Infekce močových cest patří u dospělých k nejčastějším bakteriálním infekcím, v naprosté většině je způsobena Escherichia coli. Přestože jsou k dispozici doporučené postupy k diagnostice a terapii uroinfektů, neexistuje jednotný přístup založený na důkazech. Diagnóza je často postavena klinicky. U predisponovaných jedinců mohou infekce močových cest vést k postižení horních cest močových nebo až k urosepsi. Základem léčby je antibiotická terapie, k dispozici jsou i profylaktické postupy a alternativní strategie. Závažný problém představuje rezistence na antibiotika. Ke zpřesnění diagnostiky IMC je zapotřebí dalších studií. V tomto přehledném článku diskutujeme prevalenci, diagnostiku včetně jejích úskalí, léčbu a prevenci infekcí močových cest a asymptomatické bakteriurie u dospělých., Urinary tract infections are one of the most frequent clinical bacterial infections in adults. Escherichia coli is the organism that causes urinary tract infections in most patients. Consensus guidelines have been published to assist clinicians with diagnosis and treatment of urinary tract infection; however, a single evidence‑based approach to diagnosis of urinary tract infection does not exist. There is no gold-standard definition of urinary tract infection that clinicians agree upon. In a subgroup of individuals with coexisting morbid conditions, complicated urinary tract infections can lead to upper tract infections or urosepsis. Although the initial treatment is antimicrobial therapy, use of different prophylactic regimens and alternative strategies are available to reduce exposure to antibiotics. Future studies improving the diagnostic accuracy of urinary tract infections are needed. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. The disease by itself is a poor prognostic factor in a one-year mortality rate. Key words: incidence infective – infective endocarditis, and Rudolf Koubek

Při výběru individuální léčby mohou být využity krátce i dlouze působící přípravky. Kromě glykemie ovlivňují i další faktory, z nichž v současnosti dominuje příznivý vliv na hmotnost a hypertenzi. Jsou popsány patofyziologické mechanizmy jejich účinku, farmakologické a klinické rozdíly a jejich využití v klinické praxi. These are the new drugs for the treatment of type 2 diabetes, which have a significant antihyperglycemic effect. For the individual treatment can be used short- acting and long-acting drugs. In addition to glucose effect there are influenced another factors, especially weight and hypertension. The patophysiological mechanism of their action, pharmacological and clinical differences and their use in clinical practice are described. Tento přehledový článek vychází z kurzu prezentovaného na kongresu Americké urologické asociace v červnu 2014, který byl zaměřen na problematiku intersticiální cystitidy a syndromu bolestí v močovém měchýři. Autoři uvádějí nejnovější úpravy v taxonomii a názvosloví těchto syndromů včetně aktuálních definic a nabízejí pohled na bolestivé syndromy z gynekologické i urologické perspektivy. Článek je doplněn přehlednými diagramy a algoritmy uznávaných mezinárodních společností, které mohou sloužit jako vodítko při diagnostice a léčbě těchto komplexních syndromů., This review is based on the course presented on AUU meeting held in July 2014 focusing on the conditions known as interstitial cystitis and bladder pain syndrome. The authors are introducing last updates in taxonomy and nomenclature of these syndromes, including current definitions and offering an overview of painful syndromes from both gynaecologic and urologic perspective. PURPOSE: A variety of anti-glaucomatous shunt designs have been proposed so far. This study evaluates the feasibility of a novel shunt design, the intra-scleral parathalamus implant (IPI), in a rabbit eye model. METHODS: Ten healthy albino rabbits were included. Measurements of the IOP, using TonoPenXL, as well as ocular rigidity (OR) and aqueous outflow facility (AO), using a previously described methodology, were performed in both eyes of each animal. Subsequently, the IPI was implanted at the left eye of all animals whereas the right eye served as control. Measurements of IOP were repeated at weekly intervals for 2 months postoperatively whereas measurements of OR and AO were repeated at the 1st and 2nd postoperative week. RESULTS: The IOP decreased significantly whereas the AO increased significantly postoperatively at the operated eyes. A significant postoperative decrease in OR was also recorded at the operated eyes. Respective differences at the fellow eyes were statistically not significant. Two animals developed postoperative complications and were excluded from follow-up. CONCLUSIONS: Results imply that the implantation of the IPI is feasible in rabbit eyes with significant favorable effects on the IOP, AO and OR. and E. T. Detorakis, K. Loggaki, N. Karyotakis, H. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Karotická endarterektomie byla dlouho považována za zlatý standard v léčbě a prevenci ischemické cévní mozkové příhody u pacientů s významnou stenózou vnitřních karotických arterií. Karotický stenting, i přes počáteční limitace, dosahuje v současné době výsledků srovnatelných s karotickou endarterektomií a u selektovaných pacientů je považován za rovnocennou volbu. Bezpečnost obou výkonů je dána zejména rizikem periprocedurálních komplikací, které jsou významně ovlivněny zkušenostmi operatéra. Transkraniální dopplerovská ultrasonografie je zavedenou diagnostickou a monitorovací metodou, jejíž využití přispívá ke zkvalitnění péče o pacienty podstupující karotické intervence. Tento článek shrnuje znalosti týkající se invazivní léčby karotických stenóz se zaměřením na možnosti optimalizace pomocí transkraniální dopplerovské ultrasonografie., Surgical carotid endarterectomy was for a long time considered the standard approach to the treatment of atherosclerotic carotid artery disease. Despite its initial limitations and slow acceptance, carotid artery stenting has evolved into an elaborate method currently considered to be equivalent and in some patients even preferable to carotid endarterectomy. Given the fact that the long‑term outcomes of carotid artery stenting and carotid endarterectomy are comparable, the major risk of both procedures is derived from the periprocedural complications, which are highly operator dependent. Transcranial Doppler ultrasound is a well‑established diagnostic and monitoring tool allowing optimising the care of patients with carotid artery disease.
