Invazivní metody léčby chronické formy ischemické choroby srdeční byly po dlouhou dobu považovány za kauzální terapii, která bezpochyby snižuje morbiditu a mortalitu nemocných. Na druhé straně životní styl a farmakologická terapie byla nahlížena jako spíše doplňující, ale nikoliv rozhodující léčba. V posledních letech je možno vidět změnu v pohledu na ischemickou chorobu srdeční. Je to mnohem difuznější proces v koronráním řečišti a není to tedy jen otázka kritických stenóz, ale stejně nemocného ohrožují i další úseky věnčitých tepen, kde se hovoří o vulnerabilních placích. Překvapením pro experty byly závěry řady studií potvrzujících neschopnost invazivch metod prodloužit život nemocným s chronickou formou ICHS. Na druhé straně přibývají důkazy o významném vlivu úpravy životního stylu a rozrůstajících se možnostech farmakolgoické terapie s pozitivním vlivem na morbiditu a mortalitu. Článek prezentuje současné názory na tyto otázky, které jsou nyní živě diskutovány., Invasive methods for treatment of coronary artery disease in chronic form were, for a long time, believed to be causal treatment that definitely diminish patient morbidity and mortality. By contrast, lifestyle and pharmacological therapy were seen as valuable but not lifesaving. In the last few years a change can be seen in the view on coronary artery disease. It is a much more diffusive and complicated process in the coronary tree. It is not merely a question of the critical stenotic areas; the new idea of vulnerable plaque, which presents the same danger of complication as critically stenotic areas, is also important. A great surprise for experts was the result of various studies confirming the inability of invasive procedures to prolong life in patients with chronic coronary disease. On the other hand, there has been more evidence that lifestyle changes and expanding pharmacological possibilities can have a significant positive effect on morbidity and mortality. Vzniku trombózy v arteriálním nebo venózním cévním systému předchází složitá souhra faktorů vnějšího prostředí a faktorů genetických. Rozsáhlé genomové asociační studie se ukazují jako velice přínosné v identifikaci lokusů spojených s kardiovaskulárními onemocněními a souvisejícími rizikovými faktory. Ještě zbývá mnoho práce v identifikaci určujících genů a jejich kauzálních variant, stejně jako v definici mechanizmů, které ovlivňují vývoj a progresi onemocnění. Z výsledků studií lokusů spojených s dříve identifikovanými onemocněními se očekává zlepšení našich znalostí o patofyziologii kardiovaskulárních onemocnění a možnost identifikovat nové cíle pro léčbu. V oblasti aterotrombózy a trombofilie jsou významné výsledky z asociačních studií zaměřených na oblast ischemické choroby srdeční, ischemické cévní mozkové příhody a žilního tromboembolizmu. INTRODUCTION: The issue of resistance to antiplatelet therapy has raised many questions in the area of neurovascular diseases. The first objective of this work was to determine the prevalence of aspirin resistance in neurovascular patients with clinical non-responsiveness to aspirin treatment and a high-risk of atherothrombotic complications using two interpretable and independent methods (aggregation and PFA 100). The second objective was to find the correlation between both assays and to evaluate the results in groups at risk for various cerebrovascular diseases. MATERIAL AND METHODS: Laboratory tests of aspirin resistance were performed in 79 patients with clinical non-responsiveness to aspirin treatment suffering from neurovascular diseases. Patients were divided into the two groups: expected low risk for aspirin resistance due to the first manifestation of a neurovascular disease (n = 34) and expected high risk due to the second clinical manifestation of a neurovascular disease (n = 45). RESULTS: The prevalence of aspirin resistance in both groups combined as determined by the PFA-100 and CPG techniques were 50.6% and 17.7%, respectively. No correlation was found between the two techniques. CONCLUSIONS: No significant prevalence of aspirin resistance was demonstrated by either method despite the heterogeneous pathophysiological mechanisms. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). We included standardized testing material, translation of administration and scoring, and assessment of normative data for the MIST in the Czech population. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V. Stenóza cervikálního kanálu může vést ke kompresi krční míchy a představuje nejvýznamnější mechanický faktor v patofyziologii cervikální spondylogenní myelopatie. Míšní tkáň je však vůči kompresi poměrně odolná a k rozvoji manifestní myelopatie dochází až při vyšším stupni komprese a v kombinaci s dalšími patofyziologickými faktory, zejm. dynamickým faktorem a zevním traumatem. Asymptomatická spondylogenní komprese krční míchy (ASCCC) je velmi častým nálezem zvláště v populaci vyššího věku nad 50 let. Objektivizace a kvantifikace cervikální stenózy a komprese krční míchy jsou však nejednotné a nejsou jasně stanoveny prediktory progrese do manifestní myelopatie a s tím související indikace případné preventivní chirurgické dekomprese. Přehled obsahuje nejčastěji používané metody stanovení cervikální stenózy a cervikální míšní komprese pomocí zobrazovacích metod. Nativní radiogram a výpočetní tomografie mají význam v objektivizaci cervikální stenózy, zatímco dominantní metodou objektivizace míšní komprese je magnetická rezonance (MR). Z parametrů, které nejvíce korelují s klinickou manifestací komprese, jsou uváděny příčná plocha míchy v místě komprese a přítomnost intenzitních změn míšního signálu. Z novějších zobrazovacích modalit je nejcitlivější MR zobrazení tenzorů difúze. Z prognostických faktorů rozvoje manifestní myelopatie u ASCCC jsou obecně akceptovány přítomnost manifestní radikulopatie a abnormita motorických a senzomotorických evokovaných potenciálů. Význam zobrazovacích metod v predikci rozvoje manifestní myelopatie a benefit provedení preventivní dekomprese ve skupině nemocných s asymptomatickou kompresí a vyšším rizikem rozvoje manifestní myelopatie bude nutné ověřit dalšími studiemi., Cervical spinal stenosis may lead to cervical cord compression and represents the most important mechanical factor in pathophysiology of cervical spondylotic myelopathy. Medullar tissue is, however, rather resistant to compression and development of symptomatic myelopathy occurs only when higher degree stenosis is present and in combination with other pathophysiological factors, mainly dynamic compression and trauma. Asymptomatic spondylotic cervical cord compression (ASCCC) is a quite frequent finding in older population above the age of fifty. However, reliability of the methods used to verify and quantify cervical stenosis and cervical cord compression is low and clear predictors of development of symptomatic myelopathy and related indication of potential preventive surgical decompression in ASCCC have not been determined yet. The overview discusses the most frequently used methods to establish cervical spinal stenosis and cervical cord compression using imaging methods. Radiogram and especially computed tomography are important for verification of cervical spinal stenosis, while magnetic resonance imaging (MRI) is a preferable method to detect cervical cord compression. The cross‑sectional spinal cord area and T2 MRI spinal cord hyperintensity are among the parameters considered to be the most closely correlated with clinical manifestation of spinal cord compression. Among newly introduced imaging modalities, MRI diffusion tensor imaging seems to be the most promising one. The presence of symptomatic radiculopathy and abnormality of motor and somatosensory evoked potentials are among generally accepted predictors of symptomatic myelopathy. The importance of imaging methods as predictors of symptomatic myelopathy development as well as benefits of preventive surgical decompression in ASCCC individuals with high risk of developing symptomatic myelopathy is to be established in future studies. Kyselina močová je u človeka biologicky aktívny koncový produkt metabolizmu purínov. Zvýšené koncentrácie kyseliny močovej sprevádzajú metabolický syndróm a jeho komponenty už u detí a mladistvých. K hyperurikémii prispieva konzumácia nápojov prisládzaných fruktózou, nakoľko je jediným sacharidom, pri ktorého metabolizme vzniká kyselina močová. Súčasné vedomosti neumožňujú odlíšiť, či je hyperurikémia kauzálnou príčinou komponentov metabolického syndrómu, alebo len sprievodným znakom naznačujúcim zvýšené riziko vzniku kardiovaskulárnych ochorení. V súčasnosti neexistuje konsenzus či a ako liečiť asymptomatickú hyperurikémiu u detí a mladistvých. Zaujať zásadný postoj k týmto otázkam umožnia len výsledky randomizovaných kontrolovaných prospektívnych štúdií, ktoré poskytnú dôkazy o prospešných účinkoch tejto liečby na prevalenciu metabolického syndrómu a incidenciu kardiovaskulárnych chorôb., In humans, uric acid represents a biologically active end-product of purine nucleotides. Several studies in children and adolescents documented an association between hyperuricaemia and the components metabolic syndrome. High intake of fructose-sweetened beverages might increase uricaemia, since fructose is the only saccharide which metabolism results in the formation of uric acid. Current knowledge does not allow distinguishing whether hyperuricaemia is causally related to the components of metabolic syndrome, or rather represents a marker of an enhanced risk, and poor outcome. No guidelines exist whether or not to treat asymptomatic hyperuricaemia in the adolescents. Cíl práce: Prezentujeme kazuistiku pacienta, u kterého došlo k rozvoji oboustranného seminomu na podkladě intratubulární germinální neoplazie – neklasifikovatelný typ (IGCNU – intratubular germ cell neoplasia unclassified type). Kazuistika: Pacient s anamnézou seminomu levého varlete klinického stadia I a IGCNU po radikální orchiektomii a adjuvantní radioterapii byl odeslán k urologickému vyšetření pro suspektní tumor solitárního pravého varlete dle sonografického vyšetření. Indikovali jsme 3biopsii varlete, která prokázala přítomnost IGCNU. Následovala radikální orchiektomie, kde byl histologicky verifikován klasický seminom. Závěr: Intratubulární germinální neoplazie představuje vysoké riziko rozvoje invazivního germinálního tumoru varlete. Léčba zahrnuje radioterapii či chemoterapii cisplatinou. Alternativou je pečlivá dispenzarizace a pravidelné sonografické kontroly, zejména u pacientů se solitárním varletem, kteří chtějí zachovat fertilitu a endokrinní funkci. Biopsie kontralaterálního varlete u pacientů s germinálním tumorem k vyloučení IGCNU by měla být provedena v případě kryptorchizmu, poruchy spermiogeneze a atrofií varlete s objemem pod 12 ml, vhodná je rovněž u pacientů s mikrolitiázou varlete a předchozím výskytem IGCNU., Objective: We are presenting a case report of young man who developed an asynchronous bilateral seminoma based on an intratubular germ cell neoplasia of unclassified type (IGCNU). Case report: A patient, with history of a stage I seminoma of the left testis, based on an IGCNU of the left testis, after a radical orchidectomy and adjuvant radiotherapy, was referred to the urology clinic with a suspected tumor of the right testis. A testicular biopsy was made and the histological examination showed an IGCNU. A radical orchidectomy was performed and a seminoma was found. Conclusion: IGCNU is a precursor lesion of germ cell testicular tumors. Treatment of the IGCNU includes radiotherapy or chemotherapy with cisplatin. Surveillance remains an option for those with a solitary testicle who wish to preserve fertility and endocrine function. A contralateral testicular biopsy, in patients with a history of germ cell tumors, should be made in cases of cryptorchidism, poor spermatogenesis, and testicular atrophy with a testicular volume of less than 12 ml.
