pacientů s mikrolitiázou varlete a předchozím výskytem IGCNU., Objective: We are presenting a case report of young man who developed an asynchronous bilateral seminoma based on an intratubular germ cell neoplasia of unclassified type (IGCNU). Case report: A patient, with history of a stage I seminoma of the left testis, based on an IGCNU of the left testis, after a radical orchidectomy and adjuvant radiotherapy, was referred to the urology clinic with a suspected tumor of the right testis. A testicular biopsy was made and the histological examination showed an IGCNU. A radical orchidectomy was performed and a seminoma was found. Conclusion: IGCNU is a precursor lesion of germ cell testicular tumors. Treatment of the IGCNU includes radiotherapy or chemotherapy with cisplatin. Surveillance remains an option for those with a solitary testicle who wish to preserve fertility and endocrine function. A contralateral testicular biopsy, in patients with a history of germ cell tumors, should be made in cases of cryptorchidism, poor spermatogenesis, and testicular atrophy with a testicular volume of less than 12 ml. Pseudocysta pankreatu patří mezi nejčastější cystické léze pankreatu. Pseudocystu lze definovat jako tekutinovou intrapankreatickou, peripankreatickou nebo extrapankreatickou kolekci s definovanou stěnou vznikající nejčastěji na pokladě chronického nebo akutního zánětu pankreatu. Jde o poměrně častou komplikaci akutní nebo chronické pankreatitidy. V diagnostice má zásadní význam UZ pro svou neinvazivitu a dostupnost. Mezi další vyšetření vykazující nejvyšší přesnost, specificitu a senzitivitu patří MDCT a ERCP. Na základě MDCT a ERCP vyšetření lze stanovit klasifikaci a na základě této klasifikace následně algoritmus léčby. MDCT vyšetření by optimálně mělo předcházet ERCP. K odlišení od maligních cystických lézí pak prokazuje největší význam EUS s FNAB nebo aspirací obsahu se stanovením onkomarkerů a AMS v aspirátu. Funkční klasifikace pseudocyst je důležitým předpokladem pro úspěšný algoritmus léčby. Klasifikace vycházejí zejména z anatomického vztahu pseudocysty k pankreatickému vývodu a morfologii parenchymu pankreatu. V léčbě se uplatňují zejména perkutánní drenáž, endoskopické a chirurgické metody., The pseudocyst of the pancreas is one of the most common cystic lesions of the pancreas. The pseudocysts can be defined as intrapancreatic, peripancreatic or extrapancreatic fluid collections with a defined wall formed mostly on the basis of acute or chronic pancreatitis. This is a relatively common complication of acute or chronic pancreatitis. Sonography is essential for the diagnosis given its noninvasivity and availability. Other assessments that show the highest accuracy, specificity and sensitivity include MDCT and ERCP. The MDCT and ERCP assessments can be used to classify the pseudocyst; the treatment algorithm is then determined based on this classification. MDCT should ideally be preceded by ERCP. EUS-FNAB or aspiration with the determination of oncomarkers and AMS in the aspirate have been shown to be most important to distinguish the pseudocyst from malignant cystic lesions. Functional classification of the pseudocyst is an important prerequisite for successful algorithm of the treatment. The classification is based mainly upon anatomical relations of the pseudocyst to the pancreatic outlet and morphology of the pancreatic parenchyma.
