Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. Takotsubo cardiomyopathy is an important entity in the differential diagnosis of acute coronary syndrome. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Castlemanova choroba je vzácnou nosologickou jednotkou, která byla v literatuře poprvé popsána v roce 1954 Castlemanem a Lownovou. Jde o onemocnění primárně postihující lymfatické uzliny, u kterého se rozlišují čtyři histomorfologické podtypy a dva klinické obrazy (uni a multicentrický). Nejčastěji se klinicky projeví jako lymfadenopatie mediastina, krku či retroperitonea. Příznaky nemoci jsou výrazně heterogenní a záleží na daném histologickém podtypu. Taktéž prognóza se liší podle typu nemoci, kdy může jít jak o zcela benigní nálezy (zejména forma unicentrická), tak o potenciálně maligní formu, při které nemoc progreduje až do stadia maligní lymfoproliferace (multicentrická forma). Autoři prezentují případ 29letého muže s klinicky manifestním (ikterus, poruchy střevní pasáže) objemným tumorem retroperitonea lokalizovaným subhepatálně, který byl úspěšně radikálně exstirpován. Pooperační průběh byl nekomplikovaný. Definitivní histopatologická diagnóza potvrdila hyalinně-vaskulární typ Castlemanovy nemoci. Vzhledem k tomu, že se jednalo o multicentrickou formu onemocnění, bude následovat adjuvantní onkologická léčba. It is rare sickness which primary affects lymfatic nodes. There are four histo-morfologic subtypes and two clinic manifestations (uni and mulicentric). Comon clinical picture is simple lymfadenopathy of neck, medistinal a retroperitonal lymfatic nodes. Signs of this disease are heterogenic and depend on histological subtype. Also prognosis depends on type of illness. It should take place as benign diagnosis (unicentric form) but also can be potencionally malignant form which shlould exacerbate to malignant lymfoproliferation (multicentric form). Authors present the case of 29 years old men with clinical manifestation (jundice, intermitent bowel obstruction) of lagre retroperitoneal tumor in subhepatal localization. The treatment was radical surgical extirpation, surgery was without complications. Definitive histological diagnosis was confirmed as hyaline-vascular type of Castleman disease. Cíl práce: Upozornit na možnosti laparoskopického chirurgického řešení náhlé příhody břišní v graviditě. Typ práce: Kazuistika. Vlastní pozorování: Formou kazuistiky prezentujeme případ těhotné ve 23. týdnu gravidity s náhlou příhodou břišní neznámé etiologie a její laparoskopické řešení. Závěr: Laparoskopie v rukou zkušeného operatéra se jeví jako velmi bezpečná metoda v případech nejasné bolesti podbříšku, a to i v graviditě v závislosti na velikosti dělohy, tedy stáří gravidity. V tomto případě byla příčinou torze fibromu tuby, která byla řešena salpingektomií., The aim of the study: Draw attention to possibilities of LPSC surgical solutions of acute abdominal evens in pregnancy. Type of paper: Case study. Own observation: We are presenting a case of a pregnant woman at 23 weeks who suffered an acute abdominal event of unknown etiology and its laparoscopic solution. Conclusion: Laparoscopy in the hands of an experienced surgeon appears as a very safe method in cases of lower abdominal pain of unclear origin also in pregnancy, depending on the size of the uterus, i.e. the stage of pregnancy. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Úvod: Bolesti v pravém podbřišku jsou nejčastějším důvodem hospitalizace na chirurgických pracovištích obecně. Zároveň akutní apendicitida je nejčastěji se vyskytující náhlá příhoda břišní. Včasná indikace k apendektomii je jedinou metodou léčby, obvykle s nekomplikovaným průběhem a krátkou délkou hospitalizace. V rámci diferenciálně-diagnostické rozvahy je nutné pátrat i po dalších symptomech, zejména dlouhodobých, při akutní exacerbaci často ustupujících do pozadí. Kazuistika: Prezentujeme případ 38leté pacientky, která byla přijata pro akutní bolesti břicha v pravém podbřišku. Klinický i laboratorní nález nasvědčoval pro typicky probíhající akutní apendicitidu, byla proto indikována k apendektomii. Při operaci byl nalezen hemorhagický výpotek v pánvi, na apendixu makroskopicky netypický nález pro akutní zánět. Překvapivý byl nález ztluštění stěny terminálního ilea a zvětšení pravých adnex. Nález si vyžádal rozšíření operačního výkonu na ileocekální resekci a pravostrannou adnexektomii. Definitivní diagnózu stanovilo histologické vyšetření potvrzující endometriózu apendixu, terminálního ilea a pravého ovaria. Dlouhodobou dispenzarizací nemocné byla následně zjištěna prekanceróza, resp. mikroadenokarcinom hrdla děložního. Závěr: Stanovení diagnózy endometriózy apendixu předoperačně je prakticky nemožné. V rámci diferenciální diagnostiky lze na ni pomýšlet u fertilních žen s chronickými gynekologickými obtížemi neobjasněné etiologie. Diagnostickou, resp. léčebnou metodou volby je laparoskopie, která dovolí přehlédnutí celé peritoneální dutiny, včetně malé pánve, provedení apendektomie a eventuálně excizi podezřelých ložisek z endometriózy. Definitivní stanovení diagnózy je doménou histopatologického vyšetření. Při průkazu endometriózy apendixu nebo jakékoli jiné lokalizace je nezbytné předání pacientky do péče gynekologa., Introduction: In general, pain in the lower right abdomen is the most frequent reason for hospital surgical admissions, acute appendicitis representing the most common cause of operation for acute abdomen. Timely appendectomy remains the only treatment in the early stages of inflammation and is usually uncomplicated, requiring only a short hospital stay. A differential diagnostic analysis necessitates a search for other, particularly long-term symptoms that might be driven to the background in cases of acute exacerbation. Case report: The case report presents a 38-year-old female patient who was admitted for lower right abdominal pain. Clinical examination and a blood test both suggested typical acute uncomplicated appendicitis, and therefore the patient underwent appendectomy. Haemorrhagic peritoneal fluid and nodularity of the appendix not typical for appendicitis was found. Oedema of the terminal ileum and a right adnexal tumour were a surprising finding. A more extensive surgical procedure involving ileocaecal resection and right-side adnexectomy was finally performed with regard to the intraoperative finding. The definitive diagnosis of appendiceal endometriosis, endometrial mass in the terminal intestine and ovarian endometriosis was established by histological evaluation. Long-term follow-up revealed microadenocarcinoma of cervix uteri. Conclusion: It is generally very difficult to confirm appendiceal endometriosis before operation, and revealing primary appendiceal endometriosis is virtually impossible. It is advisable to consider endometriosis in fertile women with chronic abdominal pain of unclear aetiology and gynaecological symptoms in their personal history. The best diagnostic and therapeutic method, respectively, is laparoscopy enabling exploration of the entire peritoneal cavity including the minor pelvis, and performing appendectomy as well as excision of suspicious endometrial lesions. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci. Granulomy často imitují maligní tumory v jaterní krajině, a chirurg tak může být lékařem, který dané onemocnění odhalí jako první při resekčním výkonu či cholecystektomii., The authors present two case reports to demonstrate wide differential diagnosis of granulomatous disorders of the hepatobiliary tract. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma.
