Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Podáváme přehled současných názorů na kombinační léčbu a na postavení fixních kombinací v léčbě hypertenze podle doporučení ESH/ESC a ČSH z roku 2013. Mezi nejčastěji doporučované dvojkombinace patří blokátor renin-angiotenzinového systému (ACEI nebo sartan) a blokátor vápníku, dále blokátor renin-angiotenzinového systému a diuretikum a blokátor vápníku a diuretikum. V roce 2014 se na českém trhu objevila fixní kombinace ACE inhibitor (perindopril), blokátor vápníku (amlodipin) a diuretikum (indapamid). Ve studii the PIANIST u 4 731 nedostatečně kontrolovaných hypertoniků fixní trojkombinace perindopril, amlodipin a indapamid vedla ke snížení krevního tlaku o 28,3/13,8 mm Hg a k dostatečné kontrole hypertenze u 92 % nemocných. Výhoda fixních kombinací je především ve větší komplianci nemocného, a tím v lepší kontrole hypertenze. Trojkombinaci k dobré kontrole tlaku potřebuje asi třetina hypertoniků., We present an overview of the present views on combination treatment and on fixed combinations in the treatment of hypertension according to guidelines of ESH/ESC and ČSH from 2013. The most frequently recommended dual combinations include a blocker of the renin-angiotensin system (ACEI or sartan) and a calcium channel blocker, and further a blocker of the renin-angiotensin system and a diuretic and a calcium channel blocker and a diuretic. In 2014 a fixed-dose combination of an ACE inhibitor (perindopril), a calcium channel blocker (amlodipine) and an diuretic (indapamide) appeared on the Czech market. Within the PIANIST study including 4 731 insufficiently controlled hypertensives, a fixed-dose triple combination of perindopril, amlodipine and indapamide led to a decrease in blood pressure by 28.3/13.8 mm Hg and to a sufficient control of hypertension in 92 % of patients. The advantage of fixed combinations primarily consists in greater compliance of patients and thereby in a better control of hypertension. Na indikace zavedení kaválního filtru v léčbě žilní tromboembolie je stále nejednotný názor. Předkládaný literární přehled je pokusem usnadnit rozhodování v klinické praxi. K problematice lze najít vysoký počet publikací, ale jen jedna splňuje podmínky prospektivní, randomizované, srovnávací studie – PREPIC 1. Během 8 let sledování pacientů byla plicní embolie méně častá ve skupině s kaválním filtrem než ve skupině bez filtru, ovšem za cenu častější návratnosti hluboké žilní trombózy. Jiné observační studie s použitím permanentních filtrů jako ve studii PREPIC 1 došly k podobným výsledkům. Zhruba posledních 10 let jsou k dispozici odstranitelné kavální filtry, které dávají možnost vyhnout se po vynětí pozdním komplikacím zaznamenaným ve studii PREPIC 1. Další studie PREPIC 2 provedená s odstranitelnými filtry má uspořádání, aby dala odpověď, je-li tomu tak. Výsledky jsou dosud k dispozici jen jako abstrakt z konference a mají tedy jen informativní povahu. Nicméně opakování plicní embolie a opakování hluboké žilní trombózy bylo méně časté ve skupině bez filtru než ve skupině se zavedeným filtrem. Vyhodnocení případů z registru RIETE ukázalo jen trend ve prospěch zavedeného filtru v parametru celkové mortality. Zřetelnější je protekční účinek zavedeného filtru u hemodynamicky nestabilních pacientů (kardiogenní šok, masivní embolizace, prudký pokles krevního tlaku), a to jak s trombolytickou léčbou, tak bez ní, jak ukázala analýza z jiného registru hospitalizovaných pacientů v USA. Metaanalýza studií provedených u pacientů s polytraumaty došla k závěru, že filtr je účelné zavést zejména u pacientů, u nichž je kontraindikována antikoagulační léčba. Uvedená data jsou krátce diskutovaná na pozadí existujících doporučení odborných společností. Doporučení Evropské kardiologické společnosti (2014) uvádí, že kavální filtry mohou být použity, když je absolutní kontraindikace k antikoagulační léčbě a vysoké riziko rekurence hluboké žilní trombózy., The indication for vena cava filters (VCF) in treatment of venous thromboembolism is still controversial. The presented overview should support the practical decisions. Beside of large volume of observational date there is only one study – PREPIC 1 – fulfilling requirements of prospective randomized design. During 8 years of follow up, pulmonary embolism (PE) was less frequent in the group with VCF than in the group without VCF, but at the cost of more frequent deep vein thrombosis (DVT). Other, but observational studies, showed similar results. Since last 10 years retrievable VCF are available. PREPIC 2 study was settled to prove, if use of retrievable VCF and their early removal will decrease the frequency of late complications observed in PREPIC 1. The results are available as conference abstract only, but it was presented that recurrence of DVT and PE was less frequent in group without IVC than with inserted VCF. Evaluation of impact of VCF insertion on mortality from RIETE registry showed only a trend which was in favour of VCF. On the other hand, a protective effect of VCF was demonstrated in hemodynamically unstable patients with PE (cardiogenic shock, massive embolism) with or without thrombolytic therapy evaluating cases from US NIS registry. Metaanalysis of studies in patients with polytrauma showed VCF protection mainly in patients where anticoagulation was contraindicated. Data gained from literature are discussed with existing guidelines. 2014 Recommendations of European Cardiologic Society is thatVC filters may be used when there are absolute contraindications to anticoagulation and a high risk of VTE recurrence.
