More than 60 neuropeptides that inhibit juvenile hormone synthesis by the corpora allata have been isolated from the brains of various insect species. Most of them are characterized by a common C-terminal pentapeptide sequence Y/FXFGL/I/V (allatostatin A family, allatostatin superfamily). Besides the allatostatin A family, allatostatic neuropeptides belonging to other two peptide families (W2W9)-allatostatins or allatostatin B family; lepidopteran allatostatin) were reported. Four peptides with allatostatic activity were isolated from brains of the Mediterranean field cricket, Gryllus bimaculatus. Three of them (Grb-AST A3: AGMYSFGL-NH2; Grb-AST A4: SRPFGFGL-NH2; Grb-AST A5: GPDHRFAFGL-NH2) belong to the wide-spread family of Y/FXFGL/I-amide peptides, the fourth (Grb-AST B5: AWDQLRPGW-NH2) is a member of the W2W9 - amide family of neuropeptides. All of these peptides are potent inhibitors of juvenile hormone (JH) biosynthesis by cricket corpora allata in vitro, causing 50% inhibition of JH biosynthesis at 0.4-3 × 10-8 M. The two peptides Grb-AST A5 and Grb-AST B5 have virtually the same potency and efficacy in inhibiting JH biosynthesis in vitro. No synergistic effect of the two peptide families with respect to the inhibition of JH biosynthesis could be observed. Allozyme polymorphism was investigated in adult males of the stenotopic peat bog (tyrphobiontic) noctuid moth, Coenophila subrosea Stephens, from three isolated peat bog localities in Austria (Styria) and Czech Republic (South and North Bohemia). Of the eighteen enzyme loci examined, twelve were polymorphic and six monomorphic. Significant deviations of genotype frequencies from Hardy-Weinberg equilibrium were observed at about one third of polymorphic loci within the populations. The average heterozygosities for the populations from three geographically distinct localities ranged from 0.192 to 0.245, and 61% of the loci were polymorphic. The FST mean value of 0.0675 was higher than that found in most other Lepidoptera. The genetic distances based on allozyme heterozygosity ranged from 0.019 to 0.051, with the population from South Bohemia being the most distant. The genetic distances and FST values do not reflect the geographic distances between the populations. Morphometric analysis revealed a difference between the Austrian Purgschachen Moor and Bohemian populations. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Žilní tromboembolická nemoc (TEN), zahrnující žilní trombózu stejně jako plicní embolii, je časté a potencionálně fatální onemocnění. S uvedením nízkomolekulárních heparinů (LMWH) do praxe bylo prokázáno, že ambulantní léčba žilní trombózy je efektivní a bezpečná pro velkou část nemocných s TEN. Vzrůstající data o ambulantní léčbě LMWH v posledních letech ukazují, že až 50 % pacientů s klinicky stabilní plicní embolií může být léčeno doma. I přes tato fakta dosud neměla domácí léčba plicní embolie pevnou pozici v běžné praxi. Pokud bychom měli sumarizovat podmínky pro domácí léčbu, potom by bylo možné uvažovat o ambulantní léčbě u nemocných s nízkým rizikem dle pomocných kritérií, bez hemodynamické nestability (především bez šokového stavu), bez selhávání pravé komory, bez předchozího chronického srdečního či plicního onemocnění, bez závažných komorbidit (onemocnění gastrointestinálního traktu, onemocnění ledvin, krevní choroby, pokročilá nádorová onemocnění), s nízkým rizikem časné rekurence tromboembolické nemoci, bez jiných indikací pro hospitalizaci (bolest vyžadující parenterální analgetika, infekce apod), s nízkým rizikem krvácení a s garantovanou spoluprací nemocného a dobře organizovanou domácí péčí., Venous thromboembolic disease which includes both venous thrombosis and pulmonary embolism, is a frequent and potentially fatal disease. Based on the introduction of low-molecular-weight heparins (LMWH) into practice it has been proved that outpatient treatment of venous thrombosis is effective and safe for a large number of patients with VTE. The growing volume of data on LMWH outpatient treatment in recent years shows that up to 50 % of patients with clinically stable pulmonary embolism can be treated at home. In spite of these facts home treatment of pulmonary embolism has not been established as part of common practice as yet. If we were to summarize the conditions for home treatment, we would consider outpatient care for patients at low risk based on auxiliary criteria, free from hemodynamic instability (primarily without a shock state), free from right ventricular failure, prior chronic heart or lung disease, serious comorbidities (gastrointestinal tract disease, kidney disease, blood diseases, advanced cancers), at low risk of early thromboembolism recurrence, free from other indications for hospitalization (pain requiring parenteral analgesics, infections etc.), at low risk of bleeding and with guaranteed patient‘s cooperation and well-organized home care., and Radovan Malý, Jaroslav Malý
Úvod: Výsledky poskytované chirurgické péče v širším přesahu jsou obtížně dosažitelné. Práce testovala systém účtování zdravotní pojišťovně z hlediska využitelnosti dat. Metoda: Analyzováno bylo 6879 operací tříselné kýly v období 2008–2015. Využit byl systém účtování v DRG Vojenské zdravotní pojišťovny České republiky. Výsledky: Operace provedlo 156 pracovišť. V režimu jednodenní chirurgie bylo provedeno 3,8 % (248) výkonů, z toho 77 % (193) bylo operováno laparoskopicky. Za hospitalizace bylo provedeno 96,2 % (6631) výkonů, medián délky hospitalizace byl 4 dny. U hospitalizovaných pacientů byla otevřená plastika bez síťky provedena v 36,54 % (2423), otevřená plastika se síťkou v 19,84 % (1316), laparoskopická plastika v 43,61 % (2892). Reoperace pro recidivu v souboru byla v 1,85 % (127), po otevřené plastice bez síťky v 2,76 % (66), po otevřené plastice se síťkou v 1,66 % (22), po laparoskopické plastice v 1,19 % (37). Predikce recidiv do 5 let byla 2,42 % celkem. Hodnoceny byly i další parametry. Závěr: Studie potvrdila využitelnost systému pojištění k získání relevantních dat o léčbě vybrané diagnózy., Introduction: Information about actual results of provided surgical care is difficult to obtain. We tested the insurance company billing system in terms of data usability. Methods: 6,879 groin hernia surgeries carried out from 2008 to 2015 were analysed using registry data of one health insurance company. Results: Groin hernia surgeries were performed by 156 providers in the Czech Republic. Day surgeries were used in 3.8% cases (248), of which laparoscopic surgeries were used in 77% (193) of patients. The other 96.2% (6,631) surgeries were carried out on inpatients with the median hospital stay of 4 days. Of this number: open procedures without the use of a mesh were done in 36.54% (2,423), open procedures using a mesh in 19.84% (1,316), and laparoscopic procedures in 43.61% (2,892) cases. Reoperations for recurrence after hernia repair were done in 1.85% (127) of all cases, while 2.76% (66) reoperations were done after open procedures without the use of a mesh, 1.66% (22) after open procedures using a mesh, and 1.19% (37) after laparoscopic surgeries. The prediction rate for recurrent hernia incidence within 5 years was 2.42% in all cases. Others parameters were also evaluated. Cíl práce: Analyzovat výsledky indukovaných porodů u primipar po ukončeném 41. týdnu těhotenství bez další zátěže. Typ studie: Retrospektivní klinická studie. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 1. LF UK v Praze, Nemocnice Na Bulovce. Materiál: V letech 2011–2013 porodilo na Gynekologicko-porodnické klinice v Nemocnici Na Bulovce celkem 6 317 žen. U 203 prvorodiček byl porod indukován po ukončeném 41. týdnu těhotenství prostaglandinem dinoproston. Do kontrolní skupiny bylo zařazeno 291 primipar se spontánním začátkem porodu po termínu. Metodika: Porovnali jsme u obou skupin věk rodiček, celkovou délku porodu, celkovou krevní ztrátu, rozdíly ve způsobu ukončení porodu, výskyt hypotonie děložní a manuální vybavení placenty, porodní poranění, četnost epidurální analgezie a poporodní adaptaci novorozenců (Apgarové skóre a pH z pupečníkové krve novorozenců). Údaje byly retrospektivně získány ze zdravotnické dokumentace a statisticky zpracovány. Výsledky: Ve sledované skupině jsme zjistili signifikantně vyšší četnost ukončení porodu císařským řezem (21,2 % vs 11,0 %, p < 0,002), statisticky významně vyšší krevní ztrátu (341 ml vs 314 ml, p = 0,002) a četnost podání epidurální analgezie (25,1 % vs 11,7 %, p < 0,001). Další sledované parametry byly srovnatelné u obou skupin. Předpokládaný vliv věku nad 35 let na frekvenci císařského řezu nebyl v této studii prokázán. Závěr: U primipar s indukovaným porodem jsme zjistili výrazně vyšší frekvenci císařských řezů. Délka porodů a frekvence hypotonie děložní byly srovnatelné s kontrolní skupinou. Farmakologická indukce porodu prostaglandinem E2 z důvodu potermínové gravidity představuje u primipar silný rizikový faktor k ukončení císařským řezem., Objective: To analyse effect of induced labor in primiparous women after completed 41st gestation week without any other pathology. Design: Retrospective clinical study. Setting: Department of Gynaecology and Obstetrics, First Faculty of Medicine, Charles Univesity in Prague, Hospital Na Bulovce. Material: Altogether 6 317 women delivered at Hospital Na Bulovce in Prague between years 2011–2013. In 203 primiparous women after completed 41st gestation week, labor was induced by prostaglandin dinoproston. The control group consisted of 291 primiparae with spontaneus onset of labor after the term. Methods: We compared the groups in respect to patient´s age, parity, total duration of labor, total blood loss, frequency of different ways of delivery, frequency of analgesia, frequency of diverse delivery injuries, manual removal of placenta and uterine hypotonia, neonatal outcomes (Apgar score of newborns, and the pH of umbilical arterial and vein blood). Data were retrospectively obtained from the medical documentation and statistically processed. Results: We proved significantly higher frequency of Caesarean sections (21,2 % vs 11,0 %, p < 0,002), statistically significant higher blood loss (341 ml vs 314 ml, p = 0,002), and frequency of epidural analgesia applications (25,1 % vs 11,7 %, p < 0,001) in the study group. Other analysed parameters were similar in both groups. Expected effect of age more than 35 years on frequency of Caesarean section was not proved in the study group. Conclusion: We found a substantially higher frequency of Cesarean sections in primiparous women with induced labor. The duration of delivery and the frequency of uterine hypotonia were similar as in the control group. Úvod: Cílem práce je zhodnocení našich zkušeností a výsledků s použitím hybridních rekonstrukcí v terapii multietážového postižení tepenného systému dolních končetin a vyhodnocení vlivu indikace a typu hybridní rekonstrukce na jejich průchodnost. Metoda: Retrospektivní analýza souboru pacientů s prospektivním sběrem dat. Na základě typu hybridní rekonstrukce rozdělení pacientů do 5 skupin. Skupina pacientů „1“ podstoupila transluminální angioplastiku (TA) ? implantaci stentu do pánevního řečiště v kombinaci s plastikou tepen v třísle (35 pacientů; 27,6 %). Skupina pacientů „2“ podstoupila TA ? implantaci stentu do pánevního řečiště v kombinaci s infrainguinálním bypassem (15 pacientů; 11,8 %). Skupina pacientů „3“ podstoupila plastiku tepen v třísle v kombinaci s TA výtokového traktu končetiny: AFS ? AP ? bércové tepny (52 pacientů; 40,9 %). Skupina pacientů „4“ podstoupila provedení infrainguinálního bypassu v kombinaci s TA distálně od místa distální anastomózy rekonstrukce (3 pacienti; 2,4 %). Skupina pacientů „5“ představuje heterogenní skupinu hybridních rekonstrukcí, které nespadaly do některé ze 4 výše uvedených skupin (22 pacientů; 17,3 %). Na základě indikace k výkonu rozdělení souboru do 3 skupin: skupina „ALI“ pacienti s akutní končetinovou ischemií, skupina „CLI“ pacienti s kritickou končetinovou ischemií, skupina „II“ pacienti s klaudikačními obtížemi. Analýza průchodností provedena s použitím Kaplan-Meier life tables. Porovnání průchodnosti mezi skupinami provedeno pomocí log-rank testu. Statistická analýza provedena s použitím softwaru IBM SPSS Statistics 21.0 . Výsledky: Soubor prezentuje 127 pacientů, kteří podstoupili 127 hybridních intervencí. Technický a klinický úspěch hybridní rekonstrukce byl 96,9 % a 98,4 %. 30denní perioperační mortalita byla 3,1 %. Výsledky primární (PP), primární asistované (PAP) a sekundární (SP) průchodnosti ve 2 letech byly nejlepší u pacientů s hybridním výkonem typu „1“ a „3“, tedy kombinací plastiky tepen v třísle a transluminální angioplastiky vtokového nebo výtokového traktu končetiny. Výsledky skupiny „1“ v pořadí PP, PAP a SP ve 2 letech: 80,6 %, 84,3 % a 84,3 %. Výsledky skupiny „3“ ve 2 letech ve stejném pořadí: 81,1 %, 82,8 % a 86,3 %. O poznání horších výsledků bylo dosaženo u pacientů ze skupiny „2“ a „5“. Výsledky PP, PAP a SP ve 2 letech u pacientů ze skupiny „2“ byly: 39,4 %, 59,1 % a 59,1 %. Výsledky průchodnosti pacientů z heterogenní skupiny „5“ byly ve 2 letech: 30,4 %, 49,2 % a 70,7 %. Při zohlednění vlivu indikace na průchodnost rekonstrukce ve 2 letech byly nejlepší výsledky primární asistované a sekundární průchodnosti dosaženy u pacientů ze skupiny „CLI“: 81,6 % a 86,2 %. Jenom primární průchodnost ve 2 letech byla nejlepší u pacientů s klaudikacemi (skupina „II“), a to 71,1 %. Pacienti indikováni k hybridnímu výkonu pro akutní končetinovou ischemií (ALI) dosahovali nejhorších výsledků z uvedených 3 skupin, a to PP, PAP a SP ve 2 letech: 33,7 %, 46,2 % a 45,5 %. Amputation-free survival ve 2 letech měl nejlepší hodnotu pro pacienty ze skupiny „CLI“, a to 89,1 % („II“ 81,9 %; „ALI“ 61,4 %). Overall-survival a amputation-free survival ve 2 letech za celý soubor dosahovaly hodnot 93,2 % a 82,3 %. Závěr: Hybridní revaskularizace je kombinovaný výkon, který umožňuje řešení multietážového postižení tepenného systému dolních končetin v rámci jednoho sezení. Je zatížen nízkým rizikem periprocedurální mortality a morbidity pro pacienta. Lze jím dosáhnout dobrých výsledků průchodnosti tepenného systému a záchrany končetiny bez ohledu na typ chronické ischemie (klaudikace, kritická končetinová ischemie). Výsledky u pacientů s akutní končetinovou ischemií jsou inferiorní. Jako výkon s nejlepšími výsledky nám vychází kombinace plastiky tepen v třísle spojená s transluminální angioplastikou vtokového nebo výtokového traktu končetiny., Introduction: The goal of this study was to evaluate our experience and results of single staged hybrid procedures for revascularization of lower extremities affected by multilevel arterial occlusive disease. We analysed the effect of the indication and type of reconstruction on patency. Method: Patients were retrospectively reviewed. Data collection was conducted prospectively. Patients were divided into 5 groups based on the type of hybrid reconstruction. Group “1” included patients who underwent transluminal angioplasty (TA) ? stenting of iliac arteries with endarterectomy (EA) and patch arterioplasty of the femoral bifurcation (35 patients; 27.6 %). Group “2” included patients who underwent TA ? stenting of iliac arteries with infrainguinal bypass (15 patients; 11.8 %). Group “3” consisted of patients who underwent TA ? stenting of outflow lower extremity arteries: superficial femoral artery ? popliteal artery ? crural arteries in combination with EA and patch arterioplasty of the femoral bifurcation (52 patients; 40.9 %). Group “4” represented patients who underwent infrainguinal bypass surgery in combination with TA distal to the site of open reconstruction (3 patients; 2.4%). Group “5” represented a heterogenous population of patients who underwent a hybrid reconstruction which did not belong to any of the 4 previously mentioned groups (22 patients; 17.3 %). The patients were divided into 3 groups based on the indication criteria of the intervention: Patients with acute limb ischemia (ALI) were put into group “ALI”. Patients with critical limb ischemia (CLI) were put into group “CLI”. Patients with claudications were put into group “II”. Patency analyses were performed using Kaplan-Meier life tables. Differences in patency rates between the different groups of patients were determined using the log-rank test. Statistical analysis was performed using the software IBM SPSS Statistics 21.0. Results: The study included 127 patients who underwent 127 hybrid arterial procedures. Technical and clinical success rates were 96.9% and 98.4%. 30-day perioperative mortality rate was 3.1%. The primary (PP), assisted-primary (APP) and secondary patency (SP) results at 2 years were the best amongst the patients from groups “1” and “3”. These patients underwent patch arterioplasty ? EA of the femoral bifurcation in combination with transluminal angioplasty of inflow or outflow arteries of the lower extremity. The resulting PP, APP and SP rates at 2 years in group “1” were 80.6%, 84.3% and 84.3%. Patency rates in group “3“ in the same order at 2 years were 81.1 %, 82.8% and 86.3%. Significantly lower patency rates were achieved in patients from groups “2“ and “5“. The resulting PP, APP and SP rates in group “2” at 2 years were 39.4%, 59.1% and 59.1%; in group “5“ at 2 years they were 30.4%, 49.2% and 70.7%. Taking into account the effect of the indication on patency rates, we found that patients from group “CLI” had the best APP and SP rates at 2 years: 81.6% and 86.2%. Only the PP rate at 2 years was the best in group “II” (patients with claudications): 71.1%. Patients who underwent their operations because of acute limb ischemia (group “ALI”) achieved the worst results among these 3 groups. Their PP, APP and SP rates at 2 years were 33.7%, 46.2% and 45.5%. Patients from the group „CLI“ had the best amputation-free survival at 2 years: 89.1% amongst the 3 groups subdivided according to the indication for the intervention (“II“ 81.9%; “ALI“ 61.4%). Overall survival and amputation-free survival at 2 years were 93.2% and 82.3%, respectively, for the whole studied population. Conclusion: The hybrid procedure is an intervention which combines endovascular and open reconstruction performed at the same time. Hybrid procedures provide effective therapy for patients with multilevel lower extremity arterial disease. They have low complication rates, periprocedural morbidity and mortality. Using these procedures, we are able to achieve good limb salvage rates and patency rates irrespective of the form of chronic limb ischemia (claudications, critical limb ischemia). Results for patients with acute limb ischemia are inferior. The combination of patch arterioplasty ? endarterectomy of the femoral bifurcation with transluminal angioplasty of inflow or outflow limb arteries is our hybrid procedure of choice with the most favourable results., and E. Úvod: V České republice (ČR) jsou pacienti s diagnózou diabetes mellitus (DM) sledováni a léčeni převážně u specialistů (přibližně 80 % v odborné diabetologické ambulanci), menší část praktickými lékaři (do 20 %). Doposud nebyl zhodnocen dlouhodobý vývoj změn preskripce metforminu a sulfonylurey v České republice a jeho konkordance s doporučenými postupy. Cíl: Porovnání vývoje preskripce metforminu (MET) a sulfonylurey (SU) v letech 2002–2006 s vývojem v letech 2010–2014 v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u které bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2006 a 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10 (308 962 evidovaných v roce 2002 a 426 695 evidovaných v roce 2014). Pro každý rok byl vyhodnocen počet osob, které měly předepsán nejméně jednou MET nebo SU. Počet osob léčených MET nebo SU byl pak vyjádřen v procentech ze všech, kteří měli v daném roce předepsanou jakoukoliv terapii ze skupiny A10. Výsledky: Preskripce metforminu stoupá lineárně ze 43 % na 77 %, preskripce sulfonylurey se lineárně snižuje z 65 % na 37 %. Závěr: Naše analýza je prvním zhodnocením vývoje preskripce metforminu v ČR a zhodnocením její konkordance s doporučenými postupy pro léčbu DM. Preskripce metforminu v ČR se zvýšila mezi roky 2002–2014 ze 43 % na 77 % a preskripce SU se snížila z 65 % na 37 %. Tento vývoj a aktuální poměr preskripce MET a SU je potvrzením implementace doporučených postupů do praxe a důkazem vysoké kvality péče o pacienty s DM2T v České republice. Long-term development of the changes in prescribing metformin and sulfonylurea in the Czech Republic and its concordance with recommended procedures has not been evaluated until now. Goal: Comparison of the development of metformin (MET) and sulfonylurea (SU) prescriptions in the period of 2002–2006 with that of 2010–2014 in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech Republic population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2006 and 2010–2014. A cohort of patients was extracted for analysis, who had an agent from A10 group prescribed at least once in a relevant year (n = 308 962 in 2002; n = 426 695 in 2014). A number of patients was evaluated for each year, who had at least once MET or SU prescribed. The number of patients treated with MET or SU was then expressed as a percentage of all who had any therapy from A10 group prescribed in the year in question. Results: Metformin prescriptions have linearly risen from 43% to 77%, while sulfonylurea prescriptions have linearly decreased from 65% to 37%. Conclusion: The analysis presents the first evaluation of the development of metformin prescriptions conducted in the Czech Republic and evaluation of its concordance with the recommended procedures for the treatment of DM. The amount of metformin prescribed in the Czech Republic increased from 43% to 77% while the amount of SU prescribed decreased from 65% to 37% between 2002 and 2014. This development and the current ratio between the prescribed amounts of MET and SU demonstrate the implementation of the recommended procedures into practice and prove the high quality of care for patients with DM2T in specialists – diabetologists‘ surgeries.
