Úvod: Systémový lupus erythematodes (SLE) je chronické autoimunitní onemocnění charakterizované multisystémovým postižením. Cílem naší studie bylo ozřejmit výskyt snížených hodnot koncentrací imunoglobulinů (s-Ig) v séru v populaci nemocných se SLE. V retrospektivní studii bylo zhodnoceno 799 výsledků vyšetření s-Ig získaných od 157 nemocných splňujících revidovaná klasifikační kritéria. Výsledky: Sérová koncentrace imunoglobulinů nižší než dolní hranice normy byla zjištěna celkem u 29 ze 157 nemocných (18,5 %). Nejčetnější byl výskyt izolovaného snížení IgG 12 ze 157 (7,6 %), 2 nemocné naplňovaly kritéria selektivního deficitu IgA a v jednom případě se jednalo o vzácnou koincidenci s běžnou variabilní imunodeficiencí (common variable immunodeficiency – CVID). Článek podrobněji prezentuje 2 případy nemocných se SLE komplikované těžkou hypogamaglobulinemií a infekcemi vyžadujícími dlouhodobou substituční léčbu imunoglobuliny. V prvním případě se jedná o vysoce pravděpodobný případ běžné variabilní imunodeficience a v případě druhém pak o léky indukovanou hypogamaglobulinemii. Tato žena s anamnézou maligního lymfomu a současným závažným multiorgánovým postižením při SLE byla léčena rituximabem po selhání konvenční léčby. Závěr: S protilátkovými deficiencemi se můžeme setkat i u nemocných se SLE. Opakované měření sérové koncentrace imunoglobulinů by mělo být součástí rutinního sledování těchto nemocných. Na možnost CVID bychom měli myslet v případě opakujících se sinopulmonárních infekcí, zejména v kombinaci s absencí aktivity SLE. Léčba rituximabem může vést i u nemocných se SLE k dlouhodobé supresi B-lymfocytů s rozvojem sekundárního protilátkového deficitu, který v některých případech vyžaduje substituční léčbu imunoglobuliny., Introduction: Systemic lupus erythematosus (SLE) is a chronic autoimmune multisystem disease. The aim of our study was to clarify the frequency of decreased serum immunoglobulin levels in SLE patients. There were evaluated 799 results of serum immunoglobulin levels gained from 157 patients fulfilling revised ACR criteria in the retrospective study. Results: The immunoglobulin levels under the normal range were found in 29/157 (18.5 %) patients. The most frequent was isolated reduction of IgG 12/157 (7.6 %), two persons fulfilled criteria for selective IgA deficiency, and one case possible diagnosis of common variable immunodeficiency (CVID). Additionally we report two cases of SLE patients complicated by severe hypogammaglobulinaemia and infectious complications with necessity of long-term immunoglobulin substitution therapy. The diagnosis of CVID is highly probable in the first case. The second case presents sever drug-induced hypogammaglobulinaemia. This female with lymphoma history and multiorgan impairment due to acute SLE was treated with rituximab after convention therapy failure. Conclusion: Humoral immunodeficiency may occur in SLE patients. The monitoring of serum immunoglobulin levels could be a routine in these patients. The CVID diagnosis is possible in patients suffering from recurrent sinopulmonary infections, especially in combination with absence of lupus activity. Autoři prezentují kazuistiku pacientky s časným karcinomem sigmoidea v těhotenství a problematiku diskutují ve světle současných literárních poznatků. Syndrom HELLP velmi závažná a život ohrožující komplikace, která je přímo vázána na těhotenství. Projevuje se nejčastěji bolestmi v pravém podžebří, bolestmi hlavy, neklidem ženy, nauzeou a typickými laboratorními nálezy. Práce popisuje případ rodičky po císařském řezu ve 38. týdnu gravidity pro HELLP-syndrom z pohledu ošetřovatelské péče. Spinální arteriovenózní malformace jsou skupina vzácných onemocnění, jejichž klinické projevy mohou být velmi rozmanité. Optimální léčebný postup je nutno vždy volit individuálně a pacienty směřovat do specializovaných center. Cílem sdělení je uvést přehlednou klasifikaci SAVM, jejich diferenciální diagnostiku a terapii a dále prezentovat dvě kazuistiky. První kazuistika se zmiňuje o případu 27letého muže, u kterého se rozvinuly silné bolesti bederní páteře s dominující meningeální symptomatikou v klinickém obraze. Na MR hrudního úseku páteře (Th11/12) byla nalezena AVM zásobovaná hypertrofickou radikulopiální tepnou. Na specializovaném pracovišti byla provedena úspěšná embolizace AV zkratu. Druhá kazuistika uvádí případ 41leté ženy, které vznikla při lyžování náhlá prudká bolest za krkem. V neurologickém obraze došlo k rozvoji inkompletní léze míšní s kvadruparézou s levostrannou akcentací do těžkého stupně s hranicí čití ve výši C1. Na provedeném MR a MRAG krční míchy a následně na DSA vertebrálních tepen byla detekována AVM zásobovaná levou arteria spinalis posterior. Terapeuticky byla indikována elektivní endovaskulární embolizace a pacientka byla ošetřena na specializovaném pracovišti ve Francii., Spinal arteriovenous malformations are a group of rare diseases with highly diverse clinical manifestations. The optimal therapeutic practice should always be selected individually and patients should be sent to specialized centres. The objective of this paper is to provide a structured classification of spinal AVMs, their differential diagnosis and therapy, and to present two case reports. The first report is concerned with a case of a 27-year-old man who developed severe pain in the lumbar spine with predominantly meningeal symptomatology. An AVM fed by a hypertrophic radiculopial artery was found on the thoracic spine (Th11/12) MR. A specialized centre performed successful embolization of an arteriovenous shunt. The second report presents a case of a 41-year-old woman, who felt sudden sharp pain in the neck while skiing. Neurologically, she developed an incomplete spinal cord lesion with quadriparesis with left-sided accentuation of severe intensity and sensation threshold at C1. The MR and MRAG of the cervical spinal cord and the following DSA of vertebral artery detected AVM fed by the left posterior spinal artery. Úvod: Splenóza predstavuje autotransplantáciu tkaniva sleziny do atypických miest v brušnej dutine, hrudníku alebo iných častiach organizmu. Kazuistika: Autori popisujú prípad splenózy u 35 ročnej ženy, ktorá bola hospitalizovaná s bolesťami brucha, a u ktorej bola pred 19 rokmi vykonaná splenektómia pre traumatickú ruptúru s implantáciou časti sleziny do omenta. Pre intenzívne ťažkosti bola indikovaná chirurgická revízia. Histopatológ potvrdil diagnózu splenózy v exstirpovanom tkanive. Záver: Napriek upusteniu od autotransplantácie tkaniva sleziny po splenektómii sa môže chirurg stále stretnúť s diagnózou splenózy u pacientov po traumatických ruptúrach sleziny. Každý náhodný nález splenózy pri inej operácii je vhodné histologizovať., Introduction: Splenosis is the autotransplantation of splenic tissue in atypical locations in the abdomen, chest or other parts of the body. Case report: Authors present a case of splenosis in a 35 years old woman hospitalized with abdominal pain, who underwent splenectomy for traumatic rupture with splenic tissue implantation in the omentum 19 years ago. Surgical revision was indicated for intensive pain. Histopathology confirmed the diagnosis of splenosis in the excised tissue. Conclusion: Despite the abandonment of splenic tissue autotransplantation after splenectomy, surgeons may still encounter the diagnosis of splenosis in patients after traumatic splenic rupture. Behçet's disease is a multisystem chronic inflammatory disease generally presenting as recurrent oral-genital ulcers and uveitis. Vascular involvement is a common presentation. However, cardiac involvement is rare in this disorder. In this case report, we present a large right ventricular thrombus detected both in computed tomography and echocardiography in a 32 year-old male patient with complaints of fever, haemoptysis and weight loss. Buddův-Chiariho syndrom je soubor onemocnění s velice heterogenním spektrem příčin vzniku podmíněný blokádou odtoku krve z jater. V tomto kazuistickém sdělení je popsán případ 32leté pacientky se symptomatickou stenózou jaterní žíly idiopatického původu, která byla léčena na našem pracovišti v letech 2014 a 2015. Žena trpěla refrakterním fluidothoraxem, přičemž po neúspěšné diuretické léčbě byla odeslána na angiointervenční oddělení naší nemocnice. Do stenotické pravé jaterní žíly, při hypoplastické střední a levé, byl implantován balonexpandibilní stent. Po 6 měsících bez obtíží se znovu objevila dušnost na podkladě fluidothoraxu, při provedené angiografii byla diagnostikována restenóza ve stentu a byla provedena dilatace balónkem. K dosažení regrese nebylo nutné vytvoření transjugulární portosystémové spojky (TIPS), ale postačilo pouze zavedení balonexpandibilního stentu, což se ukázalo jako dobré řešení pro tento vzácný typ Buddova-Chiariho syndromu., Budd-Chiari syndrome is a group of rare disorders with heterogenous spectrum of causes based on liver outflow blocation. There is a case report of a 32 year old female patient with syptomatic hepatic vein stenosis of idiopathic origin. She was treated in our department in 2014 and 2015. She suffered from refractory hydrothorax and after diuretic therapy was sent to angiointerventional unit of our hospital. The left and middle hepatic veins were hypoplastic. The stenotic right hepatic vein was treated with implantation of balloon expandable stent. After 6 months without symptoms she suffered from shortness of breath with hydro-thorax, on angiography in-stent restenosis was found and balloon dilatation was performed. Autoři referují o vzácném případu traumatické totální avulze očního bulbu s optickým nervem v úrovni chiazmatu. Kazuistické sdělení retrospektivně popisuje průběh ročního sledování mladé ženy, která byla zraněna při hasičských závodech. Avulze bulbu a optiku, kombinovaná s poraněním chiazmatu, rezultovala v hemianopický defekt druhostranného zorného pole. Při traumatu vzniklo subarachnoidální krvácení do oblasti cisteren chiazmatu, jež spontánně ustoupilo. Nedošlo k rozvoji vaskulárních spazmů mozku. Celková kortikoterapie a neuroprotekce neměla vliv na průběh onemocnění. Během sledovací doby objektivně došlo k descendentní atrofii korespondujících axonů a retinálních gangliových buněk. V současné době neexistuje efektivní terapie poranění v oblasti chiazmatu., The authors report a rare case of complete traumatic avulsion of the eyeball with the optic nerve at the level of chiasm. The case report retrospectively describes a year-long monitoring of a young woman who was injured in fire-fighter races. Avulsion of the globe and optic nerve combined with injured chiasm resulted in contralateral hemianopic visual field defect. The trauma led to subarachnoid hemorrhage in cisterns of chiasm, which spontaneously resolved. There was no development of vascular spasms in the brain. Systemic corticosteroid therapy and neuroprotection did not affect the course of the disease. During the observation period, descending atrophy of the corresponding axons and retinal ganglion cells developed. There is currently no effective therapy of chiasm injuries., and L. Hejsek, V. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy. Vazospazmy jsou považovány za hlavní příčinu vysoké mortality a špatné prognózy pacientů se subarachnoidálním krvácením způsobeným rupturou aneuryzmatu. Námi popisovaná kazuistika se týká 30leté pacientky, která byla přijata s Glasgow Coma Scale 14 a těžkou levostrannou hemiparézou. Počítačová tomografie odhalila subarachnoidální krvácení. Jeho příčinou byla ruptura aneuryzmatu na arteria cerebri media vpravo. Aneuryzma bylo zaklipováno. Pro symptomatické vazospazmy jsme opakovaně provedli mechanickou i farmakologickou angioplastiku. Přesto došlo k rozvoji ischemie v pravé mozkové hemisféře, což nás vedlo k dekompresivní kraniektomii. Finálně vazospazmy regredovaly a nastala úplná úprava neurologického deficitu. Čtyři roky po operaci je pacientka plně soběstačná, bez poruchy hybnosti končetin, s nedotčenými kognitivními funkcemi., Vasospasms are considered to be the major cause of high mortality and bad prognosis in patients with subarachnoid hemorrhage caused by a rupture of an aneurysm. Our case report describes a 30-year-old patient admitted with Glasgow Coma Scale 14 and severe left-sided hemiparesis. Computed tomography revealed subarachnoid hemorrhage. Its etiology was ruptured aneurysm of the right middle cerebral artery. The aneurysm was clipped. Mechanical and pharmacological angioplasty was repeatedly performed to manage symptomatic vasospasms. Despite these interventions, an ischemic lesion developed in the right brain hemisphere that prompted us to perform decompressive craniectomy. Finally, vasospasms, as well as the neurological deficit, disappeared.
