Náplní sdělení je aktuální informace o přínosu nové, vysoce senzitivní metody analýzy imunoglobulinů séra (Hevylite™), umožňující separátní kvantitativní vyšetřování párů těžkých/lehkých řetězců imunoglobulinu (HLC), tj. monoklonálního (involved) a polyklonálního (noninvolved) izotypu včetně jejich poměru (HLC-r) u monoklonálních gamapatií. Hlubší pozornost je věnována východiskům a charakteristice této analytické metody, srovnání jejího klinického přínosu se standardními technikami používanými v diagnostice, sledování průběhu, časné detekce relapsu a progrese nemoci, stratifikace, hodnocení léčebné odezvy a prognózy monoklonální gamapatie nejistého významu, mnohočetného myelomu, Waldenströmovy makroglobulinemie, systémové AL-amyloidózy a některých nehodgkinských lymfomů. Z dosavadních výsledků vyplývá, že ve srovnání s dosavadními standardně používanými metodami obohacuje metoda Hevylite™ klinickou praxi o vyšetření hladiny noninvolved Ig s možností vyhodnocení hloubky imunoparézy, stanovení indexu HLC-r uplatňujícího se při stratifikaci MM do rizikových skupin a přispívajícího k odhadu prognózy a zlepšení hodnocení hloubky léčebné odezvy, zatímco v případě MGUS poskytuje informaci o riziku maligní evoluce. Ke zpřesnění indikací metody Hevylite™ u MG v běžné klinické praxi zřejmě přispěje zveřejnění výsledků aktuálně probíhajících validačních studií., The aim of the paper is to inform about the contribution of novel, highly sensitive analytic technique for the assessment of serum immunoglobulins (Hevylite™), enabling separate quantitative assessment of heavy/light chain pairs of immunoglobulin (HLC), i. e. the monoclonal („involved“) and polyclonal („noninvolved“) isotype including their ratio (HLC-r) in monoclonal gammopathies. We particularly target the characteristics of this technique, the comparison of its clinical contribution with standard methods used in the diagnostics, course and the detection of relapse and progression of the disease, as well as the stratification, assessment of therapeutic outcome and prognosis in monoclonal gammopathy of undetermined significance, multiple myeloma, Waldenström´s macroglobulinemia, systemic AL-amyloidosis and some non-Hodgkin lymphomas. Present results show that in comparison with existing routinely used techniques the Hevylite™ method enriches clinical practice with the assessment of serum levels of „uninvolved“ Ig. It enables the evaluation of the depth of „immunoparesis“, and the determination of HLC-r index that is needful for the stratification of MM into „risk cohorts“. It also contributes to prognostic assessment and improvement of the evaluation of the depth of therapeutic response. In MGUS individuals the HLC-r index provides information about the risk of malignant transformation. Cílem pilotní studie bylo vyhodnotit kvalitu života (Quality of Life – QoL) u pacientek s karcinomem prsu ve vztahu k léčebným modalitám a jejich kombinacím. Ve studii QoL participovalo 71 pacientek s karcinomem prsu, QoL byla hodnocena pomocí Amerického dotazníku zdraví (SF36), Dotazníku životní spokojenosti a české výzkumné verze Functional Assessment of Breast Cancer Therapy (FACT-B). Průměrná doba od diagnózy do doby hodnocení QoL ve výzkumném souboru byla 4,5 roku. Statistická významnost rozdílů v kvalitě života mezi pacientkami s různou léčbou byla testována Mannovým-Whitneyovým testem pro jednotlivé typy terapie a Kruskalovým-Wallisovým testem pro kombinace terapie. Vztah mezi kvalitou života a léčebnou modalitou byl zjištěn pro dotazník SF36 – pro dimenzi emocionální funkční role a operační léčbu (vyšší kvalita života u operovaných, p = 0,049), pro dotazník FACT-B – fyzický stav a chemoterapii (vyšší kvalita života u pacientek bez chemoterapie, p = 0,012), pro FACT-B – sociální/rodinný stav a operační léčbu (vyšší kvalita života u operovaných, p = 0,036), pro FACT-G skóre a operační léčbu (vyšší kvalita života u operovaných, p = 0,049) a dále pro Dotazník životní spokojenosti – pro dimenzi zdraví a kombinaci léčby (nejvyšší kvalita života u kombinace operace, chemoterapie, radioterapie, biologické a hormonální terapie, nejnižší u kombinace předešlých bez biologické terapie, p = 0,027). Pro úplnost uvádíme typ a četnost vedlejších efektů léčebných modalit. Zjistili jsme vyšší QoL u operovaných pacientek a nižší QoL u pacientek vystavených chemoterapii, avšak naše závěry vyžadují další výzkumná ověření. A total of 71 patients with breast carcinoma participated in this study. QoL was assessed using the 36-item Short-Form Health Survey (SF-36), Life Satisfaction Questionnaire and Czech research version of Functional Assessment of Breast Cancer Therapy (FACT-B). The average time from diagnosis at the time of QoL assessment was 4.5 years. Mann-Whitney U test was used to test the differences in QoL between groups of patients with different type of therapy and Kruskal-Wallis test for combinations of therapies. Generally we found higher QoL in operated patients and lower in patients who underwent chemotherapy, however our findings require further research verifications. Cílem studie bylo prozkoumat vztah mezi sociální oporou, socioekonomickými determinantami a průběhem onkologické léčby. Byl sledován výskyt komplikací souvisejících s toxicitou protinádorové terapie, výskyt viróz během 6 měsíců po operaci a pooperační komplikace. Ve studii participovalo 139 pacientek s karcinomem prsu (medián věku 65 let), sociální opora (SO) a socioekonomický status (SES) byly hodnoceny na základě strukturovaného interview. SES byl odhadován na základě kategorií ekonomického příjmu a dosaženého vzdělání. Ze zdravotnické dokumentace byly do analýz zařazeny údaje o výskytu obecně léčebných komplikací, komplikací po operaci a o výskytu infekčních onemocnění typu viróz do 6 měsíců po operaci. Statistická významnost rozdílů v kategoriálních proměnných mezi testovanými skupinami byla hodnocena Fisherovým přesným testem. Statisticky významné rozdíly mezi skupinami s a bez SO byly zjištěny u celého souboru pacientek v méně častém výskytu viróz u pacientek se SO (bez viróz 70,3 % vs 48,3 % bez SO). U pacientek se SO ve věku do 60 let byl zjištěn nižší výskyt pooperačních komplikací (56,7 % vs 0 %) i obecně nižší počet komplikací v souvislosti s onkologickou léčbou (více než 4 komplikace se projevily u 47,1 % se SO vs 100 % bez SO). U pacientek se SO ve stadiu nemoci II–III byl zjištěn nižší výskyt viróz (bez viróz 63,2 % vs 28,6 %) a nižší četnost komplikací (více než 4 komplikace 35,5 % vs 73,3 %). Statisticky významný vztah (P = 0,015) byl zjištěn mezi vzděláním a počtem komplikací u pacientek se stadiem II–III, u pacientek se základním a učňovským vzděláním byl vyšší podíl více než 4 komplikací (55,9 %) oproti skupině se středním a vyšším vzděláním (32,6 %). Zvýšený trend byl nalezen pro vztah mezi SES a výskytem komplikací souvisejících s onkologickou léčbou (P = 0,06) ve skupině pacientek s karcinomem prsu ve stadiu II–III.Zjistili jsme indicie pro vztah sociální opory a průběh onkologické léčby pro starší pacientky napříč klinickými stadii a pro vztah SES a výskyt léčebných komplikací pro starší ženy ve stadiu II–III. Sociální opora je velmi důležitý faktor pro zdraví pacientek, významnou pozitivní roli hraje také vyšší socioekonomický status a vzdělání. Klíčová slova: karcinom prsu – onkologická léčba – sociální opora – socioekonomický status, The objective of the study was to investigate the relationship between social support, socioeconomic determinants and oncological treatment progress in patients with complications due to the toxicity of antitumor therapy, viral diseases during 6 months following surgery, as well as postoperative complications. The study sample consisted of 139 female patients with breast cancer (age median: 65 years), and social support (SS) and socioeconomic status (SES) were evaluated based on a structured interview. SES was estimated based on income and education classification. The analyses performed included information from patients’ documentation, including data on the occurrence of general therapy complications, postoperative complications, and the occurrence of infectious viral diseases within the first 6 months following surgery. The level of statistical significance of the differences within category-specific variables between the analysed groups was evaluated with Fisher’s exact test. The differences in statistical significance between groups with and without SS were found within the entire group of patients, while viral diseases occurred less in patients with SS (no viral diseases 70.3% vs 48.3% without SS). Patients with SS and below 60 years of age experienced a lower occurrence of postoperative complications (56.7% vs 0%), as well as generally lower numbers of complications resulting from oncological treatment (more than 4 complications occurred in 47.1% of patients with SS vs 100% of patients without SS). Patients with SS in the Stage II–III experienced a lower occurrence of viral diseases (63.2% no virosis vs 28.6%) and a lower occurrence of complications (35.5% more than four complications vs 73.3%). A statistically significant relationship (P = 0.015) was determined between the education of patients and the number of complications in patients in Stage II–III: more than four complications occurred in patients with basic and vocational education (55.9%), compared to patients with secondary and higher education (32.6%). An increasing trend was determined for the relationship between SES and the occurrence of complications due to oncological treatment (P = 0.06) within the group of patients with Stage II–III breast cancer. We have found evidence supporting the idea of a relationship between social support and oncological treatment for older patients across clinical stages and for the relationship between SES and the occurrence of complications for older women in Stage II–III. Social support is a factor which is of great importance for patient health, while higher socioeconomic status and education play a significant role, too. This cross sectional study focused on how Postural Stability (PS) indicators: body sway deviation (BSD) and body sway velocity (BSV), change with age and their association with levels of social and physical activity. Observational study: 80 older adults (aged: 60-96) were purposefully recruited from two sources: the University of the Third Age (TAU) (n = 35) and a residential care home (CH) (n = 45). Differences in the indicators of PS, approximated through Centre of Pressure (COP) measurements, were assessed by the Romberg Stance Test (Test A) subsequently repeated on 10 cm foam surface (Test B), using a Kistler Dynamometric Platform. The RCH Group was older, had higher BMI and was less socially and physically active, showed more body sway in all indicators compared to TAU group. For all participants body sway velocity (BSV) was significantly correlated with age. The strength of correlation of body sway deviation (BSD) with age was also significant but not as strong. Úvod: Kompletní resekce je obecně uznávaný standard léčby GII ependymomů IV. komory u dospělých, zatímco role adjuvantní radioterapie zůstává nejistá. Účelem předkládané studie je zhodnotit lokální kontrolu onemocnění a klinický stav pacientů s tímto onemocněním po chirurgické terapii. Materiál a metody: Retrospektivně jsme zhodnotili 15 pacientů (sedm mužů a osm žen) s histologicky verifikovaným GII ependymomem IV. komory v časovém období leden 2001–prosinec 2011. Výsledky: Kompletní resekce tumoru byla dosažena u 12 pacientů, u žádného z nich nebyla indikována adjuvantní radioterapie. Medián dalšího sledování byl 51 měsíců (min.–max.; 11–123 měsíců). Ke grafické recidivě došlo u jednoho pacienta, ten byl reoperován s časovým odstupem 51 měsíců od prvního zákroku. Nový neurologický deficit po operaci byl zaznamenán ve dvou případech. U dvou ze tří pacientů po subtotální resekci došlo ke grafické progresi rezidua po 11 a 16 měsících od první operace, jeden z nich byl reoperován s odstupem 27 měsíců. Adjuvantní radioterapie byla indikována u obou pacientů po reoperaci. Závěr: Dle našich dat je kompletní resekce efektivní a bezpečná léčba GII ependymomů IV. komory u dospělých. Následná radioterapie by měla být rezervována pro případy subtotální resekce s následnou progresí rezidua tumoru a reoperace., Introduction: Surgical resection is a generally accepted standard treatment modality in GII posterior fossa ependymomas in adults, while the role of adjuvant radiotherapy remains unclear. The purpose of this study was to evaluate local control of tumor growth and clinical outcome of patients after surgical treatment of GII fourth ventricle ependymomas. Material and methods: We conducted a retrospective study of 15 adult patients (seven males and eight females) with histologically confirmed GII fourth ventricular ependymoma between January 2001 and December 2011. Results: Gross total resection was achieved in 12 patients, none of them received adjuvant treatment after the initial surgery. Median follow-up after GTR was 51 months (min. – max.; 11–123 months). One patient showed radiographic signs of tumor recurrence and was re-operated 51 months after the initial surgery. Remaining patients were without any signs of tumor recurrence. After the surgery, a new neurological deficit occurred in two patients. Two of the three patients after subtotal tumor resection showed residual mass progression after 11 and 16 months postoperatively, one of them was re-operated 27 months after the initial surgery. Adjuvant radiotherapy was indicated in both re-operated patients. Conclusion: According to our data, gross total resection is effective and safe treatment of GII fourth ventricle ependymomas. Adjuvant radiotherapy should be reserved for the cases where complete removal of tumor mass is not possible and residual tumor is growing, or after repeated resection. Cíl: Cílem práce bylo provést komplexní zhodnocení faktorů ovlivňujících úspěšnost zachování sluchu při mikrochirurgické léčbě vestibulárního schwannomu (VS). Soubor a metodika: Prospektivně jsme hodnotili soubor 81 pacientů, kteří v období 2008–2010 podstoupili radikální exstirpaci VS retrosigmoidním‑transmeatálním přístupem a v době zákroku měli zachovaný sluch. Hodnotili jsme faktory stran pacienta, nádoru, audiometrické parametry a technické aspekty operace. Parametry byly statisticky vyhodnoceny včetně regresivní analýzy nezávislých proměnných. Výsledky: Sluch se podařilo zachovat u 17 pacientů (tzv. užitečný sluch: devět; tzv. neužitečný sluch: osm). U dvou pacientů jsme zaznamenali zlepšení z neužitečné úrovně na sluch užitečný. Velikost VS byla ve skupině se zachovaným sluchem menší (p < 0,001), převažovaly tak nádory nižších stadií (p = 0,001). Hloubka zasahování do vnitřního zvukovodu nevykazovala vztah k úspěšnosti zachování sluchu. K významným faktorům ovlivňujícím výsledek patřila předoperační úroveň sluchu (p < 0,001) včetně typu odpovědi v BERA (p = 0,03). Užití intraoperační BERA bylo pro výsledek klíčové (p < 0,001). Úspěšnost zachování BERA však s výsledkem nekorelovala. Ke klíčovým faktorům ovlivňujícím uchování sluchu patřila úspěšná identifikace kochleárního nervu (p < 0,001), užití endoskopické techniky (p < 0,001), měkká konzistence tumoru (p = 0,05) a absence adhezí k neurovaskulárním strukturám (p < 0,001). Krvácivost ani přítomnost cystické komponenty úspěšnost uchování sluchu neovlivnily. Závěry: Mimo zkušenost chirurgického týmu patří k hlavním faktorům ovlivňujícím uchování sluchu velikost nádoru, úroveň sluchu před operací a možnost užití intraoperačního monitoringu sluchu. Klíčová je identifikace kochleární porce n. VIII. Z tohoto hlediska se jeví přínosné užití endoskopické techniky. K významným intraoperačním faktorům pak náleží měkká konzistence tumoru a absence adhezí k neurovaskulárním strukturám., Aim: The aim of the study was to conduct a comprehensive analysis of the factors influencing the success rate of hearing preservation in patients undergoing microsurgical removal of vestibular schwannoma (VS). Materials and methods: A total of 81 consecutive patients with serviceable hearing underwent surgical removal of unilateral vestibular schwannoma via retrosigmoid-transmeatal approach between 2008 and 2010. Patient, tumor and audiometric parameters together with intraoperative findings were prospectively analyzed. Data were statistically processed including a regression analysis of independent variables. Results: Serviceable hearing was preserved in 17 patients (useful hearing in nine and non-useful in eight cases). We observed improvement of hearing from non-useful to useful level in two patients. Average size of VS was smaller in the preserved hearing group (p < 0.001) where lower stage tumors prevailed (p = 0.001). Extent of internal auditory canal filling did not correlate with the result. Preoperative hearing level (p < 0.001) and the type of brainstem auditory evoked response (p = 0.03) arose as significant prognostic factors. Employment of intraoperative BERA was crucial for the outcome (p < 0.001) but success rate of its preservation was not correlated with the result. Identification of the cochlear nerve (p < 0.001), employment of endoscopic technique (p < 0.001), soft consistency of the tumor (p = 0.05) and the lack of adhesions to the neurovascular structures (p < 0.001) proved to be crucial factors affecting the preservation of hearing. Neither hypervascularity nor the presence of cystic component affected outcome. Conclusions: Apart from the experience of the surgical team, size of the tumor, preoperative hearing level and possibility of intraoperative hearing monitoring represented the main factors for hearing preservation. Early identification of the cochlear portion of the CN VIII is key. For this, the use of endoscopic technique seems to be beneficial. Soft consistency of tumor and lack of adhesions to the neurovascular structures represent crucial intraoperative factors. Životní prognóza pacientů s diabetes mellitus 1. typu (DM1T) se dramaticky změnila po objevu inzulinu v roce 1922, ale stále byla o 25 let kratší ve srovnání s nediabetickou populací. Někteří lidé s DM1T však žili stejně dlouho jako nediabetická populace a neměli pozdní komplikace diabetu. Začali být oceňováni medailemi za dlouhý život s diabetem. Stali se však i předmětem výzkumu zabývajícího se otázkou, proč žijí tak dlouho, čím se odlišují od těch pacientů s DM1T, kteří žili krátce. Práce se zabývá odlišnostmi medailistů a diskutuje různé hypotézy, které by mohly tyto odlišnosti vysvětlit. Zdá se, že velice důležitá je kvalitní kontrola diabetu v prvních 20 letech od stanovení diagnózy, protože existuje tzv. glykemická paměť, která může v dalších letech životní prognózu významně ovlivnit. Život lidí se lineárně prodlužuje od počátku 19. století, prodlužuje se však i život osob s DM1T. Je to způsobeno jednak kvalitnější kontrolou glykemie, jednak kvalitnější prevencí a léčbou komplikací. Ukazuje se, že klesá výskyt terminálních stadií diabetické nefropatie, zlepšuje se primární i sekundární prevence kardiovaskulárních komplikací i kardiologická a kardiochirurgická léčba. Jako nejvýznamnější prognostické faktory zvýšené mortality se jeví manifestní proteinurie, diabetická neuropatie a hypertenze. Pokud nejsou přítomny tyto ukazatele, neliší se životní prognóza pacientů s DM1T významně od životní prognózy nediabetické populace. Klíčová slova: diabetická nefropatie – diabetes mellitus 1. typu – glykemie – glykemická paměť – ICHS – mortalita – pozdní komplikace diabetu – prognóza, The life expectancy of Type 1 diabetes mellitus (T1DM) dramatically improved after the discovery of insulin in 1922, but was still 25 years shorter than that of non-diabetic population. Some people with T1DM, however, lived to the same age as a non-diabetic population and had no late complications of diabetes. They began to be awarded medals in appreciation of their long life with diabetes. They also became the subject of a research examining why they lived so long and what was the difference between them and those patients with T1DM, whose lives were much shorter. The paper deals with the differences observed in the ´medallists´ and discusses various hypotheses that might account for them. It seems that reliable control of diabetes within the first 20 years following the diagnosis is very important, in relation to the existence of “glycemic memory“ which may significantly affect life expectancy in the following years. Human lifespan in general has been linearly extended since the early 19th century and the same holds for lifespans of people with T1DM. This is due to the higher quality control of glycemia on the one hand, and a better prevention and treatment of complications. It is observed that the incidence of terminal stages of diabetic nephropathy has been dropping, the primary as well as secondary prevention of cardiovascular complications, cardiological treatment and heart surgery have been improving. Manifest proteinuria, diabetic neuropathy and hypertension appear to be major prognostic factors of increased mortality. If these indicators are not present, the life expectancy of patients with T1DM does not significantly differ from that of the non-diabetic population. Cíl: Analyzovat efektivitu postupů vedoucích ke zkrácení Door-to-Needle Time (DNT) u pacientů léčených intravenózní trombolýzou (IVT). Důvodem je skutečnost, že DNT je jediný parametr, který dokáže zlepšit efektivitu IVT a který je současně modifikovatelný. Soubor a metodika: Data pochází z Iktového centra (IC) Oblastní nemocnice Kladno z období 07/2011 do 06/2013. Opatření směřující ke zkrácení DNT byla zaváděna od 05/2012 a zahrnovala: propracování spolupráce se složkami záchranného systému, zlepšení koordinace péče v rámci nemocnice, u vybraných pacientů podání IVT bezprostředně po provedení výpočetní tomografie (CT). DNT cílové skupiny pacientů léčených po zavedení nových standardů byla srovnávána s DNT historické kontrolní skupiny pacientů léčených trombolýzou od 7/2011 do 4/2012. Regresní metody byly použity pro adjustaci na vstupní charakteristiky pacientů. Výsledky: V cílové skupině došlo ke klinicky zásadnímu zkrácení DNT z 65 min na 45 min (p = 0,001), k mírnému zkrácení mediánu Onset to Treatment Time (OTT) ze 144 min na 130 min (p = 0,66) a také k mírnému poklesu výskytu symptomatických intrakraniálních krvácení (SICH) oproti skupině kontrolní – z 4,40 % na 3,70 % (OR 0,90; 95% CI: 0,11–7,03). U pacientů léčených bez znalosti kompletních výsledků laboratoře (n = 17) jsme nezaznamenali žádné SICH. Pacienti léčení v pracovní době měli 6,8krát vyšší šanci na zahájení léčby do 60 min oproti pacientům léčeným v době ústavní pohotovostní služby (ÚPS) (OR 6,78; 95% CI: 2,34–19,64). Závěry: Zavedení opatření ke zkrácení DNT bylo vysoce účinné a bezpečné. Podání IVT bez kompletních výsledků laboratoře u vybraných pacientů bylo bezpečné a významně se podílí na zkrácení mediánu DNT celého souboru., Aim: To analyze the effectiveness of interventions intended to reduce door-to-needle time (DNT) for patients treated with intravenous thrombolysis (IVT), considering that DNT is the only parameter we can influence to improve the efficacy of IVT. Material and methods: The data were collected from the Stroke Centre of the Kladno Regional Hospital over the period 07/2011 to 06/2013. Interventions intended to reduce the DNT have been implemented in 05/2012 and include: well-developed cooperation with the emergency medical service, improved coordination of care within the hospital, administration of IVT immediately after CT in selected patients. DNTs in the target group (patients treated after implementation of the interventions) were compared to the DNTs in a historical control group (patients treated with thrombolysis from 7/2011 to 4/2012). Multiple logistic regression analysis was used for adjusting the baseline characteristics of the patients. Results: The target group showed clinically crucial shortening of DNT from 65 min to 45 min (p = 0.001), a slight reduction of the median onset-to-treatment time (OTT) from 144 min to 130 min (p = 0.66) and a slight decrease in symptomatic intracranial hemorrhage (SICH) compared to the control group – from 4.40% to 3.70% (OR 0.90, 95% CI: 0.11–7.03). We did not detect any SICH in the group of patients who were treated even though the complete laboratory results were not known (n = 17). Patients treated during working hours had a 6.8–times higher chance of treatment initiation within 60 min compared to patients treated during “on call” hours (OR 6.78, 95% CI: 2.34–19.64). Conclusions: The interventions implemented to reduce DNT were highly effective and safe. IVT administration without complete laboratory results in selected patients was safe and significantly contributed to reducing the median DNT of the entire group. Úvod: I přes medicínský pokrok studie z řady zemí ukazují na přetrvávající zvýšené riziko perinatální morbidity i mortality potomků diabetických matek. V České republice nebyla publikována data o osudu gravidit žen s diabetes mellitus 1. a 2. typu (pregestační DM). Cílem práce bylo zhodnotit osud gravidit žen s pregestačním DM a jejich potomků a zjistit, zda se zlepšil s odstupem 10 let. Metodika: Retrospektivní zhodnocení osudu gravidit žen s pregestačním DM sledovaných ve FN Plzeň v období let 2000–2009 (skupina A, n = 107) a porovnání s obdobím let 1990–1997 (skupina B, n = 39). Ze statistických testů byl použitý Wilcoxonův test nepárový, kontingenční tabulky, logistická regrese – metoda step-wise a lineární mnohočetná regrese metoda step-wise. Výsledky: Data jsou uvedena jako medián (1., 3. kvartil). Ženy ve skupině A jsou starší 28 (25, 31) vs 25 (22, 27) let, p = 0,01. Jinak se skupiny statisticky významně neliší dobou trvání diabetu, BMI, zastoupením žen s DM 2. typu. V A skupině byla dosažena lepší kompenzace DM (dle HbA1c mmol/mol) ve všech trimestrech – 1. trimestr: 59 (47, 67) vs 66 (56, 76), 2. trimestr: 46 (40, 52) vs 54 (48, 59) a 3. trimestr: 46 (40, 51) vs 53 (47, 60), p = 0,01. Klesl počet porodů sekcí 65,2 % vs 87,5 %, p < 0,05, po adjustaci na věk a dobu trvání diabetu i výskyt respiratory distress syndromu 8,9 % vs 18,2 %, p < 0,05. Klesající trend byl zaznamenán i ve výskytu porodů před 34. gestačním týdnem (1,1 % vs 6,3 %) a neonatální mortalitě (1,1 % vs 2,9 %), ale rozdíly nejsou statisticky významné. Závěr: Dosažená lepší kompenzace diabetu vedla pouze k částečnému zlepšení průběhu gravidity a osudu potomků diabetických matek., Introduction: In spite of progress in medicine, studies from a number of countries indicate steadily increased risk of perinatal morbidity and mortality in the offspring of diabetic mothers. No data regarding the pregnancy outcome in women with diabetes mellitus type 1 and 2 (pregestational DM) have been published in the Czech Republic. The aim of the study was to evaluate the pregnancy course of women with pregestational DM and outcome of their offspring and to assess whether it has improved in ten years. Methods: A retrospective evaluation of pregnancy outcome of pregestational DM women followed up in the University Hospital Pilsen in years 2000–2009 (Group A, n = 107) and comparison with the period 1990–1997 (Group B, n = 39) were performed. Wilcoxon non-paired test, contingency tables, step-wise logistic regression and step-wise linear multiple regression methods were used for statistical analyses. Results: Data is presented as median (interquartile range). Women from the Group A were older 28 (25, 31) vs 25 (22, 27) years, p = 0.01. Otherwise, the groups did not statistically significantly differ in diabetes duration, BMI, and representation of women with type 2 diabetes. A better glycemic control (HbA1c, mmol/mol) was achieved in the Group A in all trimesters – 1st trimester: 59 (47, 67) vs 66 (56, 76), 2nd trimester: 46 (40, 52) vs 54 (48, 59) and 3rd trimester: 46 (40, 51) vs 53 (47, 60), p = 0.01. The caesarean section rate decreased (65.2 % vs 87.5 %, p < 0.05). The incidence of the respiratory distress syndrome after adjustment for age and diabetes duration also decreased (8.9 % vs 18.2 %, p < 0.05). A decreasing trend in the rate of premature delivery before 34th week of gestation (1.1 % vs 6.3 %) and neonatal mortality (1.1 % vs 2.9 %) was observed, however, the differences were not statistically significant.
